早期骶髂关节结核的诊断及治疗

目的探讨早期骶髂关节结核的诊断方法及疗效。方法分析2005年2月至2011年6月在武警北京市总队第二医院就诊并接受治疗的20例早期骶髂关节结核患者,依据临床表现、影像学特征及穿刺活检病理检查明确诊断,按照kim分型,其中Ⅱ型8例,Ⅲ型12... 详细信息

目的探讨早期骶髂关节结核的诊断方法及疗效。方法分析2005年2月至2011年6月在武警北京市总队第二医院就诊并接受治疗的20例早期骶髂关节结核患者,依据临床表现、影像学特征及穿刺活检病理检查明确诊断,按照kim分型,其中Ⅱ型8例,Ⅲ型12例。行手术病灶刮除、取自体骨进行植骨术。观察其治疗效果,采用Majeed评分系统,从疼痛(30分)、工作(36分)、就座(10分)、性生活(4分)和站立(20分)方面分别评分,总分为100分,计算总分后进行临床分级,优(>85分)、良(70～84分)、中(55～69分)、差(eed评分,其中优0例、良2例、中14例、差4例。结果20例患者全部获得随访,随访时间12～36个月,无复发、感染及骶髂关节分离脱位等并发症,所有患者均于术后3～6个月恢复正常训练及出勤。术前Majeed评分平均(61.75±6.54)分,术后末次随访平均(92.75±3.70)分,术后Majeed评分均较术前显著提高(t=3.481,P<0.001)。结论早期骶髂关节结核可依据临床表现、影像学特征及穿刺活检病理检查得以早期明确诊断,降低误诊率;进行系统规范的抗结核药物及手术治疗,预后及疗效满意。

脓毒症是一种由感染引起的异质性疾病,感染触发了一系列复杂的局部或者全身的免疫炎症反应,引起多器官功能衰竭,发病率和病死率显著升高。由于至今仍然没有诊断脓毒症的金标准,所以脓毒症的临床诊断仍是一个难题。因此,脓毒症的临床诊断需要不断改变来满足临床和研究的要求。然而,尽管有许多新型的生物标记和筛选工具去预测脓毒症发生的风险,但是这些措施的诊断价值和有效性不足以让人满意,并且没有充分的证据去建议临床使用这些新技术。因此,脓毒症的临床诊断标准需要定期更新去适应不断产生的新证据。这篇综述旨在呈现当前脓毒症的诊断和早期识别方面的最新研究证据。临床运用不同的诊断方法的推荐意见依赖于推荐、评价、发展和评估分级体系(Grades of Recommendation Assessment,Development and evaluation,GRADe),因为大部分的研究是观察性研究,并没有对这些方法进行可靠评估,采用的是两步推理方法。未来需要更多研究来确认或者反驳某一特殊的指标检测,同时应该直接采用相关病人的结果数据。

脓毒症并发急性肾损伤患儿的血清miR-182-5p、miR-21-3p表达变化及其临床意义

目的观察脓毒症并发急性肾损伤(AKI)患儿血清miR-182-5p、miR-21-3p的表达变化,并探讨其临床意义。方法113例脓毒症患儿,根据是否并发AKI分为AKI组89例和NAKI组24例,采集患儿入院后次日清晨空腹静脉血,采用实时荧光定量法检测血清miR-182-5p、miR-21-3p,采用eLISA法检测血清肾功能指标肾损伤分子-1(kim-1)、胱抑素C(Cys-C)、肌酐(Scr),分析血清miR-182-5p、miR-21-3p表达与脓毒症并发AKI患儿血清kim-1、Cys-C、Scr水平的相关性,采用多因素Logistics回归分析法分析脓毒症并发AKI的影响因素,绘制受试者工作特征曲线分析血清miR-182-5p、miR-21-3p表达对脓毒症患儿并发AKI的预测价值。结果AKI、NAKI组血清miR-182-5p相对表达量分别为1.54(0.98,1.98)、0.35(0.28,0.39),血清miR-21-3p相对表达量分别为2.66(1.83,3.28)、0.85(0.75,1.00),二者比较,P均kim-1、Cys-C、Scr水平均呈正相关(P均kim-1、Cys-C、Scr水平升高,急性生理和慢性健康评估Ⅱ评分、脓毒症相关的序贯器官衰竭评估评分增加(P均kim-1、Cys-C、Scr为脓毒症患儿并发AKI独立危险因素(OR分别为1.530,1.247,1.249,1.473,1.741;95%CI分别为1.134~1.913,1.011~1.707,1.021~1.529,1.103~2.987,1.062~3.517;P均<0.05)。miR-182-5p+miR-21-3p联合预测脓毒症患儿并发AKI的曲线下面积明显高于血清miR-182-5p、miR-21-3p单独预测(Z=3.039、2.864,P=0.002、0.004)。结论脓毒症并发AKI患儿血清miR-182-5p、miR-21-3p表达降低。血清miR-182-5p、miR-21-3p表达降低是脓毒症患儿并发AKI的独立危险因素。血清miR-182-5p、miR-21-3p可作为脓毒症患儿并发AKI的预测指标。

目的探讨强直性脊柱炎患者近端肾小管损伤的尿生物学标志物。方法选取2019年1月—2020年12月寿光市人民医院收治的70例强直性脊柱炎患者,按照尿白蛋白/肌酐比值将其分为蛋白尿组40例与无蛋白尿组30例,另外选取同期70例健康体检者为对照组。比较三组间尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平。强直性脊柱炎患者进行Bath强直性脊柱炎疾病活动指数评分,并分析该评分与尿液检测指标的关系。结果三组间尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平,差异有统计学意义(P<0.05)。相关性分析显示,Bath强直性脊柱炎疾病活动指数评分与尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平呈正相关。结论有微量白蛋白尿的强直性脊柱炎患者肾小管损伤程度更严重,尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平,可作为评估强直性脊柱炎患者早期近端肾小管损伤的指标。

小胶质细胞是调节大脑突触发育和可塑性的重要细胞类型,但其影响突触的正常功能的机制尚不清楚。在本研究中,我们通过PLX5622造成小胶质细胞耗竭,并观察其对成年野生型小鼠海马CA3-CA1突触的影响。在小胶质细胞耗竭后,与树突棘密度降低相关的自发和诱发谷氨酸能活动的减少,出现未成熟突触特征以及突触的可塑性提高。小胶质细胞耗竭的小鼠在新物体识别任务的获取方面表现出缺陷。海马星形胶质细胞出现增生,但并没有神经炎症反应。在Cx3cr1-/-小鼠中,PLX不能导致海马出现上述改变。这说明CX3cl1/CX3CR1轴在小胶质细胞对突触功能的控制中有重要作用。PLX5622停用后,小胶质细胞的重新增殖,海马突触恢复,小鼠的学习功能也出现恢复。综上所述,小胶质细胞对维持成人大脑的突触的正常功能用重要的作用,去除小胶质细胞会导致谷氨酸能突触组织和活动的可逆变化。
