Endoscopic and histopathological study on the duodenum of Strongyloides stercoralis hyperinfection

AIM:to investigate endoscopic and histopathological findings in the duodenum of patients with Strongyloides stercoralis(***)hyperinfection. MEtHODS:Over a period of 23 years(1984-2006),we investigated 25 patients *** hyperinfection who had had an esophagogastroduodenoscopy before undergoing treatment for *** clinical and endoscopic findings were analyzed retrospectively. RESULtS:twenty-four(96%)of the patients investigated were under immunocompromised condition which was mainly due to a human t lymphotropic virus type 1(HtLV-1)*** abnormal endoscopic findings,mainly edematous mucosa,white villi and erythematous mucosa,were observed in 23(92%) *** degree of duodenitis including villous atrophy/destruction and inflammatory cell infiltration corresponded to the severity of the endoscopic findings. the histopathologic yield for identifying larvae was 71.4% by duodenal *** endoscopic findings of duodenitis were more severe in patients whose biopsies were positive for larvae than those whose biopsies were negative(Endoscopic severity score:4.86±2.47vs 2.71 ±1.38,Ptudy clearly demonstrates that,in addition to stool analysis,endoscopic observation and biopsies are very *** also emphasize that *** and HtLV-1 infections should be ruled out before immunosuppressive therapy is administered in endemic regions.

A primary aortoenteric fistula (PAEF), defined as a communication between the native aorta and the gastrointestinal tract, is a rare cause of gastrointes-tinal bleeding. the preoperative diagnosis of PAEF is extremely difficult. Consequently, PAEF may cause sudden and unexpected death. We present an autopsy case of a 68-year-old man who died of massive gastro-intestinal bleeding due to a PAEF. Autopsy revealed a pinhole rupture located on the third part of the duode-nal mucosa and fistulized into the adjacent abdominal aortic aneurysm (AAA). Our case indicates that the aortoenteric fistula can result in fatal gastrointestinal bleeding. Consequently, a PAEF should be included in the differential diagnosis of gastrointestinal bleeding.

Enteropathy-associated t-cell lymphoma （EAtL） is a rare peripheral t-cell lymphoma classified into 2 types, with or without celiac disease, based on histology. type 2 EAtL is less commonly associated with celiac disease, in which cells are characterized by being monomorphic and small- to medium-sized. Cells are characterized by CD8 and CD56 expression and c-MYC oncogene locus gain. We present an atypical case of type 2 EAtL in the jejunum, with human t-lymphotropic virus-1 that was CD4- CDS＋ CD56- CD30- CD25- tIA-I＋ and granzyme B＋ on immunohistological staining. It also displayed translocation of chromosome 8p24 （c-MYC）, as de- termined by fluorescent/n situ hybridization. Mucosalspreading and intraepithelial invasion by lymphoma with villous atrophy were detected adjacent to the mucosal layer. the lymphoma may be derived from in- traepithelial CD8＋ t cells, similar to celiac disease.

Multiple lymphomatous polyposis (MLP) is an unusual form of non-Hodgkin's lymphoma characterized by polyps throughout the gastrointestinal tract. It has been reported that most MLP are observed in cases with mantle cell lymphoma of B-cell type. We herein present a case of a 66-year-old man with adult t-cell leukemia/lymphoma (AtLL). Colonoscopy revealed MLP throughout the colon and histopathological findings of AtLL cell infiltration. the patient died despite combination of chemotherapy. the literature of manifestations of colonic involvement of AtLL isreviewed and the importance of endoscopic evaluation to differentiate AtLL intestinal lesions from opportunistic infectious enterocolitis is discussed.

Immunoglobulin (Ig) M production can be induced by the interaction of thymus-independent type-2 (tI-2) antigen (Ag) with B cell Ag receptors (BCRs) without the involvement of conventional t cells;for IgG production through the same process, however, a second signal is required. Previous studies have reported that invariant natural killer t (iNKt) cells may be responsible for the second signal involved in IgG production. In the present study, we addressed whether human iNKt cells could participate in the production of Ig against tI-2 Ag in vitro. two major distinct subsets of human iNKt cells, CD4+ CD8β- (CD4) and CD4- CD8β- [double negative (DN)] cells, were generated from peripheral blood monocytes from a healthy volunteer. BCR engagement, triggered by anti-IgM antibody stimulation, examined here as a model of BCR engagement triggered by tI-2 Ag, induced abundant IgM production by B cells. Both CD4 and DN iNKt cells reduced IgM production and conversely enhanced IgG production in a dose-dependent manner. In addition, IgG production by CD19+CD27- (naïve) and CD19+CD27+ (memory) B cells was predominantly promoted by DNiNKt cells rather than CD4 iNKt cells;nevertheless, IgM production by both B cell subsets was similarly reduced by either subset of iNKt cells. these results suggest that the DN iNKt subsets may preferentially promote Ig class switching by B cells upon stimulation with tI-2 Ag.

Objectives the purpose of this study was to investigate the hypothesis that ci rculation via collateral vessels to an infarctrelated artery (IRA) is impaired w ith aging in patients with acute myocardial infarction (AMI). Background Animal experiments have shown that advanced age blunts the development of new vessels i n response to angiogenic cytokines. Methods Of 3,573 consecutive patients with A MI, 1,934 patients who fulfilled the following criteria were enrolled in this st udy: 1) coronary angiograms were obtained within 72 h after the onset of AMI; an d 2) IRA showed complete occlusion (thrombolysis In Myocardial Infarction tIMI>> flow grade 0 or 1). Collaterals to the IRA were angiographically evaluated usin g the Rentrop score. Rentrop scores 1 to 3 were defined as demonstrating significant collaterals. Results the prevalence of collaterals decreased with age, from 47.9%, 45.8%, 43.4%, to 34.0%in patients to 59 year s, 60 to 69 years, ≥70 years, respectively (p t factor predicting the absence of collateral circulation to the IRA. In contrast, time to catheterization, history of angina pectoris, and preinfarction angina were independent predictors for the presence of collaterals. Multivariat e analysis showed that the absence of collaterals was an independent predictor o f in-hospital mortality in elderly patients ≥70 years (odds ratio, 15.6; 95%c onfidence interval, 3.5 to 69.6), although this finding was not significant in p atients ted with decreased angiog raphic presence of collaterals to the IRA in patients with AMI. this abnormality may contribute to the poor prognosis of elderly patients with AMI.

this paper outlines the results of our study aiming at evaluating the actual extent of human exposure to Hg mainly in tapajos River Basin in central Amazonia, which is considered to be the oldest and most productive gold mining area in the Amazon. Human hair and blood as well as fish samples were collected from gold mining areas and various fishing villages located at different distances from main gold mining areas, and analyzed for total mercury(t Hg) and methylmercury (MeHg) with new methods recently being developed in our laboratory. the results to date showed the inhabitants of fishing villages particularly in the upstream tapajos areas were constantly exposed to MeHg at abnormally high levels, with little confounding exposure to inorganic Hg. A highly significant correlation between Hg in hair and blood was observed in fishing villages, and the ratios of hair Hg to blood Hg were very close to 250g, generally established for MeHg. While, the levels of MeHg in hair and blood of gold miners and goldshop workers were low, indicating that they were mainly exposed to Hg in inorganic form. From these data, together with the levels of MeHg in fish, Hg contamination mechanisms are discussed.
