Background:To report a surgical method for treating corneal oedema in a case of late-onset Descemet membrane detachment after penetrating *** presentation:A 55-year old patient presented with sudden visual loss in his left eye 28 years after penetrating keratoplasty for ***-lamp biomicroscopy revealed a distortion of the corneal graft anatomy with protrusion of the graft and peripheral thinning and steepening in the residual host tissue,accompanied by corneal graft *** segment optical coherence tomography revealed detachment of Descemet membrane localized to the area of the graft *** proceeded with a full-thickness,partially circumferential incision in the graft-host junction,followed by repositioning and re-suturing of the graft in place,and intracameral air injection in order to achieve reattachment of Descemet ***:Corneal graft repositioning in combination with re-bubbling may represent an effective therapeutic option in keratoconic patients with peripheral thinning in the residual host corneal tissue and subsequent Descemet membrane detachment.

The development of innovative, complex marine systems, such as autonomous ship concepts, has led to risk-based approaches indesign and operation that provide safety level quantification and continuous risk assessment. The existing approaches to dynamicrisk assessmentmainly aim at updating accident probabilities for specific risk scenarios, based on knowledge of system operation andfailure, aswell as on past accident and failure information. However, for innovative marine systems that include complex interactions,our ability to identify anything that might go wrong is very limited, which may lead to unidentified risks, and failure data may not beavailable. This paper presents the foundations of a framework for dynamic risk assessment, which is equally applicable to mannedand autonomous ships and mainly relies on information about the safe operational envelope and real-time information regardingdeviations from safety. Inspiration is drawn from how the biological immune system identifies the risk of infection in a dynamicenvironment. The objective is to show the feasibility and benefits of our approach for quantifying the operational risk of marinesystems. This paper provides the conceptual basis for developing ship specific applications and describes a process for dynamic riskassessment that is methodologically based on artificial immune systems. To demonstrate the implementation of our framework, wedescribed, an illustrative example that involves a ship in a grounding scenario. The results show that the bio-inspired assessmentprocess and risk description can reflect the changes of the risk level of a marine system.

Background:To present a method,alternative to penetrating keratoplasty,for the restoration of impaired corneal clarity with anterior stromal scarring following long-standing corneal graft *** presentation:A 48-year old female who had previously underwent Descemet stripping automated endothelial keratoplasty(DSAEK)for the treatment of pseudophakic bullous keratopathy,presented with longstanding corneal oedema and anterior corneal scarring.A significant improvement in corrected distance visual acuity was demonstrated,as corneal clarity was restored following graft exchange and phototherapeutic keratectomy(PTK).Conclusions:The combination of corneal graft exchange and phototherapeutic keratectomy may represent an effective therapeutic option for long-standing corneal oedema with concomitant anterior corneal scarring after failure of a DSAEK graft.

This study was designed to study the effect of diet supplementation with an organic acids-based formulation(OABF) on luminal-and mucosa-associated bacteria, concentration of volatile fatty acids(VFA), microbial glycolytic enzyme activity and expression of mucin 2(MUC2), immunoglobulin A(IgA)and tight junction protein, i.e., zonula occludens-1(ZO1), zonula occludens-2(ZO2), claudin-1(CLDN1),claudin-5(CLDN5) and occludin(OCLN), genes at the ileal and cecal level. A 2 × 2 factorial design was used having OABF inclusion and avilamycin as main factors. Subsequently, 544 day-old male Cobb broilers were allocated in the following 4 treatments, each with 8 replicates: no additions(CON), 1 g OABF/kg diet(OA),2.5 mg avilamycin/kg diet(AV) and combination of OA and AV(OAAV). The trial lasted for 42 days. In the ileum, OAAV resulted in lower mucosa-associated total bacteria levels(PO × A= 0.028) compared with *** addition, ileal digesta levels of Clostridium perfringens subgroup were decreased by avilamycin(PA= 0.045). Inclusion of OABF stimulated the activity of microbial glycolytic enzymes, whereas avilamycin resulted in lower acetate(PA= 0.021) and higher butyrate(PA= 0.010) molar ratios. Expression of ZO1 and CLDN5 was down-regulated by both OABF(PO= 0.016 and PO= 0.003, respectively) and avilamycin(PA= 0.016 and PA= 0.001, respectively). In addition, CLDN1 was down-regulated in AV compared with CON(PO × A= 0.012). Furthermore, OABF down-regulated MUC2(PO= 0.027), whereas avilamycin down-regulated nuclear factor kappa B subunit 1(NFKB1)(PA= 0.024), toll-like receptor 2 family member B(TLR2B)(PA= 0.011) and toll-like receptor 4(TLR4)(PA= 0.014) expression. In the ceca, OABF inclusion increased digesta levels of Clostridium coccoides(PO= 0.018) and Clostridium leptum(PO= 0.040) subgroups, while it up-regulated MUC2 expression(PO= 0.014). We have recently read with interest the mini-review article"Therapeutic endoscopy for the treatment of post-bariatric surgery complications".The abovementioned article is a brief overview of the different endoscopic modalities employed in the management of bariatric surgery complications and represents an important decision support tool for clinicians to improve their current *** we appreciate the endeavor of Larsen and Kozarek,based on our indepth analysis,we came across several minor issues in this article;thus,we present our comments in this *** case the authors contemplate these comments in their relevant research,we believe that their contribution would be considerable for future studies.

Chronic infection with hepatitis C virus(HCV) is a leading cause of liver-related morbidity and mortality worldwide and predisposes to liver fibrosis and endstage liver complications. Liver fibrosis is the excessive accumulation of extracellular matrix proteins, including collagen, and is considered as a wound healing response to chronic liver injury. Its staging is critical for the management and prognosis of chronic hepatitis C(CHC) patients, whose number is expected to rise over the next decades, posing a major health care challenge. This review provides a brief update on HCV epidemiology, summarizes basic mechanistic concepts of HCV-dependent liver fibrogenesis, and discusses meth-ods for assessment of liver fibrosis that are routinely used in clinical practice. Liver biopsy was until recently considered as the gold standard to diagnose and stage liver fibrosis. However, its invasiveness and drawbacks led to the development of non-invasive methods, which include serum biomarkers, transient elastography and combination algorithms. Clinical studies with CHC patients demonstrated that non-invasive methods are in most cases accurate for diagnosis and for monitoring liver disease complications. Moreover, they have a high prognostic value and are cost-effective. Non-invasive methods for assessment of liver fibrosis are gradually being incorporated into new guidelines and are becoming standard of care, which significantly reduces the need for liver biopsy.

Backgound Red cell distribution width(RDW) and neutrophil-to-lymphocyte ratio(NLR) are simple hematologic indices that have been used to predict adverse outcomes in different clinical settings. The aim of our study is to determine whether RDW and NLR can predict atrial fibrillation(AF) recurrence in patients undergoing AF ablation. Methods Consecutive patients, without known hematological disorders, who underwent AF catheter ablation between January 2014 and April 2017 were enrolled into this study. Blood samples were taken one day before and five hours after the ablation procedure. Results A total of 346 patients(224 males(65%), mean age: 59 ± 11 years old) were included. After a mean follow up of 26.2 ± 12.1 months, 80(23.1%) patients experienced late AF recurrence(defined as any recurrence after the blanking period of three months), while 97(28%) patients experienced early AF recurrence during the blanking period. Univariate analysis showed that early arrhythmia recurrence, type of AF and NLR after the procedure were significantly associated with late AF recurrence, while early arrhythmia recurrence and NLR remained significant in multivariate analysis. RDW was not associated with late AF recurrence. None of the parameters above predicted early arrhythmia recurrence. Conclusions Simple and inexpensive hematological indices such as NLR should be evaluated for their ability to predict AF recurrence in patients undergoing catheter ablation in larger prospective studies.
