Myocardial metastasis from ZEB1-and TWIST-positive spindle cell carcinoma of the esophagus:A case report

BACKGROUND Metastatic cardiac tumors are known to occur more frequently than primary cardiac tumors,however,they often remain asymptomatic and are commonly dis-covered on *** tumors with a relatively high frequency of cardiac metastasis include mesothelioma,melanoma,lung cancer,and breast cancer,whereas reports of esophageal cancer with cardiac metastasis are *** SUMMARY The case of a 60-year-old man who complained of dysphagia is *** gastrointestinal endoscopy showed a submucosal tumor-like elevated lesion in the esophagus causing ***-enhanced computed tomography showed left atrial compression due to the esophageal tumor,multiple liver and lung metastases,and a left pleural *** examination of a biopsy speci-men from the esophageal tumor showed spindle-shaped cells,raising suspicion of esophageal *** disease progressed rapidly,and systemic chemotherapy was deemed necessary,however,due to his poor general condition,adminis-tration of cytotoxic agents was considered *** his high Combined Positive Score,nivolumab was administered,however,the patient soon died from the *** autopsy confirmed spindle cell carcinoma(SCC)of the esophagus and cardiac metastasis with similar histological *** stem cell markers,ZEB1 and TWIST,were positive in both the primary tumor and the cardiac *** To the best of our knowledge,there have been no prior reports of cardiac metastasis of esophageal *** case highlights our experience with a patient with esophageal SCC who progressed rapidly and died from the disease,with the autopsy examination showing cardiac metastasis.

Hyper Catastrophe on 4-Dimensional Canards

When discovering the potential of canards flying in 4-dimensional slow-fast system with a bifurcation parameter, the key notion “symmetry” plays an important role. It is of one parameter on slow vector field. Then, it should be determined to introduce parameters to all slow/fast vectors. It is, however, there might be no way to explore for another potential in this system, because the geometrical structure is quite different from the system with one parameter. Even in this system, the “symmetry” is also useful to obtain the potentials classified by R. Thom. In this paper, via the coordinates changing, the possible way to explore for the potential will be shown. As it is analyzed on “hyper finite time line”, or done by using “non-standard analysis”, it is called “Hyper Catastrophe”. In the slow-fast system which includes a very small parameter , it is difficult to do precise analysis. Thus, it is useful to get the orbits as a singular limit. When trying to do simulations, it is also faced with difficulty due to singularity. Using very small time intervals corresponding small , we shall overcome the difficulty, because the difference equation on the small time interval adopts the standard differential equation. These small intervals are defined on hyper finite number N, which is nonstandard. As and the intervals are linked to use 1/N, the simulation should be done exactly.

A Case of Transient Advanced Atrioventricular Block after Aortic Valve Replacement, Report of a Case

Approximately 3%-11.8% of cases require permanent pacemaker implantation due to atrioventricular block (AVB) after aortic valve replacement (AVR), and determination of conduction disturbances such as left or right bundle branch block by preoperative electrocardiography is correlated with high risk postoperative permanent pacemaker implantation. Intraoperative risk factors include severe calcification of the aortic valve, prolonged cardiopulmonary bypass time, aortic clamp time. Recently, there have been reports of high rates of pacemaker implantation (14.2%) after transcatheter aortic valve implantation. The time of permanent pacemaker implantation after AVB is often 4-10 days, and the European Society of Cardiology guidelines recommend a period of seven days of persistent atrioventricular block postsurgery prior to permanent pacemaker implantation. We report a 79-year-old woman in which the patient developed high-degree AVB after AVR was performed for severe aortic stenosis with complete right bundle branch block. However, her pulse returned to sinus rhythm 7 days postsurgery.

Model tests and corresponding numerical simulations on cave model subjected to thermo-mechanical loading

Geological repository of high-level nuclear waste(HLNW),the most feasible approach for the safe and permanent treatment of HLNW without human intervention,has been investigated for years by many *** some countries,real facilities for the geo-logical repository are already under *** Japan,however,due to complicated geologic conditions,especially underground water and fractured rock masses,and the high risk of natural disasters,ensuring the long-term stability of the method remains a *** influential factors include underground water,heat generation from radioactive waste,and thermal and chemical weathering of the surrounding rock mass as a natural *** is difficult to estimate and verify the long-term stability for up to one hundred thousand years,a complicated thermal-hydraulic-mechanical-chemical coupling behavior,via any field *** objective of the study is to develop a numerical method for predicting the long-term stability of geological *** the first step toward realizing this objec-tive,heating and loading tests on cave model made of man-made rock specimens that are composed of diatoms,gypsum,and water,which are viscoplastic materials,were conducted,based on which a newly proposed numerical method with finite element method(FEM)was used to describe the thermal,mechanical,and time-dependent behavior of the model tests for a geological *** ensure the accuracy of the numerical calculations,all the material parameters in the thermoelasto-viscoplastic model with consideration of overconsolidation,the structure,and the influence of intermediate stress were determined via triaxial compression/creep tests under various temperatures,confining stresses,and loading ***,the validity of the numerical method was demonstrated by model tests over a limited time span.

Let us consider higher dimensional canards in a sow-fast system R2+2 with a bifurcation parameter. Then, the slow manifold sometimes shows various aspects due to the bifurcation. Introducing a key notion “symmetry” to the slow-fast system, it becomes clear when the pseudo singular point obtains the structural stability or not. It should be treated with a general case. Then, it will also be given about the sufficient conditions for the existence of the center manifold under being “symmetry”. The higher dimensional canards in the sow-fast system are deeply related to Hilbert’s 16th problem. Furthermore, computer simulations are done for the systems having Brownian motions. As a result, the rigidity for the system is confirmed.

Canards Flying on Bifurcation

There exists a property “structural stability” for “4-dimensional canards” which is a singular-limit solution in a slow-fast system with a bifurcation parameter. It means that the system includes the possibility to have some critical values on the bifurcation parameter. Corresponding to these values, the pseudo-singular point, which is a singular point in the time-scaled-reduced system should be changed to another one. Then, the canards may fly to another pseudo-singular point, if possible. Can the canards fly? The structural stability gives the possibility for the canards flying. The precise reasons why happen are described in this paper.

In this study,a thermo-elasto-viscoplastic model for soft rock is proposed to describe its fundamental mechanical behavior of soft rock such as the influences of overconsolidation,intermediate principal stress,temperature,time dependency and inherent structure with a unified set of *** the model,the concepts of subloading and superloading yielding surfaces are introduced to describe the influence of overconsolidation and structure on the deformation and strength of soft *** influence of the intermediate principal stress is also properly considered by adopting a transformed stress tensor t_(ij).To consider the temperature effect,a thermo-induced equivalent stress is introduced to consider its influence on the yielding surfaces,the evolution of the overconsolidation,and the structure when subjected to shearing loading under different *** comparing the calculated results with test results of a manmade rock under different loading and temperature conditions,the availability and accuracy of the proposed model are carefully investigated,and the performance of the proposed model is verified in detail.

Usefulness of bispectral monitoring of conscious sedation during endoscopic mucosal dissection

AIM: To assess the usefulness of bispectral index (BIS) monitoring in order to carry out endoscopic submucosal dissection (ESD) safely and with patients' satisfaction. METHODS: Three hundred sixty-six patients with an early-stage neoplasm of the digestive tract were enrolled. The BIS monitor (A-1050: Aspect medical systems/NIHON KOHDEN, Tokyo, Japan) was used. The appropriate sedative condition was set at 55 to 75 BIS levels (BIS value) during the endoscopic procedures. RESULTS: Among 366 cases, 13 were accompanied by adverse events during and/or after ESD. All episodes occurred in cases with BIS value between 56 and 65. Hypotension was observed in four cases, and bradycardia in six. Respiratory distress was observed in two cases with chronic pulmonary obstructive disease. All patients with adverse events were able to leave the hospital without extension of the hospitalization. CONCLUSION: BIS monitoring is useful to safely perform ESD. A BIS value of 70 to 75 is suitable for ESD.

On the Two Methods for Finding 4-Dimensional Duck Solutions

This paper gives the existence of a duck solution in a slow-fast system in R2+2 using two ways. One is an indirect way and the other is a direct way. In the indirect way, the original system is once reduced to the slow-fast system in R2+1. In the direct one, it has a 4-dimensional duck solution when having an efficient local model. This is already published in [1,2]. Some sufficient conditions are given to get such a good model.

In the present work,bimetallic silver/zinc was applied into the degradation of trihalomethanes,THMs:CHCl_(3),CHBrCl_(2),CHBr_(2)Cl,and CHBr_(3).The kinetics reaction rates and removal efficiencies of the THM compound mixtures,in the aqueous solutions,were *** experiments were conducted under mild conditions,ambient temperature,and *** primary degradation reaction followed a pseudo-first-order kinetic *** first-order rate constants and the degradation efficiencies followed the decreasing order of CHBr_(3)>CHBr_(2)Cl>CHBrCl_(2)>CHCl_(3).The bond dissociation energy and hydrophilic/hydrophobic characteristics of the THM compounds may become the most important parameters affecting the degradation kinetics and efficiency by bimetallic Ag/Zn.
