Paracentesis in cirrhotics is associated with increased risk of 30-day readmission

AIM To determine the readmission rate, its reasons, predictors, and cost of 30-d readmission in patients with cirrhosis and *** A retrospective analysis of the nationwide readmission database(NRD) was performed during the calendar year 2013. All adults cirrhotics with a diagnosis of ascites,spontaneous bacterial peritonitis, or hepatic encephalopathy were identified by ICD-9 codes. Multivariate analysis was performed to assess predictors of 30-d readmission and cost of *** Of the 59597 patients included in this study, 18319(31%) were readmitted within 30 d. Majority(58%) of readmissions were for liver related reasons. Paracentesis was performed in 29832(50%) patients on index admission. Independent predictors of 30-d readmission included age 3 elixhauser comorbidity(OR: 1.13; CI: 1.05-1.22), nonalcoholic cirrhosis(OR: 1.16; CI: 1.10-1.23), paracentesis on index admission(OR: 1.28; CI: 1.21-1.36) and having hepatocellular carcinoma(OR: 1.21; CI: 1.05; 1.39). Cost of index admission was similar in patients readmitted and not readmitted(P-value: 0.34); however cost of care was significantly more on 30 d readmission($30959 ± 762) as compared to index admission($12403 ± 378), P-value: < *** Cirrhotic patients with ascites have a 33% chance of readmission within 30-d. Younger patients, with public insurance, nonalcoholic cirrhosis and increased comorbidity who underwent paracentesis are at increased risk of readmission. Risk factors for unplanned readmission should be targeted given these patients have higher healthcare utilization.

Spatiotemporal Distribution of Phlebotomine Sand Flies （Diptera： Psychodidae） in a Focus of Cutaneous Leishmaniasis in Foum Jamaa （Azilal, Atlas of Morocco）

An entomological survey of Phlebotomine sand flies （Diptera： Psychodidae） was carried out in three sectors of Foum Jamgta region （province of Azilal, Morocco） during the year 2010. Morphological identification was performed on a total of 1,152 sand flies （23% females and 77% males） collected by sticky paper traps. 80% of the total collected flies were identified as Phlebotomus （Paraphlebotomus） sergenti （Parrot）（57%） and Phlebotomus （Larroussius） longicuspis （Nitzulescu）（23%）. In addition to these dominant species, four other species were found, Phlebotomus （Phlebotomus） papatasi （Scopoli）, Sergentomyia （Sergentomyia） minuta （Rondani）, Phlebotomus （Larroussius） perniciosus （Newstead） and Phlebotomus （Paraphlebotomus） chabaudi （Croset）. Overall, the population dynamics show a yearly bimodal pattern related to rainfall and temperature, and with high density around human dwellings. The spatiotemporal distribution of sand fly species was helpful to discuss strategies that might be useful in controlling cutaneous leishmaniasis transmission in this endemic focus.

随着我国一带一路战略部署的推进,我国在一带一路相关国家承担的地质调查任务也在逐步增加。地球化学填图由于其可靠性高、精度好、进度可控等原因,往往是国外地质调查工作的重要一环。地球化学调查工作主要环节包括野外样本采集、样本分析、数据处理及成图等。由于数据处理及成图决定了地球化学填图工作中地球化学异常的圈定及地球化学分布的判定,其模型和方法的研究越来越受到地质科学家的重视,并成为地球化学填图工作中的重要一环。设计了基于ArcGIS建模器的数据处理模型,并将其用于地球化学数据处理。该模型不仅实现了从地球化学采样数据分析到地球化学成图的自动化,而且模型可以在科研人员和生产人员之间共享。模型实现了地球化学填图工作中的以下主要功能:(1)地球化学各元素含量数据的批量插值;(2)元素的分类及异常提取;(3)用栅格数据表示的元素含量数据转换为矢量的等值线/面并平滑。该模型已成功应用于中国商务部援摩洛哥东阿特拉斯山地区1∶10万低密度地球化学填图项目,应用于包含3个工作区45种元素共7 974个地球化学样品的数据处理。成功地解决了项目有效工期短、数据分析任务重、输出图件质量要求高等问题,充分显示了模型的高效实用性,可望在未来地质调查工作中发挥巨大作用。

Nonlinear Propagation of Positron-Acoustic Periodic Travelling Waves in a Magnetoplasma with Superthermal electrons and Positrons

The nonlinear propagation of positron acoustic periodic （PAP） travelling waves in a magnetoplasma composed of dynamic cold positrons, superthermal kappa distributed hot positrons and electrons, and stationary positive ions is examined. The reductive perturbation technique is employed to derive a nonlinear Zakharov Kuznetsov equation that governs the essential features of nonlinear PAP travelling waves. Moreover, the bifurcation theory is used to investigate the propagation of nonlinear PAP periodic travelling wave solutions. It is found that kappa distributed hot positrons and electrons provide only the possibility of existence of nonlinear compressive PAP travelling waves. It is observed that the superthermality of hot positrons, the concentrations of superthermal electrons and positrons, the positron cyclotron frequency, the direction cosines of wave vector k along the z-axis, and the concentration of ions play pivotal roles in the nonlinear propagation of PAP travelling waves. Tile present investigation may be used to understand the formation of PAP structures in the space and laboratory plasmas with superthermal hot positrons and electrons.

Nutritional Status of Mothers and Its Determinants in Alemtidad Area, Khartoum

Background: Nutritional status of mothers is considered one of the major problems facing mothers in the world. The nutrition plays a major role in mothers and child health. Objective: This study aims to assess mothers’ nutritional status and its determinants in Alemtidad area, Khartoum. Methods: A descriptive community based survey was conducted in the Alemtidad area of Khartoum between November 2012 and December 2012. One hundred and eighty mothers were selected by simple randomization. Data were collected by using structured designed questionnaire which was then analyzed by (SPSS) program version 20. Results: The study conducted among mothers in Alemtidad area;their age ranged between 19 and 86 years with the mean age 38 ± 12 years. From those mothers about half were unemployed, and having mainly primary and secondary educational level. 81.1% of mothers were not able to select the proper food according to age and nutrients and about 71.1% of mothers took two meals per day and 23.3% one meals per day, and only 5.6 take three meals per day. The Body Mass Index (BMI) was calculated to measure the nutrition status of mothers;this study shows that mothers in Alemtidad are mainly overweight and obese, 45.6% and 18% respectively and only 35% within normal range of Body Mass Index (BMI). Conclusion: The study concludes that mothers are either overweight or obese (45.6%, 17.7% respectively). Compared to national and international statistics the findings of this study reflect high Body Mass Index (BMI) of mothers in Alemtidad.

Annular pancreas producing duodenal obstruction: A case report

Annular pancreas is a rare congenital anomaly characterized by the presence of ectopic pancreatic tissue surrounding the duodenum. This malformation is usually asymptomatic in adults, but can manifests as pancreatitis, duodenal stenosis, or duodenal or gastric ulceration. We report the case of a young patient of 18 years old hospitalized for epigastric pain and vomiting, in whom radiological investigations showed an annular pancreas. At operation, a complete obstruction of the duodenum between its first and second parts was found, caused by an annular pancreas. No other congenital anomaly of the intra abdominal organs was noted. A gastroenterostomy was performed. Both the rarity of this congenital abnormality and its successful correction by surgical means have prompted us to make the following presentation.

Hemobilia Due to an Iatrogenic Arteriobiliary Fistula Complicating Laparoscopic Cholecystectomy: A Case Report

Hemobilia is the result of a pathological communication between bile duct and intra or extrahepatic vessel. 40% to 60% of the haemobilia cases are Iatrogenic, and the other causes are either vascular malformations or hepatic blunt trauma. We describe the case history of a patient in which laparoscopic cholecystectomy was complicated 3 months later by massive hemobilia. The cause of haemorrhage was a fistula between the principal bil duct and the right hepatic artery. This complication was successfully managed by surgery and angiographic embolization with full recovery of the patient.

This present study aimed to determine the efficacy of supplementing layer diets with vitamin A(0, 8,000 and16,000 IU/kg diet) and vitamin E(0, 250 and 500 mg/kg diet) either individually or in combination on egg production and quality, and blood hematology and chemistry of birds reared under summer conditions. A total of 135 Bovans Brown laying hens were distributed to 9 treatment groups with 5 replicates of 3 hens/pen in a 3 × 3 factorial design. A significant improvement in feed conversion ratio(FCR) was observed as supplementary vitamin A or E increased(P 0.01). Hens fed diets supplemented with 16,000 IU vitamin A plus 500 mg vitamin E/kg diet had the best FCR among all groups. Egg quality traits were not significantly affected by the interaction of vitamin A and vitamin E levels. There was a significant increase in monocytes(P ≤ 0.01)and a decrease in basophils counts(P ≤ 0.05) in response to vitamin E. Significant decreases were observed in packed cell volume(PCV), thyroxine(T_4), alanine transferase(ALT), albumin, total cholesterol and total lipids((P ≤ 0.05 or P ≤ 0.01) P 0.01), and increases were observed in serum concentrations of globulin(P ≤ 0.05)and calcium(P ≤ 0.01) due to vitamin A. The combination of 0 IU vitamin A and 500 mg vitamin E/kg diet had the highest values of PCV(40.09%) and hemoglobin(Hb)(10.33 mg/100 mL) among all groups. Vitamin E raised serum values of total protein, total cholesterol and total lipids(P ≤ 0.05 or P≤ 0.01). Feed intake, FCR,PCV, Hb, lymphocytes, monocytes, eosinophils, T_4, ALT and total protein were significantly affected by the interaction of vitamins A and E(P ≤ 0.05 or P ≤ 0.01). The interaction of vitamins A and E was only significant with respect to serum total protein(P ≤ 0.05). It can be concluded that layer diets supplemented with vitamins A and E had good results in alleviating the harmful impacts of high ambient temperature. A numerical treatment for self-adjoint singularly perturbed second-order two-point boundary value problems using trigonometric quintic B-splines is presented,which depend on different engineering *** method is found to have a truncation error of O(h 6)and converges to the exact solution at O(h 4).The numerical examples show that our method is very effective and the maximum absolute error is acceptable.
