Perceived Program Components of <i>Omotenashi Family Experiences Learning Program</i>in Japan: Qualitative Study of Families of Persons with Mental Disorders

Families need sufficient learning opportunities about mental illness. Therefore, family peer education program has been developed in Japan following the U.S. and Hong Kong. We aimed to identify program components that families recognize which would provide suggestions in modifying the program. We used qualitative and inductive study design. Data were collected through group interviews for six family research participants. We coded the data to focus on relationships between program components, program processes, and impacts. Data Analysis resulted in seven assumptions based on causal relationships and three types of program components: program form, learning style and facilitation skills. The program form is family-led peer group, closed-membership, 5 times per course, and small group. The learning style is combination of textbook and experiential knowledge, family peer learning, and sharing experiences. The facilitation skills are group work focusing on strengths of family members, the acceptance of emotional disclosure, omotenashi, and teamwork skills.

High-speed measurement confronts the extreme speed limit when the signal becomes comparable to the noise *** the context of broadband mid-infrared spectroscopy,state-of-the-art ultrafast Fourier-transform infrared spectrometers,in particular dual-comb spectrometers,have improved the measurement rate up to a few MSpectra s^(-1),which is limited by the signal-to-noise ***-stretch infrared spectroscopy,an emerging ultrafast frequency-swept mid-infrared spectroscopy technique,has shown a record-high rate of 80 MSpectra s^(-1) with an intrinsically higher signal-to-noise ratio than Fourier-transform spectroscopy by more than the square-root of the number of spectral ***,it can measure no more than~30 spectral elements with a low resolution of several ***,we significantly increase the measurable number of spectral elements to more than 1000 by incorporating a nonlinear upconversion *** one-to-one mapping of a broadband spectrum from the mid-infrared to the near-infrared telecommunication region enables low-loss time-stretching with a single-mode optical fiber and low-noise signal detection with a high-bandwidth *** demonstrate high-resolution mid-infrared spectroscopy of gas-phase methane molecules with a high resolution of 0.017 cm^(-1).This unprecedentedly high-speed vibrational spectroscopy technique would satisfy various unmet needs in experimental molecular science,e.g.,measuring ultrafast dynamics of rreversible phenomena,statistically analyzing a large amount of heterogeneous spectral data,or taking broadband hyperspectral images at a high frame rate.

Use of Analogy by Public Health Nurses in Problem Solving for Individual Consultations in Japan: A Multiple Case Study

Aim: Nurses must develop good problem-solving skills (PSS) to work in the complex health care environment. This study explored PHNs’ use of analogy in PSS development. The purposes of the study were to clarify how PSS developed in one area (i.e., mental health) could be applied to another area (i.e., maternal health) and whether new PSS could develop in response to PSS gained in another area. Methods: A multiple case study was conducted using interviews. We interviewed 27 consultations from eight Japanese PHNs who consulted in mental health departments before transferring to maternal health departments. The data on how PHNs applied PSS in the selection, mapping, evaluation, and learning stages of the analogy process were extracted from transcribed interviewed data and compared. Results: PHNs provided 59 PSS used in 27 consultations. All PHNs applied past mental health PSS to solve new problems in maternal health. They tended to select past PSS based on structural similarity and to apply PSS via low-level abstraction in serious situations or preventively to avoid causing the current situation to worsen. Notably, PHNs developed maternal health PSS by using past mental health PSS;these new PSS were derived through analogy from various failures and successes. Conclusions: PSS developed in one area can be applied in another area, and new PSS can develop through applying these previous PSS. Identification of structural similarities and preventive analogies must be included in nursing education, especially for nurses working in public health fields.

We report an extremely rare case where a mesenchymal differentiation, especially embryonal sarcoma, was demonstrated in cholangiocarcinoma. At autopsy, a yellowish-white tumor （15 cm× 12 cm） was found in the right hepatic lobe, and there were several daughter nodules in both hepatic lobes. Histologically, most of the main tumor and all of the daughter nodules examined showed sarcomatous changes （spindle cells, pleomorphic ceils and hyalization）. Histologic examination of a part of the main tumor disclosed a focus of adenocarcinoma within the tumor. The frequent transitions between the adenocarcinomatous areas and the sarcomatous areas suggested that sarcomatous transformation occurred in the cholangiocarcinoma and then spread rapidly. Immunohistochemically, the adenocarcinomatous elements were positive for cytokeratin, carcinoembryonic antigen （CEA） and epithelial membrane antigen, and negative in the sarcomatous cells. Vimentin was positive only in the sarcomatous elements. The findings of the present case support the view that carcinosarcomas represent carcinomas that develop sarcomatous elements via metaplasia of the epithelial element.

Negative bHLH transcription factor Hesl can inhibit neural stem cells(NSCs)from precocious neurogenesis through repressing proneural gene expression;therefore,sustenance of Hesl expression is crucial for NSC pool ***,we find that lds,the dominant=negative regulators of proneural proteins,are expressed prior to proneural genes and share an overlapping expression pattern with Hesl in the early neural tube of chick *** of ld2 in the chick hindbrain upregulates Hesl expression and inhibits proneural gene expression and neuronal *** contrast,Hesl expression decreases,proneural gene expression expands and neurogenesis occurs precociously in Id1;Id3 double knockout mice and in Id1-3 RNAi-electroporated chick *** studies show that Id proteins interact directly with Hesl and release the negative feedback autoregulation of Hesl without interfering with its ability to affect other target *** results indicate that Id proteins participate in NSC maintenance through sustaining Hesl expression in early embryos.

Utility of Miconazole Therapy for <i>Trichosporon</i>Fungemia in Patients with Acute Leukemia

Invasive trichosporonosis is an extremely rare mycosis, but Trichosporon fungemia (TF) in patients with hematologic malignancies has been increasingly recognized to be a fulminant and highly lethal infection. Although the utility of azole therapy has been demonstrated in several observations, little is known about the efficacy of one of azoles, miconazole (MCZ). To assess its therapeutic role, we retrospectively investigated 6 cases of TF in patients with acute leukemia receiving MCZ containing regimens. Successful outcome was obtained in 4 patients [MCZ + amphotericin B (AmB) in 2, MCZ only and MCZ + fluconazole (FLCZ) + AmB in one each], but not in 2 (MCZ + FLCZ + AmB and MCZ + FLCZ in one each). Although MCZ and AmB exhibited good in vitro activities against isolates from all patients, FLCZ had such finding from only one patient. Considering the reportedly limited utility of AmB, MCZ seemed to play a critical role even in the combination therapies for TF. Despite the release of newer azoles and other classes of antifungals, the use of MCZ remains a potential therapeutic approach for TF in patients with acute leukemia.
