外伤性视神经病变24例回顾研究(英文)

目的:探讨的外伤性视神经病变的临床表现,评估在眼科第三病区住院的经过三组不同处理(保守估计,单独静脉注射皮质类固醇,静脉注射和口服皮质类固醇联合治疗)的外伤性视神经病变患者的治疗效果。方法:对2007-01/2009-12在马来西亚医科大学眼科连续住院的24例27眼外伤性视神经患者进行了回顾性研究。结果:本次研究对象为24例27眼男性外伤性视神经患者(平均年龄为33岁)。车祸是导致发病的主要原因(83%)。大部分患者的视力低下(手动/眼前～无光感约占82%),其中有22眼并发眶周血肿,20眼并发有结膜下出血。并发多于一骨(颅骨或眶骨)骨折有19例(79%),5例(21%)没有出现骨折。CT扫描或核磁共振检查显示患者均没有视神经压迫的证据。第一组: 11例(46%)患者进行静脉注射联合口服类固醇的治疗;第二组:7例患者(29%)进行保守治疗;第三组:6例(25%)患者给予静脉注射皮质类固醇治疗。92%(11/12眼)给予静脉注射和口服类固醇治疗的患者和78%保守治疗患者视力能提高1行。单独静脉注射皮质类固醇治疗的患者4例呈现无光感,1例有轻度视力改善,而其他患者视力保持不变。保守治疗患者和单独静脉注射皮质类固醇患者视力改善不显著,且两组间相比在统计学上无显著性差异(P=0.368)。静脉注射联合口服类固醇治疗的患者有明显的视觉改善(P<0.05)。没有患者接受视神经减压手术。在本研究中,随访时间为6mo～3a。结论:大部分外伤性视神经病变患者都存在有眶周血肿,结膜下出血,眶壁骨折的症状。对比保守治疗,经过静脉注射和口服皮质类固醇联合治疗的患者有更好的治疗效果。

患者,男,39岁,以右眼红、痛入院。他受到一个老者暗示将自己的右眼用木棒击伤。临床表现为右眼中度突出,完全性眼肌麻痹和严重眶周皮肤肿胀。眼科影像学表现为右眼眼窝蜂窝组织炎及眼下静脉血栓形成,两侧海绵窦综合征和海绵窦血栓。报告中无眼内及眶内异物。积极使用静脉抗生素,患者的病情并没有得到改善。在麻醉下检查发现在穹窿下有一个巨大的木质异物。异物去除及强力抗生素治疗下患者病情明显恢复。诊断中发现受伤原因可能是由这个精神病患者自我伤害引起。详细的病史、检查及合理的调查揭示其潜在的病因。敏锐的察觉及相关的临床发现与影像学研究在眶内异物及眶蜂窝组织炎处理中显得尤为有效的。及时诊断和治疗可能进一步减少患者的发病率或死亡率。

The naïve Bayes classifier is one of the commonly used data mining methods for *** its simplicity,naïve Bayes is effective and computationally *** the strong attribute independence assumption in the naïve Bayes classifier makes it a tractable method for learning,this assumption may not hold in real-world *** enhancements to the basic algorithm have been proposed in order to alleviate the violation of attribute independence *** these methods improve the classification performance,they do not necessarily retain the mathematical structure of the naïve Bayes model and some at the expense of computational *** approach to reduce the naïvetéof the classifier is to incorporate attribute weights in the conditional *** this paper,we proposed a method to incorporate attribute weights to naïve *** evaluate the performance of our method,we used the public benchmark *** compared our method with the standard naïve Bayes and baseline attribute weighting *** results show that our method to incorporate attribute weights improves the classification performance compared to both standard naïve Bayes and baseline attribute weighting methods in terms of classification accuracy and F1,especially when the independence assumption is strongly violated,which was validated using the Chi-square test of independence.

A pilot clinical study of developing an External Assist Targeting Device for rapid and precise renal calyx access during percutaneous nephrolithotomy

Objective:To design a device to increase the accuracy of the targeting process and reduce the radiation exposure to both the patients and the medical ***:We analyzed the inherent problem and designed the External Assist Targeting Device(EATD)to assist in the alignment of needle targeting on the desired renal calyx under fluoroscopic *** EATD was designed to allow rapid and precise access to calyces at all angles,with a simple two-step puncture protocol developed for puncturing a target renal *** then tested the device in a pilot human trial with four ***:In experiments with phantom models,the time for successful targeting was reduced by 31%using the *** mean fluoroscopic time was reduced by 40%.In initial human trial,the puncture time was shortened by 66%and the radiation dose was decreased by 65%compared to free-hand *** complication was observed during the ***:The EATD was found to be cost effective,portable,simple to set up,and safe to operate for assisting in the percutaneous nephrolithotomy *** preliminary tests showed high degree of accuracy in gaining precise access to a targeted renal calyx with much shorter time and lesser radiation *** EATD also has the potential to be used to access other organs with precision under fluoroscopic guidance.

A human's e-life needs multiple offline and online accounts. It is a balance between usability and security to set keys or passwords for these multiple accounts. Password reuse has to be avoided due to the domino effect of malicious administrators and crackers. However,human memorability constrains the number of keys. Single sign-on server,key hashing,key strengthening and petname system are used in the prior arts to use only one key for multiple online accounts. The unique site keys are derived from the common master secret and specific domain name. These methods cannot be applied to offline accounts such as file encryption. We invent a new method and system applicable to offline and online accounts. It does not depend on HTTP server and domain name,but numeric 4-digit passcode,key hashing,key strengthening and hash truncation. Domain name is only needed to resist spoofing and phishing attacks of online accounts.

Effective thermal control systems are essential for reliable operation of spacecraft.A dual-driven intelligent combination control strategy is proposed to improve the temperate control and heat flux tracking *** temperature regulation and heat flux tracking errors are employed to generate the final control action;their contributions are adaptively adjusted by a fuzzy fusing policy of control *** evaluate the control effects,describe a four-nodal mathematical model for analyzing the dynamic characteristics of the controlled heat pipe space cooling system（HP-SCS） consisting of an aluminum-ammonia heat pipe and a variable-emittance micro-electromechanical-system（MEMS） *** dynamical model calculates the mass flow-rate and condensing pressure of the heat pipe working fluid directly from the systemic nodal temperatures,therefore,it is more suitable for control engineering *** closed-loop transient performances of four different control schemes have been numerically *** results conclude that the proposed intelligent combination control scheme not only improves the thermal control effects but also benefits the safe operation of HP-SCS.

Objective:Men with benign prostate hyperplasia(BPH)with good urinary flow may still have bladder outlet obstruction(BOO).Intravesical prostatic protrusion(IPP)has been shown to be able to predict *** aim to investigate the use of IPP to predict BOO in men with good urinary ***:One hundred and fourteen consecutive men(>50 years old)presenting with lower urinary tract symptoms suggestive of BPH were recruited in 2001 and *** were evaluated with serum prostate specific antigen(PSA),uroflowmetry and transabdominal ultrasound measurement of IPP and prostate volume(PV).Pressure-flow urodynamic studies were performed on all men and BOO was defined by BOO index>*** with Qmax12.0 mL/s were considered to have good ***:Among the 114 men,61 patients had good urinary *** median age,PV and Qmax were 66 years,32.9 mm3 and 14.5 mL/s respectively.14/61(23.0%)patients had BOO and their distribution of IPP were as follows:Grade 1 e 0/20(0%)obstructed,Grade 2 e 6/22(27.3%)and Grade 3 e 8/19(42.1%).Sensitivity of Grade 2/3 IPP for BOO was 100% while specificity of Grade 3 IPP was 76.6%.The area-under-curve(AUC)for IPP was greater than that for PV(0.757 vs.0.696).Conclusion:Even in men with good flow,high grades of IPP were more likely to have BOO and hence,may be a useful adjunct to predict BOO.

A variable magnetization(VM) motor by incorporating magnets that can be flexibly configured with variable magnetization process is proposed to meet the emerging requirements on motor efficiency and actuator compliance in robotic applications. A generalized spin torque model is established which provides a relationship between the motor torque and two different types of motor inputs, the current inputs and the magnet magnetizations. Avariable magnetization process is proposed based on the study of the hysteresis properties of the magnetic materials and the design criteria for implementing the variable magnetization process with current pulses are established. The feasibility of the variable magnetization is validated with experimental data and the motor functions and performances are numerically demonstrated and evaluated. The results show that the VM motor can maintain high efficiency by switching between two actuation modes. Controllable stiffness at different equilibria can be also achieved with the VM motor with instantaneous magnetizing current pulses.

In particular, the dye-sensitised solar cells(DSSCs) have a high potential in the rational energy conversion efficiency to secure our sustainable energy *** the present study, advanced radio frequency(RF) magnetron sputtering technique was applied to incorporate titanium dioxide(TiO) dopants into reduced graphene oxide(rGO) nanosheet for improving the power conversion efficiency(PCE) of DSSCs device. An optimum TiOcontent incorporated onto rGO nanosheet plays an important role in improving the PCE of DSSCs by minimising the recombination losses of photo-induced charge *** on the results obtained, 40-s sputtering duration for incorporating TiOdopants onto rGO nanosheet exhibits a maximum PCE of 8.78% than that of pure rGO film(0.68%). In fact, the presence of optimum content of TiOdopants within rGO nanosheet could act as mediators for efficient separation photo-induced charge carriers. However, the excessive of sputtering duration(e.g. 60 s) of TiOdopants onto rGO nanosheet results higher charge recombination and lowers the PCE of DSSCs(5.39%).
