Immunological milieu in the peritoneal cavity at laparotomy for gastric cancer

AIM： To investigate the immunological repertoire in the peritoneal cavity of gastric cancer patients. METHODS： The peritoneal cavity is a compartment in which immunological host-tumor interactions can occur. However, the role of lymphocytes in the peri- toneal cavity of gastric cancer patients is unclear. We observed 64 patients who underwent gastrectomy for gastric cancer and 11 patients who underwent lapa-roscopic cholecystectomy for gallstones and acted as controls. Lymphocytes isolated from both peripheral blood and peritoneal lavage were analyzed for surface markers of lymphocytes and their cytokine production by flow cytometry. CD4＋CD25h＋gh T cells isolated from the patient＇s peripheral blood were co-cultivated for 4 d with the intra-peritoneal lymphocytes, and a cytokine assay was performed. RESULTS： At gastrectomy, CCR7-CD45RA- CD8＋ effector memory T cells were observed in the peritoneal cavity. The frequency of CD4＋ CD25 h＋ghT ceils in both the peripheral blood and peritoneal cavity was elevated in patients at advanced stage [control vs stage IV in the peripheral blood： 6.89 （3.39-10.4） vs 15.34 （11.37-19.31）, P 〈 0.05, control vs stage IV in the peri- toneal cavity： 8.65 （5.28-12.0） vs 19.56 （14.81-24.32）, P 〈 0.05]. On the other hand, the suppression was restored with CD4＋ CD25h＋QhT cells from their own pe- ripheral blood. This study is the first to analyze lympho- cyte and cytokine production in the peritoneal cavity in patients with gastric cancer. Immune regulation at ad- vanced stage is reversible at the point of gastrectomy. CONCLUSION： The immunological milieu in the peri- toneal cavity of patients with advanced gastric cancer elicited a Th2 response even at gastrectomy, but this response was reversible.

Recent advances in the surgical treatment of hepatocellular carcinoma

Hepatocellular carcinoma(HCC) is the most common primary liver malignancy. The treatment of HCC is complex and complicated by the severity of associated chronic liver disease, the stage of HCC, and the clinical condition of the patient. Liver resection(LR) is one of the most efficient treatments for patients with HCC, with an expected 5-year survival of 38%-61% depending on the stage of the disease. Improved liver function assessment, increased understanding of segmental liver anatomy from advanced imaging studies, and surgical technical progress are important factors that have led to reduced mortality in patients with HCC. The indication for LR may be expanded due to emerging evidences from laparoscopic hepatectomies and combined treatments with newly developed chemotherapies. Liver transplantation(LT) is considered as an ideal treatment for removal of existing tumors and the injured/preneoplastic underlying liver tissue with impaired liver function and the risk of multicentric carcinogenesis that results from chronically injured liver. However, LT is restricted to patients with minimal risk of tumor recurrence under immunosuppression. The expansion of criteria for LT in HCC patients is still under trial and discussion. Limited availability of grafts, as well as the risk and the cost of transplantation have led to considerable interest in expansion of the donor pool, living donor-related transplantation, and combined treatment involving LR and LT. This highlight presents evidence concerning recent studies evaluating LR and LT in HCC patients. In addition, alternative therapies for the treatment of early stage tumors and the management of patients on transplant waiting lists are discussed.

Duodenal stenosis resulting from a preduodenal portal vein and an operation for scoliosis

A preduodenal portal vein （PDPV） is known to be a rare cause of duodenal stenosis. We treated a 22-yearold male patient with malnutrition as a result of PDPV and a previously performed operation for scoliosis, who showed an improvement in quality of life after being treated with a combination of nutritional support and surgery. The patient with PDPV had been admitted to our department with duodenal stenosis, ranging from the first to third portions. He had suffered from vomiting since 1 year of age, and he developed malnutrition during the last 6-too period after orthopedic surgery for scoliosis. The stenosis was related to both the PDPV and the previously performed operation for scoliosis. After receiving nutritional support for 6 mo, a gastrojejunostomy with Braun＇s anastomosis for the first portion and a duodenojejunostomy for the second and third portions were performed. The postoperative course was almost uneventful. Three months later, he was discharged and able to attend university. In patients with widespread duodenal stenosis, there may be a complicated cause,such as PDPV and duodenal stretching induced by previous spinal surgery.

Further Improvement of Reflection Efficiency of a Magnetic Mirror and Replenishment against Loss of Escaping Deuteron Ions

We reported previously the idea to improve reflection-ability of a magnetic mirror by installing a cyclotron resonance space in the front part of the mirror. However, since the previous analysis was insufficient from the examination after that, we complement the following two things in this work: 1) A simpler procedure of design to make a supplemental magnetic mirror with the simplest magnetic configuration, compared with the procedure reported previously. 2) A peculiar characteristic arising only in reflection of a nonrelativistic charged particle (a deuteron ion).

Post-polypectomy bleeding and thromboembolism risks associated with warfarin vs direct oral anticoagulants

AIM To verify the validity of the endoscopy guidelines for patients taking warfarin or direct oral anticoagulants(DOAC).METHODS We collected data from 218 patients receiving oral anticoagulants(73 DOAC users, 145 warfarin users) and 218 patients not receiving any antithrombotics(age-and sexmatched controls) who underwent polypectomy.(1) We evaluated post-polypectomy bleeding(PPB) risk in patients receiving warfarin or DOAC compared with controls;(2) we assessed the risks of PPB and thromboembolism between three AC management methods: Discontinuing AC with heparin bridge(HPB)(endoscopy guideline recommendation), continuing AC, and discontinuing AC without *** PPB rate was significantly higher in warfarin users and DOAC users compared with controls(13.7% and 13.7% vs 0.9%, P < 0.001), but was not significantly different between rivaroxaban(13.2%), dabigatran(11.1%), and apixaban(13.3%) users. Two thromboembolic events occurred in warfarin users, but none in DOAC users. Compared with the continuing anticoagulant group, the discontinuing anticoagulant with HPB group(guideline recommendation) had a higher PPB rate(10.8% vs 19.6%, P = 0.087). These findings were significantly evident in warfarin but not DOAC users. One thrombotic event occurred in the discontinuing anticoagulant with HPB group and the discontinuing anticoagulant without HPB group; none occurred in the continuing anticoagulant *** PPB risk was similar between patients taking warfarin and DOAC. Thromboembolism was observed in warfarin users only. The guideline recommendations for HPB should be re-considered.

Background:The application of cryopreservation and artificial insemination technology have contributed to the advancement of animal ***,a substantial proportion of spermatozoa undergoes alterations and loses their fertility during cryopreservation,rendering the frozen-thawed semen impractical for routine *** is known to reduce sperm lifespan and *** in cryosurvival of spermatozoa from different sires and even with the individual sire is common in artificial insemination(AI)*** goal is to improve post-thawed semen quality by optimization of cryopreservation technique through sperm selection prior to cryopreservation ***:Our strategy of sperm selection based on rheotaxis and thermotaxis(SSRT)on macrosale in a rotating fluid flow demonstrated the ability to maintain the original pre-freezing structural integrity,viability and biological function related to fertilization *** strategy has a positive effect on the cryosurvival and fertilizing abilities of spermatozoa as supported by the improvement on pregnancy rate of Japanese Black heifers and Holstein repeat *** technique protected further sublethal damage to bovine spermatozoa(higher%cryosurvival than the control)and resulted in the improvement of DNA *** selected spermatozoa demonstrated slower and controlled capacitation than unprocessed control which is thought to be related to sperm longevity and consequently to appropriate timing during in vivo ***:These results provide solid evidence that improvement of post-thawed semen quality by SSRT method is beneficial in terms of cryosurvival,longevity of post-thawed sperm,and optimization of in vivo fertilization,embryo development and calving as supported by the favorable results of field fertility study.

A power-supply system was developed for Ohmic heating(OH)to double×10^(18)the amount of change magnetic flux in the primary central solenoid(CS)on the QUEST spherical *** power supplies are connected with stacks of insulated-gate bipolar transistors,and sequentially operated to generate positive and negative CS *** bipolar power-supply system is controlled via a field-programmable gate array,which guarantees the safety of the entire system *** new OH system,assisted by electron cyclotron heating,enables the stable generation of plasma currents exceeding 100 k ***,the achieved electron density over the wide range in the major radial direction exceeds the cut-off density for one of the highpower microwave sources in *** strategy yields target plasmas for future experiments with the electron Bernstein wave.
