The impact of emergency department length of stay on the outcomes of trauma patients requiring hospitalization: a retrospective observational study

BACKGROUND: We aimed to explore the impact of the emergency department length of stay(EDLOS) on the outcome of trauma ***: A retrospective study was conducted on all trauma patients requiring hospitalization between 2015 and 2019. Patients were categorized into 4 groups based on the EDLOS(24 h). Data were analyzed using Chi-square test(categorical variables), Student’s t-test(continuous variables), correlation coefficient, analysis of variance and multivariate logistic regression analysis for identifying predictors of short EDLOS and hospital ***: The study involved 7,026 patients with a mean age of 32.1±15.6 years. Onefifth of patients had a short EDLOS(24 h were older(P=0.001) and had more comorbidities(P=0.001) and fewer deaths(P=0.001). Multivariate regression analysis showed that the predictors of short EDLOS were female gender, GCS, SI, hemoglobin level, ISS, and blood transfusion. The predictors of mortality were TTA-1(odds ratio [OR]=4.081, 95%CI: 2.364–7.045), head injury(OR=3.920, 95%CI: 2.413–6.368), blood transfusion(OR=2.773, 95%CI: 1.668–4.609), SI(OR=2.132, 95%CI: 1.364–3.332), ISS(OR=1.077, 95%CI: 1.057–1.096), and age(OR=1.040, 95%CI: 1.026–1.054). CONCLUSIONS: Patients with shorter EDLOS had different baseline characteristics and hospital outcomes compared with patients with longer EDLOS. Patients with prolonged EDLOS had better outcomes;however, the burden of prolonged boarding in the ED needs further elaboration.

Epilepsy is a common neurological disorder in neurology clinic. Levetiracetam is considered as one of common antiepileptic drugs used to manage epilepsy with good efficacy and tolerability profile. It is renally excreted and not depending on the cytochrome p450. It has adverse effects reported as somnolence, headaches, dizziness, depression and anxiety. also, it was reported that levetiracetam can cause Acute kidney injury (AKI), renal profile disturbance, that may be related to its way of excretion and possible nephrotoxicity especially with high loading dose. We are reporting a young female patient with epilepsy presented to hospital with status epileptcus and started on loading dose of levetiracetam 3 grams and then maintenance dose of 1 gram twice daily seizure were controlled but she developed acute kidney injury that improved after discontinue leveriracetam and medical management without renal dialysis and discharged home in stable condition. Physician and health care providers should be aware of such rare adverse reaction and available management options for better patient care and outcome.

There is a distinct lack of online atlases to visualize and explore cancer incidence in Middle Eastern countries despite the clear benefit that such tools can deliver. This paper describes the development and implementation of a cancer Atlas of Saudi Arabia, which is a web-based client-server application with built-in analysis functions for analyzing patterns of cancer incidence. Built using ESRI’s ArcGIS Server API and ***, the atlas contains 45,532 incidences of cancer for the period from 1998 to 2004, which were provided by the Saudi Arabian National Cancer Registry. This tool is aimed at health care practitioners and researchers, who can use this tool for exploring cancer distribution and investigating trends, and as a decision support tool for service allocation. The tool allows users to map cancer incidence and undertake analyses at four spatial scales from city to national level.

The purpose of this study is the accurate prediction of undrained shear strength using Standard Penetration Test results and soil consistency indices,such as water content and Atterberg *** this study,along with the conventional methods of simple and multiple linear regression models,three machine learning algorithms,random forest,gradient boosting and stacked models,are developed for prediction of undrained shear *** models are employed on a relatively large data set from different projects around Turkey covering 230 *** an improvement over the available studies in literature,this study utilizes correct statistical analyses techniques on a relatively large database,such as using a train/test split on the data set to avoid overfitting of the developed ***,the validity and consistency of the prediction results are ensured with the correct use of statistical measures like p-value and cross-validation which were missing in previous *** compare the performances of the models developed in this study with the prior ones existing in literature,all models were applied on the test data set and their performances are evaluated in terms of the resulting root mean squared error(RMSE)values and coefficient of determination(R^2).Accordingly,the models developed in this study demonstrate superior prediction capabilities compared to all of the prior ***,to facilitate the use of machine learning algorithms for prediction purposes,entire source code prepared for this study and the collected data set are provided as supplements of this study.

This paper investigates the problem of controlling half-vehicle semi-active suspension system involving a magnetorheological(MR)*** features a hysteretic behavior that is presently captured through the nonlinear Bouc-Wen *** control objective is to regulate well the heave and the pitch motions of the chassis despite the road *** difficulty of the control problem lies in the nonlinearity of the system model,the uncertainty of some of its parameters,and the inaccessibility to measurements of the hysteresis internal state *** Lyapunov control design tools,we design two observers to get online estimates of the hysteresis internal states and a stabilizing adaptive state-feedback *** whole adaptive controller is formally shown to meet the desired control *** theoretical result is confirmed by several simulations demonstrating the supremacy of the latter compared to the skyhook control and passive suspension.

Mobile computing has fast emerged as a pervasive technology to replace the old computing paradigms with portable computation and context-aware *** software systems can be migrated(while preserving their data and logic)to mobile computing platforms that support portability,context-sensitivity,and enhanced *** recent years,some research and development efforts have focused on a systematic migration of existing software systems to mobile computing *** investigate the research state-of-the-art on the migration of existing software systems to mobile computing *** aim to analyze the progression and impacts of existing research,highlight challenges and solutions that reflect dimensions of emerging and futuristic *** followed evidence-based software engineering(EBSE)method to conduct a systematic mapping study(SMS)of the existing research that has progressed over more than a decade(25 studies published from 1996–2017).We have derived a taxonomical classification and a holistic mapping of the existing research to investigate its progress,impacts,and potential areas of futuristic research and *** SMS has identified three types of migration namely Static,Dynamic,and State-based Migration of existing software systems to mobile computing *** to mobile computing platforms enables existing software systems to achieve portability,context-sensitivity,and high ***,mobile systems may face some challenges such as resource poverty,data security,and *** emerging and futuristic research aims to support patterns and tool support to automate the migration *** results of this SMS can benefit researchers and practitioners-by highlighting challenges,solutions,and tools,etc.,-to conceptualize the state-of-the-art and futuristic trends that support migration of existing software to mobile computing.

Lie symmetry method is applied to analyze a nonlinear elastic wave equation for longitudinal deformations with third-order anharmonic corrections to the elastic energy. Symmetry algebra is found and reductions to second-order ordinary differential equations （ODEs） are obtained through invariance under different symmetries. The reduced ODEs are further analyzed to obtain several exact solutions in an explicit form. It was observed in the literature that anharmonic corrections generally lead to solutions with time-dependent singularities in finite times singularities, we also obtain solutions which along with solutions with time-dependent do not exhibit time-dependent singularities.

BACKGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to *** typically presents as a rash on the face,neck,and upper torso after intravenous administration of *** is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been ***,we add another case of VFS developed after slow local absorption of vancomycin in cement *** SUMMARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the *** pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and *** patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl *** hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to ***,the rash disappeared after 96 h with no further *** VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our *** orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring.
