BACKGROUND Somatostatin analogues are an established first-line therapy for well differentiated small bowel neuroendocrine tumours(Wd-SBNETs),while and peptide receptor radionuclide therapy(PRRT)is frequently used as a second-line *** treatment selection of third-line treatment remains challenging due to the limited prospective data currently available on the best therapeutic *** To understand current practice and rationale for decision-making by physicians in the 3rd-line setting by building an online *** Weighted average(WA)of likelihood of usage between responders(1 very unlikely;4 very likely)was used to reflect the relevance of factors *** Replies from representatives of 28 centers were received(5/8/2020-21/9/2020);medical oncologist(53.6%),gastroenterologist(17.9%);United Kingdom(21.4%),Spain(17.9%),Italy(14.3%).Majority from European Neuroendocrine Tumor Society(ENETS)Centres of Excellence(57.1%),who followed ENETS guidelines(82.1%).Generally speaking,3rd-line treatment for Wd-SBNETs was:everolimus(EVE)(66.7%),PRRT(18.5%),liver embolization(LE)(7.4%)and interferon-alpha(IFN)(3.7%);chemotherapy(0%);decision was based on clinical trial data(59.3%),or personal experience(22.2%).EVE was most likely used if Ki-67<10%(WA 3.27/4)or age<70 years(WA 3.23/4),in the 3rd-line setting(WA 3.23/4);regardless of presence/absence of carcinoid syndrome(CS),rate of progression or extent of *** was mainly utilised only if rapid progression(within 6 mo)(WA 3.35/4),Ki-6710%-20%(WA 2.77/4),negative somatostatin receptor imaging(WA 2.65/4)or high tumour burden(WA 2.77/4);temozolomide or streptozocin was used with capecitabine or 5-fluorouracil(5-FU)(57.7%),FOLFOX(5-FU combined with oxaliplatin)(23.1%).LE was selected if presence of CS(WA 3.24/4)or Ki-67<10%(WA 2.8/4),after progression to other treatments(WA 2.8/4).IFN was rarely used(WA 1.3/4).CONCLUSION Everolimus was the most frequently used therapeutic option in the t

Awareness of the adverse impact of air pollution on attention-related performance such as learning and driving is rapidly ***,there is still little known about the underlying neurocognitive *** an adapted dot-probe task paradigm and event-related potential(ERP)technique,we investigated how visual stimuli of air pollution influence the attentional allocation *** were required to make responses to the onset of a target presented at the left or right visual *** probable location of the target was forewarned by a cue(pollution or clean air images),appearing at either the target location(attention-holding trials)or the opposite location(attention-shifting trials).Behavioral measures showed that when cued by pollution images,subjects had higher response accuracy in attention-shifting *** analysis results revealed that after the cue onset,pollution images evoked lower N300 amplitudes,indicating less attentioncapturing effects of dirty *** the target onset,pollution cues were correlated with the higher P300 amplitudes in attention-holding trials but lower amplitudes in attention-shifting *** indicates that after visual exposure to air pollution,people need more neurocognitive resources to maintain attention but less effort to shift attention *** findings provide the first neuroscientific evidence for the distracting effect of air *** conclude with several practical implications and suggest the ERP technique as a promising tool to understand human responses to environmental stressors.

In digital soil mapping(DSM),a fundamental assumption is that the spatial variability of the target variable can be explained by the predictors or environmental *** to adequately sample the predictors have been well documented,with the conditioned Latin hypercube sampling(cLHS)algorithm receiving the most attention in the DSM *** advances in sampling design,a critical gap remains in determining the number of samples required for DSM *** propose a simple workflow and function coded in R language to determine the minimum sample size for the cLHS algorithm based on histograms of the predictor variables using the Freedman-Diaconis rule for determining optimal bin *** preprocessing was included to correct for multimodal and non-normally distributed data,as these can affect sample size determination from the *** on a user-selected quantile range(QR)for the sample plan,the densities of the histogram bins at the upper and lower bounds of the QR were used as a scaling factor to determine minimum sample *** technique was applied to a field-scale set of environmental covariates for a well-sampled agricultural study site near Guelph,Ontario,Canada,and tested across a range of *** results showed increasing minimum sample size with an increase in the QR *** sample size increased from 44 to 83 when the QR increased from 50% to 95% and then increased exponentially to 194 for the 99%*** technique provides an estimate of minimum sample size that can be used as an input to the cLHS algorithm.

功率谱密度被广泛用于表征路面不平度,进而用于评估路面振动环境下武器装备系统(如火炮、战车)的可靠性和疲劳寿命,其隐含的假设条件是路面不平度服从高斯分布。然而,实测路面不平度常常具有非高斯特性,导致武器装备系统更快地发生疲劳失效。为提高评估精度,提出一种基于实测国际粗糙度指数的非高斯路谱建模方法。首先由ISO标准功率谱密度生成高斯路谱,然后由实测国际粗糙度指数生成具有相同统计特性的幅值调制信号,最后利用幅值调制法生成非高斯路谱。案例分析表明,直接采用高斯模型描述路面不平度会导致欠试验;而利用提出的方法可得到与实测IRI相同的统计特性,可有效提高路谱激励下武器装备系统的可靠性和耐久性评估精度。

Due to a lack of resources, rural communities often face challenges when planning catastrophic events. This project involved applying systems thinking and model-based systems engineering to develop a proof-of-concept, multi-method computer simulation and then determining whether the simulation could be used to assess the efficacy of disaster planning approaches on health outcomes in rural communities, as a function of primary healthcare. The project focus was a rural or non-urban healthcare system experiencing a natural hazard. Both system dynamics and discrete event models were incorporated to represent subsystem operations, crucial disaster responses, as well as three key response systems: public health, emergency management, and healthcare. The subsystem models included several components: policies/procedures, communications,resources, exercises/drills/training, healthcare space and staff, and the flow of affected people into and through the *** combined simulation can serve as a first step to a more comprehensive approach to helping rural communities achieve more efficient and effective healthcare planning for disaster responses.

A total hysterectomy is a common *** to published data,in the European Union,laparoscopic and laparoscopicassisted hysterectomies are performed more frequently than abdominal hysterectomies.A newtechnique in endoscopic surgery is the natural orifice approach-vaginal natural orifice transluminal endoscopic surgery(vNOTES).Performing a hysterectomy using the vNOTES method hasa numberofadvantages compared with the traditional one,including a decrease in the intensity of pain in the early postoperative period,the absence of a cosmeticdefect on the anterior abdominal wall and improved visualisation,which reduces the risk of intraoperative complications.
