A 20-Year Follow-up after the Fontan Operation in a Population with Hypoplastic Left Heart Syndrome

Background:Thromboembolic events are a cause of significant morbidity and mortality in the Fontan population.We previously reported on coagulation profile changes in a cohort of patients with hypoplastic left heart syndrome(HLHS)from Stage I through Fontan completion.In this report,we examine their clinical status,anticoagulation and incidence of thromboembolic events up to 20 years post Fontan.Methods:A retrospective chart review was conducted for twenty(20)surviving patients,from 1998 through December 2020.Patients who underwent orthotopic heart transplantation(OTx)were followed until their transplant.Patients who were found in the original study to have a factor VIII activity level>160%,were examined separately.Results:Most patients had follow-up within the last two years(2018–2020).Two patients underwent OTx and two patients died.Anticoagulation strategy was variable.Most patients were on aspirin monotherapy.There was a total of twelve thrombotic events(63.2%).These included six cerebrovascular accidents(two of which were fatal).Three out of the seven patients with elevated factor VIII activity from the original study had thromboembolic events(42.9%).Fontan complications were variable.Some degree of Fontan Associated Liver Disease was universal.conclusions:This retrospective review of a group of single-ventricle patients post Fontan,illustrates the variability in anticoagulation therapy that exists in this population.A large proportion of patients suffered a significant thromboembolic event,including the patients with elevated factor VIII.Further investigation into the patients with elevated factor VIII may help determine whether a different antithrombotic strategy post Fontan would be beneficial.

Background Recent infection with SARS‑coV-2 in children has been associated with multisystem inflammatory syndrome in children(MIS-c).SARS‑coV-2 has undergone different mutations.Few publications exist about specific variants and their correlation with the severity of MIS-c.Methods This was a single-center,retrospective study including all patients admitted with MIS-c at Rady children’s Hospital-San Diego between May 2020 and March 2022.Local epidemiologic data,including viral genomic information,were obtained from public records.Demographics,clinical presentation,laboratory values,and outcomes were obtained from electronic medical records.Results The analysis included 104 pediatric patients.Four MIS-c waves were identified.circulating variants in San Diego during the first wave included clades 20A to c.During the second wave,there were variants from clades 20A to c,20G,21c(Epsilon),20I(Alpha),and 20J(Gamma).The third wave had Delta strains(clades 21A,21I,and 21J),and the fourth had Omicron variants(clades 21K,21L,and 22c).MIS-c presented with similar symptoms and laboratory findings across all waves.More patients were admitted to the pediatric intensive care unit(PIcU)(74%)and required inotropic support(63%)during the second wave.None of the patients required mechanical circulatory support,and only two required invasive ventilatory support.There was no mortality.conclusions The various strains of SARS-coV-2 triggered MIS-c with differing severities,with the second wave having a more severe clinical course.Whether the differences in disease severity across variants were due to changes in the virus or other factors remains unknown.

Purpose:The study aimed to examine the reporting completeness of randomized controlled trials(RcTs)of non-pharmacological interventions following concussion.Methods:We searched MEDLINE,Embase,PsycInfo,cINAHL,and Web of Science up to May 2022.Two reviewers independently screened studies and assessed reporting completeness using the Template for Intervention Description and Replication(TIDieR),consensus on Exercise Reporting Template(cERT),and international consensus on Therapeutic Exercise aNd Training(i-cONTENT)checklists.Additional information was sought my study authors where reporting was incomplete.Risk of bias(ROB)was assessed with the cochrane ROB-2 Tool.RcTs examining non-pharmacological interventions following concussion.Results:We included 89 RcTs(n=53 high ROB)examining 11 different interventions for concussion:sub-symptom threshold aerobic exercise,cervicovestibular therapy,physical/cognitive rest,vision therapy,education,psychotherapy,hyperbaric oxygen therapy,transcranial magnetic stimulation,blue light therapy,osteopathic manipulation,and head/neck cooling.Median scores were:TIDieR 9/12(75%;interquartile range(IQR)=5;range:5-12),cERT 17/19(89%;IQR=2;range:10-19),and i-cONTENT 6/7(86%;IQR=1;range:5-7).Percentage of studies completely reporting all items was TIDieR 35%(31/89),cERT 24%(5/21),and i-cONTENT 10%(2/21).Studies were more completely reported after publication of TIDieR(t_(87)=2.08;p=0.04)and cERT(t_(19)=2.72;p=0.01).Reporting completeness was not strongly associated with journal impact factor(TIDieR:rs=0.27;p=0.01;cERT:r_(s)=-0.44;p=0.06;i-cONTENT:r_(s)=-0.17;p=0.48)or ROB(TIDieR:rs=0.11;p=0.31;cERT:rs=0.04;p=0.86;i-cONTENT:rs=0.12;p=0.60).conclusion:RcTs of non-pharmacological interventions following concussion demonstrate moderate to good reporting completeness,but are often missing key components,particularly modifications,motivational strategies,and qualified supervisor.Reporting completeness improved after TIDieR and cERT publication,but publication in highly cited journals and low ROB

The last 20 years have produced developments in the treatment for patients with non-obstructive azoospermia(NOA)who were once considered to be infertile.The combination of intracytoplasmic sperm injection together with various testicular sperm retrieval techniques,including conventional testicular sperm extraction(TESE),microdissection TESE(micro-TESE)and fine needle aspiration(FNA),have revolutionized treatment for these men.In men with NOA,isolated regions of spermatogenesis within the testis are common.The goal for all types of sperm retrieval procedures is locating the focal region(s)of spermatogenesis,and harvesting the sperm for assisted reproduction.This review article explores the surgical management of men with NOA and describes all techniques that can be used for testicular sperm retrieval.A PubMed search was conducted using the key words:“sperm extraction”,“NOA”,“testicular FNA”,“testicular mapping”,“TESE”,and“testicular biopsy”.All articles were reviewed.Articles were included if they provided data on sperm retrieval rates.The methods for performing sperm retrieval rates and outcomes of the various techniques are outlined.Micro-TESE has a higher sperm retrieval rates with fewer postoperative complications and negative effects on testicular function compared with conventional TESE.

Wingless/Int (Wnt) signaling pathways are signal transduction mechanisms that have been widely studied In the field of embryogen- esis. Recent work has established a critical role for these pathways in brain development, especially of midbrain dopaminergic neu- rones, However, the fundamental importance of Wnt signaling for the normal function of mature neurones in the adult central nervous system has also lately been demonstrated by an increasing number of studies. Parkinson's disease (PD) is the second most prevalent neurodegenerative disease worldwide and is currently incurable. This debilitating disease is characterized by the progres- sive loss of a subset of midbrain dopaminergic neurones in the substontla nigm leadingto typical extrapyramidal motor symptoms. The aetiology of PD is poorly understood but work performed over the Last two decades has identified a growing number of genetic defects that underlie this condition. Herewe review a growing body of data connecting genes implicated in PD-most notablythe PARKgenes- with Wnt signaling. These observations provide clues to the normal function of these proteins in healthy neurones and suggest that deregulated Wnt signaling might be a frequent pathomechanlsm leading to PD. These observations have implications for the patho- genesis and treatment of neurodegenerative diseases in general.

Landscape and scale-dependent spatial niches of bats foraging above intensively used arable fields

Introduction:Bats are threatened by agricultural intensification,and although bat ecology in agricultural landscapes is in the focus of current research,the effects of interacting spatiotemporal factors on species-specific bat activity above farmland remain understudied.Our aim was to identify spatiotemporal factors and their interactions relevant for the activity of bat species above conventionally managed arable fields.Methods:We repeatedly monitored relative bat activity above open arable fields in Germany using acoustic monitoring.We used site-related biotic and abiotic factors and landscape characteristics across five spatial scales,their combinations,and interactions to identify those factors which best explain variation in bat activity.Results:Numerous interactions between landscape characteristics and the insect abundance affected bat activity above fields.For instance,Pipistrellus pipistrellus became more active with increasing insect abundance,but only above fields with a low proportion of woody vegetation cover in the surroundings.Additionally,the level of bat activity in summer depended on landscape characteristics.For example,the activity of Pipistrellus nathusii was relatively low in summer above fields that were surrounded by vegetation patches with a high degree of edge complexity(e.g.,hedgerow).However,the activity remained at a relatively high level and did not differ between seasons above fields that were surrounded by vegetation patches with a low degree of edge complexity(e.g.,roundly shaped forest patch).conclusions:Our results revealed that landscape characteristics and their interactions with insect abundance affected bat activity above conventionally managed fields and highlighted the opportunistic foraging behavior of bats.To improve the conditions for bats in agricultural landscapes,we recommend re-establishing landscape heterogeneity to protect aquatic habitats and to increase arthropod availability.

carbon Dioxide and Nitrous Oxide Emissions from Naturally Occurring Sulfate-Based Saline Soils at Different Moisture contents

Soil salinization may negatively affect microbial processes related to carbon dioxide(cO_2) and nitrous oxide(N_2O) emissions. A short-term laboratory incubation experiment was conducted to investigate the effects of soil electrical conductivity(Ec) and moisture content on cO_2 and N_2O emissions from sulfate-based natural saline soils. Three separate 100-m long transects were established along the salinity gradient on a salt-affected agricultural field at Mooreton, North Dakota, USA. Surface soils were collected from four equally spaced sampling positions within each transect, at the depths of 0–15 and 15–30 cm. In the laboratory, artificial soil cores were formed combining soils from both the depths in each transect, and incubated at 60% and 90% water-filled pore space(WFPS) at 25?c. The measured depth-weighted Ec of the saturated paste extract(Ec_e) across the sampling positions ranged from 0.43 to 4.65 dS m^(-1). Potential nitrogen(N) mineralization rate and cO_2 emissions decreased with increasing soil Ec_e, but the relative decline in soil cO_2 emissions with increasing Ece was smaller at 60% WFPS than at 90% WFPS. At 60% WFPS, soil N_2O emissions decreased from 133 μg N_2O-N kg^(-1) soil at Ec_e c_e = 4.65 dS m^(-1). In contrast, at 90% WFPS,soil N_2O emissions increased from 262 μg N_2O-N kg^(-1) soil at Ec_e = 0.81 dS m^(-1) to 849 μg N_2O-N kg^(-1) soil at Ec_e = 4.65 dS m^(-1), suggesting that N_2O emissions were linked to both soil Ece and moisture content. Therefore, spatial variability in soil Ec_e and pattern of rainfall over the season need to be considered when up-scaling N_2O and cO_2 emissions from field to landscape scales.

The translocon on the outer membrane of mitochondria （TOM） facilitates the import of nuclear-encoded proteins. The principal machinery of mitochondrial protein transport seems conserved in eukaryotes; however, divergence in the composition and structure of TOM components has been observed between mammals, yeast, and plants. TOM9, the plant homolog of yeast Tom22, is significantly smaller due to a truncation in the cytosolic receptor domain, and its precise function is not understood. Here we provide evidence showing that TOM9.2 from Arabidopsis thaliana is involved in the formation of mature TOM com- plex, most likely by influencing the assembly of the pore-forming subunit TOM40. Dexamethasone- induced RNAi gene silencing of TOM9.2 results in a severe reduction in the mature TOM complex, and the assembly of newly imported TOM40 into the complex is impaired. Nevertheless, mutant plants are fully viable and no obvious downstream effects of the loss of TOM complex, i.e., on mitochondrial import ca- pacity, were observed. Furthermore, we found that TOM9.2 can bind calmodulin （cAM） in vitro and that caM impairs the assembly of TOM complex in the isolated wild-type mitochondria, suggesting a regula- tory role of TOM9.2 and a possible integration of TOM assembly into the cellular calcium signaling network.

Gender gap in acute coronary heart disease:Myth or reality?

AIM: To investigate potential gender differences in the prevalence of cardiovascular risk factors, cardiovascular disease (cVD) management, and prognosis in acute coronary syndrome (AcS). METHODS: A systematic literature search was performed through Medline using pre-specified key words . An additional search was performed, focusing specifically on randomized controlled clinical trials in relation to therapeutic intervention and prognosis. In total, 92 relevant articles were found. RESULTS: Women with cVD tended to have more hypertension and diabetes at the time of presentation, whereas men were more likely to smoke. coronary angiography and revascularization by percutaneous coronary intervention were performed more often in men. Women were at a greater risk of short-term mortality and complications after revascularization. Interestingly, women under 40 years presenting with AcS were athighest risk of cardiovascular death compared with men of the same age, irrespective of risk factors. This disadvantage disappeared in older age. The long-term mortality risk of AcS was similar in men and women, and even in favor of women. cONcLUSION: Mortality rates are higher among young women with AcS, but this difference tends to disappear with age, and long-term prognosis is even better among older women.

Heat inactivation of foot-and-mouth disease virus,swine vesicular disease virus and classical swine fever virus when air-dried on plastic and glass surfaces

Foot-and-mouth disease virus(FMDV),swine vesicular disease virus(SVDV)and classical swine fever virus(cSFV)all cause important animal diseases.FMDV affects many different cloven-hoofed animals,whereas SVDV and cSFV are restricted to domestic and feral pigs together with wild boar.Europe is normally free of these diseases,but occasionally outbreaks happen,which can cause huge economic losses.Handling of these viruses,in particular FMDV and cSFV,is only allowed within high containment laboratories and stables.Periodically such facilities need to be decommissioned for repair or closing down,which is done by cleaning and chemical disinfection,followed by fumigation due to residual risk from virus on surfaces in inaccessible places.However,building materials in older laboratories or stables that have housed infected animals may not be well-suited for fumigation.Heat treatment is another way of inactivating viruses.In this study,we have determined the survival of infectivity in air-dried virus samples on glass and plastic surfaces incubated at room temperature or heated to 70°c for 1,2,3,5 and 7 days.Each of the tested viruses was inactivated to below the limit of detection after 24 h or 48 h of incubation at 70°c;in contrast,some of these viruses were still infectious after 7 days of incubation at room temperature.This study provides important information that can be used in relation to decontamination of buildings and in risk-assessments.
