The Problem of Rehospitalisation for Heart Failure at the Cardiology Department of the Hôpital National Ignace Deen

Introduction: Despite current therapeutic advances, heart failure in sub-Saharan Africa remains a common, serious and costly disease, particularly due to rehospitalizations. The objective of this work was to determine the proportion of rehospitalizations for heart failure and to identify etiological factors. Methodology: This was a retrospective descriptive study with a duration of 8 months from April 1 to November 30, 2021. This study included all patients rehospitalized in the department for Heart Failure and who agreed to participate in our study. Results: During the period of our study, 437 patients were hospitalized in the HF department, among which we collected 126 cases of rehospitalization for HF with a frequency of 28.83%. The mean age of our patients was 46.32 ± 18.98 years with the extremes of 15 to 84 years. The most affected age group was between 35 and 44 years old in 24 cases, i.e. a frequency of 19%. We observed a female predominance of 64 cases, i.e. a frequency of 50.8% compared to 62 cases, i.e. a frequency of 49.2% with a sex ratio (M/F) equal to 0.96. 98 cases of our patients, i.e. a frequency of 77.8%, were mutual insurance companies who felt they had the necessary support from those around them. In our sample, the underlying heart disease was mainly represented by valvular heart disease in 59 cases, followed by hypertensive heart disease in 42 cases with the respective frequencies of 46.82% and 33.33%. The majority of our patients were rehospitalized (1 - 3) times after a first episode of HF flare-up in 117 cases or 92.9%. Irregularity at control and therapeutic break were the most common decompensation factors with frequencies of 75.8% and 74.2% respectively. Introduction: Neonatal mortality remains a major public health concern. According to the World Health Organisation (WHO), the number of newborns dying each year has fallen from 5 million in 1990 to 2.4 million in 2019. The aim of our study was to describe the main morbid conditions and their lethality in newborns admitted to the paediatric ward of the CSReF in Commune V. Methodology: We conducted a 12-month prospective descriptive and analytical study from August 2020 to July 2021. Results: During the 12 months of the study, out of 2654 neonates admitted to hospital, 216 met our inclusion criteria, i.e. 8%. Newborns admitted in the first few hours of life represented 99.54% of cases. Full-term newborns accounted for 65.74% of the total. Forty-eight percent of newborns had hypothermia on admission. The most common diagnosis was asphyxia in 55.56% of cases, followed by neonatal infection in 27.78% and prematurity in 10.65%. Death was recorded in 22.2% of cases. Asphyxia was the main cause of neonatal death, followed by prematurity in our context. Conclusion: Improving the prognosis of newborn babies will require a thorough understanding of neonatal pathologies and the implementation of a pre- and perinatal prevention policy.

In Mali, cholesteatoma surgery remains a challenge due to the limited number of otological practitioners, and the low socio-economic level of patients reflecting the difficulties in performing CT or MRI for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatoma. Materials and Method: This was a retrospective and prospective, descriptive study that took place in the eNT and head and neck surgery department of the Gabriel Touré University Hospital over 36 months from November 2020 to October 2023. These were patients admitted to the department for chronic cholestematous otitis media. Result: In total, we collected 34 files of patients admitted for cholesteatoma. This represented 9% of cases compared to all otological surgeries carried out during the same period, i.e. 362 cases. The average age of our patients was 35.31 years, with extremes ranging from 7 years to 80 years, there were 24 men and 10 women, i.e. a M/F ratio of 2.4. The average time to diagnosis was 7 years. The main functional signs were dominated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of meningeal complication and three cases of cerebral empyema were reported. Clinically, a postero-superior and marginal tympanic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-marginal tympanic perforation in 32%. Pure-tone audiometry performed preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a mini box. An ossiculoplasty was performed immediately with fragments of cartilage. Increasing the detail of european land use/cover data by combining heterogeneous data sets

Data on land use and land cover(LULC)are a vital input for policy-relevant research,such as modelling of the human population,socioeconomic activities,transportation,environment,and their *** europe,CORINe Land Cover has been the only data set covering the entire continent consistently,but with rather limited spatial *** data sets have provided much better detail,but either have covered only a fraction of europe(*** Atlas)or have been thematically restricted(*** High Resolution Layers).In this study,we processed and combined diverse LULC data to create a harmonised,ready-to-use map covering 41 *** doing so,we increased the spatial detail(from 25 to one hectare)and the thematic detail(by seven additional LULC classes)compared to the CORINe Land ***,we decomposed the class‘Industrial and commercial units’into‘Production facilities’,‘Commercial/service facilities’and‘Public facilities’using machine learning to exploit a large database of points of *** overall accuracy of this thematic breakdown was 74%,despite the confusion between the production and commercial land uses,often attributable to noisy training data or mixed land *** learnt from this exercise are discussed,and further research direction is proposed.

The ultrafine particles of silver chloride (AgCl) were obtained by mixing two separately prepared reversed micelle systems both containing a non-ionic surfactant,cyclohexane and an aqueous solution of either silver nitrate (AgNO3) or potassium chloride (KCl). The size distribution of spherical AgCl particles depended on the solubility behavior of aqueous electrolytes in surfactant solution. The NMR, near infrared,light-scattering, X-ray, UV spectroscopy and TeM (transmission electron microscopy) were used to examine the relationship between the size of particles and the states of solubilized water or reversed micelles. In the solubilized regions, it was found that three states of water were related to the formation of reversed and swollen micelles or W/O microemulsions. i.e, monodisperse particles were formed in reversed micelle and swollen micelle systems, whereas in W/O microemulsion system the particles became polydisperse.

目的观察γ-酮醛清道夫水杨胺(salicylamine,SAM)对尿毒症Apoe-/-小鼠动脉粥样硬化(atherosclerosis,AS)及其表型的影响。方法Apoe-/-小鼠通过5/6肾切除术建立尿毒症模型,对照组仅行假手术。实验动物分为对照组、尿毒症组及尿毒症SAM干预组,分别以SAM或安慰剂干预6周。干预结束后检测体质量、血压、血脂、肾功能、血清SAM浓度、主动脉AS斑块面积及斑块组分。结果与对照组相比,尿毒症组主动脉AS病变面积明显增大,AS斑块组分中坏死面积增大,巨噬细胞含量增多,胶原含量减少。SAM干预6周,尿毒症小鼠AS损伤面积、AS斑块中坏死面积及巨噬细胞含量减少,胶原含量增加,但不影响体质量、血压、血脂和肾功能,未发生药物蓄积和毒性作用。结论SAM能减轻尿毒症Apoe-/-小鼠加速发展的AS病变并使斑块的稳定性增加,这些作用不依赖于对血压、血脂或肾功能的影响,同时不引起药物蓄积及毒性作用。

Otological Injuries Observed in Head and Neck Trauma in a Second-Line Hospital

Otological injuries are common during cervicofacial trauma. The aim of this study is to describe the epidemiological characteristics and to map otological injuries during head and neck trauma. This was an observational, descriptive, prospective study conducted at the eNT unit of the Referral Health Center Hospital in Mali from January 2017 to September 2018. Patients with middle and/or inner ear injuries due to trauma were included. The prevalence of otologic injuries was 34.54% of cases. The mean age was 25 years and the sex ratio was 1.11. Students accounted for 42.10%. The mechanism of occurrence was an intentional assault in 68.42% of the cases followed by road traffic accidents (15.79%). The most common functional signs were hearing loss (68.42%), tinnitus (57.89%), and facial paralysis in 5.26%. Otoscopy showed tympanic perforation (47.83%), otorrhagia (21.05%) and otoliquorrhea (5.26%). Rock fractures with osteodural breach (10.53%) and pneumencephaly were found on a CT scan of the rock. The evolution was marked by the regression of the facial paralysis and the healing of the tympanum in 45.45% of the cases. Sequelae persisted in the form of sensorineural deafness (17%). The otological lesions observed during cervicofacial trauma are varied. The challenges in our context are felt at three levels: the improvement of the technical platform, the long-term follow-up of patients, and the cost of care in terms of equipment. These lesions are responsible for disabling deafness, a source of difficulty in social and school integration.

Association between preoperative high sensitive troponin I levels and cardiovascular events after hip fracture surgery in the elderly

Objective Cardiovascular complications contribute to postoperative morbidity and mortality in elderly hip fracture patients. Limited data are available regarding which preoperative risk factors predict cardiovascular course following hip fracture surgery （HFS）. We used high sensitive troponin I （hs-TnI） assays and clinical parameters to identify preoperative risk factors associated with major adverse cardiac events （MACe） in elderly hip frac^u＇e patients. Method From August 2014 to November 2016, 575 patients with hip fracture were enrolled in a retrospective, single-center registry. A total of 262 of these patients underwent HFS and hs-TnI assays. MACe was defined as postoperative all-cause deaths, heart failure （HF）, new-onset atrial fibrillation （AF）, myocardial infarction （MI） and cardiovascular re-hospitalization that occurred within 90 days postoperative. Results Of 262 HFS patients, MACe developed following HFS in 65 （24.8%）. Patients with MACe were older and had higher rates of renal insufficiency, coronary artery disease, prior HF, low left ventricular ejection fraction and use of beta blockers; higher levels of hs-Tnl and N-terminal pro-brain natriuretic peptide （NT-proBNP） and higher revised cardiac risk index. A preoperative hs-TnI≥ 6.5 ng/L was associated with high risk of postoperative HF, new-onset AF and MACe. In multivariable analysis, pre-operative independent predictors for MACe were age 〉 80 years [adjusted hazard ratio （HR）： 1.79, 95% confident interval （CI）： 1.03-3.13, P = 0.04], left ventricular ejection fraction （LVeF）〈 50% （adjusted HR： 3.17, 95% CI： 1.47-5.82, P 〈 0.01） and hs-TnI 〉 6.5 ng/L （adjusted HR： 3.75, 95% CI： 2.09~5.17, P 〈 0.01）. Conclusion In elderly patients with hip fracture who undergo HFS, a preoperative assessment of hs-TnI may help the risk refinement of cardiovascular complications.
