Paracentesis in cirrhotics is associated with increased risk of 30-day readmission

AIM To determine the readmission rate, its reasons, predictors, and cost of 30-d readmission in patients with cirrhosis and *** A retrospective analysis of the nationwide readmission database(NRD) was performed during the calendar year 2013. All adults cirrhotics with a diagnosis of ascites,spontaneous bacterial peritonitis, or hepatic encephalopathy were identified by ICD-9 codes. Multivariate analysis was performed to assess predictors of 30-d readmission and cost of *** Of the 59597 patients included in this study, 18319(31%) were readmitted within 30 d. Majority(58%) of readmissions were for liver related reasons. Paracentesis was performed in 29832(50%) patients on index admission. Independent predictors of 30-d readmission included age 3 elixhauser comorbidity(OR: 1.13; CI: 1.05-1.22), nonalcoholic cirrhosis(OR: 1.16; CI: 1.10-1.23), paracentesis on index admission(OR: 1.28; CI: 1.21-1.36) and having hepatocellular carcinoma(OR: 1.21; CI: 1.05; 1.39). Cost of index admission was similar in patients readmitted and not readmitted(P-value: 0.34); however cost of care was significantly more on 30 d readmission($30959 ± 762) as compared to index admission($12403 ± 378), P-value: < *** Cirrhotic patients with ascites have a 33% chance of readmission within 30-d. Younger patients, with public insurance, nonalcoholic cirrhosis and increased comorbidity who underwent paracentesis are at increased risk of readmission. Risk factors for unplanned readmission should be targeted given these patients have higher healthcare utilization.

Evaluation of 3D-CRT and VMAT Radiotherapy Plans for Left Breast Cancer with Regional Lymph Nodes Irradiation

Introduction: Radiation therapy after breast surgery is an integral part of the treatment of early breast cancer. The goal of radiation therapy is to achieve the best possible coverage of the planning target volume (PTV), while reducing the dose to organs at risk (OARs) which are normal tissues whose sensitivity to irradiation could cause damage that can lead to modification of the treatment plan. In the last decade, radiation oncologist started to use the Intensity Modulated Radiotherapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT) for irradiating the breast, in order to achieve better dose distribution and target dose to the PTV and OAR. The aim of this study is to compare 2 external radiotherapy techniques (VMAT vs 3D) for patients with node-positive left breast cancer. Patients and Methods: We randomly selected 10 cases of postoperative radiotherapy for breast cancer in our hospital. The patients are all female, the average age was 45.4 years old, and the primary lesions are left breast. The ANOVA test was used to compare the mean difference between subgroups, and the p value Results: Dose volume histogram (DVH) was used to analyze each evaluation dose of clinical target volume (CTV) and organs at risk (OARs). Compared to 3DCRT plans, VMAT provided more uniform coverage to the breast and regional lymph nodes. The max point dose for tVMAT was lower on average (106.4% for VMAT versus 109% for 3DCRT). OAR sparing was improved with tVMAT, with a lower average V17Gy for the left lung (27.91% for VMAT versus 30.04% for 3DCRT, p and lower for V28Gy (13.75% for VMAT versus 22.34% for 3DCRT, p = 0.01). We also found a lower V35Gy for the heart on VMAT plan (p = 0.02). On the contrary, dose of contralateral breast was lower in 3DCRT than VMAT (0.59 Gy vs 3.65 Gy, p = 0.00). AIM: To assess the prevalence of nutritional disorders in children with inflammatory bowel disease(IBD) in Saudi ***: The data from a national cohort of children newly diagnosed with IBD between 2003 and 2012 were analyzed. The diagnosis of IBD and the differentiation between Crohn's disease(CD) and ulcerative colitis(UC) were confirmed by gastroenterologists according to the standard criteria. The body mass index(BMI) of each child [weight(kg)/height^2(m)] was calculated at the time of diagnosis. The World Health Organization standards and references were used and the BMI for age > +1 and ely. Age stratification analysis was performed to investigate any age-related variation in the prevalence of nutritional status between children ely, with a significantly higher prevalence of thinness in children with CD than in children with UC(P = 0.037) only in the age group of 10-17 years(P = 0.030). The prevalence of overweight was 16 %, 15% and 20 % in the children with IBD, CD and UC, respectively, indicating a higher prevalence in UC that was statistically significant only in the age group of 10-17 years(P = 0.020). CONCLUSION: A high proportion of children with IBD presented with overweight instead of the classical underweight. Awareness of this finding is important for patient care.

Spatiotemporal Distribution of Phlebotomine Sand Flies （Diptera： Psychodidae） in a Focus of Cutaneous Leishmaniasis in Foum Jamaa （Azilal, Atlas of Morocco）

An entomological survey of Phlebotomine sand flies （Diptera： Psychodidae） was carried out in three sectors of Foum Jamgta region （province of Azilal, Morocco） during the year 2010. Morphological identification was performed on a total of 1,152 sand flies （23% females and 77% males） collected by sticky paper traps. 80% of the total collected flies were identified as Phlebotomus （Paraphlebotomus） sergenti （Parrot）（57%） and Phlebotomus （Larroussius） longicuspis （Nitzulescu）（23%）. In addition to these dominant species, four other species were found, Phlebotomus （Phlebotomus） papatasi （Scopoli）, Sergentomyia （Sergentomyia） minuta （Rondani）, Phlebotomus （Larroussius） perniciosus （Newstead） and Phlebotomus （Paraphlebotomus） chabaudi （Croset）. Overall, the population dynamics show a yearly bimodal pattern related to rainfall and temperature, and with high density around human dwellings. The spatiotemporal distribution of sand fly species was helpful to discuss strategies that might be useful in controlling cutaneous leishmaniasis transmission in this endemic focus.

随着我国一带一路战略部署的推进,我国在一带一路相关国家承担的地质调查任务也在逐步增加。地球化学填图由于其可靠性高、精度好、进度可控等原因,往往是国外地质调查工作的重要一环。地球化学调查工作主要环节包括野外样本采集、样本分析、数据处理及成图等。由于数据处理及成图决定了地球化学填图工作中地球化学异常的圈定及地球化学分布的判定,其模型和方法的研究越来越受到地质科学家的重视,并成为地球化学填图工作中的重要一环。设计了基于ArcGIS建模器的数据处理模型,并将其用于地球化学数据处理。该模型不仅实现了从地球化学采样数据分析到地球化学成图的自动化,而且模型可以在科研人员和生产人员之间共享。模型实现了地球化学填图工作中的以下主要功能:(1)地球化学各元素含量数据的批量插值;(2)元素的分类及异常提取;(3)用栅格数据表示的元素含量数据转换为矢量的等值线/面并平滑。该模型已成功应用于中国商务部援摩洛哥东阿特拉斯山地区1∶10万低密度地球化学填图项目,应用于包含3个工作区45种元素共7 974个地球化学样品的数据处理。成功地解决了项目有效工期短、数据分析任务重、输出图件质量要求高等问题,充分显示了模型的高效实用性,可望在未来地质调查工作中发挥巨大作用。

Nonlinear Propagation of Positron-Acoustic Periodic Travelling Waves in a Magnetoplasma with Superthermal electrons and Positrons

The nonlinear propagation of positron acoustic periodic （PAP） travelling waves in a magnetoplasma composed of dynamic cold positrons, superthermal kappa distributed hot positrons and electrons, and stationary positive ions is examined. The reductive perturbation technique is employed to derive a nonlinear Zakharov Kuznetsov equation that governs the essential features of nonlinear PAP travelling waves. Moreover, the bifurcation theory is used to investigate the propagation of nonlinear PAP periodic travelling wave solutions. It is found that kappa distributed hot positrons and electrons provide only the possibility of existence of nonlinear compressive PAP travelling waves. It is observed that the superthermality of hot positrons, the concentrations of superthermal electrons and positrons, the positron cyclotron frequency, the direction cosines of wave vector k along the z-axis, and the concentration of ions play pivotal roles in the nonlinear propagation of PAP travelling waves. Tile present investigation may be used to understand the formation of PAP structures in the space and laboratory plasmas with superthermal hot positrons and electrons.

Annular pancreas producing duodenal obstruction: A case report

Annular pancreas is a rare congenital anomaly characterized by the presence of ectopic pancreatic tissue surrounding the duodenum. This malformation is usually asymptomatic in adults, but can manifests as pancreatitis, duodenal stenosis, or duodenal or gastric ulceration. We report the case of a young patient of 18 years old hospitalized for epigastric pain and vomiting, in whom radiological investigations showed an annular pancreas. At operation, a complete obstruction of the duodenum between its first and second parts was found, caused by an annular pancreas. No other congenital anomaly of the intra abdominal organs was noted. A gastroenterostomy was performed. Both the rarity of this congenital abnormality and its successful correction by surgical means have prompted us to make the following presentation.
