BACKGROUND Primary hyperparathyroidism is typically caused by a single parathyroid *** parathyroid adenomas occur as well,with cases involving various sites,including the mediastinum,presenting in varying *** hyperparathyroidism develops in the context of chronic kidney disease,primarily due to vitamin D deficiency,hypocalcemia,and *** is frequently diagnosed in patients undergoing *** article presents a rare case of hyperparathyroidism involving multiple hyperplastic parathyroid glands with pulmonary seeding in a 50-year-old female patient undergoing hemodialysis(HD).CASE SUMMARy The patient had a history of parathyroidectomy 10 years prior but developed recurrent hyperparathyroidism with symptoms of pruritus and cough with sputum during a period of routine *** imaging revealed multiple nodules in both lungs,with the largest measuring approximately 1.35 *** histopathology confirmed the presence of hyperplastic parathyroid glands within the pulmonary *** tumor resection surgery via videoassisted thoracic surgery with wedge resection,the patient was discharged in stable condition and in follow-up her symptoms showed *** This article describes hyperparathyroidism presenting as pulmonary nodules in a patient undergoing postparathyroidectomy HD,highlighting diagnostic challenges and a positive outcome from tumor resection surgery.

Background:We clarified the predictive factors for changes in the status ofmedications for lower urinary tract symptoms(LUTS)2 years after local radiotherapy for nonmetastatic prostate *** and methods:We retrospectively included patients who underwent local external radiotherapy for nonmetastatic prostate cancer in 8 institutions between April 2001 and March *** were divided into themedication and no-medication group based on the use of drugs for LUTS before *** defined improvement of LUTS as when the patient did not require medication for LUTS at 24 months after radiotherapy in the medication group and as deterioration whenmedication was required in the no-medication *** regression analysis was used to evaluate predictive factors for changes in medication ***:Altogether,505 patients were divided into a no-medication group(n=352)and a medication group(n=153).The number of patients with deterioration and improvement in LUTS was 49(14%)and 36(23%),*** the multivariate analysis,the predictive variables for deterioration were the International Prostate SymptomScore(≥8;odds ratio[OR],2.21;p=0.014)and the biopsy Gleason score(≤3+4=7;OR,2.430;p=0.008)in the no-medication group,whereas those for improvement were age(years old;OR,5.81;p=0.002),the quality of life score(y core rate(≥50%;OR,2.530;p=0.027)in the medication ***:These predictive factors for changes in the status ofmedications for LUTS at 2 years after external radiotherapymay help determine the definitive therapy for nonmetastatic prostate cancer.

Influence of overhanging tool length and vibrator material on electromechanical impedance and amplitude prediction in ultrasonic spindle vibrator

This study presents the development of an ultrasonic transducer with a radius horn for an ultrasonic milling spindle(UMS)*** ultrasonic transducer was intended to have a working frequency of approximately 30 *** different materials were considered in the study:stainless steel(SS 316L)and titanium alloy(Ti-6Al-4V).Titanium alloy gave a higher resonance frequency(33 kHz)than stainless steel(30 kHz)under the same preload compression *** electromechanical impedance simulation was carried out to predict the impedance resonance frequency for both materials,and the effect of the overhanging toolbar was *** to the electromechanical impedance simulation,the overhanging toolbar length affected the resonance frequency,and the error was less than 3%.Harmonic analysis confirmed that the damping ratio helps determine the resonance ***,damping ratios of 0.015-0.020 and 0.005-0.020 were selected for stainless steel and titanium alloy,respectively,with an error of less than 1.5%.Experimental machining was also performed to assess the feasibility of ultrasonic-assisted milling;the result was a lesser cutting force and better surface topography of Al 6061.

Impact of steroid maintenance on the outcomes in firsttime deceased donor kidney transplant recipients: Analysis by induction type

AIM: To analyze the impact of steroid maintenance on the outcomes in kidney transplant recipients stratified by induction agent ***: Patients who underwent first-time deceased donor kidney transplantation between 2000 and 2008 after receiving induction therapy with rabbitantithymocyte globulin(r-ATG), alemtuzumab or an interleukin-2 receptor blocker(IL-2B) and discharged on a calcineurin inhibitor(CNI)/mycophenolate mofetil(MMF)-regimen along with or without steroids were identified from the Organ Procurement and Transplant Network/United Network of Organ Sharing *** each induction type, adjusted overall and deathcensored graft as well as patient survivals were compared between patients discharged on steroid vs no steroid. Among r-ATG induced patients, analysis was repeated after splitting the group into low and high immune risk ***: Among the 37217 patients included in the analysis, 17863 received r-ATG(steroid = 13001, nosteroid = 4862), 3028 alemtuzumab(steroid = 852, no-steroid = 2176) and 16326 IL-2B(steroid = 15008, no-steroid = 1318). Adjusted overall graft survival was inferior(HR = 1.16, 95%CI: 1.06-1.27, P = 0.002) with similar death-censored graft survival(HR = 0.99, 95%CI: 0.86-1.14, P = 0.86) for steroid vs no-steroid groups in r-ATG induced patients. Both adjusted overall and death-censored graft survivals for steroid vs nosteroid groups were similar in alemtuzumab(HR = 0.92, 95%CI: 0.73-1.15, P = 0.47 and HR = 0.87, 95%CI: 0.62-1.22, P = 0.43 respectively) and IL-2B(HR = 1.05, 95%CI: 0.91-1.21, P = 0.48 and HR = 0.94, 95%CI: 0.75-1.18, P = 0.60 respectively) induced groups. Adjusted patient survivals were inferior for steroid vs nosteroid groups in r-ATG induced(HR = 1.31, 95%CI: 1.15-1.49, P < 0.001) but similar in alemtuzumab(HR = 1.02, 95%CI: 0.75-1.38, P = 0.92) and IL-2B(HR = 1.17, 95%CI: 0.97-1.40, P = 0.10) induced patients. Influence of Production Factors on the Physico-Chemical Characteristics of Fermented Cassava Dough and Sensory Evaluation of Attieke

Attieke is an Ivorian semolina which obtained by fermenting, pressing and steaming cassava dough. Attieke production remains a traditional activity carried out by less literate women. However, perceived differences in measurable factors and attieke qualities require an investigation of their influence on the characteristics of the pressed dough and attieke. The aim of this study is to improve the quality of the dough in relation to that of the attieke produced. The experiment was carried out on 4 production factors, namely the type of boiled or braised ferment, the incorporation rate of the ferment between 8 and 10%, the addition of oil from 0.1 to 1% and the fermentation time from 12 to 15 hours applied to the Improved African Cassava (IAC) variety. A complete experiment design of 16 samples of fermented dough and attieke was employed. These samples underwent physic-chemical analyses for the fermented dough and sensory evaluation for the attieke. It was found that, except for titratable acidity, reducing sugar content and ash content, the physico-chemical characteristics of the dough of IAC variety were significantly influenced by all production factors and their interaction. Fermentation time significantly influences 60% of the physico-chemical characteristics of the fermented dough. The type of ferment, the oil addition and the ferment rate have a significant influence at 40% of these characteristics. At the sensory level, color, acidity and grain binding with an explained variance of 34.60% were essential for the appreciation of the attieke samples. Thus, these production factors could be considered for the improvement of the fermented dough and attieke production process.

Chronic Subdural Hematoma Associated with an Arachnoid Cyst in Elderly, an Intraoperative Finding after Re-Bleeding: Case Report

Intracranial arachnoid cysts (AC) are believed to be congenital and chronic subdural hematomas tend to occur in elderly patients with a history of mild head injury. The association between these two entities sporadically occur in relatively young patients but rare in elderly patients. We report a 65-year-old man who presented with headache and dizziness of 2 months’ duration with a history of head injury. Brain computed tomography (CT) a CSDH in right side and a hygroma in left side. After first operation with burr holes in both sides, the patient underwent an early recurrence of acute subdural hematoma in the right side. The evacuation of this hematoma by a craniotomy allowed seeing an AC that we resected partially with complete recovery of the patient. The association CSDH/AC is rare and possible in elderly patients and there is no consensus on treatment.

BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal *** IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial *** SUMMARy A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized *** initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral *** the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower *** evaluation revealed findings consistent with acute motor sensory *** initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor *** the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal ***,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening *** IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.
