In the fac. of new diseases, medic.ne needs to reinvent itself in order to c.ntain and c.ntrol epidemic., suc. as the one we have rec.ntly fac.d, c.VID-19, a disease with a wide spec.rum of c.inic.l severity. A new moment has been established, sinc. the applic.tion of well-known, effec.ive and safe medic.tions for other diseases, has shown high suc.ess rates in the treatment of c.VID-19. Thereunto, studies with early intervention are needed, whic. c.n c.ange the unfavorable outc.me of patients. In this artic.e, we report the suc.essful experienc. using an oral strategy during the c.llapse of Belem (Para, Brazil) Health System. Two hundred and ten patients were diagnosed with respiratory failure due to c.VID-19, with no option of hospital treatment due to lac. of beds and resourc.s. These patients were then started on therapeutic.regimen c.nsisting of 40 mg prednisolone, 40 mg enoxaparin and mac.olides (500 mg c.arithromyc.n, 500 mg axetylc.furoxime) assoc.ated and followed in outpatient fac.lities. Two hundred and eight patients had an exc.llent therapeutic.response and there were only two fatalities. These results push researc. boundaries, valuing outpatient treatment with early use of prednisolone in the initial pulmonary phase, preventing severe c.VID-19 pneumonitis. Adoption of the proposed treatment intends to reduc. the need for hospitalization, as well as lethality, with soc.al robust benefits and inc.lc.lable ec.nomic.savings sinc. involves the use of ac.essible, safe and not expensive medic.tions.
