Development and validation of a simple and multifaceted instrument,GERD-TEST,for the clinical evaluation of gastroesophageal reflux and dyspeptic symptoms

To evaluate the psychometric properties of a newly developed questionnaire, known as the gastroesophageal reflux and dyspepsia therapeutic efficacy and satisfaction test (GERD-TEST), in patients with *** patients with predominant GERD symptoms recruited according to the Montreal definition were treated for 4 wk using a standard dose of proton pump inhibitor (PPI). The GERD-TEST and the Medical Outcome Study Short Form-8 Health Survey (SF-8) were administered at baseline and after 4 wk of treatment. The GERD-TEST contains three domains: the severity of GERD and functional dyspepsia (FD) symptoms (5 items), the level of dissatisfaction with daily life (DS) (4 items), and the therapeutic efficacy as assessed by the patients and medication compliance (4 items).RESULTSA total of 290 patients were eligible at baseline; 198 of these patients completed 4 wk of PPI therapy. The internal consistency reliability as evaluated using the Cronbach’s α values for the GERD, FD and DS subscales ranged from 0.75 to 0.82. The scores for the GERD, FD and DS items/subscales were significantly correlated with the physical and mental component summary scores of the SF-8. After 4 wk of PPI treatment, the scores for the GERD items/subscales were greatly reduced, ranging in value from 1.51 to 1.87 and with a large effect size (P < 0.0001, Cohen’s d; 1.29-1.63). Statistically significant differences in the changes in the scores for the GERD items/subscales were observed between treatment responders and non-responders (P < 0.0001).CONCLUSIONThe GERD-TEST has a good reliability, a good convergent and concurrent validity, and is responsive to the effects of treatment. The GERD-TEST is a simple, easy to understand, and multifaceted PRO instrument applicable to both clinical trials and the primary care of GERD patients.

In mammals,the livers regenerate after chemical injury or resection of hepatic lobe by *** liver regeneration is initiated after mass loss remains to be *** we report that following liver injury,activated platelets deploy SDF-1 and VEGF-A to stimulate CXCR7+liver sinusoidal endothelial cell(LSEC)and VEGFR1^(+)myeloid cell,orchestrating hepatic *** carbon tetrachloride injection or hepatectomy,platelets and CD11b+VEGFR1^(+)myeloid cells were recruited to LSECs,and liver regeneration in both models was impaired in thrombopoietin-deficient(Thpo^(−/−))mice repressing production of circulating *** impeded regeneration phenotype was recapitulated in mice with either conditional ablation of Cxcr7 in LSEC(Cxcr7iΔ/iΔ)or Vegfr1 in myeloid cell(Vegfr1^(lysM/lysM)).Both Vegfr1lysM/lysM and Cxcr7i^(Δ/iΔ)mice exhibited suppressed expression of hepatocyte growth factor and Wnt2,two crucial trophogenic angiocrine factors instigating hepatocyte *** note,administration of recombinant thrombopoietin restored the prohibited liver regeneration in the tested genetic *** such,our data suggest that platelets and myeloid cells jointly activate the vascular niche to produce pro-regenerative endothelial paracrine/angiocrine *** this‘hematopoietic–vascular niche’might help to develop regenerative therapy strategy for hepatic disorders.

Dipeptidyl peptidase Ⅳ (DPPIV), which belongs to the prolyl oligopeptidase family of serine proteases, is known to have a variety of regulatory biological functions and has been shown to be implicated in type 2 diabetes. It is therefore important to develop selective human DPPIV (hDPPIV) inhibitors. In this study, we determined the crystal structure of apo hDPPIV at 1.9 A resolution. Our high-resolution crystal structure of apo hDPPIV revealed the presence of sodium ion and glycerol molecules at the active site. In order to elucidate the hDPPIV binding mode and substrate specificity, we determined the crystal structure of hDPPIV-diprotin B (Val-Pro-Leu) complex at 2.1 A resolution, and clarified the difference in binding mode between diprotin B and diprotin A (Ile-Pro-Ile) into the active site of hDPPIV. Comparison between our crystal structures and the reported apo hDPPIV structures revealed that positively charged functional groups and conserved water molecules contributed to the interaction of ligands with hDPPIV. These results are useful for the design of potent hDPPIV inhibitors.

Surgically treated diaphragmatic perforation after radiofrequency ablation for hepatocellular carcinoma

We review 6 cases of diaphragmatic perforation, with and without herniation, treated in our institution. All patients with diaphragmatic perforation underwent radiofrequency ablation(RFA) treatments for hepatocellular carcinoma(HCC) performed at Kurume University Hospital and Tobata Kyoritsu Hospital. We investigated the clinical profiles of the 6 patients between January 2003 and December 2013. We further describe the clinical presentation, diagnosis, and treatment of diaphragmatic perforation. The change in the volume of liver and the change in the Child-Pugh score from just after the RFA to the onset of perforation was evaluated using a paired t-test. At the time of perforation, 4 patients had herniation of the viscera, while the other 2 patients had no herniation. The majority of ablated tumors were located adjacent to the diaphragm, in segments 4, 6, and 8. The average interval from RFA to the onset of perforation was 12.8 mo(range, 6-21 mo). The median Child-Pugh score at the onset of perforation(8.2) was significantly higher compared to the median Child-Pugh score just after RFA(6.5)(P = 0.031). All patients underwent laparotomy and direct suture of the diaphragm defect, with uneventful post-surgical recovery. Diaphragmatic perforation after RFA is not a matter that can be ignored. Clinicians should carefully address this complication by performing RFA for HCC adjacent to diaphragm.

WS2 exhibits tremendous potentials for Na-ions storage owing to high capacity(433 mAh g^(-1)). Nevertheless, WS2 layered structure is often exfoliated with rapid capacity decay and sluggish reaction *** this work, WS2 nanosheets with different crystallinities are controlled by different synthesis *** high crystallinity WS2 exhibits high degree of interlayer order and strong interlayer force. It exhibits superior electrochemical properties, at the current density of 200 mA g^(-1) after 300 cycles with reversible capacity of 471 mAh g^(-1). Even at 5.0 A g^(-1), the capacities can still arrive at 240 mAh g^(-1) after 250 cycles, exhibiting stable cycling performance. Further electrochemical research finds that the high degree of interlayer order of layered WS2 structure can perform highly conducive Na+insertion/extraction with greatly improved contribution of intercalation capacity. Moreover, the strong interlayer force can effectively restrain the exfoliating of the WS2 nanosheets, guaranteeing the stability of the structure. Combining the above result reveals that controlling the order and force of the interlayer is an effective way to enhance the electrochemical properties of WS2 as SIBs anode materials. This work can provide new insight for inhibiting the exfoliation of layered compounds to pursue excellent electrochemical performance in Na-ion storage systems.

I report the reason why Tensor satisfying Binary Law has relations toward physics in this article. Q: The n th-order covariant derivative of the Vector {Aμ, Aμ}:(n=1) satisfying Binary Law. R: The n th-order covariant derivative of the Vector {Aμ, Aμ}:(n≥2) satisfying Binary Law. I have reported in other articles about Q. I report R in this article. I obtained the following results in this. I got the conclusion that derived function became 0. The derived function becoming 0 in the n th-order covariant derivative of the covariant vector Aμhere in the case of n=2. Similarly, in the n th-order covariant derivative of the contravariant vector Aμin the case of n=4 .

Barros discusses that [Jose Acacio de Barros, Int. J. Theor. Phys. 50, 1828 (2011)] Nagata derives inconsistencies from quantum mechanics [K. Nagata, Int. J. Theor. Phys. 48, 3532 (2009)]. Barros considers that the inconsistencies do not come from quantum mechanics, but from extra assumptions about the reality of observables. Here we discuss the fact that there is a contradiction within the quantum theory. We discuss the fact that only one expected value in a spin-1/2 pure state 〈σx〉rules out the reality of the observable. We do not accept extra assumptions about the reality of observables. We use the actually measured results of quantum measurements (raw data). We use a single Pauli observable. We stress that we can use the quantum theory even if we give up the axiomatic system for the quantum theory.
