Stroma-targeting strategies in pancreatic cancer:Past lessons,challenges and prospects

Pancreatic ductal adenocarcinoma(PDAC)is projected to emerge as the second leading cause of cancer-related death after *** treatment resistance is perhaps the most significant factor that underlies the poor prognosis of *** date,combination chemotherapy remains the mainstay of treatment for most PDAC *** to other cancer types,treatment response of PDAC tumors to similar chemotherapy regimens is clearly much lower and *** from typically harboring genetic alterations that to date remain undruggable and are drivers of treatment resistance,PDAC tumors are uniquely characterized by a densely fibrotic stroma that has well-established roles in promoting cancer progression and treatment ***,emerging evidence also suggests that indiscriminate targeting and near complete depletion of stroma may promote PDAC aggressiveness and lead to detrimental *** conflicting results undoubtedly warrant the need for a more in-depth understanding of the heterogeneity of tumor stroma in order to develop modulatory strategies in favor of tumor *** advent of novel techniques including single cell RNA sequencing and multiplex immunohistochemistry have further illuminated the complex heterogeneity of tumor cells,stromal fibroblasts,and immune *** new knowledge is instrumental for development of more refined therapeutic strategies that can ultimately defeat this ***,we provide a concise review on lessons learned from past stromatargeting strategies,new challenges revealed from recent preclinical and clinical studies,as well as new prospects in the treatment of PDAC.

BACKGROUND:Pyogenic liver abscess(PLA)is commonly seen in patients in the Far ***,hepatobiliary neoplasms are also *** as the initial manifestation of hepatobiliary neoplasms is extremely rare. METHOD:Three patients with PLA were identified from the ward registion files and were retrospectively reviewed. RESULTS:These patients(two men aged 74 and 80 years, and one woman aged 35 years)were treated(in the right lobe in two patients and in the left lobe in one)and subsequently followed up for 11-22 *** patients were diagnosed with hepatocellular carcinoma and one with metastatic *** patients were subjected to biopsies showing features of inflammatory pseudotumor before a diagnosis of hepatocellular *** patient underwent hepatic resection with good *** the other two patients who declined any further interventions,one died a few weeks after diagnosis and the other is still alive on conservative treatment. CONCLUSIONS:This series of cases highlights the importance of considering underlying hepatobiliary malignancies in patients with PLA and a close follow-up necessary for non-resolving hepatic lesions.

BACKGROUND:Gallstone disease is common,and complications that are frequently encountered include acute cholecystitis and acute pancreatitis,but rarely gallbladder perforation. METHOD:Data were retrospectively collected from clinical case notes and a literature review is presented. RESULTS:A 72-year-old lady presented with spontaneous gallbladder perforation,pericholecystic abscess and cholecystoduodenal fistula as the first manifestations of gallstone *** was previously well and had no abdominal *** condition was successfully managed with initial antibiotic therapy followed by interval cholecystectomy and fistula repair. CONCLUSIONS:Our case highlighted some uncommon but severe complications which occurred simultaneously as the first manifestations of previously asymptomatic gallstone *** complications need to be considered in patients suspected of intra-abdominal sepsis,even when there are no characteristic symptoms.

Nasopharyngeal carcinoma(NPC)is a malignant epithelial tumor ubiquitously associated with the Epstein-Barr virus(EBV),which is highly prevalent in South China,Southeast Asia,and North *** being a highly radio-sensitive and treatable cancer,a majority of NPC patients are diagnosed in their advanced stage,and locoregional and distant relapses following definitive treatment contribute largely to cancer-specific mortality among these *** that EBV-driven NPC is the predominant variant seen in endemic regions,various EBV detection methods have been developed and are utilized in screening,prognostication,and post-treatment surveillance of NPC *** the Immunoglobulin A(IgA)serology assay is the most extensively studied EBV detection method,the detection of plasma EBV DNA released during replication or cellular apoptosis has shown superior outcomes in endemic population screening,prognostication,and detection of distant ***,there is emerging evidence on the use of circulating tumor cells,microRNAs,DNA hypermethylation,and combination assays in various clinical ***,this paper provides a comprehensive overview of the relevant studies using various EBV detection techniques in the management of ***,the recent advances,clinical evidence,and challenges associated with the clinical application of EBV liquid biopsies in population screening,prognostication,and surveillance of NPC are presented.

介绍新加坡中央医院针灸门诊的规范化管理。通过对门诊基础建设逐渐改进,医务人员再培训,操作过程严格消毒,医疗仪器定期检查,病历书写、保存和处理统一管理,开展公众中医知识教育等,逐渐形成了一套与现代化医院相适应的规范化管理制度,使临床治疗效率和质量不断提高。自1998年4月至2012年12月,治疗患者74 654人次,日诊量渐增,避免了针灸异常状况的发生,多次通过国内外国际医疗卫生机构认证联合委员会定期的严格审查。此外,针灸门诊还承担了3项新加坡健保集团的科研项目。
