A 10-year-old Chinese boy who had a history of congenital thrombocytopathy presented with severe iron deficiency anemia secondary to chronic gastric inflammation and duodenal ulcerations. Subtle oculocutaneous albinism led to the finding of diminished dense bodies in the platelets under electron microscopy, hence the diagnosis of Hermansky-Pudlak syndrome (HPS). Biopsies from the stomach and duodenum revealed a lymphocytic infiltration in the submucosa, but H pylori infection was absent. The gastroduodenitis responded to the treatment with omeprazole while iron deficiency anemia was corrected by oral iron therapy. HPS is a rare cause of congenital bleeding disorder with multisystemic manifestations. Upper gastrointestinal involvement is rare and should be distinguished from a mere manifestation of the bleeding diathesis.

Coronaviruses have developed various measures to evade innate immunity, We have previously shown that severe acute respiratory syndrome （SARS） coronavirus M protein suppresses type I interferon （IFN） production by impeding the formation of functional TRAF3-containing complex. In this study, we demonstrate that the IFN-antagonizing activity is specific to SARS coronavirus M protein and is mediated through its first transmembrane domain （TM 1） located at the N terminus. M protein from human coronavirus HKU 1 does not inhibit IFN production. Whereas N-linked glycosylation of SARS coronavirus M protein has no influence on IFN antagonism, TM1 is indispensable for the suppression of IFN production. TM 1 targets SARS coronavirus M protein and heterologous proteins to the Golgi apparatus, yet Golgi localization is required but not sufficient for IFN antagonism. Mechanistically, TM 1 is capable of binding with RIG-I, TRAF3, TBK1 and IKK~, and preventing the interaction of TRAF3 with its downstream effectors. Our work defines the molecular architecture of SARS coronavirus M orotein reouired for suooression of innate antiviral re^nnn^e.

利用附加电子转移因子和开关函数的半经典近似和量子分子轨道方法,在0 3～3 0KeV能区和2 0～20 0(a u )核间距的条件下,对Si5+离子与He原子相互作用过程中5个singlet态和5个triplet态的双电子捕获势能进行了理论计算。通过参数优化,在比较了基通道电子转移势能和实验值之后,研究了主通道的电子转移势能。其结果说明了这一动力学过程中的避交叉(avoidedcrossings)性质,给出了双电子容易被捕获的不同核间距离点。

BACKGROUND: Intravenous fluid(IVF) is commonly used in acute clinical management. This study aimed to review the choice and primary considerations in IVF prescriptions and to evaluate the adequacy of guidelines and trainings on it in the New Territories West Cluster(NTWC) of Hong ***: This is a descriptive study based on data collected from an online survey. Data were processed by SPSS for statistical analysis. This study focused on a general description and doctor-nurse between group comparison. Participants were asked the choice of IVF for nine acute clinical scenarios and provide reason. A 1–10 scale was used to assess the sufficiency of guideline, training and information, and time for revision on IVF ***: 0.9% sodium chloride was the most familiar IVF(36%), followed by 5% Dextrose solution(26%). In the nine scenarios, the most chosen IVF was 0.9% sodium chloride(37%–61%). There was significant difference in the choice of IVF between doctors and nurses in 7 cases. The second most chosen IVF for doctors was Plasma-Lyte A while that for nurses was Gelofusine. Departmental practice was the most chosen reason to account for the prescription. The adequacy of guideline, information and training, and time for revision was rated 5. Doctors had significantly more time at work than nurses to update knowledge in IVF prescription(5.41 versus 4.57).CONCLUSION: 0.9% sodium chloride was mostly chosen. The choice of IVF was mainly based on departmental practice. Adequacy of guideline, information and training, and time for revision on IVF prescription were average, indicating significant training deficit.

目的探討三陰乳腺癌PD-L1(SP142)與CTLA-4、PTEN表達的關係和意義。方法收集鏡湖醫院2008年1月~2018年6月間病理科136例乳腺癌標本,按乳腺癌分子分型分為三陰乳腺癌(TNBC)、管腔A型、管腔B型及HER2型乳腺癌,採用免疫組化檢測各型乳腺癌PD-L1(SP142)、CTLA-4和PTEN表達情況,並分析它們之間的關係。結果136例乳腺癌中,32.4%的病例PD-L1(SP142)陽性,其中48%(24/50)TNBC、11.9%(5/42)管腔A型、20%(3/15)管腔B型和41.4%(12/29)HER2型PD-L1(SP142)陽性;43.4%的病例CTLA-4陽性,其中52.0%(26/50)TNBC、31.0%(13/42)管腔A型、33.3%(5/15)管腔B型和50.9%(15/29)HER2型CTLA-4陽性;28.0%的病例PTEN陽性,其中52.0%(26/50)TNBC、31.0%(13/42)管腔A型、33.3%(5/15)管腔B型和50.9%(15/29)HER2型PTEN陽性。TNBC的PD-L1(SP142)表達與CTLA-4表達呈正相關(Spearman=0.409,P<0.05),與PTEN表達呈負相關(Spearman=-0.318,P<0.05)。結論TNBCPD-L1(SP142)與CTLA-4表達的正相關關係,提示聯合免疫治療可能使TNBC患者獲益,而TNBCPD-L1(SP142)與PTEN表達的負相關關係,提示TNBC同時遭受免疫逃逸和基因突變的雙重打擊,因此適合免疫治療和靶向PTEN聯合治療。

In order to alleviate unstable factor-caused bifurcation and reduce oscillations in traffic flow,a feedback control with consideration of time delay is designed for the solid angle model(SAM).The stability and bifurcation condition of the new SAM is derived through linear analysis and bifurcation analysis,and then accurate range of stable region is *** order to explore the mechanism of the influence of multiple parameter combinations on the stability of controlled systems,a definite integral stabilization method is provided to determine the stable interval of time delay and feedback *** simulations are explored to verify the feasibility and effectiveness of the proposed model,which also demonstrate that feedback gain and delay are two key factors to alleviate traffic congestion in the SAM.
