Heterogeneity in supraglacial debris thickness and its role in glacier mass changes of the Mount Gongga

In the Tibetan Plateau, many glaciers have extensive covers of supraglacial debris in their ablation zones, which affects glacier response to climate change by altering ice melting and spatial patterns of mass loss. Insufficient debris thickness data make it difficult to analyze regional debris-cover effects. Maritime glaciers of the Mount Gongga have been characterized by a substantial reduction in glacier area and ice mass in recent decades. The thermal property of the debris layer estimated from remotely sensed data reveals that debris-covered glaciers are dominant in this region, on which the proportion of debris cover to total glacier area varies from 1.74% to 53.0%. Using a physically-based debris-cover effect assessment model, we found that although the presence of supraglacial debris has a significant insulating effect on heavily debris-covered glaciers, il accelerates ice melting on -10.2% of total ablation zone and produces rapid wastage of -25% of the debris-covered glaciers, leading to the similar mass losses between the debris-covered and debris-free glaciers. Widespread debris cover also facilitates the development of active terminus regions. Regional differences in debris-cover effects are apparent, highlighting the im- portance of debris cover for understanding glacier mass changes in the Tibetan Plateau and other mountain ranges around the world.

Is hepatic arterial infusion chemotherapy effective treatment for advanced hepatocellular carcinoma resistant to transarterial chemoembolization?

AIM:To evaluate the effectiveness of hepatic arterial infusion chemotherapy(HAIC) for advanced hepatocellular carcinoma(HCC) resistant to transarterial chemoembolization(TACE).METHODS:This study was conducted on 42 patients who received HAIC for advanced HCC between 2001and 2010 at our hospital.5-fluorouracil(5-FU) was administered continuously for 24 h from day 1 to day 5 every 2-4 wk via an injection ***-arterial cisplatin or subcutaneous interferon was administered in combination with the *** patients enrolled in this retrospective study were divided into two groups according to whether or not they fulfilled the criteria for resistance to TACE proposed by the Japan Society of Hepatology in 2010(written in Japanese);one group of patients who did not fulfill the criteria for TACE resistance(group A,n = 23),and another group who fulfilled the criteria for TACE resistance(group B,n = 19).We compared the outcomes in terms of the response and survival rates between the two ***:Both the response rate and tumor suppression rate following HAIC were significantly superior in group A than in group B(response rate:48% vs 16%,P = 0.028,tumor suppression rate:87% vs 53%,P = 0.014).Furthermore,both the progression-free survival rate and survival time were significantly superior in group A than in group B(3-,6-,12-,and 24-mo = 83%,70%,29% and 20% vs 63%,42%,16% and 0%,respectively,P = 0.040,and 9.8 mo vs 6.2 mo,P = 0.040).A multivariate analysis(Cox proportional hazards regression model) showed that resistance to TACE was an independent predictor of poor survival(P = 0.007).CONCLUSION:HAIC administrating 5-FU was not effective against advanced HCC resistant to *** tools for treatment,i.e.,molecular-targeting agents may be considered for these cases.

Diabetes mellitus and metformin in hepatocellular carcinoma

Hepatocellular carcinoma(HCC) is the leading cause of cancer-related death worldwide. Diabetes mellitus, a risk factor for cancer, is also globally endemic. The clinical link between these two diseases has been the subject of investigation for a century, and diabetes mellitus has been established as a risk factor for HCC. Accordingly, metformin, a first-line oral anti-diabetic, was first proposed as a candidate anti-cancer agent in 2005 in a cohort study in Scotland. Several subsequent large cohort studies and randomized controlled trials have not demonstrated significant efficacy for metformin in suppressing HCC incidence and mortality in diabetic patients; however, two recent randomized controlled trials have reported positive data for the tumor-preventive potential of metformin in non-diabetic subjects. The search for biological links between cancer and diabetes has revealed intracellular pathways that are shared by cancer and diabetes. The signal transduction mechanisms by which metformin suppresses carcinogenesis in cell lines or xenograft tissues and improves chemoresistance in cancer stem cells have also been elucidated. This review addresses the clinical and biological links between HCC and diabetes mellitus and the anti-cancer activity of metformin in clinical studies and basic experiments.

The Association between Awareness of Sexual Behavior and Cognitive Social Capital among High School Students in Vietnam

Background: Globally, adolescent sexual behavior is a major health concern and the focus of many researches. However, negative attitude towards premarital sex and sense of community belonging have not been examined in relation to adolescent sexual behavior. The purpose of this study was to examine whether these two factors are associated with adolescent sexual behavior. Methods: We conducted a cross-sectional survey of 1672 eleventh grade students from three public high schools in Hanoi, Vietnam. Participants’ sense of community belonging and negative attitude towards premarital sex were assessed through a self-administered questionnaire. We performed multiple logistic regression analyses to examine associations between the cognitive social capital concepts, such as a sense of community belonging. Results: 92.3% of students agreed with a sense of community belonging, but only 14.1% of students accepted premarital sex. A high sense of community belonging was significantly associated with negative attitude towards premarital sex OR (95% CI), male 3.16 (2.3-4.34), female 8.77 (5.38-14.29). After adjusting for a sense of community belonging, the association disappeared between acceptance of premarital sex and sexual experience of peers, proper time to wear a condom, and Rosenberg Self-Esteem Scale score. Conclusion: Cognitive social capital, such as sense of community belonging, was associated with negative attitude towards premarital sex. Therefore, cognitive social capital concepts should be included in sexual behavior and health promotions, especially with adolescents.

Self-Efficacy as a Suicidal Ideation Predictor: A Population Cohort Study in Rural Japan

Background: Self-efficacy has been widely studied in suicide, both in its causality and treatment effects. However, the evidence of causality is still ambiguous and there is a lack of generalizability. The aim of this study was to examine the relationship between self-efficacy and suicidal ideation through a population cohort study. Methods: The community-based household survey using a self-administered questionnaire was conducted in a rural area of Japan, Happo Town, in Akita Prefecture with community residents aged 30 and over at two respective time points by local health volunteers. The baseline survey was conducted in 2010 with a response rate of 88.9% (n = 6044). Among them, 3812 residents met the inclusion criteria for the follow-up survey in 2012 where the response rate was 75.3% (n = 2869). Exposure variables to suicidal ideation included demographic details, depression and self-efficacy. The Institutional Review Board and the Ethics Committee of Akita University approved the study protocol and all subjects signed informed consent. Results: A total number of 2105 participants (76.4%) without suicidal ideation in the baseline study were enrolled into the follow-up study, and 8.2% of them had developed suicidal ideation. These participants with suicidal ideation were significantly less likely to be married/cohabitant;they had worse subjective health, poorer self-perceived economic status, stronger depressive mood, and lower self-efficacy scores. The odds ratio of the self-efficacy scores at follow-up survey for participants who had developed suicidal ideation were about 2 times lower than at baseline (95% confidence interval = 1. 53 - 3.06). After adjusting for all confounding factors, the association was still significant (OR = 1.66, 95% CI = 1.15 - 2.42). Conclusion: This result suggests that suicidal ideation may be prevented by increasing self-efficacy. Wind Tunnel Test for Videogrammetric Deformation Measurement of UAV for Mars Airplane Balloon Experiment-1 (MABE-1)

This paper reports the results of the aerodynamic deformation measurements of the meter-scale, entire shape, actual UAV in the wind tunnel using a video grammetry technique. The measured airplane was the airplane for Mars exploration being developed by Japan Aerospace Exploration Agency (JAXA) and Japanese universities. Its main wing span length was 2.4 m. The video grammetry measurement was performed using VICON’s system. Retroactive markers and stickers were put on the airplane. JAXA’s 6.5 m × 5.5 m Low-Speed Wind Tunnel was used. The airplane was mounted on the strut support with pitch-free or pitch-locked conditions. The deformations of the main wing bending, the main wing twisting, the tail boom bending, and the elevator deflection angle change were revealed quantitatively. The bending stiffness of a main wing spar that was designed as a safety factor of 2.8 at load factor of 5 was sufficient. The main wing spar was located around a center of pressure of an airfoil and it showed enough stiffness for twisting at nominal condition. The effects of the main wing bending and twisting, and the tail boom bending on the aerodynamic performance were estimated but they were in an acceptable range from the standpoint of the controllability of the aerodynamic performance using control surfaces. Even though the servo motor was located near the elevator and the linkage between the servo motor and the elevator was short, the measured elevator deflection angle was at most 4% smaller than the angle at no-wind condition. The obtained results and presented method are useful for control, flight data analysis, and design of lightweight airplanes.

Surgical Repair of Dorsal Capsulo-Ligamentous Complex for Dislocation of the Thumb Carpometacarpal Joint

Background: Acute dislocation of the thumb carpometacarpal (CMC) joint is a rare and challenging injury, and its treatment remains controversial. The purpose of the study was to evaluate the clinical outcome after surgical repair of the dorsal capsulo-ligamentous complex. Materials and methods: Eight patients with a mean age of 39.8 years (range, 23 - 56 years) treated for the thumb CMC joint were treated in our hospital between 2008 and 2018. The ruptured dorsal capsulo-ligamentous complex was repaired using suture anchors in all patients, and the joint was immobilized with a Kirschner wire pinning and splint for 3 weeks. The clinical outcome was assessed by measuring the range of motion of the thumb CMC joint and the Japanese Society for Surgery of the Hand version of the Quick Disability of the Arm, Shoulder, and Hand (Q-DASH-JSSH) score. Results: The mean radial abduction of the thumb CMC joint was 66°, and the volar abduction was 63°. The mean Q-DASH-JSSH score was 4.8 (range, 0 - 15.6). There were no cases of infection, nerve disturbance, or osteoarthritis. Anatomical reduction was observed in seven patients at the final follow-up and in a rugby football player at 6 months of follow-up;however, subluxation of the thumb CMC joint at 12 months of follow-up due to reinjury was observed. Conclusion: Surgical repair of the dorsal capsulo-ligamentous complex with suture anchors is a reliable and simple treatment for acute unstable dislocation of the thumb CMC joint.

Levovist ultrasonography imaging in intracystic hemorrhage of simple liver cyst

The differential diagnosis between intracystic hemorrhage and cystadenocarcinoma of the liver is often difficult even with the use of various imaging modalities. A 73-year-old woman was admitted to our hospital with the complaint of right upper quadrant pain. Ultrasonography (US) demonstrated a heterogeneous echogenic cyst measuring 11 cm × 8 cm in size in S2 of the liver, indicated intracystic hemorrhage of simple liver cyst or cystadenocarcinoma, but the differential diagnosis was considerably difficult. Levovist (Schering, Berlin, Germany) US revealed no enhancement of the intracystic structures, suggesting a clot in the case of intracystic hemorrhage. An operation was performed and the resected lesion showed a solitary benign liver cyst, measuring 5.5 cm × 4.7 cm × 8.5 cm containing a large blood clot. The patient had an uneventful recovery after the surgery. Levovist US may play an important role in discrimination between intracystic hemorrhage of simple hepatic cysts and cystadenocarcinoma of the liver.
