Disc degeneration primarily contributes to chronic low back and neck pain.Consequently,there is an urgent need to understand the spectrum of disc degeneration phenotypes such as fibrosis,ectopic calcification,herniation,or mixed phenotypes.Amongst these phenotypes,disc calcification is the least studied.Ectopic calcification,by definition,is the pathological mineralization of soft tissues,widely studied in the context of conditions that afflict vasculature,skin,and cartilage.Clinically,disc calcification is associated with poor surgical outcomes and back pain refractory to conservative treatment.It is frequently seen as a consequence of disc aging and progressive degeneration but exhibits unique molecular and morphological characteristics:hypertrophic chondrocyte-like cell differentiation;TNAP,ENPP1,and ANK upregulation;cell death;altered Pi and PPi homeostasis;and local inflammation.Recent studies in mouse models have provided a better understanding of the mechanisms underlying this phenotype.It is essential to recognize that the presentation and nature of mineralization differ between AF,NP,and EP compartments.Moreover,the combination of anatomic location,genetics,and environmental stressors,such as aging or trauma,govern the predisposition to calcification.Lastly,the systemic regulation of calcium and Pi metabolism is less important than the local activity of PPi modulated by the ANK-ENPP1 axis,along with disc cell death and differentiation status.While there is limited understanding of this phenotype,understanding the molecular pathways governing local intervertebral disc calcification may lead to developing disease-modifying drugs and better clinical management of degeneration-related pathologies.

Objective This study aimed to evaluate the associations of serum folate and/or vitamin B12 concentrations with obesity among Chinese children and adolescents.Methods A cross-sectional study was conducted including 3,079 Chinese children and adolescents,aged 6 to 17 years,from Jiangsu,China.Anthropometric indices,such as,children's body mass index(BMI),BMI z-scores,waist circumference,and waist-to-height ratio were utilized.Multivariable linear regression and generalized additive models were used to investigate the associations of serum folate and vitamin B12 levels with anthropometric indices and odds of obesity.Results We observed that serum vitamin B12 concentrations were inversely associated with all anthropometric indices and the odds of general obesity[odds ratio(OR)=0.68;95%confidence interval(CI)=0.59,0.78]and abdominal obesity(OR=0.68;95%CI=0.60,0.77).When compared to participants with both serum vitamin levels in the two middle quartiles,those with both serum folate and vitamin B12 levels in the highest quartile were less prone to general(OR=0.31,95%CI=0.19,0.50)or abdominal obesity(OR=0.46,95%CI=0.31,0.67).Conversely,participants with vitamin B12 levels in the lowest quartile alongside folate levels in the highest quartile had higher odds of abdominal obesity(OR=2.06,95%CI=1.09,3.91).Conclusion Higher serum vitamin B12 concentrations,but not serum folate concentrations,were associated with lower odds of childhood obesity.Children and adolescents with high levels of vitamin B12 and folate were less likely to be obese.

Application of the Shrinking Particle Model for the Evaluation of Molecular Recyclability of PET versus Semi-Aromatic Polyesters

The molecular recyclability of poly (ethylene terephthalate) (PET) and three semi-aromatic polyesters poly (phloretic acid) (poly-H), poly (dihydroferulic acid) (poly-G), and poly (dihydrosinapinic acid) (poly-S) is evaluated in this study. PET is an extensively used aromatic polyester, and poly-H, poly-G, and poly-S can be considered semi-aromatic poly (lactic acid) modifications. All these polyesters have been depolymerized at neutral pH and by acid- and base-catalyzed hydrolysis at two temperatures, i.e., 50˚C and 80˚C. Base-catalyzed depolymerization of virgin PET leads to an isolated yield of 38% after 48 hours of reaction at 80˚C. Contrary to these results for PET, almost all the monomers of the semi-aromatic polyesters poly-H, poly-G, and poly-S are recovered with isolated yields larger than 90% at the same temperature after 15 minutes in a facile manner. A shrinking particle model used to determine the global kinetics of the base-catalyzed depolymerization showed that the rate rises with increasing temperature. Using the shrinking particle model, the intrinsic reaction rate constants were determined. It has been demonstrated that the rate coefficients of the depolymerization of the semi-aromatic polyesters poly-H, poly-G, and poly-S are between 2 and 3 orders of magnitude higher than those for PET.

BACKGROUND Acetabular component positioning in total hip arthroplasty(THA)is of key importance to ensure satisfactory post-operative outcomes and to minimize the risk of complications.The majority of acetabular components are aligned freehand,without the use of navigation methods.Patient specific instruments(PSI)and three-dimensional(3D)printing of THA placement guides are increasingly used in primary THA to ensure optimal positioning.AIM To summarize the literature on 3D printing in THA and how they improve acetabular component alignment.METHODS PubMed was used to identify and access scientific studies reporting on different 3D printing methods used in THA.Eight studies with 236 hips in 228 patients were included.The studies could be divided into two main categories;3D printed models and 3D printed guides.RESULTS 3D printing in THA helped improve preoperative cup size planning and post-operative Harris hip scores between intervention and control groups(P=0.019,P=0.009).Otherwise,outcome measures were heterogeneous and thus difficult to compare.The overarching consensus between the studies is that the use of 3D guidance tools can assist in improving THA cup positioning and reduce the need for revision THA and the associated costs.CONCLUSION The implementation of 3D printing and PSI for primary THA can significantly improve the positioning accuracy of the acetabular cup component and reduce the number of complications caused by malpositioning.

Background:The AbSeS-classification defines specific phenotypes of patients with intra-abdominal infection based on the(1)setting of infection onset(community-acquired,early onset,or late-onset hospital-acquired),(2)presence or absence of either localized or diffuse peritonitis,and(3)severity of disease expression(infection,sepsis,or septic shock).This classification system demonstrated reliable risk stratification in intensive care unit(ICU)patients with intra-abdominal infection.This study aimed to describe the epidemiology of ICU patients with pancreatic infection and assess the relationship between the components of the AbSeS-classification and mortality.Methods:This was a secondary analysis of an international observational study(“AbSeS”)investigating ICU patients with intra-abdominal infection.Only patients with pancreatic infection were included in this analysis(n=165).Mortality was defined as ICU mortality within 28 days of observation for patients discharged earlier from the ICU.Relationships with mortality were assessed using logistic regression analysis and reported as odds ratio(OR)and 95%confidence interval(CI).Results:The overall mortality was 35.2%(n=58).The independent risk factors for mortality included older age(OR=1.03,95%CI:1.0 to 1.1 P=0.023),localized peritonitis(OR=4.4,95%CI:1.4 to 13.9 P=0.011),and persistent signs of inflammation at day 7(OR=9.5,95%CI:3.8 to 23.9,P<0.001)or after the implementation of additional source control interventions within the first week(OR=4.0,95%CI:1.3 to 12.2,P=0.013).Gramnegative bacteria were most frequently isolated(n=58,49.2%)without clinically relevant differences in microbial etiology between survivors and non-survivors.Conclusions:In pancreatic infection,a challenging source/damage control and ongoing pancreatic inflammation appear to be the strongest contributors to an unfavorable short-term outcome.In this limited series,essentials of the AbSeS-classification,such as the setting of infection onset,diffuse peritonitis,and severity of disease expression,

The phase behaviour of lanthanide(Ⅲ) dodecylsulphates, Ln(C 12 H 25 SO 4) 3, by thermo optical microscopy using Lawrence penetration technique was investigated. The lyotropic phase behaviour of lanthanide(Ⅲ) dodecylsulphates in ethylene glycol water in mixtures hereof, depends on the composition of the solvent. For pure ethylene glycol and mixtures of ethylene glycol and water three different mesophases are formed, i.e. a lamellar, a cubic and a hexagonal phase, whereas when water is used as solvent no cubic phase is formed. The size of the lanthanide ion has no influence on the mesomorphism of these metallomesogens, although the smaller the lanthanide ion the lower the solubility.

Current study evaluated the effect of a fine and coarsely ground insoluble dietary fibre source on the gastrointestinal development of suckling pigs.Oat hulls(OH)were selected as a model feedstuff,rich in cellulose,lignin,and insoluble dietary fibre.Three experimental supplemental diets were formulated:a finely ground,low fibre and nutrient dense diet served as control(CON).For the 2 high fibre diets,15%heat-treated starch in CON was exchanged with OH,either finely(OH-f)or coarsely ground(OH-c).Litters of 10 primi-and multiparous sows(mean litter size 14.6±0.84)were used.Within a litter,experimental diets were allotted to triplets of 4 piglets.From approximately 12 d of age,piglets’individual feed intakes were recorded 2 times per day when separated from their dam for 70 min.Piglets could suckle with their dam for the remainder of the day.On d 24 and 25,from the total pool of 120 piglets,seven healthy well-eating piglets per treatment were selected for post-mortem evaluation,resulting in 14 replicates per treatment.Consumption of OH-c and OH-f did not impede clinical health and production performance of piglets.The full stomach weights tended to be greater for OH-c compared to OH-f whereas CON was intermediate(P=0.083).Supplementing OH significantly increased ileal villus height and caecal dry matter concentration(P<0.05).For the colon,OH increased its length,contents weight,short-chain fatty acid concentration and reduced total bacterial count as well asγ-proteobacteria count and proportion(P<0.05).The OH-c treatment specifically increased full gastrointestinal tract weight and caecum contents weight compared to piglets fed CON and OH-f.Furthermore,OH-c reduced colonic crypt depth when compared to OH-f(P=0.018).In conclusion,supplementing OH to a diet for suckling piglets exerted subtle developmental effects on gastrointestinal morphology and colonic microbial community.These effects were largely independent from the particle size of the OH.

For reservoir operation, maintaining a quasi-natural flow regime can benefit river ecosystems, but may sacrifice human interests. This study took the Qingshitan Reservoir in the Lijiang River as a case, and developed an optimization model to explore a trade-off solution between social-economic interests and nature flow maintenance on a monthly base. The objective function considered irrigation, cruise navigation and water supply aspects. An index of flow alteration degree was proposed to measure the difference between the regulated discharge and the natural flow. The index was then used as an additional constraint in the model besides the conventional constraints on reservoir safety. During model solving, different criteria were applied to the index, representing various degrees of alteration of the natural flow regime in the river. Through the model, a relationship between social-economic interests and flow alteration degree was established. Finally, a trade-off solution of the reservoir operation was defined that led to a favorable social-economic benefit at an acceptable alteration of the natural flow.
