Opinions and Perceptions of Gynecologists and Midwives on the Use of the Intrauterine Device in Senegal

Objective: The purpose of this study was to analyze the opinions of gynecologists, midwives, and interns/Specialist students practicing in Senegal about the use of IUD, and to assess factors that stand as obstacles to IUD prescribing. Patients and methods: An online questionnaire was developed to assess providers’ IUD practices and attitudes. The questionnaire was based on the one used in a Swiss study on the same topic. The platform used was Google forms. The questionnaire was shared in associative platforms involving gynecologists, midwives and interns/Specialist students. Results and comments: We received 292 feedbacks. Gynecologists represented 13.7% of the sample, interns and specialist students 11.3% and midwives 76%. Parity is a determining factor in IUD selection. The care-providers were concerned about the followings, which might have prevented the use of IUD, even though it was indicated: infections (75.7%), pelvic pain (61.3%), expulsion (48.3%), increased risk of perforation (46.9%), women’s appreciation (45.2%), ectopic pregnancy (33.9%) and sexual behavior (28.4%). Epidemiological, Clinical Aspects and Treatment of Victims of Sexual Abuse in the Gynecology and Obstetrics Department of Pikine National Hospital in 183 Cases

Introduction: Sexual abuse or sexual assault is part of violence against women, as is physical violence, female genital mutilation and forced marriage. According to the Criminal Code, this is any sexual offense committed with violence, coercion, threat or surprise on the person of others. It is a criminal act punishable by the laws in force in almost every country in the world. Study Objectives: 1) draw up an epidemiological profile of female victims of sexual abuse;2) develop the treatment protocol. Methodology: This is a 7-year descriptive retrospective study from January 1, 2010 to December 31, 2016 at the Center Hospitalier National de Pikine (Dakar). Included in this study were all of the patients admitted during our investigation period who were the victims of suspected or certain sexual abuse, with or without requisition. For each case, the following parameters were studied: the epidemiological characteristics (age, gestation, parity and place of residence), the circumstances of the attack (time and place), the characteristics of the aggressor (age, link with the victim, number of aggressor), the type of sexual contact, the possible lesions found during the physical examination (genital and extra-genital), the mode of admission, the delay of the consultation, the attitude after the sexual abuse, the gynecological and obstetrical status of the victim, the repercussions and psychological follow-up, the paraclinical assessment, the treatments administered. Data were collected using a survey form and analyzed with SPSS software. Results: During the study period, 183 victims of sexual abuse were received and treated at the level of our structure out of a total of 39,760 patients, representing a frequency of 0.46%. The age of the victims varied between 2 and 36 years with an average of 13 years. The 11 to 15 age group was the most represented. Students were the most represented among victims of sexual abuse (87.4%). Workers represented 9.3% of the victims. Infants

Surgical Treatment of Relapsed Clubfoot Using the Ilizarov Method: Anatomical and Functional Outcomes—A Report of 16 Cases at the Order of Malta’s Hospital Center

Introduction: Relapsed Congenital Talipes Equino Varus (CTEV) refers to clubfoot diagnosed after walking age. We refer to this entity as residual deformities of a previously treated clubfoot or a complete recurrence following treatment. For its treatment, several techniques have been used to improve the foot’s functional outcome while remaining minimally invasive. Correction by distraction using the Ilizarov external fixator is a promising technique. This single-center study aims to evaluate the anatomical and functional outcomes of treatment by distraction and compression with the Ilizarov fixator in relapsed club feet. Patients and Method: We carried out a retrospective, single-center study over 5 years. In total 16 clubfeet in 14 patients, including 9 males and 5 females were retained for final analysis. The average age of the patients at the time of the intervention was 15.16 years. K-wires were placed using the original technique. The average duration of the fixator in our series was 4.27 months. The functional assessment of patients was based on pre-and post-operative evaluation of the Foot and ankle function according to the AOFAS score. Results: Anatomic assessment was done according to the Dimeglio classification. There was a significant improvement at post-operative period with an average score of 4.47 against 10.07 at pre-operative period. We obtained 43.8% good and very good functional outcomes according to the global AOFAS score increasing from 63.07 at pre-operative period to 77.88 at post-operative period. Two cases of reinterventions were recorded. Conclusion: Distraction with the Ilizarov external fixator occupies a place of choice in the management of relapsed club foot. A good mastery of the technique and patient selection produce good functional outcomes.

Comparative Characteristics of Hydrated Lime with Fine Sewage Sludge Ash (FSSA) and Coal Fly Ash (CFA)

The disposal of waste has become an environmental issue due to the limited available landfilling space. This paper aims to compare the characteristics of hydrated lime with fine sewage sludge ash (FSSA) and coal fly ash (CFA). Multiple techniques, X-ray fluorescence (XRF), X-ray diffraction (XRD), the Fourier transform infrared (FTIR), compressive strengths, thermophysical properties, and setting time were used to assess the physicochemical characteristics of the lime-based materials. X-ray fluorescence and X-ray diffraction were used to determine the chemical composition and phases of ashes, lime and binders. The results showed that the chemical composition of ashes is similar to that of cement. Besides glass, the main minerals identified in CFA and FSSA are quartz (SiO2) and anhydrite (CaSO4). Moreover, calcium aluminium oxide (Ca3Al2O6) was detected for CFA and phosphorus calcium silicate (Ca2SiO4-Ca3(PO4)2) for FSSA and minor phases were detected for both. FTIR measurements were carried out to characterize the inorganics components of different samples. Compressive strengths of mortars with different formulations have shown that both have a long-term positive effect which might be related to a pozzolanic activity. For the CFA the L3 binder consisting of 60% of coal fly ash and 40% lime has a higher compressive strength than the others while for the FSSA the L4 binder consisting of 80% fine ash and 20% lime has a higher compressive strength than the others. Both binders setting start times are greater than that of cement but shorter than that of lime. The study of the thermophysical properties of binders shows that they have a higher thermal resistance than cement mortar. Sociodemographic Factors Associated with Delays in Breast Cancer

Objectives: The aim of our study was to investigate the relation of some sociodemographic factors and delays presentation in breast cancer. Patients and method: This was an observational study in Breast Unit of Aristide Le Dantec Teaching Hospital in Dakar. Were included in the study all patients followed for breast cancer from January 2008 to December 2014. The following variables were selected and stratified: age, place of living, educational attainment, menopausal status, socioeconomic status and family history of breast cancer. For each variable, an association with the stage and presentation delay was assessed. Results: Two hundred and fifty nine (259) patients met the inclusion criteria. No correlation was found between the studied socio-demographic factors and delay. Negative history of family breast cancer was associated with presentation delay. The only factor associated with locally advanced breast cancer after adjusting confounding factors was low level of income. Conclusion: A significant percentage of women with breast cancer in Senegal are experiencing presentation delay. Coordinated efforts with public health department are needed to educate the focused groups and mitigate the barriers.

Knee Osteoarthritis Progression after Distal Femur Closing Wedge Osteotomy

Background: Despite the conservative treatment of tibio-femoral osteoarthritis through realignment osteotomies, the rate of total knee replacements following an osteotomy is increasing. The aim of this study was to identify the factors associated with the progression of knee osteoarthritis after a medial closing-wedge distal femoral osteotomy. Methods: Hospital-based observational study on 20 patients who underwent a medial closing-wedge distal femoral osteotomy evaluating the progression of osteoarthritis using the Kellgren and Laurence classification. The Wilcoxon test was used to compare the variation in the progressive stage of the Kellgren and Laurence classification of knee osteoarthritis preoperatively and at the final follow up. Univariate analysis made it possible to determine the factors associated with progression. The final significance threshold for statistical tests was set at 5% (p Results: Overall, the mean follow-up of 46 months ± 6.6 months, with a mean age of 43 years (range: 27 - 69 years) and a female predominance (M: F = 3/7). The progression of tibiofemoral osteoarthritis following a medial closing-wedge distal femoral osteotomy is associated with valgus or varum malalignment been a moderate valgus (OR 6.2 [1.5 - 42.7] at 95% CI;p-value = 0.02), a correction of the mechanical deviation angle with a valgus alignment (OR 2.7 [0.9 - 8.3] at 95% CI), and loss of correction (OR 3.8 [1.3 - 11.6] at 95% CI;p -value) for the lateral compartment while varus alignment (OR 1.7 [0.9 - 8.3] 95% CI, p-value = 0.05) and with rupture of the lateral cortex (OR 2.8 [1.7 - 11.5] 95% CI, p-value = 0.02) were those of the medial compartment. Conclusion: Distal femur closing wedge osteotomy does not definitively interrupt the progression of valgus knee osteoarthritis. The factors associated with the progression of this pathology are modifiable. Taking them into account when performing this surgical technique could improve the osteotomy survival curve.

A Hospital-Based Cross-Sectional Study Assessing the Relation between Time of Birth and Perinatal Outcome

Objectives:?Investigating the relation between perinatal outcomes and?hospital working shifts.?Methods:?We conducted a cross-sectional study at Philippe Maguilen Senghor health center (PMSHC) in Dakar, Senegal from January, 1st?2011 to December, 31th 2018. The study population was comprised of all mothers who had delivered at PMSHC and their newborns after completing 22 weeks of gestation. Time of delivery was?divided into three periods of working hours: morning shift (deliveries occurred between 7 am and 4:59 pm);evening shift from 5 pm to 10:59 pm and night shift from 11?pm to 6:59 am.?Maternal outcomes were assessed by mode of delivery, epsisotomy and perineal injuries.?The Apgar scoring system was used to assess newborns at first minute after they were born. Other adverse perinatal outcomes included fresh stillbirth, neonatal referral and early neonatal death. Data were analyzed using Statistical Package for Social Science software (SPSS 24, Mac version).?Results:?A total of 48,270 mothers and their newborns met eligibility criteria. Caesarean section deliveries were less likely to occur during evening (OR 0.84, 95% CI;0.79?-?0.89, p = 0.001) and night shifts (OR 0.45, CI;0.47?-?0.53, p = 0.001).?Evening shift deliveries had 1.1 the odds of poor perinatal outcome (Apgar score ?- 1.18, p = 0.012). No significant difference was found in the odds of neonate referrals and deaths across the three shifts.?Night shift deliveries had 1.1 the odds of perineal injuries compared to morning shift deliveries (OR 1.11, 95% CI;1.04?- 1.18, p = 0.001, for episiotomy and OR, 1.14;95% CI, 1.04?- 1.26, p = 0.008, for perineal tears). Conclusion:?Off-hours deliveries, particularly during the night shift, were significantly associated with higher proportions of perineal injuries compared to morning shift.?However, no significant difference was found in the odds of neonate referrals and deaths across the three shifts.?Our findings suggest to set up a Neonatology unit at the CSPMS as
