Informational Needs in Women Undergoing Multifraction High Dose-Rate Brachytherapy for Cervical Cancer: A Prospective Qualitative Study

Objective: To identify the informational needs of Moroccan women receiving intracavitary brachytherapy for locally advanced cervical cancer as part of a process to develop guidelines for quality patient-centered care. Methodology: A prospective, qualitative study with a phenomenological approach was carried out at the Brachytherapy unit of the National Institute of Oncology in Rabat, Morocco. Purposive sampling was used to recruit 31 patients undergoing high dose-rate brachytherapy for cervical cancer from July 2020 to August 2020. Semi-structured, one-to-one interviews were conducted by a female radiation oncologist in Arabic, guided by a theme list. The interviews were translated and a thematic analysis was performed. Results: Data saturation was achieved having interviewing 31 participants, aged 27 - 70 years. Findings on patients’ informational needs were the overarching theme and form the focus of this article. The informational needs included: providing patients with disease- and treatment-related information in their home language;adequate information concerning pre-treatment preparation, possible side-effects, and sexual intercourse;and providing patients with informative material adapted to their needs as standard procedure. Conclusion: This study has identified unmet women’s informational needs during brachytherapy for cervical cancer. Providing patients with sufficient and understandable information, adequate preparation before the procedure, more sensitive support during the procedure, and debriefing afterward could lessen feelings of fear and anxiety towards treatment delivery. Guidelines with a patient-centered approach could thus be developed to be used as a tool to assist members of multidisciplinary teams in providing quality care to this group of women.

Objective: The aim of this study was to establish cervical cancer patients’ expectations and experiences during high dose-rate (HDR) intracavitary brachytherapy procedure, as part of a process to develop guidelines for quality patient-centered care. Methodology: A prospective, qualitative study with a descriptive phenomenological approach was used. Purposive sampling was carried out to recruit 31 women undergoing HDR brachytherapy for cervical cancer from June to August 2020 at the National Institute of Oncology in Rabat. Semi-structured, one-to-one interviews guided by a theme list were conducted by a female radiation oncologist in Arabic before, during and after treatment. The following aspects were discussed: expectations, experiences in the waiting room, in the treatment room, and suggestions for improvement. Data was transcribed, translated and thematic analysis performed. Results: Most of the patients felt unprepared and did not have a clear understanding of brachytherapy. Brachytherapy was a difficult experience causing fear and anxiety throughout treatment. Most women dreaded the procedure, before receiving the first treatment and even after having had one. Pain was a major problem for the participants. Some women compared this pain to childbirth, a process they preferred to brachytherapy. Patients agreed that the preventative medication received was not efficient to relieve the pain. Despite these negative experiences, patients were left with a positive outlook. Dialogue with the healthcare professionals, support from their family and fellow patients, envisaged outcomes and desires to heal were used to cope, whilst faith and spirituality gave them strength to endure the procedure. Conclusion: Women undergoing uterovaginal brachytherapy for cervical cancer experience pain and emotional distress. Sarcomatoid Carcinoma of the Mandible: A Case Report and Literature Review

Background: Sarcomatoid carcinoma is a rare biphasic malignancy with an epithelial and mesenchymal component. Given its histological particularity, the diagnosis of this tumor still represents a challenge for pathologists and surgeons. Sarcomatoid carcinoma of the mandible is a very rare and aggressive entity. To date, only sporadic cases have been reported in the literature. Aim: To report a rare case of aggressive mandibular sarcomatoid carcinoma treated with radical surgery and adjuvant chemoradiation, and discuss the related literature. Case Report: We introduce a rare case of mandibular sarcomatoid carcinoma occurring in a 34-year-old woman. Treatment consisted of radical surgery and re-excision for positive margins, adjuvant radiation therapy (66 Gy in 33 fractions in simultaneous integrated boost delivered using Volumetric Modulated Arc Therapy) and Cisplatin-based concurrent chemotherapy. The tumor displayed aggressive behavior and high metastatic potential, causing tumour recurrence and extensive lung metastasis. Conclusion: Sarcomatoid carcinoma of the mandible is an extremely rare malignant entity, with a high tendency for local recurrence, distant metastasis. Treatment involves radical surgery and adjuvant chemoradiation, although the outcome and prognosis might be poor.

Hepatic hydatid cyst is a rare parasitic affection which constitutes a major health problem in countries endemic to hydatidosis. Rupture of hydatid cyst into gallbladder is an exceptional complication first described in 1952 by Atlas and Kamenear. We report the case of a hepatic hydatid cyst rupture into gallbladder in a 31-year-old patient with no history of disease.

Renewable energy is one of the essential elements of the social and economic development in any civilized *** use of fossil fuels and the non-renewable form of energy has many adverse effects on the most of *** the high potential of renewable energy sources in Yemen and the absence of similar studies in the region,this study aimed to examine the wind energy potential of Hodeidah-Yemen Republic by analyzing wind characteristics and assessment,determining the available power density,and calculate the wind energy extracted at different *** average wind speed of Hodeidah was obtained only for the data currently available for the five years 2005–2009(due to the current economic and the political situation in Yemen).The results show that the average wind speed in the five years is(25.2 W/m2 at 10 m,93.9 W/m2 at 30 m,and 173.5 W/m2 at 50 m).The average yearly wind power density(25.2 W/m2 at 10 m,93.9 W/m2 at 30 m and 173.5 W/m2 at 50m),and the average yearly energy density(220.8 KWh/m2/year at 10 m,822.6 KWh/m2/year at 30 m and 1519.9 KWh/m2/year at 50 m).This research is a preliminary assessment of the potential of wind energy in Hodeidah,which provides useful information for developing wind energy and an efficient wind *** to the International Wind Energy Rating criteria,the region of Hodeidah falls under‘Class 2’and is classified as‘Marginal’for most of the year.

Modeling of instability and collision of nonlinear dust-acoustic（NDA） envelope solitons in strongly coupled dusty plasmas（SCDPs） is theoretically investigated. The SCDPs consists of strongly correlated negatively variable-charged dust grains and weakly correlated Boltzmann electrons and ions. Using the derivative expansion perturbation technique, a nonlinear Schr dinger-type（NLST） equation for describing the propagation of NDA envelope solitons is derived. Moreover,the extended Poincar′e–Lighthill–Kuo（EPLK） method is employed to deduce the analytical phase shifts and the trajectories after the collision of NDA envelope solitons. In detail, the results show that both modulation instability and phase shift after collision of NDA envelope solitons will modify with the increase in the effects of the viscosity, the relaxation time, and the dust charge fluctuation. Crucially, the modeling of dust-acoustic envelope solitons collision, as reported here, is helpful for understanding the propagation of NDA envelope solitons in strongly coupled dusty plasmas.

Background: Metal stents for unresectable pancreatic cancer are associated with longer patency and superior cost-effectiveness. However, they are too expensive to be recommended routinely in developing countries. Moreover, a debate on outcome results in these patients who receive plastic biliary endoprothesis versus surgical bypass as palliation of obstructive jaundice. We aimed to compare retrospectively the outcomes in patients treated with plastic stent or surgical bypass as a palliative option for these patients. Patients and Methods: We have examined data for patients (n = 86) who received endoscopic stenting (n = 64) or surgical bypass (n = 22), from January 2013 to November 2016, as a palliative treatment for obstructive jaundice from inoperable cancer head pancreas. Results: Serum bilirubin and CA19.9 levels were comparable in age and gender matched patient groups. Moreover, post-operative major complications and 30-days mortality showed no significant differences among patient groups. However, surgical bypass treated patients showed longer initial hospital stay (9 vs. 6 days, p = 0.014), higher cost ($1600 vs. $1088) and longer survival (192 vs. 101 days, p = 0.003) compared to endoscopy-stenting treated patients. Re-hospitalization was required for 5 stented patients (averaged $448). Conclusion: Biliary bypass surgery for unresectable pancreatic cancer may improve patient survival, although prolongs hospital stay. It may be recommended for relatively fit patients with a life expectancy of 6 months and more.

Primary breast leiomyosarcoma: case report and literature review

PURPOSE: Leiomyosarcoma of the breast is extremely rare. To date, the factors that are predictive of patient prognosis have not been identified. To clarify the nature of leiomyosarcoma of the breast, and also to establish the proper treatment strategy, we report this case We report this case while discussing The Clinical presentation, diagnosis, therapy and pathologic feature. INTRODUCION: Sarcomas comprise less than 1% of all primary breast neoplasms and only a minority of these are leiomyosarcomas. There were only 35 cases being reported in English literature. The mainstay of treatment is surgical excision with clear margins and longterm followup is essential. Its prognosis is better compared to other breast sarcoma. CASE PRESENTATION: We report a case of primary leiomyosarcoma of the breast occurring in young female patient which was successfully treated by surgery and radiotherapy. CONCLUSION: According to literature data, primary leiomyosarcoma is characterized by a better prognosis compared with other breast sarcomas, To date, the factors that are predictive of patient prognosis have not been identified. .The rarity and diagnostic difficulty imposed a multidisciplinary approach.
