Novel technologies have been necessary for improving fruit quality and productivity of citrus,labor-saving and orchard conservation on steep slope lands since aging of growers and decrease in the number of successors is remarkable in mountain areas of southwestern *** purpose of this paper is to introduce new technologies for improving citrus production that have been developed in recent years.A new fruit quality control system using drip irrigation and liquid fertilization technique combined with year-round plastic mulching was developed,and it enables high quality and stable citrus fruit *** and/or nutrient solution is automatically supplied through drip tubes that are laid under the mulching sheets to give adequate water stress,so as to improve sugar and acid content of fruit.A new transportation system for steep sloping citrus orchards,which is a combination of the monorail system and contour narrow paths,was suggested.A small walking cultivator was developed to explain the procedure of narrow path *** introducing the narrow path,working hours for fertilizer and chemical herbicide application were *** prevention mapping of citrus orchards on slope lands was developed based on computer-aided seepage estimation and topographic *** mapping can show zones of both ascending flow and descending flow of underground water during heavy rains in citrus *** mapping is considered to be effective for the management of orchards and prevention of erosion on slope lands.

Surgical resection for colorectal liver metastases(CRLM)may offer the best opportunity to improve ***,only about 20% of CRLM cases are indicated for resection at the time of diagnosis(initially resectable),and the remaining cases are treated as unresectable(initially unresectable).Thanks to recent remarkable developments in chemotherapy,interventional radiology,and surgical techniques,the resectability of CRLM is ***,some metastases are technically resectable but oncologically questionable for upfront *** pancreatic cancer,such cases are categorized as"borderline resectable",and their definition and treatment strategies are ***,in CRLM,although various poor prognosis factors have been identified in previous reports,no clear definition or treatment strategy for borderline resectable has yet been *** the efficacy of hepatectomy for CRLM was reported in the 1970 s,multidisciplinary treatment for unresectable cases has improved resectability and prognosis,and clarifying the definition and treatment strategy of borderline resectable CRLM should yield further improvement in *** review outlines the present status and the future perspective for borderline resectable CRLM,based on previous studies.

Prevention of esophageal strictures after endoscopic submucosal dissection

Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) have recently been accepted as less invasive methods for treating patients with early esophageal cancers such as squamous cell carcinoma and dysplasia of Barrett’s esophagus. However, the large defects in the esophageal mucosa often cause severe esophageal strictures, which dramatically reduce the patient’s quality of life. Although preventive endoscopic balloon dilatation can reduce dysphagia and the frequency of dilatation, other approaches are necessary to prevent esophageal strictures after ESD. This review describes several strategies for preventing esophageal strictures after ESD, with a particular focus on anti-inflammatory and tissue engineering approaches. The local injection of triamcinolone acetonide and other systemic steroid therapies are frequently used to prevent esophageal strictures after ESD. Tissue engineering approaches for preventing esophageal strictures have recently been applied in basic research studies. Scaffolds with temporary stents have been applied in five cases, and this technique has been shown to be safe and is anticipated to prevent esophageal strictures. Fabricated autologous oral mucosal epithelial cell sheets to cover the defective mucosa similarly to how commercially available skin products fabricated from epidermal cells are used for skin defects or in cases of intractable ulcers. Fabricated autologous oral-mucosal-epithelial cell sheets have already been shown to be safe.

Clinical management of gastrointestinal amyloidosis

Amyloidosis is characterized by extracellular deposition of abnormal protein, consisting of primary, secondary, hemodialysis-related, hereditary, senile and localized type. Primary amyloidosis is associated with monoclonal light chains. Secondary amyloidosis is associated with inflammatory, infectious, and neoplastic diseases. Amyloid deposition in the gastrointestinal tract can manifest the symptoms including diarrhea, steatorrhea, or constipation. For diagnosis, one should obtain an immunofixation of serum or urine as well as biopsy sampling of gastrointestinal mucosa stained specifically. While most gastrointestinal complications are managed symptomatically, treatment depends upon the type of amyloidosis. Causal therapy is reserved for a select few from various subtypes of this disorder.

＜正＞The closed loop equations of three cylindrical rollers, the spider of three spherical ends and the housing of the tripod constant velocity joints are deduced. They are solved for prescribed positions of its input and output shafts and relative motion characteristics of components are made clear. Moreover, a procedure is established for solving simultaneously a set of conditional equations with respect to forces and moments acting on three cylindrical rollers, the spider and the housing, for any values of friction coefficients between cylindrical rollers and its grooves and spherical ends. The established numerical procedure simulates the normal force acting on the roller groove with a period of π and the housing thrust force with a period of 2π/3 for given values of the joint angle. These results are inspected by experiments.

Custom Made Fenestrated Stent Graft Collapse after Thoracic Endovascular Aortic Repair: A Case Report

We present a case of stent graft collapse after performing thoracic endovascular aortic repair with a custom-made fenestrated stent graft. The patient was a 70-year-old woman with an asymptomatic aneurysm of the distal aortic arch, and thoracic endovascular aortic repair was performed. The patient showed a blood pressure difference between the left arm and the right arm on postoperative day (POD) 17 prompting the performance of a chest computed tomography scan which revealed stent graft collapse. She then underwent staged debranching of thoracic endovascular aortic repair. Stent graft collapse is a rare but well-described complication of thoracic endovascular repair. Therefore, patients who undergo such a procedure should be carefully monitored for signs and symptoms, which suggest the possibility of stent collapse.

Stanford Linear Accelerator Center （SLAC） is in northern California, USA. Radiological environmental protection is one of the main elements of the radiation protection program. One of SLAC’s accelerator facilities is B-Factory, whose PEP-II accelerator ring has been operating since 1997 and is being upgraded to higher luminosity operation. Four radiological issues associated with high luminosity operation up to CY2008 are re-evaluated: 1) annual doses in IR halls, 2) annual skyshine doses at the SLAC site boundaries, 3) potential radioactive air releases, and 4) potential groundwater activation. This paper presents the skyshine doses and air emission doses to the Maximally Exposed Individual （MEI） at SLAC site boundaries. The normal beam loss scenarios around PEP-II ring are presented first. In CY2008, the luminosity is 2x1034 cm-2s-1, and the stored current is 4.0-A for low-energy ring （LER ） and 2.2-A for high-energy ring （HER）. The beam losses around PEP-II ring include those near injection region in IR10 and IR8 and those at collimators （e.g., HER collimators in IR12, LER collimators in IR4 and IR6）. The beam losses in IR8 and IR10 （where injection into ring occurs） are further divided into septum, BAD （beam abort dump） and TD （tune-up dump）, as well as apertures. The skyshine prompt dose rate distributions as a function of distance from an IR hall at four directions were calculated using the MARS15 Monte Carlo code. For skyshine dose to the MEI, the annual dose （7200 h/y occupancy） is calculated to be 2.9 mrem/y at Sand Hill Road （from e- losses in IR12 HER collimators） and 1.2 mrem/y at Horse Track Offices near IR6 （from e+ losses in IR8, IR6 and IR4）. These are lower than the SLAC skyshine limit of 5 mrem/y for any single facility within SLAC. Radionuclide productions in the air at the PEP-II IR10 were calculated using MARS15. Generation of Arbitrary Pressure Pulsation of Wide Frequency Range for Flow Meter Testing in a Laminar Gas Pipeline

The present paper presents a device to test flow meters under an arbitrary pressure pulsation in a gas pipeline with a laminar flow containing frequency components up to 50 Hz, with the amplitude reaching hundreds of pascals. In order to reduce flow noise, the device has a strainer-like element connected to a pipeline under test and uses an open-loop control law based on the frequency response test. The control signal is calculated by adding the inputs to obtain each of the sinusoidal waves included in the original wave, which was decomposed by Fourier analysis. The validity of the developed method is demonstrated through the generation tests of superimposed pressure waves containing frequency components up to 50 Hz. Analysis of the relative uncertainty demonstrated the relative uncertainty to be less than 10% when the generated pressure is larger than 360 Pa.

Soluble Fiber Improves Management of Diarrhea in Elderly Patients Receiving Enteral Nutrition

Dietary fiber is a non-digestible carbohydrate providing beneficial effects for bowel health. The aim of this study was to evaluate the clinical effects of fiber supplementation in enteral feeding on elderly patients suffering from diarrhea. This study was conducted in 15 patients (7 men and 8 women, 79.0 ± 7.5 years) who had loose stools or diarrhea during enteral nutrition. The enteral formula was supplemented with soluble dietary fiber (5.2 g/day) for 3 weeks, which was then discontinued for 1 week to confirm its effects. The effects of soluble dietary fiber on stool frequency, the Bristol Stool Form Scale (which is designed to measure stool consistency), plasma diamine oxidase (DAO) activity, and concentrations of plasma short-chain fatty acids (SCFA) were evaluated. After supplementation with soluble dietary fiber, there were no significant differences in stool frequency but there was a significant improvement in stool consistency (P < 0.05). Furthermore, ingestion of soluble dietary fiber resulted in increased plasma DAO activity and significantly increased levels of plasma SCFA (P < 0.05). Supplementation with soluble dietary fiber may be beneficial for improving stool consistency in patients suffering from diarrhea during enteral nutrition. A further controlled trial is warranted to examine the preventive effects of soluble dietary fiber in patients suffering from diarrhea.
