Background Asthma is a significant chronic health problem worldwide. Management aims at disease control by reducing functional impairment and exacerbations and improving quality of life (QoL). We report a multi-center study to survey asthma control and QoL in four cities in the Pearl River Delta. Methods The conjoint survey involved ten Hong Kong pediatric hospitals/units, two Shenzhen hospitals, two Macao hos-pitals, and two Guangzhou hospitals on asthma control (using Asthma Control Test) and QoL (Pediatric Allergic Disease Quality of Life Questionnaire, PADQLQ). Acceptability of a treatment is graded as very good/good/fair/poor. Results Good asthma control was only reported in 80% subjects in Hong Kong, but higher in sister cities (85–94%, P<0.001). Allergic rhinitis, 'incense burning', and 'smoker in family' were prevalent among the four cities. Logistic regres-sion showed better control of asthma was associated with better PADQLQ (B=?0.029, P < 0.001), better acceptability of bronchodilator (B=?1.488, P = 0.025), negatively with 'smoker in family' (B=?0.83, P = 0.015) and various PADQLQ domains. Conversely, worse PADQLQ was associated with allergic rhinitis severity (B=4.77, P <0.001), poor control of asthma (B=7.56, P <0.001), increased frequency of traditional Chinese medicine use (B=1.7, P < 0.05), increased fre-quency of bronchodilator usage (B=1.05, P < 0.05), 'smoker in family' (B=4.05, P < 0.05), and incense burning at home (B=3.9, P < 0.05). Conclusions There are some clinical and cultural differences among the four southern Chinese cities within the Guangdong province. This study identifies potentially modifiable environmental and treatment factors associated with poor asthma control and QoL for health-care interventions. Having a smoker in the family is independently associated with poor asthma control and QoL.

Within-Cluster Variability Exponent for Identifying Coherent Structures in Dynamical Systems

We propose a clustering-based approach for identifying coherent flow structuresin continuous dynamical systems. We first treat a particle trajectory over a finitetime interval as a high-dimensional data point and then cluster these data from differentinitial locations into groups. The method then uses the normalized standarddeviation or mean absolute deviation to quantify the deformation. Unlike the usualfinite-time Lyapunov exponent (FTLE), the proposed algorithm considers the completetraveling history of the particles. We also suggest two extensions of the method. To improvethe computational efficiency, we develop an adaptive approach that constructsdifferent subsamples of the whole particle trajectory based on a finite time interval. Tostart the computation in parallel to the flow trajectory data collection, we also developan on-the-fly approach to improve the solution as we continue to provide more measurementsfor the algorithm. The method can efficiently compute the WCVE over adifferent time interval by modifying the available data points.

On Effective Campus Attack Response:Insights from Agent-Based Simulation for Improving Emergency Information Sharing System Design and Response Strategy

Information sharing systems are a critical component of emergency response—especially in campus attack situations that unfold very *** design of effective information sharing systems is often difficult,however,due to a lack of data on these assault *** work takes an agent-based approach to simulate three campus emergency information sharing system design alternatives in the context of a college campus knife attack,and incorporates data from on-campus student surveys and parameter tuning *** are evaluated according to:(1)improved student attack response outcomes;and(2)effective institutional response to the *** results confirm that increased awareness supports rapid emergency reporting,but an important gap exists between students'awareness and their ability to respond effectively,which depends on a number of campus-specific factors.A strong positive impact is seen from safe and efficient information sharing with *** impact depends largely on reporting system implementation qualities,as opposed to campus-specific *** a campus in China,WeChat was used as a basis for messaging *** simulation results show a 9%drop in casualties and a 22%faster police response time from a text-based reporting system using‘‘base''WeChat features instead of traditional phone *** results also project a 30%drop in casualties and 52%faster police response time using a system designed around a WeChat Mini Program or standalone campus emergency reporting *** outcomes suggest a number of recommendations for improving outdated campus emergency information-sharing systems and response strategies.

Organic materials are of great interest for the development of low cost electronic and optoelectronic devices. Although majority of research on organic materials is concerned with synthesis of novel compounds and organic thin films, organic nanostructures are attracting increasing interest in recent years. We briefly review different growth methods of organic nanostructures, which can be roughly divided into vapor deposition methods and self-assembly techniques in solution. Then we highlight some interesting properties of organic nanostructures, as well as possible applications, includinf field emission, electronic and optoelectronic devices.

Value of gastrografin in adhesive small bowel obstruction after unsuccessful conservative treatment: A prospective evaluation

AIM: Gastrografin is a hyperosmolar water-soluble contrast medium. Besides its predictive value for the need for operative treatment, a potential therapeutic role of this agent in adhesive small bowel obstruction has been suggested. This study aimed at evaluating the effectiveness of gastrografin in adhesive small bowel obstruction when conservative treatment failed. METHODS: Patients with adhesive small bowel obstruction were given trial conservative treatment unless there was fear of bowel strangulation. Those responded in the initial 48 h had conservative treatment continued. Patients who showed no improvement in the initial 48 h were given 100 mL of gastrografin through nasogastric tube followed by serial abdominal radiographs. Patients with the contrast appeared in large bowel within 24 h were regarded as having partial obstruction and conservative treatment was continued. Patients in which the contrast failed to reach large bowel within 24 h were considered to have complete obstruction and laparotomy was performed. RESULTS: Two hundred and twelve patients with 245 episodes of adhesive obstruction were included. Fifteen patients were operated on soon after admission due to fear of strangulation. One hundred and eighty-six episodes of obstruction showed improvement in the initial 48 h and conservative treatment was continued. Two patients had subsequent operations because of persistent obstruction. Forty-four episodes of obstruction showed no improvement within 48 h and gastrografin was administered. Seven patients underwent complete obstruction surgery. Partial obstruction was demonstrated in 37 other cases, obstruction resolved subsequently in all of them except one patient who required laparotomy because of persistent obstruction. The overall operative rate in this study was 10%. There was no complication that could be attributed to the use of gastrografin. Ridge-furrow plastic mulching with a suitable planting density enhances rainwater productivity,grain yield and economic benefit of rainfed maize

Soil surface mulching and planting density regulation are widely used for effective utilization of limited rainwater resources and improvement of crop productivity in dryland ***,the combined effects of mulching type and planting density on maize growth and yield have been seldom studied,especially in different hydrological years.A field experiment was conducted to evaluate the effects of mulching type and planting density on the soil temperature,growth,grain yield(GY),water use efficiency(WUE)and economic benefit of rainfed maize in the drylands of northern China during *** fluctuated over the three *** were four mulching types(NM,flat cultivation with non-mulching;SM,flat cultivation with straw mulching;RP,plastic-mulched ridge plus bare furrow;RPFS,plastic-mulched ridge plus straw-mulched furrow)and three planting densities(LD,low planting density,45.0×10^3 plants/hm^2;MD,medium planting density,67.5×10^3 plants/hm^2;HD,high planting density,90.0×10^3 plants/hm^2).Results showed that soil temperature was higher with RP and lower with SM compared with NM,but no significant difference was found between RPFS and *** soil water was retained by soil mulching at the early growth stage,but it significantly varied at the middle and late growth *** growth was significantly improved by soil *** increasing planting density,stem diameter,net photosynthetic rate and chlorophyll content tended to decline,whereas a single-peak trend in biomass yield was *** type and planting density did not have significant effect on evapotranspiration(ET),but GY and WUE were significantly *** were significant interacting effects of mulching type and planting density on biomass yield,GY,ET and *** with NM,RPFS,RP and SM increased GY by 57.5%,50.8%and 18.9%,and increased WUE by 66.6%,54.3%and 18.1%,*** MD,GY increased by 41.4%and 25.2%,and WUE increased by 38.6%and 22.4%compared with thos

Apparently balanced chromosomal structural rearrangements are known to cause male infertility and account for approximately 1%of azoospermia or severe ***,the underlying mechanisms of pathogenesis and etiologies are still largely ***,we investigated apparently balanced interchromosomal structural rearrangements in six cases with azoospermia/severe oligospermia to comprehensively identify and delineate cryptic structural rearrangements and the related copy number *** addition,high read-depth genome sequencing(GS)(30-fold)was performed to investigate point mutations causative of male ***-pair GS(4-fold)revealed additional structural rearrangements and/or copy number changes in 5 of 6 cases and detected a total of 48 ***,the breakpoints caused truncations of 30 RefSeq genes,five of which were associated with ***,the breakpoints disrupted 43 topological-associated *** disruptions or potential dysregulations of genes,which play potential roles in male germ cell development,apoptosis,and spermatogenesis,were found in all cases(n=6).In addition,high read-depth GS detected dual molecular findings in case MI6,involving a complex rearrangement and two point mutations in the gene ***,our study provided the molecular characteristics of apparently balanced interchromosomal structural rearrangements in patients with male *** demonstrated the complexity of chromosomal structural rearrangements,potential gene disruptions/dysregulation and single-gene mutations could be the contributing mechanisms underlie male infertility.

Successful conservative management of Class III iatrogenic aortic dissection

Iatrogenic aortic dissection （IAD） is a rare complication of percutaneous coronary intervention （PCI） with an estimated incidence of 0.02%--0.04%. lAD occurs more frequently in the setting of urgent PCI for acute myocardial infarction （AMI）（0.19%） than for elective procedures （0.01%）.[11 The mortality risk associated with IAD is high （16%） and is comparable to that for spontaneous aortic dissection （16%）.Urgent surgery is the treatment of choice for extensive dissection or hemodynamic instability. In this report, we present a case of severe IAD that was managed conservatively with good clinical outcome.
