Efficacy of percutaneous radiofrequency ablation for hepatocellular carcinoma treatment in patients aged≥80 years

Background:Although radiofrequency ablation(RFA)is a minimally invasive treatment for early-stage hepatocellular carcinoma(HCC),it remains unclear whether RFA achieves favorable outcomes in pa-tients aged≥80 *** study aimed to determine the efficacy and safety of RFA for HCC in patients aged≥80 ***:A total of 512 naïve patients with HCC who had undergone RFA from January 2001 to December 2016 were *** were categorized into the≥80-year-old group and the control group(aged<80 years).The primary endpoint was overall survival(OS),and the secondary endpoints were recurrence-free survival,complications associated with RFA,and cause of *** score matching was performed to adjust for patients’sex,liver function,tumor number,tumor diameter,and hepatitis C virus ***,the data of 68 patients in the≥80-year-old group and 68 in the control group were analyzed;their baseline characteristics,primary endpoint,and secondary endpoints were ***:There were significant differences in the alanine aminotransferase level and prothrombin time between the *** cumulative OS rate was not significantly different between the groups(P=0.83):98.5%,87.9%,and 50.5%in the≥80-year-old group and 94.1%,72.8%,and 49.3%in the control group at 1,3,and 5 years,***≥80 years was not significantly associated with OS in multivariate ***-related death occurred in 17 patients in the≥80 year-old group and in 16 patients in the control group(P=1.00).Conclusions:RFA is safe and effective for the treatment of patients with HCC aged≥80 years.

Efficacy of tolvaptan in patients with refractory ascites in a clinical setting

AIM: To elucidate the efficacies of tolvaptan(TLV) as a treatment for refractory ascites compared with conventional treatment. METHODS: We retrospectively enrolled 120 refractory ascites patients between January 1, 2009 and September 31, 2014. Sixty patients were treated with oral TLV at a starting dose of 3.75 mg/d in addition to sodium restriction(> 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-60 mg/d furosemide and 25-50 mg/d spironolactone) and 60 patients with large volume paracentesis in addition to sodium restriction(less than 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-120 mg/d furosemide and 25-150 mg/d spironolactone). Patient demographics and laboratory data, including liver function, were not matched due to the small number of patients. Continuous variables were analyzed by unpaired t-test or paired t-test. Fisher's exact test was applied in cases comparing two nominal variables. We analyzed factors affecting clinical outcomes using receiver operating characteristic curves and multivariate regression analysis. We also used multivariate Cox's proportional hazard regression analysis to elucidate the risk factors that contributed to the increased incidence of ***: TLV was effective in 38(63.3%) patients. The best cut-off values for urine output and reduced urine osmolality as measures of refractory ascites improvement were > 1800 mL within the first 24 h and > 30%, respectively. Multivariate regression analysis indicated that > 25% reduced urine osmolality [odds ratio(OR) = 20.7; P < 0.01] and positive hepatitis C viral antibodies(OR = 5.93; P = 0.05) were positively correlated with an improvement of refractory ascites, while the total bilirubin level per 1.0 mg/dL(OR = 0.57;P = 0.02) was negatively correlated with improvement. Prior minimal endoscopic sphincterotomy to prevent pancreatitis related to endoscopic balloon sphincteroplasty

AIM To investigate the efficacy of prior minimal endoscopic sphincterotomy(EST) to prevent pancreatitis related to endoscopic balloon sphincteroplasty(EBS).METHODS After bile duct access was gained and cholangiogram confirmed the presence of stones < 8 mm in the common bile duct at endoscopic retrograde cholangiography, patients were subjected to minimal EST(up to one-third of the size the papilla) plus 8 mm EBS(ESTEBS group). The incidence of pancreatitis and the difference in serum amylase level after the procedure were examined and compared with those associated with 8-mm EBS alone in 32 patients of historical control(control group).RESULTS One hundred and five patients were included in the EST-EBS group, and complete stone removal was accomplished in all of them. The difference in serum amylase level after the procedure was- 25.0(217.9) IU/L in the EST-EBS group and this value was significantly lower than the 365.5(576.3) IU/L observed in the control group(P < 0.001). The incidence of post-procedure pancreatitis was 0%(0/105) in the EST-EBS group and 15.6%(5/32) in the control group(P < 0.001).CONCLUSION Prior minimal EST might be useful to prevent the elevation of serum amylase level and the occurrence of pancreatitis related to EBS.

Microporous molecular crystals are promising materials because of their designable porosity as well as their resistance to chemical and other ***,we developed microporous molecular cocrystals by taking advantage of the specific interactions between porphyrins and fullerene ***-crystal X-ray diffraction analysis revealed that one nickel(Ⅱ)porphyrin interacts with two fullerene molecules to form a two-dimensional honeycomb network with an eclipsed stacking mode,providing onedimensional void *** the pores were activated by heat treatment or mechanical grinding,the prepared cocrystal can incorporate gas and solvent molecules reversibly while maintaining its ***,it retained its single-crystallinity in the presence of water,acid−base,or high *** findings in this study expand the availability of molecular crystals based on intermolecular interactions as porous materials,which are expected to work under conditions that have not been applicable to other molecule-based porous materials.

Near Range Radar and Its Application to Near Surface Geophysics and Disaster Mitigation

In this paper, we discuss about the near range radar applied to various environmental applications and disaster mitigation issues. Synthetic aperture radar（SAR） processing or migration is the key technology in near range radar imaging, which can be used in ground penetrating radar（GPR） and ground-based synthetic aperture radar（GB-SAR）. We demonstrate some applications which include GPR for humanitarian demining, GPR for archaeological survey, GB-SAR for landslide monitoring and nondestructive inspection of wooden buildings. We also demonstrate a new array GPR system “Yakumo”, which was used for archaeological survey for demonstration of advanced multi-static radar signal processing for better radar imaging.

Parallelization and sustainability of distributed genetic algorithms on many-core processors

Purpose–The purpose of this paper is to propose a fault-tolerant technology for increasing the durability of application programs when evolutionary computation is performed by fast parallel processing on many-core processors such as graphics processing units(GPUs)and multi-core processors(MCPs).Design/methodology/approach–For distributed genetic algorithm(GA)models,the paper proposes a method where an island’s ID number is added to the header of data transferred by this island for use in fault ***–The paper has shown that the processing time of the proposed idea is practically negligible in applications and also shown that an optimal solution can be obtained even with a single stuck-at fault or a transient fault,and that increasing the number of parallel threads makes the system less susceptible to ***/value–The study described in this paper is a new approach to increase the sustainability of application program using distributed GA on GPUs and MCPs.
