Laparoscopic surgery for colorectal disease has become widespread as a minimally invasive *** is important because the increasing availability of new devices allows us to perform procedures with a reduced length of surgery and decreased blood *** herein report the results of a literature review of energy sources for laparoscopic colorectal surgery,focused especially on 6 studies comparing ultrasonic coagulating shears(UCS) and other *** also describe our laparoscopic dissection techniques using UCS for colorectal *** short-term outcomes of surgeries using UCS and Ligasure for laparoscopiccolorectal surgery were superior to conventional *** authors have reported that the length of surgery or blood loss when Ligasure was used for laparoscopic colorectal surgery is less than when UCS was *** the other hand,a recent study demonstrated that there were no significant differences between the short-term outcomes of UCS and Ligasure for laparoscopic colorectal *** is therefore suggested that the choice of technique used should be made according to the surgeon’s *** also describe our laparoscopic dissection techniques using UCS(Harmonic ACE) for colorectal cancer with regard to the retroperitoneum dissection,dissection technique,dissection technique around the feeding artery,and various other dissection *** therefore review the outcomes of using various energy sources for laparoscopic colorectal surgery and describe our laparoscopic dissection techniques with UCS(Harmonic ACE) for colorectal cancer.

Influence of Kupffer cells and platelets on ischemia-reperfusion injury in mild steatotic liver

AIM: To investigate the effect of mild steatotic liver on ischemia-reperfusion injury by focusing on Kupffer cells (KCs) and platelets. METHODS: Wistar rats were divided into a normal liver group (N group) and a mild steatotic liver group (S group) induced by feeding a choline-deficient diet for 2 wk. Both groups were subjected to 20 min of warm ischemia followed by 120 min of reperfusion. The number of labeled KCs and platelets in sinusoids and the blood perfusion in sinusoids were observed by intravital microscopy (IVM), which was performed at 30, 60 and 120 min after reperfusion. To evaluate serum alanine aminotransferase as a marker of liver deterioration, blood samples were taken at the same time as ***: In the S group, the number of platelets adhering to KCs decreased significantly compared with the N group (120 after reperfusion; 2.9±1.1 cells/acinus vs 4.8±1.2 cells/acinus, P<0.01). The number of KCs in sinusoids was significantly less in the S group than in the N group throughout the observation periods (before ischemia, 19.6±3.3 cells/acinus vs 28.2±4.1 cells/acinus, P<0.01 and 120 min after reperfusion, 29.0±4.3 cells/acinus vs 40.2±3.3 cells/acinus, P<0.01). The blood perfusion of sinusoids 120 min after reperfusion was maintained in the S group more than in the N group. Furthermore, elevation of serum alanine aminotransferase was lower in the S group than in the N group 120 min after reperfusion (99.7±19.8 IU/L vs 166.3±61.1 IU/L, P=0.041), and histological impairment of hepatocyte structure was prevented in the S group. CONCLUSION: Ischemia-reperfusion injury in mild steatotic liver was attenuated compared with normal liver due to the decreased number of KCs and the reduction of the KC-platelet interaction.

A novel class of amphiphiles with sulfate groups at the C-6 position and palmitoyl groups at the C-2, 3 positions of α-, β-, and γ-cyclodextrin (CD) were efficiently synthesized. These compounds formed stable monolayers with high collapse pressures at the air-water interface. The mixed monolayer behaviors of the 6-O-sulfated CD amphiphiles (SO3-CDC16) in the presence of dipalmitoyl phosphatidylcholine (DPPC) and cholesterol were discussed using the surface pressure-molecular area (π-A) isotherms. The collapse pressures showed maxima at molar ratios of SO3-CDC16 lower than 10 mol%. A morphological analysis of the liposomes containing DPPC and 4 mol% SO3-CDC16 formed in PBS was carried out using transmission electron microscopy with negative staining, and vesicles with maximum diameters of 350-500 nm were observed. Moreover, the releasing ability of these liposomes was examined using a fluorescent compound, calcein. It was clearly shown that liposomes containing SO3-CDC16 could release encapsulated calcein more easily than liposomes consisting only of DPPC, and that the release rate depended on the phase transition temperature of the SO3-CDC16 included in the liposome membrane.

The clinical outcomes of adolescents with avoidant/restrictive food intake disorder(ARFID)remain ***,no report has compared the characteristics of ARFID and restricting-type anorexia nervosa(R-AN)in elementary-school students on total parenteral nutrition(TPN).This study retrospectively reviewed inpatients diagnosed with ARFID or R-AN between 2005 and *** with ARFID(two boys and seven girls)and R-AN(13 girls)were hospitalized because of rapid physical deterioration,and nutrition therapy was continued without *** ARFID group exhibited significantly lower body weights at admission than the R-AN group and gained an average of 6.5 kg during hospitalization;furthermore,the monthly weight gain during hospitalization was significantly higher,and no relapse was *** physical improvement in ARFID resulted in good *** conclusion,TPN can be easily introduced to patients with ARFID,in whom aversive eating is a concern,and is a suitable treatment for ARFID.

We report a successful case of a pregnant female positive for anti-cardiolipin antibodies who experienced two abnormal pregnancies with postpartum cerebral hemispheric infarctions. A 38-year-old female diagnosed as being positive for anti-cardiolipin antibodies was referred to our hospital due to her strong desire to have a baby. The administration of Japanese herbal medicine, Sairei-to, as immunosuppressive therapy, and low dose aspirin, as anti-coagulation therapy, were initiated prior to the patient’s pregnancy. Five months after beginning the treatment, the patient conceived spontaneously. At 34 weeks of gestation, emergency cesarean section was performed due to increasing genital bleeding resulting from coincidental placenta previa and the patient delivered an appropriate-for-date female infant (1970 g). Treatment with Japanese herbal medicine (Sairei-to) and low-dose aspirin is considered to be an effective treatment option for patients positive for anti-phospholipid antibodies with past histories of abnormal pregnancies.

The global increase in the prevalence of drug-resistant bacteria has necessitated the development of alternative treatments that do not rely on conventional antimicrobial *** bacteriophage-derived lytic enzymes in antibacterial therapy shows promise;however,a thorough comparison and evaluation of their bactericidal efficacy are *** study aimed to compare and investigate the bactericidal activity and spectrum of such lytic enzymes,with the goal of harnessing them for antibacterial ***,we examined the bactericidal activity of spanins,endolysins,and holins derived from 2 Escherichia coli model phages,T1 and *** these,T1-spanin exhibited the highest bactericidal activity against ***,we expressed T1-spanin within bacterial cells and assessed its bactericidal activity.T1-spanin showed potent bactericidal activity against all clinical isolates tested,including bacterial strains of 111 ***,2 Acinetobacter spp.,3 Klebsiella spp.,and 3 Pseudomonas *** contrast,T1 phage-derived endolysin showed bactericidal activity against *** and ***,yet its efficacy against other bacteria was inferior to that of ***,we developed a phage-based technology to introduce the T1-spanin gene into target *** synthesized non-proliferative phage exhibited strong antibacterial activity against the targeted *** potent bactericidal activity exhibited by spanins,combined with the novel phage synthetic technology,holds promise for the development of innovative antimicrobial agents.

背景AirwayScope（AWS）和Airtraq（ATQ）喉镜的镜片结构相似，在本次研究中，我们评估了无操作经验的人员在模拟人困难气道模型上分别应用AWS、ATQ和Macintoshlamgoscope（ML）喉镜所完成气管插管的难易性。方法24名无气管插管经验的5年级医学生参加了此项研究，采用人体模型（SimMan）来模拟各种不同的困难气道，包括颈椎强直、张口受限、咽部阻塞3种场景。气管插管装置及困难气道类型的顺序是随机的。我们分析了气管插管的成功率，声门暴露、气管插管、张肺所需的时间，调整手法使用次数和摩擦牙齿的喀声次数。24名学生用3种不同的插管装置在4种不同的气道条件下进行操作。结果与ML组相比，AWS组、ATQ组均有较高的气管插管成功率（AWS组为100％，ATQ组为98％，ML组为89％，AWS组、ATQ组与ML组相比P〈0．05），AWS组所需的插管时间显著少于ATQ组和ML组（AWS组为11±6秒，ATQ组为16±12秒，ML组为16±11秒），与ATQ组和ML组相比，AWS组需要优化调整的次数显著减少（P〈0．05），ML组插管过程中可闻及的牙齿喀声比AWS组和ATQ组显著增多。结论对于在模拟困难气道下进行气管插管的无插管经验的人员，AWS和ATQ可能是更适合的插管装置，有必要在今后的临床实践中进一步证实此项发现。

目的观察原因不明习惯性流产(RSA)患者的封闭抗体效应与辅助性T细胞(Th)1型免疫之间的关系。方法以20例连续发生过3次以上早期自然流产的患者作为观察对象。采用比重离心法从外周血中提取淋巴细胞,以1×106/ml浓度加入含有1%小牛血清的RPMI中。每毫升淋巴细胞悬浮液中加入植物血凝素(PHA)2μg培养72 h。采用酶联免疫吸附试验(ELISA法)检测培养液中肿瘤坏死因子(TNF)-α的水平。采用混合淋巴细胞培养方法检测研究对象的封闭抗体效应(MLR-BE),>22%为阳性,≤22%为阴性。分析TNF-α水平与MLR-BE的关系。结果20例患者TNF-α的平均浓度为(561.0±522.9)pg/ml,平均MLR-BE为(-6.5±37.6)%(阴性14例、阳性6例)。MLR-BE阴性患者的TNF-α水平为(676.1±585.0)pg/ml,显著高于MLR-BE阳性患者的(292.5±159.5)pg/ml(P0.05)。结论MLR-BE可能影响母体Th1/Th2的平衡,从而导致习惯性流产的发生。
