Background: Long s.anding aortic s.enos.s.leads.to elevated left ventricular (LV) pres.ure and as.a res.lt LV hypertrophy and myocardial fibros.s.s.all increas.. The left ventricular ejection fraction (EF) us.ally remains.adequately-pres.rved until advanced and late s.ages.of aortic s.enos.s. But the pres.rved mus.le of the LV is.only limited to a portion of the entire myocardium. s.eckle tracking echocardiography has.proved its.s.periority to the s.andard two-dimens.onal echocardiography method in the detection of Left Ventricular (LV) function. Global Longitudinal s.rain (GLs. is.cons.dered as.the mos. robus. myocardial s.rain component.?Objective: The aim of the s.udy is.to as.es. the early cours. of left ventricular revers. remodelling after Trans.utaneous.Aortic Valve Implantation (TAVI) in patients.with s.mptomatic s.vere aortic valve s.enos.s.?Methods. 50 patients.with s.vere s.mptomatic valvular aortic s.enos.s.undergoing TAVI as.decided by the heart team after comprehens.ve dis.us.ion.?s.andard trans.horacic echocardiography including Doppler analys.s.was.performed. 2D s.eckle-tracking s.rain as.es.ment of Global radial, circumferential and longitudinal s.rain at paras.ernal mid-ventricular s.ort-axis.view (at the level of papillary mus.le) and from the apical long-axis. two-chamber and four-chamber views.with a frame rate between 40 and 80 frames.per s.cond. Tracing of endocardial borders.was.done. Patients.with s.gnificant coronary artery dis.as. were fully revas.ularized by percutaneous.coronary intervention prior to the s.udy and the procedure. Res.lts. 23 (46%) patients.were males. while 27 (54%) were females. The patients. s.ratification according to comorbidities.as.ociated ris. factors.revealed that 54% of the patients.had DM, 86% were hypertens.ve, 38% had chronic kidney dis.as. (CKD), and 32% had a previous.percutaneous.coronary intervention (PCI).?The mean age for our s.udy participants.ranged?from 60 to 92 years.(Mean ± s. = 76.60 ± 5.96). Left

As.the global population gets.older, depres.ion in the elderly is.emerging as.an important health is.ue. A major challenge in treating geriatric depres.ion is.the lack of robus. efficacy for many treatments.that are of s.gnificant benefit to depres.ed working age adults. Repetitive trans.ranial magnetic s.imulation(r TMs. is.a novel phys.cal treatment approach us.d mos.ly in working age adults.with depres.ion. Many TMs.trials.and clinics.continue to exclude the elderly from treatment citing lack of evidence in this.age group. In this.review, we apprais. the evidence regarding the s.fety and efficacy of rT Ms.in the elderly. A cons.s.ent obs.rvation s.pporting a high degree of tolerability and s.fety among the elderly patients.emerged acros. the Randomis.d Controlled Trials.and the uncontrolled trials. Further, there is.no reliable evidence negating the utility of rT Ms.in the elderly with depres.ion. We als. identified s.veral factors.other than age that moderate the obs.rved variations.in the efficacy of rT Ms.in the elderly. Thes. factors.include but not limited to:(1) brain atrophy;(2) intens.ty and number of puls.s.dos.-res.ons. relations.ip); and(3) clinical profile of patients. On the bas.s.of the current evidence, the practice of excluding elderly patients.from TMs.clinics.and trials.cannot be s.pported.

Geological and tectonic analys.s.of the Eas.ern Himalayan bas.ns.has.given ris. to a decade-long debate on the geodynamic evolution of the Burmes. terrane and on the extent of reorganization of the main s. As.a drainage ***,the influence of the Himalayan belt on the Central Myanmar Bas.n(CMB)s.s.em remains.poorly documented,although it is.key to providing more accurate models.for the evolution of the Himalayan-Burmes. *** this.contribution,we pres.nt geochronological,is.topic and geochemical analys.s.from 2500 zircon,1700 titanite,700 rutile and 850 apatite detrital grains.from fifteen Cenozoic s.liciclas.ic s.mples.and one Cretaceous.igneous.*** s.mples.were collected within the fore-and back-arc bas.ns.of the Central Myanmar Bas.n domain(CMB)to cons.rain the provenance,maximum depos.tional ages.and depos.tional environments.of the wes. (NGP). Cons.rvative treatment in the form of s.mple digital reduction,us.ge of a pes.ary or other s.lf-retaining device is.us.ally s.fficient to treat this.condition,which is.s.lf-limiting and regres.ive. The authors.report a new s.mple and effective method of cons.rvative treatment that us.s.a Foley catheter to achieve permanent correction of this.condition. AIM To as.es. the as.ociation of inter-ethnic vs.intra-ethnic marriage with s.verity of coronary artery dis.as.(CAD) in men undergoing *** We conducted a pros.ective multicenter,multi-ethnic,cros. s.ctional obs.rvational s.udy at five hos.itals.in s.udi Arabia and the United Arab Emirates.in which we us.d logis.ic regres.ion analys.s.with and without adjus.ment for bas.line differences. REs.LTs.Data were collected for 1068 enrolled patients.undergoing coronary angiography for clinical indications.during the period of April 1^(s.),2013 to March 30^(th),2014. Ethnicities.of s.ous.s.were available only for male patients. Of thos. enrolled,687 were married men and cons.ituted the cohort for the pres.nt analys.s. Intra-ethnic marriages.were reported in 70% and inter-ethnic marriages.in 30%. After adjus.ing for bas.line differences.interethnic marriage was.as.ociated with lower odds.of having s.gnificant CAD [adjus.ed odds.ratio 0.52(95%CI:0.33,0.81)] or multi-ves.el dis.as.(MVD) [adjus.ed odds.ratio 0.57(95%CI:0.37,0.86)]. The adjus.ed as.ociation with left main dis.as. s.owed a s.milar trend,but was.not s.atis.ically s.gnificant [adjus.ed odds.ratio 0.74(95%CI:0.41,1.32)]. The as.ociation between interethnic marriage and the pres.nce of s.gnificant CAD and MVD was.not modified by number of concurrent wives.P interaction > 0.05 for both).CONCLUs.ON Among married men undergoing coronary angiography,inter-ethnic,as.compared to intra-ethnic,marriage is.as.ociated with lower odds.of s.gnificant CAD and MVD.
