A cadaveric model for transesophageal echocardiography transducer placement training: A pilot study

BACKGrOUND: Transesophageal echocardiography(TEE) is used in the emergency department to guide resuscitation during cardiac arrest. Insertion of a TEE transducer requires manual skill and experience, yet in some residency programs cardiac arrest is uncommon, so some physicians may lack the means to acquire the manual skills to perform TEE in clinical practice. For other infrequently performed procedural skills, simulation models are used. However, there is currently no model that adequately simulates TEE transducer insertion. The aim of this study is to evaluate the feasibility and efficacy of using a cadaveric model to teach TEE transducer placement among novice ***: A convenience sample of emergency medicine residents was enrolled during a procedure education session using cadavers as tissue models. A pre-session assessment was used to determine prior knowledge and confidence regarding TEE manipulation. Participants subsequently attended a didactic and hands-on education session on TEE placement. All participants practised placing the TEE transducer until they were able to pass a standardized assessment of technical skill(SATS). After the educational session, participants completed a post-session assessment. rESULTS: Twenty-five residents participated in the training session. Mean assessment of knowledge improved from 6.2/10 to 8.7/10(95% confidence interval [CI] of knowledge difference 1.6–3.2, Proved from 1.6/5 to 3.1/5(95% CI of confidence difference 1.1–2.0, Pre was no relationship between training level and the delta in knowledge or ***: In this pilot study, the use of a cadaveric model to teach TEE transducer placement methods among novice users is feasible and improves both TEE manipulation knowledge and confidence levels.

Novel effective treatment is direly needed for patients with pancreatic ductal adenocarcinoma(PDAC).Therapeutics that target the driver mutations,especially the KrAS oncoprotein and its effector cascades,have been *** is increasing clear that the extensive fibro-inflammatory stroma(or desmoplasia)of PDAC plays an active role in the progression and therapeutic resistance of *** desmoplastic stroma is composed of dense extracellular matrix(ECM)deposited mainly by the cancer-associatedfibroblasts(CAFs)and infiltrated with various types of immune *** dense ECM functions as a physical barrier that limits tumor vasculatures and distribution of therapeutics to PDAC *** addition,mounting evidence have demonstrated that both CAFs and ECM promote PDAC cells aggressiveness through multiple mechanisms,particularly engagement of the epithelial-mesenchymal transition(EMT)*** of a mesenchymal-like phenotype renders PDAC cells more invasive and resistant to therapyinduced ***,we critically review seminal and recent articles on the signaling mechanisms by which each stromal element promotes EMT in *** discussed the experimental models that are currently employed and best suited to study EMT in PDAC,which are instrumental in increasing the chance of successful clinical translation.

High-performance polymers(HPPs)are increasingly used in different industrial machinery components,particularly for rubbing parts that demand reliable and durable operation at extreme sliding conditions such as elevated temperature environments,where the use of conventional lubricants is not *** current study investigates the role of environmental temperature on the tribological properties of three advanced HPPs,namely aromatic thermosetting copolyester(ATSP),polyimide(PI),and polyether ether ketone(PEEK)based polymer *** experiments were carried out at different environmental temperatures from room temperature up to 300℃,and under dry sliding *** attention was given to the role of temperature on the development of transfer films and its subsequent effect on the tribological *** ATSP composite was recommended as the best performing material,based on the overall tribological performance at all examined operating temperatures.

Polymer transfer film formation from cryogenic to elevated temperatures

This study reports on the tribological performance of aromatic thermosetting co-polyester(ATSP)and polyether ether ketone(PEEK)-based polymer composite coatings mixed with PTFE *** coatings were tested across a wide temperature range from−180 to 110℃ to simulate the environmental temperatures on Titan,Moon,and Mars,which are of particular interest for NASA’s future exploratory *** experimental setup was developed to conduct the pin-on-disk experiments under dry sliding conditions and extreme temperature and contact *** film formation and its characteristics were found to play significant roles in the tribological performance,and the characteristics of the film were *** XPS and SEM analysis indicated the increase of the PTFE content in the transfer film as the temperature decreased to cryogenic *** coefficient of friction did not follow a linear trend with temperature and was minimum at 110℃ and maximum at−180℃.ATSP coating showed superior performance with lower friction and unmeasurable wear at all temperatures,whereas PEEK coating exhibited maximum wear at 25℃ followed by−180,and 110℃.

BACKGrOUND:Pyogenic liver abscess(PLA)is commonly seen in patients in the Far ***,hepatobiliary neoplasms are also *** as the initial manifestation of hepatobiliary neoplasms is extremely rare. METHOD:Three patients with PLA were identified from the ward registion files and were retrospectively reviewed. rESULTS:These patients(two men aged 74 and 80 years, and one woman aged 35 years)were treated(in the right lobe in two patients and in the left lobe in one)and subsequently followed up for 11-22 *** patients were diagnosed with hepatocellular carcinoma and one with metastatic *** patients were subjected to biopsies showing features of inflammatory pseudotumor before a diagnosis of hepatocellular *** patient underwent hepatic resection with good *** the other two patients who declined any further interventions,one died a few weeks after diagnosis and the other is still alive on conservative treatment. CONCLUSIONS:This series of cases highlights the importance of considering underlying hepatobiliary malignancies in patients with PLA and a close follow-up necessary for non-resolving hepatic lesions.

Enhancing endothelial progenitor cell for clinical use

Circulating endothelial progenitor cells(EPCs) have been demonstrated to correlate negatively with vascularendothelial dysfunction and cardiovascular risk factors. However, translation of basic research into the clinical practice has been limited by the lack of unambiguous and consistent definitions of EPCs and reduced EPC cell number and function in subjects requiring them for clinical use. This article critically reviews the definition of EPCs based on commonly used protocols, their value as a biomarker of cardiovascular risk factor in subjects with cardiovascular disease, and strategies to enhance EPCs for treatment of ischemic diseases.
