Awareness and attitudes of Greek medical students on colorectal cancer screening

AIM: To prospectively assess the knowledge and attitudes of medical students (MS), as tomorrow’s physicians, about colorectal cancer (CRC) and its screening ***: Three hundred fourth year MS of the University of Athens were enrolled in this survey. Their selection was random, based on student identification card number. All participants completed an anonymous written questionnaire over a 4 month period. The questionnaire was divided into 4 sections and included queries about CRC-related symptoms, screening with colonoscopy and MS awareness and attitudes in this field. Following collection and analysis of the data, the results are presented as percentages of answers for each separate question. RESULTS: Two hundred and sixty-five students an-swered the questionnaire over a 4 mo period. Interestingly, only 69% of the study population considered CRC to be a high-risk condition for public health. However, the vast majority of participants identified CRC-related symptoms and acknowledged its screening to be of great value in reducing CRC incidence and mortality. A very small proportion (38%) had received information material regarding CRC screening (either during their medical training or as a part of information provided to the general public) and only 60% of the participants declared willingness to receive further information. Regarding colonoscopy, 85% would prefer an alternative to colonoscopy methods for CRC screening. Moreover, 53% considered it to be a painful method and 68% would appreciate more information about the ***: MS in Greece need to be better informed about CRC screening and screening colonoscopy.

AIM: To investigate the effect of rosuvastatin monotherapy on non-alcoholic steatohepatitis(NASH). At present there is no effective treatment for non-alcoholic fatty liver disease or its advanced form ***: This prospective study included 20 biopsy proven patients with NASH, metabolic syndrome(Met S) and dyslipidaemia. Biochemical parameters of the blood of the patients and an ultrasonography of the liver were performed at baseline. Then patients receivedlifestyle advice and were treated for a 12 mo period with rosuvastatin(10 mg/d) monotherapy. Patients were re-evaluated during the study at 3 mo intervals, during which biochemical parameters of the blood were measured including liver enzymes. A repeat biopsy and ultrasonography of the liver were performed at the end of the study in all 20 patients. Changes in liver enzymes, fasting plasma glucose, serum creatinine, serum uric acid(SUA), high sensitivity C reactive protein(hs CRP) and lipid profile were assessed every 3 mo. The primary endpoint was the resolution of NASH and the secondary endpoints were the changes in liver enzyme and lipid ***: The repeat liver biopsy and ultrasonography showed complete resolution of NASH in 19 patients, while the 20 th, which had no improvement but no deterioration either, developed arterial hypertension and substantial rise in triglyceride levels during the study, probably due to changes in lifestyle including alcohol abuse. Serum alanine transaminase, aspartate transaminase, and γ-glutamyl transpeptidase were normalised by the 3rd treatment month(ANOVA P < 0.001), while alkaline phosphatase activities by the 6th treatment month(ANOVA, P = 0.01). Fasting plasma glucose and glycated haemoglobin were significantly reduced(P < 0.001). Lipid values were normalised by the 3rd treatment month. No patient had Met S by the 9th treatment month. AIM: To identify plasma metabolites used as biomarkers in order to distinguish cirrhotics from controls and ***: A clinical study involving stable cirrhotic patients with and without overt hepatic encephalopathy was designed. A control group of healthy volunteers was used. Plasma from those patients was analysed using 1H- nuclear magnetic resonance spectroscopy. We used the Carr Purcell Meiboom Gill sequence to process the sample spectra at ambient probe temperature. We used a gated secondary irradiation field for water signal suppression. Samples were calibrated and referenced using the sodium trimethyl silyl propionate peak at 0.00 ppm. For each sample 128 transients(FID's) were acquired into 32 K complex data points over a spectral width of 6 KHz. 30 degree pulses were applied with an acquisition time of 4.0 s in order to achieve better resolution, followed by a recovery delay of 12 s, to allow for complete relaxation and recovery of the magnetisation. A metabolic profile was created for stable cirrhotic patients without signs of overt hepatic encephalopathy and encephalopathic patients as well as healthy controls. Stepwise discriminant analysis was then used and discriminant factors were created to differentiate between the three ***: Eighteen stabled cirrhotic patients, eighteen patients with overt hepatic encephalopathy and seventeen healthy volunteers were recruited. Patients with cirrhosis had significantly impaired ketone body metabolism, urea synthesis and gluconeogenesis. This was demonstrated by higher concentrations of acetoacetate(0.23 ± 0.02 vs 0.05 ± 0.00, P < 0.01), and b-hydroxybutarate(0.58 ± 0.14 vs 0.08 ± 0.00, P < 0.01), lower concentrations of glutamine(0.44 ± 0.08 vs 0.63 ± 0.03, P < 0.05), histidine(0.16 ± 0.01 vs 0.36 ± 0.04, P < 0.01) and arginine(0.08 ± 0.01 vs 0.14 ± 0.02, P < 0.03) and higher concentrations of glutamate(1.36 ± 0.25 vs 0.58 ± 0.04, P < 0.01), lactate(1.53 ± 0.11 vs 0.42 ± 0.05, P < 0.01),

AIM:To investigate the adipokine levels of leptin,adiponectin,resistin,visfatin,retinol-binding protein 4(RBP4),apelin in alcoholic liver cirrhosis(ALC).METHODS:Forty non-diabetic ALC patients[median age:59 years,males:35(87.5%),Child-Pugh(CP)score:median 7(5-12),CP A/B/C:18/10/12,Model for End-stage Liver Disease(MELD):median 10(6-25),follow-up:median 32.5 mo(10-43)]were prospectively *** serum adipokine levels were estimated in duplicate by *** characteristics were assessed with tetrapolar bioelectrical impedance ***’s rank correlation coefficient was used to assess possible associations with adipokine *** and multivariate Cox regression analysis was used to determine independent predictors for ***:Body mass index:median 25.9(range:20.1-39.3),fat:23.4%(7.6-42.1),fat mass:17.8(5.49-45.4),free fat mass:56.1(39.6-74.4),total body water(TBW):40.6(29.8-58.8).Leptin and visfatin levels were positively associated with fat mass(P<0.001/P=0.027,respectively)and RBP4 with TBW(P=0.025).Median adiponectin levels were significantly higher in CPC compared to CPA(CPA:7.99±14.07,CPB:7.66±3.48,CPC:25.73±26.8,P=0.04),whereas median RBP4 and apelin levels decreased across the spectrum of disease severity(P=0.006/P=0.034,respectively).Following adjustment for fat mass,visfatin and adiponectin levels were significantly increased from CPA to CPC(both P<0.001),whereas an inverse correlation was observed for both RBP4 and apelin(both P<0.001).In the multivariate Cox regression analysis,only MELD had an independent association with overall survival(HR=1.53,95%CI:1.05-2.32;P=0.029).CONCLUSION:Adipokines are associated with deteriorating liver function in a complex manner in patients with alcoholic liver cirrhosis.

AIM: To investigate CYP2E1 IgG4 autoantibody levels and liver biochemical markers in adult patients after anesthesia with desflurane. METHODS: Forty patients who were > 18 years old and undergoing elective surgery under general anes-thesia with desflurane were studied. Alpha-glutathione-S-transferase(aGST) and IgG4 antibodies againstCYP2E1 were measured preoperatively and 96 h post-operatively, as well as complete blood count, prothrom-bin time(PT), activated partial thromboplastin time(aPTT), international normalized ratio(INR), aspartate aminotransferase(SGOT), alanine aminotransferase(SGPT), g-glutamyl-transpeptidase(gGT), alkaline phosphatase, total serum proteins, albumin and bili-rubin. A separate group of 8 patients who received re-gional anesthesia was also studied for calibration of the methodology used for CYP2E1 IgG4 and aGST measure-ments. Smart lighting system using ANN-IMC for personalized lighting control and daylight harvesting

Lighting contributes a significant portion to the overall energy consumption in an office building. It is thus important to reduce the energy consumption of lighting systems especially for Net Zero Energy Buildings( NZEB). Maximizing daylight harvesting can significantly increase the energy savings. With increase in demand for satisfying occupant preferences in visual comfort,the need for personalized lighting in the office space is also rising. In this paper,a novel lighting control system for Net Zero Energy Buildings( NZEB) is proposed which models the lighting system using Artificial Neural Netw ork( ANN) and utili-zes this model with the Internal Model Control( IMC) principle for controller design. Modeling the lighting system using ANN reduces the challenge of modeling a large and complex system with inherent process variability without the need to analyze extensive data-sets. The proposed ANN-IMC controller uses feedback from sensors on the task table to maintain desired illuminance,is easy to tune with just one parameter and is robust to process variability. The proposed control design is applicable to square systems where the number of lights and number of sensors are equal. How ever,the proposed architecture can also be extended for controlling other lighting accessories such as roller blinds. The performance of the proposed lighting control system to harvest the daylight effectively is demonstrated using both simulation results and an experimental setup in test-bed environment. The versatility of the proposed system will allow an operator to deploy personalized lighting in an office space.

AIM: To investigate donor site's area histological and immunohistochemical knee cartilage appearances after resurfacing iatrogenic defects with biosynthetic plugs orautografts. METHODS: Thirty New Zealand White rabbits were used in this study. A full-thickness cylindrical defect of 4.5 mm(diameter) × 7 mm(depth) was created with a hand drill in the femoral groove of every animal. In Group A(n = 10) the defect of the donor site was re-paired with a biosynthetic osteochondral plug, in Group B(n = 10) with an osteochondral autograft, while in Group C(control group of 10) rabbits were left untreated. RESULTS: Twenty-four weeks postoperatively, smooth articular cartilage was found macroscopically in some trocleas' surfaces; in all others, an articular surface with discontinuities was observed. Twenty-eight out of 30 animals were found with predominantly viable chondrocytes leaving the remaining two-which were found only in the control group- with partially viable chondrocytes. However, histology revealed many statistical differences between the groups as far as the International Cartilage Repair Society(ICRS) categories are concerned. Immunofluoresence also revealed the presence of collagen Ⅱ in all specimens of Group B, whereas in Group A collagen Ⅱ was found in less specimens. In Group C collagen Ⅱwas not found. CONCLUSION: The matrix, cell distribution, subchondral bone and cartilage mineralization ICRS categories showed statistically differences between the three groups. Group A was second, while group B received the best scores; the control group got the worst ICRS scores in these categories. So, the donor site area, when repairing osteochondral lesions with autografting systems, is better amended with osteochondral autograft rather than bone graft substitute implant.

地点:昆士兰结核病(TB)控制中心(QTCC)。目的:调查昆士兰地区活动性肺结核诊断中的病人自身延误和医疗机构延误情况。设计:对摘取自QTCC数据库的资料进行分析并作病历回顾。经细菌学或组织学诊断的有症状病人构成一个总体组和一个涂阳肺结核(PSP)病例组。结果:病人自身延误的中位数是29天(总体组)和30天(PSP组)。医疗机构延误的中位数是22天(总体组)和11天(PSP组)。随着病例年龄增加以及移民定居时间的延长,医疗机构延误呈现明显上升趋势。45岁以上女性病人的医疗机构延误明显为长。与非土著澳人病例相比,来自高结核病发病率地区以及土著澳人病例的医疗机构延误要短。造成90天以上诊断延误的常见原因是未对病人进行适当的TB调查以及胸部X线检查误诊。结论:医生在对老年病人以及来澳定居时间较长的移民病人进行鉴别诊断时应考虑结核病的可能。对于有持续性咳嗽症状的病人宜适当降低进行X线检查和痰菌检查的标准。
