Molecular interactions of a representative pyrrolidinium-based ionic liquid 1-butyl-l-methyl-pyrrolidinium bis（triflorosulfonyl）- imide （[BMPyrr][TFSI]） with dimethyl sulfoxide （DMSO） and acetonitrile （AN） have been analyzed in this work. Attenuated total reflection Fourier transform infrared spectroscopy （ATR-FTIR） and density functional theory （DFT） calculations are used in the investigation, while excess infrared spectra and two-dimensional correlation spectroscopy are used to explore the data in detail. It has been found that the molecular solvents can interact with TFSI- （mainly with S=O and weakly with S-N-S group）. AN interacts feebly with BMPyrr＋ as compared with the strong interaction of DMSO. The strength of the interactions depends on the electron donating ability of the solvent. Upon mixing, hydrogen bonds regarding C-Hs in cation and S-N-S in anion are weakened, while that regarding S=O in anion is strengthened. Among the C-Hs which are connected directly with the N of the cation, Cl-H is the main interaction site for both DMSO and AN. This means that Cl-H is the most acidic hydrogen in pyrrolidinium cation.

A number of orthopedic injuries can occur during epileptic seizures. Anterior shoulder dislocation is one such orthopedic injury that is quite rare. The shoulder dislocation may injure the brachial plexus. Besides seizures, the nerves can also be damaged by anticonvulsive therapy. Muscle wasting following a seizure can misguide a clinician to investigate only neural or muscular pathologies. We report here an individual with epilepsy who was referred to us for electrodiagnostic evaluation of proximal muscle wasting related to a suspected proximal neuropathy. He was found to have a normal electrodiagnostic evaluation and later on discovered to have had bilateral shoulder dislocation on X-rays. This report advocates a thorough clinical appraisal, radiographs, and electrodiagnostic evaluation in a case with muscle wasting following a seizure.

We present a case of Retinal Cavernous Haemangioma treated with Intravitreal Bevacizumab, which was initially labelled as persistent proliferative diabetic retinopathy with multiple episodes of vitreous haemorrhage. These lesions can be confused with new retinal vessels in diabetics and if correctly diagnosed unnecessary photocoagulation can be avoided. Our patient received a course of three intravitreal Bevacizumab injections (1.25 mg/0.05 ml) in order to stop the leakage from the retinal cavernous haemangioma lesions and prevent another episode of vitreous haemorrhage. No intraoperative or postoperative complications were seen. Twenty-two months following treatment there was no recurrence of vitreous haemorrhage.

This study presents the intercomparison of the outdoor environmental gamma dose rates measured using a Nal （TI） based survey meter along with thermoluminescent dosimeters （TLDs） and estimation of excess lifetime cancer risk （ELCR）, for the inhabitants of Poonch division of the Azad Kashmir, Pakistan. CaF2： Dy （TLD-200） card dosimeters were installed at height of 1 m from ground at fifteen different locations covering the entire Poonch division comprising of three districts.

A ventriculoperitoneal shunt(VPS) is a medical device used to deflect cerebrospinal fluid from cerebral ventricles to the peritoneal cavity. Since 1908, ventriculoperitoneal shunts have been considered the best option in hydrocephalus management. Manifestation related to VPS may arise anywhere whilst tunneling the VPS from the ventricle to the peritoneal cavity, but complications associated with the distal catheter are especially common and a leading cause of revision surgeries. Among the many complications associated with peritoneal shunts, migration of the catheter into other parts of the body is still common. Scrotal migration mainly takes place in children aged 4 days to 5 years. Patent processus vaginalis, formation of an inguinal hernia, and increased intraabdominal pressure are leading risk factors associated with migration of the peritoneal catheter into the scrotum. Thoracic migration of peritoneal catheters predominantly occurs in adult patients. Important mechanisms in thoracic migration include(1) iatrogenic perforation of the external or internal jugular veins by the surgeon during tunneling and(2) gradual destruction of the venous structures by the catheter due to routine extension and flexion of the neck. After entering the vessel, negative intrathoracic pressure combined with positive intraabdominal pressure aids migration into the thoracic cavity. Sharp tips or shunt types contribute to intraabdominal migration and perforation.

Bypass surgery for ischemic stroke caused by intracranial artery stenosis or occlusion

Surgical revascularization may be beneficial in patients with ischemic stroke caused by intracranial stenosis or occlusion who are ineligible for thrombolysis. Objective: To evaluate the outcome of superficial temporal artery to middle cerebral artery(STA-MCA) bypass in ischemic stroke caused by intracranial artery stenosis or occlusion. Methods: We retrospectively studied successive case series of 19 patients who underwent surgical treatment between 2013–2017 of STA-MCA bypass. Surgical procedure was performed for the patients with acute ischemic stroke who were ineligible for thrombolysis. Results: Of the 19 patients enrolled, symptom aggravation occurred during medical treatment, the patients were ineligible for thrombolysis despite being within 8 hours of symptom onset. Bypass significantly improved National Institutes of Health Stroke Scale scores, mean patient age was 78.05 years(range, 39-78 y). However, male 11(57.95%)out of nineteen patients were presented with left-sided-lesions while female 8(42%) had right-sided lesions with significant infarction growth by diffusion weighted imaging achieved, after surgical maneuver. No major complications occurred intraoperatively, in contrast to 2(10.5%) minor manifestation were suffering minor complications probably they included the remote infarction(posterior cerebral artery territory). Pooled analysis with our patients showed a significant neurological improvement and a good outcome in 13(68.4%) patients without hemorrhage or any other complication, 6(31.6%) patients with unfavorable outcome(severe disability 2; vegetative state 4, non of them are died 0;). Conclusion: STA-MCA bypass may be beneficial to patients with acute stroke or stenosis in progress who are ineligible for medical therapy. Furthermore, it appears safe when the infarction is small. 背景:腹腔镜辅助下置入脑室腹腔分流(VPS)导管的方法已被广泛实施,目前,有多种放置腹腔中VPS导管远端的放置方法。最近,肝脏的镰状韧带已被用作远端分流的支撑物,而不再夹持或连接导管。然而,尽管分流方式不断改善,腹部并发症,如感染,迁移,梗阻/阻塞,穿孔和假性囊肿仍然是非计划再手术的主要原因。为克服远端分流相关问题,我们对前述“镰状技术”进行了改进。方法:2013年6月至2017年3月期间,共有55例患者进行VPS,32例采用镰状韧带固定技术,23例采用开放技术。开放技术是腹腔远端导管的传统钝性插入方法,而在镰状韧带固定技术中,插入腹腔内导管之前,在导管用丝线结扎固定。在镰状韧带钻孔,将结扎线作为固定点,将远端导管固定于肝上间隙。比较镰状韧带固定技术组和开放技术组中的术前临床特征如年龄、性别、脑积水类型、脑积水病因、术后并发症如术后感染、术后梗阻、脑脊液引流及非计划再手术的发生率。结果:术前资料如年龄,性别,脑积水类型及脑积水病因在改善的镰状韧带固定技术组和开放技术组之间无统计学差异。在术后脑脊液引流量上两组差异有统计学意义(P= 0.036),开放技术组中,脑脊液引流量过多和脑脊液过少发生率分别为9%和4%,而在改良镰状韧带固定技术组中,未出现脑脊液引流量过多或过少的病例。在其他术后并发症如术后感染、术后梗阻、非计划再手术及非计划再入院方面、改善的镰状韧带固定技术组发生率均小于开放技术组,但两组间差异未达到统计学差异。结论:应用改良的镰状技术将导管远端置于肝上间隙能够显著减少诸如感染、梗阻、脑脊液引流量过多或过少以及VPS修正手术并发症的发生。然而,也需进一步改善以减少上腹部不适。改良的镰状技术是将导管远端置于肝上间隙的合理有效的方法,因此,认为改良的镰状技术是脑积水患者较优的选择。
