Adverse Maternal Outcomes in High-Order Multiple Pregnancies in a Private Health Facility in nigeria: A 10-Year Experience

Background: naturally occurring high-order multiple (HOM) pregnancies are rare (1 - 7 per 10,000) even in nigeria noted to have the highest twinning rates. Worldwide multiple pregnancies are increasing and are associated with increased adverse maternal outcomes. Materials and Methods: This was a retrospective study of all patients with HOM pregnancy in Prime Medical Consultants between Jan 2004 to Dec 2013. We identified all the patients with high-order multiple pregnancy and extracted relevant data from the ante-natal and labour ward records, theatre records, and patients case notes. Demographic characteristics, gestational age at delivery, preterm rupture of membranes, preterm labour, hypertensive disease in pregnancy, proteinuria, anaemia, and postpartum haemorrhage were data extracted from the records. Results: The incidence of HOM pregnancy in this hospital was 0.24%. The mean maternal age was 35.7 ± 1.81 years. Over 85% of all the high-order multiple pregnancies in this hospital were from in-vitro fertilization and embryo transfer treatments for infertility. About 11% of the HOM pregnancy ended as spontaneous miscarriages. The mean gestational age at birth was 31 ± 1.5 weeks, and primary postpartum haemorrhage (22.2%) was the most common maternal adverse outcome from birth. Conclusion: The high incidence of HOM pregnancy in this hospital was mainly from in-vitro fertilization and embryo transfer. The HOM pregnancies are associated with high occurrence of preterm labour, caesarean sections and primary postpartum haemorrhage.

This study examined the sources,health risk and pollution status of selected inorganic contaminants such as iron(Fe),nickel(ni),cadmium(Cd),cobalt(Co),arsenic(As),manganese(Mn),lead(Pb),copper(Cu),zinc(Zn),and chromium(Cr) in representative roadside uncontaminated soil(n=72) from the Egi crude oil mining areas(Oboburu,Obagi,and Ogbogu),niger Delta,nigeria,using a combination of multiple models(Principle component analysis(PCA),correlation analysis,geo-accumulation index(Igeo),contamination factor(CF),enrichment factor(EF),pollution load index(PLI),cancer risk(CR),hazard quotient(HQ),and metal daily intake(MDIm)).A random sampling method was applied in the sampling and analysis using inductively coupled plasma mass *** result revealed that the mean(mg/kg) for Oboburu,was the highest for Fe(89.95 ± 28.83) and Mn(26.12 ± 7.642),whereas Obagi,had less Fe(28.90 ± 7.601) and Mn(16.80 ± 5.021),and Ogbogu,Fe(44.67 ± 28.15) and Pb(19.38± 8.731) was the second highest heavy *** order of contamination was F> Mn> Zn> Pb> Cu> Cr> ni> Cd> *** Igeo and CF had cadmium as a metal of *** contamination level was low from the PLI *** EF revealed anthropogenic sources for all the metals except for ni,which was *** correlations at p nd p nificance amongst the *** of the cumulative percentages,loading percentages,and eigenvalues from the regression plot gave a coefficients of determination(r2) of 0.9936,0.9995,and 0.9994,*** PCA indicated that Cd and ni were of anthropogenic origin;Mn,Fe,and Co were biogenic or natural,and Zn and Pb were of mixed *** CR assessment indicated a possible cancer risk due to Cr,Cd,and ni,and the rest of elements have no carcinogenic *** findings suggest there should be regular monitoring,source control,and integrated environmental management of the study area.

Designing highly efficient and low-cost electrocatalysts for oxygen evolution reaction is important for many renewable energy *** particular,strain engineering has been demonstrated as a powerful strategy to enhance the electrochemical performance of catalysts;however,the required complex catalyst preparation process restricts the implementation of strain ***,we report a simple self-template method to prepare hierarchical porous Co_(3)O_(4)nanowires(PnWs)with tunable compressive strain via thermal-oxidation-transformation of easily prepared oxalic acid-cobalt nitrate(Co(nO_(3))_(2))composite *** on the complementary theoretical and experimental studies,the selection of proper solvents in the catalyst preparation is not only vital for the hierarchical structural evolution of Co_(3)O_(4) but also for regulating their compressive surface *** of the rich surface active sites and optimized electronic Co d band centers,the PnWs exhibit the excellent activity and stability for oxygen evolution reaction,delivering a low overpotential of 319 mV at 10 mA·cm^(−2)in 1 M KOH with a mass loading 0.553 mg·cm^(−2),which is even better than the noble metal catalyst of RuO_(2).This work provides a cost-effective example of porous Co_(3)O_(4)nanowire preparation as well as a promising method for modification of surface strain for the enhanced electrochemical performance.

Hybrid organolead halide perovskites have attracted tremendous attention due to their recent success as high efficiency solar cell materials and their fascinating material properties uniquely suitable for optoelectronic devices. However, the poor ambient and operational stability as well as the concern of lead toxicity greatly hamper their practical utilization. In this work, crystalline, all-inorganic and lead-free Cs_(3)Sb_(2)I_(9) perovskite microplates are successfully synthesized by a two-step chemical vapor deposition method. As compared with other typical lead-free perovskite materials, the Cs_(3)Sb_(2)I_(9) microplates demonstrate excellent optoelectronic properties, including substantial enhancements in the Stokes shift, exciton binding energy and electron-phonon coupling. Simple photoconductive devices fabricated using these microplates exhibit an ultra-fast response with the rise and decay time constants down to 96 and 58 µs, respectively. This respectable photoconductor performance can be regarded as a record among all the lead-free perovskite materials. Importantly, these photodetectors show superior thermal stability in a wide temperature range, capable to function reversibly between 80 and 380 K, indicating their robustness to operate under both low and high temperatures. All these results evidently suggest the technological potential of inorganic lead-free Cs_(3)Sb_(2)I_(9) perovskite microplates for next-generation high-performance optoelectronic devices.

ng>Introduction:ng> Malaria control becomes effective if countries adopt the World Health Organization & Global Malaria Programme (WHO/GMP) recommendation with includes diagnosis of malaria cases and treatment with effective medicines, distribution of insecticide-treated nets (ITns) and indoor residual spraying (IRS). Ownership and utilization of mosquito nets for malaria prevention is still sub-optimal with only 62% ownership of at least one mosquito net and only 37% of children using the nets. This seeks to investigate the willingness to buy and the use ITn use among caregivers of under five children attending immunization clinic in Bingham University Teaching Hospital. ng>Methodology:ng> A descriptive cross sectional study design carried out among 242 caregivers of all ages and sex of under-five children attending immunization clinic at Bingham University Teaching Hospital (BHUTH) in Jos north Local Government Area of Plateau State, nigeria. A Simple Random Sampling technique was used to select respondents. ng>Findings:ng> Two hundred and sixteen 216 (89.3%) of the children use ITns, majority of the children 190 (78.5%) slept under ITn the night before the interview, 226 (93.4%) owned ITns, 156 (64.5%) got the ITns for free while 60 (24.8%) paid for it and majority 172 (71.1%) of the children use ITn every night. Tertiary education, having much younger children (less than 3 years) and perception as a means of malaria prevention were associated with higher use of ITns. Majority 220 (90.9%) the caregivers were willing to buy ITn and also recommend ITn to others. Caregivers who were traders and civil servants were willing to buy ITn than farmers, tertiary education, having children 1 - 3 months old, and owning 3 or more ITns was associated with willingness to buy ITns. ng>Conclusion: ng>About 90% of the children of caregivers use ITns and over three quarters slept under ITn the night before the interview and majority use i

Modulations in Anti-Oxidant Activities of Selected Gastro-Intestinal Tissues in Alloxan-Induced, Silymarin Treated Diabetic Wistar Rats

Diabetes mellitus (DM) is reportedly the commonest metabolic disorder with multi organ involvement. By inducing DM (with Alloxan) in Wistar rats, current study investigated the changes in antioxidant activities of selected gastrointestinal (GI) tissues [stomach, duodenum, pancreas and liver], upon treatment with Silymarin and/or Vitamin C. One hundred and twenty five (125) adult male wistar rats of between 130 to 180 grams were procured for the study. Five units of one control and four experimental units were designated with twenty five (25) rats per group (n = 25);Unit 1: Control rats, Unit 2 were DM induced, Silymarin untreated rats, and Units 3, 4 and 5 were DM induced, vitamin C, Silymarin and Vitamin C + Silymarin treated respectively. Following four (4) weeks of administration of test substance(s), rats were euthanized and blood samples obtained for biochemical and antioxidant assay on aforementioned GI tissues. One way analysis of variance (AnOVA) and Students t-test at p p n of extract treated unit to control. Study also observed a significant change in pancreatic, liver, and duodenal anti-oxidant marker levels with Vitamin C, Silymarin and Vitamin C + Silymarin co-administrations to diabetic rats. It can therefore be said, that DM caused a destructive alteration pancreatic histo-architecture with improved functional capabilities in wistar rats at administration of Silymarin and vitamin C. Thus, Silymarin posed antioxidant potentials, with ameliorated pancreatic dysfunctions.

Obesity is known to be a major risk factor of type 2 diabetes （T2D） and responsible for most lipid abnormalities associated with the disease but limited data on such association are available for diabetic patients of Igbo ethnicity in the South East region of nigeria. A case-control study involving 72 T2D patients and 75 non-diabetic （nD） patients （control） ofIgbo ethnicity was conducted. Demographic and anthropometric data were obtained followed by blood collection for the determination of fasting blood sugar （FBS）, total cholesterol （TC）, triglyceride （TG）, high density lipoprotein （HDL） and low density lipoprotein （LDL）. Obesity based on waist circumference （WC） was significantly higher （p 〈 0.001） in T2D patients compared to their non-diabetic counterparts. Similarly, TC, TG and LDL levels were significantly （p 〈 0.001） higher in T2D patients while HDL was significantly lower （p 〈 0.001） in T2D patients compared to the control. The proportion of dyslipidaemia characterized by high TC, high TG, high LDL and low HDL was significantly higher （p 〈 0.001） in T2D patients. BMI correlated positively （p 〈 0.05） with WC, TC, and LDL while FBS correlated positively （p 〈 0.05） with TG but negatively with HDL. In conclusion, dyslipidaemia characterised by hypercholesterolaemia, hypertriglyceridaemia, elevated LDL and reduced HDL, as well as obesity were associated with T2D and correlated with FBS in this population.

ng>Background:ng> Alcohol Use Disorders (AUDs) in the elderly are associated with impairments in physical, psychological, social and cognitive functioning. Both the elderly men and women who drink alcohol are at a greater risk of social isolation and economic deprivation and the women in particular are in the danger of rapid development of dependence a phenomenon termed “telescoping” dependence. This study seeks to determine the prevalence of alcohol use disorders as well as their associated factors among the elderly in Jos South LGA, Plateau State, nigeria. ng>Methods:ng> A community-based cross-sectional study of elderly (≥60 years) men and women. A total of 400 out of a total of 422 elderly community dwellers identified were interviewed with the aid of a composite questionnaire incorporating the socio-demography, Alcohol Smoking and Substance Involvement Screening Test (ASSIST), Mini International neuropsychiatric Interview (MInI) and the 12-item General Health Questionnaire (GHQ-12). ng>Results:ng> The lifetime and current prevalence of alcohol use were 69.8% and 45.5% respectively. The prevalence of AUDs was (n = 52, 13.0%). AUDs was significantly associated with being male (OR = 2.57, 95%CI: 1.35 - 3.77, p nts with AUDs were more likely to report difficulty ambulating (χ2 = 0.34, 0.56), cardiovascular (χ2 = 0.42, p = 0.52), respiratory (χ2 = 0.55, p = 0.46), and digestive (χ2 = 0.17, p = 0.68) problems, though not statistically significant. Similarly, no significant associations were seen with probable psychiatric co-morbidity (χ2 = 2.35, p = 0.13) and chronic pain (χ2 = 0.52, p = 0.47). ng>Conclusions:ng> Alcohol use and use disorders were common in this elderly cohort. AUDs were associated with poorer health indices.

Performance Evaluation of Real Time Polymerase Reaction Assay (SCODA) for Detection of SARS-CoV-2

In order to robustly detect and quantify gene expression from small amounts of RnA, amplification of the gene transcript is necessary. Real Time PCR is useful for detection and quantification of genetic constitution of pathogens. This technique amplifies a tiny DnA target million or billion times in such a way that it can be easily studied by scientists. Availability of highly sensitive and specific assay for the detection of SARS-Cov-2 and easy accessibility of such was necessary for early diagnosis and effective management of COVID-19 infection. The aim of this study was to evaluate the performance characteristic of SCODA. Validation of SCODA was performed using synthesized standards and clinical samples previously tested using a commercially approved COVID-19 RT-qPCR detection kit (LifeRiver). The assay showed a linearity of 98.2% on the ORF1ab target and 99.8% on the n-gene target. The sensitivity and specificity were both 100%. Analysis for the LoD95 produced 74.04 (CI: 25 - 1000) cp/μl on ORF1ab gene and 1.119 (CI: 1 - 1) cp/μl on n-gene target with a precision of CV ≤ 3%. SCODA showed high comparable performance in comparison with LifeRiver and other commercial COVID-19 RT-qPCR test kits.
