Successful biliary drainage using a metal stent through the gastric stoma

We report a case of biliary drainage for malignant stricture using a metal stent with an ultrathin endoscope through the gastric stoma. A 78-year-old female was referred to our hospital for jaundice and fever. She had undergone percutaneous endoscopic gastrostomy(PEG) for esophageal obstruction after radiation therapy for cancer of the pharynx. Abdominal contrast-enhanced computed tomography showed a 3-cm enhanced mass in the middle bile duct and dilatation of the intrahepatic bile duct. We initially performed endoscopic retrograde cholangiopancreatography(ERCP) with a trans-oral approach. However, neither the side-viewing endoscope nor the ultrathin endoscope passed through the esophageal orifice. Thus, we eventually performed ERCP via the PEG stoma using an ultrathin endoscope. We performed biliary drainage with a 6F introducer selfexpanding metal stent. The cytology findings obtained by brush cytology showed malignancy. Her laboratory results were restored to normal levels after drainage and no complication occurred.

The management of jaundice and cholangitis is important for improving the prognosis and quality of life of patients with unresectable malignant hilar biliary strictures(UMHBS). In addition, effective chemotherapy, such as a combination of gemcitabine and cisplatin, requires the successful control of jaundice and cholangitis. However, endoscopic drainage for UMHBS is technical demanding, and continuing controversies exist in the selection of the most appropriate devices and techniques for stent deployment. Although metallic stents(MS) are superior to the usual plastic stents in terms of patency, an extensive comparison between MS and "inside stents", which are deployed above the sphincter of Oddi, is necessary. Which techniques are preferred remains as yet unresolved: for instance, whether to use a unilateral or bilateral drainage, or a stent-in-stent or side-by-side method for the deployment of bilateral MS, although a new cell design and thin delivery system for MS allowed us to accomplish successful deployments of bilateral MS. The development of techniques and devices for re-intervention after stent occlusion is also imperative. Further critical investigations of more effective devices and techniques, and increased randomized controlled trials are warranted to resolve these important issues.

Bilioenteric or pancreatoenteric anastomotic stric-tures often occur after surgery for a pancreaticobiliary disorder. Therapeutic endoscopic retrograde cholan-giopancreatography using balloon enteroscopy has been shown to be feasible and effective in patients with such strictures. However, when a benign anas-tomotic stricture is severe, a dilation catheter cannot pass through the stricture despite successful insertion of the guidewire. We report on the usefulness of the Soehendra Stent Retriever over a guidewire for dilating a severe bilioenteric or pancreatoenteric anastomotic stricture under short double-balloon enteroscopy, in two patients with surgically altered anatomies.

Low-temperature thermochronology is a widely used tool for revealing denudation histories of mountain ranges. Although this technique has been applied mainly to continental orogens, such as the European Alps, Himalayas, and Andes, recent technological development of low-temperature thermochronology has made it applicable to a wider variety of mountain ranges with various sizes and tectonic histories. The Japanese Islands comprise young and active island arcs, where an early stage of mountain range formation is observed. Numerous attempts have been made to constrain the uplift and denudation histories of the mountains in the Japanese Islands using geologic, geomorphologic, or geodetic methods. However, the number of thermochronometric attempts has been limited primarily due to the small amount of total denudation since the initiation of the uplift. In this review paper, we introduce the tectonic and geomorphic settings of the mountain ranges in the Japanese Islands, and discuss previous attempts to estimate uplift or denudation of the Japanese mountains using methods other than ther- mochronology. Furthermore, we discuss problems of the thermochronometric applications in revealing denudation histories of the Japanese mountains. Finally, we present a case study of the Kiso Range in central Japan and discuss the current effectiveness and applicability of low-temperature thermochronology to the Japanese mountainous areas.

Geochemistry of Rare Earth Elements(REE) in the Weathered Crusts from the Granitic Rocks in Sulawesi Island, Indonesia

We report for the first time the geochemistry of rare earth elements（REE） in the weathered crusts of I-type and calc-alkaline to high-K（shoshonitic） granitic rocks at Mamasa and Palu region, Sulawesi Island, Indonesia. The weathered crusts can be divided into horizon A（lateritic profile） and B（weathered horizon）. Quartz, albite, kaolinite, halloysite and montmorrilonite prevail in the weathered crust. Both weathered profiles show that the total REE increased from the parent rocks to the horizon B but significantly decrease toward the upper part（horizon A）. LREE are enriched toward the upper part of the profile as shown by La/YbN value. However, HREE concentrations are high in horizon B1 in Palu profile. The total REE content of the weathered crust are relatively elevated compared to the parent rocks, particularly in the lower part of horizon B in Mamasa profile and in horizon B2 in Palu profile. This suggests that REE-bearing accessory minerals may be resistant against weathering and may remain as residual phase in the weathered crusts. The normalized isocon diagram shows that the mass balance of major and REE components between each horizon in Mamasa and Palu weathering profile are different. The positive Ce anomaly in the horizon A of Mamasa profile indicated that Ce is rapidly precipitated during weathering and retain at the upper soil horizon.

Hepatocellular carcinoma(HCC), the fifth most common cancer in the world, shows increasing incidence worldwide. Curative treatments such as hepatectomy,liver transplantation, and radiofrequency ablation are applied in only 30%-60% of cases. Most remaining patients receive transarterial chemoembolization(TACE).Patients with intermediate-stage HCCs are regarded as good candidates for TACE. Does foreign direct investment （FDI） into developing countries affect the growth of local firms in host countries？ Using a dataset of 38 sectors in China ＇s electrical and electronics industry, in this paper, we analyze whether FDI has a positive effect on loeal firms, with technology spillovers, added value and increasing total faetor productivity, or a negative, market stealing, effect. Estimating the relationship between growth of local firms and investment offoreign firms, our results show that FDI is likely to have a negative impact on the growth of local firms in sectors with large disparities in technology and less experience in business. Therefore, local firms lacking in technology need to find markets with no competition from foreign firms or determine strategies to compensate technology disparities.
