Use of three-dimensional printing in preoperative planning in orthopaedic trauma surgery: A systematic review and meta-analysis

BACKGROUND With the increasing complexity of surgical interventions performed in orthopaedic trauma surgery and the improving technologies used in threedimensional(3D)printing,there has been an increased interest in the *** has been shown that 3D models allow surgeons to better visualise anatomy,aid in planning and performing complex *** is however not clear how best to utilise the technique and whether this results in better *** To evaluate the effect of 3D printing used in pre-operative planning in orthopaedic trauma surgery on clinical *** We performed a comprehensive systematic review of the literature and a ***,Ovid and Embase were searched from inception to February 8,*** controlled trials,case-control studies,cohort studies and case series of five patients or more were included across any area of orthopaedic *** primary outcomes were operation time,intra-operative blood loss and fluoroscopy *** Seventeen studies(922 patients)met our inclusion criteria and were *** use of 3D printing across all specialties in orthopaedic trauma surgery demonstrated an overall reduction in operation time of 19.85%[95%confidence intervals(CI):(-22.99,-16.71)],intra-operative blood loss of 25.73%[95%CI:(-31.07,-20.40)],and number of times fluoroscopy was used by 23.80%[95%CI:(-38.49,-9.10)].CONCLUSION Our results suggest that the use of 3D printing in pre-operative planning in orthopaedic trauma reduces operative time,intraoperative blood loss and the number of times fluoroscopy is used.

Metal-on-metal(MoM) hip replacements have proven to be a modern day orthopaedic failure. The early enthusiasm and promise of a hard, durable bearing was quickly quashed following the unanticipated wear rates. The release of metal ions into the blood stream has been shown to lead to surrounding soft tissue complications and early failure. The devastating destruction caused has led to a large number of revision procedures and implant extractions. The resulting research into this field has led to a new area of interest; that of the wear at the trunnion of the prosthesis. It had been previously thought that the metal debris was generated solely from the weight bearing articulation, however with the evolution of modularity to aid surgical options, wear at the trunnion is becoming more apparent. The phenomenon of "trunnionosis" is a rapidly developing area of interest that may contribute to the overall effect of metallosis in Mo M replacements but may also lead to the release of metal ions in non Mo M hip designs. The aim of this paper is to introduce, explain and summarise the evidence so far in the field of trunnionosis. The evidence for this phenomenon, the type of debris particles generated and a contrast between Mo M, non Mo M and resurfacing procedures are also presented.

Anterior cruciate ligament reconstruction best practice: A review of graft choice

There is much literature about differing grafts used in anterior cruciate ligament(ACL) reconstruction. Much of this is of poor quality and of a low evidence base. We review and summarise the literature looking at the four main classes of grafts used in ACL reconstruction; bone-patella tendon-bone, hamstrings, allograft and synthetic grafts. Each graft has the evidence for its use reviewed and then compared, where possible, to the others. We conclude that although there is no clear "best" graft, there are clear differences between the differing graft choices. Surgeon's need to be aware of the evidence behind these differences, in order to have appropriate discussions with their patients, so as to come to an informed choice of graft type to best suit each individual patient and their requirements.

Purpose:The COVID-19 pandemic has caused 1.4 million deaths globally and is associated with a 3-4 times increase in 30-day mortality after a fragility hip fracture with concurrent COVID-19 ***,death from COVID-19 infection occurs between 15 and 22 days after the onset of symptoms,but this period can extend up to 8 *** study aimed to assess the impact of concurrent COVID-19 infection on 120-day mortality after a fragility hip ***:A multi-centre prospective study across 10 hospitals treating 8%of the annual burden of hip fractures in England between 1st March and 30th April,2020 was *** whose surgical treatment was payable through the National Health Service Best Practice Tariff mechanism for"fragility hip fractures"were included in the ***'120-day mortality was assessed relative to their perioperative COVID-19 *** analysis was performed using SPSS version ***:A total of 746 patients were included in this study,of which 87(11.7%)were COVID-19 *** rates at 30-and 120-day were significantly higher for COVID-19 positive patients relative to COVID-19 negative patients(p<0.001).However,mortality rates between 31 and 120-day were not significantly different(p=0.107),16.1%and 9.4%respectively for COVID-19 positive and negative patients,odds ratio 1.855(95%CI 0.865-3.978).Conclusion:Hip fracture patients with concurrent COVID-19 infection,provided that they are alive at day-31 after injury,have no significant difference in 120-day *** the growing awareness and concern of "long-COVID"and its widespread prevalence,this does not appear to increase mediumterm mortality rates after a hip fracture.

Structural analysis by Rietveld refinement of calcium and lanthanum phosphosilicate apatites

Britholites with the general formula Ca10-xLnx（PO4）6-x（SiO4）xF2, （0≤x≤6） are considered to be promising matrices for the confinement of the by-products in the nuclear industry. A thermodynamic study showed that the stability of these compounds de-creased as the substitution rate increased. The present work was an attempt to gain more information about the structural changes in-duced by the substitution, in order to understand the observed stability decrease. The samples were successfully synthesized as a sin-gle-phase apatite by a solid-state reaction between 1200 and 1400 °C. The structural refinement indicated that the La3＋ions preferen-tially occupied the 6（h） sites. A progressive shift of F-along the c-axis outside its ideal position occurred as a result of the substitution increase. This might be the cause of the observed stability decrease, especially as the energies of the La-O, La-F and Si-O bonds are higher than those of Ca-O, Ca-F and P-O. The distribution of La3＋ between the two non-equivalent sites was confirmed by the charge distribution method.

Ab initio study of structural,electronic,thermo-elastic and optical properties of Pt3Zr intermetallic compound

Structural,elastic,electronic and optical properties of the Pt3Zr intermetallic compound are investigated using first principles calculations based on the density functional theory(DFT)within the generalized gradient approximation(GGA)and the local density approximation(LDA).The Pt3Zr compound is predicted to be of cubic L12 and hexagonal D024 *** calculated equilibrium ground-state properties(lattice parameters a and c,bulk modulus B and its pressure derivative B',formation enthalpy ΔH)of the Pt3Zr compound,for both cubic and hexagonal phases,show good agreement with the experimental results and other theoretical *** constants(C11,C12,C13,C33,C44,and C55)are *** predicted elastic properties such as Young's modulus E and shear modulus GH,Poisson ratioν,anisotropic ratio A,Kleinman parameter ξ,Cauchy pressure(C12-C44),ratios B/C44 and B/G,and Vickers hardness Hv indicate the stiffness,hardness and ductility of the *** characteristic parameters such as Debye temperature θD and melting temperature Tm are *** properties such as density of states(DOS)and electronic specific heat γ are also *** calculated results reveal that the Fermi level is on the psedogap for the D024 structure and on the antibonding side for the L12 *** optical property functions(real partε1(ω)and imaginary part ε2(ω)of dielectric function),optical conductivity σ(ω),refraction index n(ω),reflectivity R(ω),absorption α(ω)and extinction coefficients k(ω)and loss function L(ω)are also investigated for the first time for Pt3Zr in a large gamme of energy from 0 to 70 eV.
