Childhood asthma biomarkers including zinc: An exploratory crosssectional study

BACKGROUND Childhood bronchial asthma(BA)is a chronic inflammatory respiratory disease.Nutritional conditions,including zinc deficiency,can affect such allergic disorders.AIM To outline the difference in serum zinc levels between asthmatic children and healthy controls.METHODS A cross-sectional study was carried out at Children’s Hospital,Cairo University,investigating serum zinc levels in children with BA(n=40)and healthy children(n=21).Other markers included serum ferritin,iron,hemoglobin(Hb),and immunoglobulin E(IgE)levels.Independent t-tests and Mann-Whinny tests were used for comparisons.The Kruskal-Wallis test was applied to compare serum ferritin and IgE levels with regard to asthma severity.Spearman's rank correlation was performed to explore the relationship between serum ferritin levels and both iron and Hb levels in asthmatic children.RESULTS Children with BA had higher levels of zinc,yet the difference was not significant(P=0.115).Serum ferritin and IgE levels were significantly higher in asthmatic children(P=0.006 and 0.001,respectively),yet their levels did not differ significantly by severity(P=0.623 and 0.126,respectively).There was a nonsignificant weak correlation between serum ferritin levels and both serum iron and Hb levels.CONCLUSION Serum zinc levels do not seem to differ between asthmatic children and healthy children.Serum ferritin levels may be a marker of asthma control.Serum IgE levels are not markers of asthma severity.

Delayed Grafting as a Valid Technique in Burn Management: Experience from a Burn Unit in Oman

Background: Early excision and grafting has been the preferred method of managing major burns around the world since 1970. Considering the advances in health care and the development of new antibiotics over the past 50 years, delayed grafting as a technique for the management of burns over 15%-20% of total body surface area (TBSA) could have comparable results to that of early excision. This study aims to highlight the outcomes of practicing delayed grafting in burn patients. Methods: A case series analysis was performed of 51 patients who were admitted to the burns unit in Sultan Qaboos Hospital Salalah with over 20% TBSA between January 2014 and December 2019. The patients received prophylactic antibiotics and silver sulphadiazine dressing until the burn eschar had completely separated, followed by grafting. Results: Two patients were lost during the entire duration of the study. The mortality rate was comparable to that of early excision, while the rate of hypertrophic scarring was lower than the range reported by other studies. Conclusion: In the management of patients with over 20% TBSA, delayed grafting after complete separation of eschar is still a valid technique.

Predictors of renal angioembolization outcome:A retrospective analysis with 148 patients at a tertiary urology institute

Objective:The aim of this study was to evaluate the predictor of unsuccessful outcome of renal angioembolization(RAE).Knowing those predictors may help in avoiding unnecessary RAE procedures and their associated side effects,while helping to prepare for an alternate procedure and improving patient's overall satisfaction.Methods:A retrospective analysis between January 2006 and December 2018 was performed,and the indications for RAE were classified into post-traumatic,iatrogenic,renal tumors,and spontaneous.Patients who underwent RAE prior to nephrectomy were eliminated.Computed tomography angiography was performed in patients with normal renal function and those who had no contrast allergy,otherwise magnetic resonance angiography was performed.For the purpose of statistical analysis,we stratified patients into two main categories based on the final outcome—successful or failed.Results:Of 180 patients,32 with negative angiography were eliminated,leaving 148 patients;136(91%)had successful outcomes after one or more trials and 12 had unsuccessful outcomes.The mean age was 45±15 years,and 105(71%)were male.Neither gender,side of the lesion,presence of hematuria,indication for RAE,nor the type of lesion affected the outcome.On the other hand,renal anatomy with presence of accessory artery was the only predictor to failed RAE(p=0.001).Failed RAE trial was a predictor for nephrectomy as a secondary procedure(p=0.03).Conclusion:No pre-procedural predictors could anticipate the RAE outcome,and different indications can be scheduled to RAE,which is equally effective.The presence of accessory renal artery on diagnostic angiography is the only factor that may predict the failure of the procedure.

Most children and elderly people worldwide die from pneumonia,which is a contagious illness that causes lung ulcers.For diagnosing pneumonia from chest X-ray images,many deep learning models have been put forth.The goal of this research is to develop an effective and strong approach for detecting and categorizing pneumonia cases.By varying the deep learning approach,three pre-trained models,GoogLeNet,ResNet18,and DenseNet121,are employed in this research to extract the main features of pneumonia and normal cases.In addition,the binary dipper throated optimization(DTO)algorithm is utilized to select the most significant features,which are then fed to the K-nearest neighbor(KNN)classifier for getting the final classification decision.To guarantee the best performance of KNN,its main parameter(K)is optimized using the continuous DTO algorithm.To test the proposed approach,six evaluation metrics were employed namely,positive and negative predictive values,accuracy,specificity,sensitivity,and F1-score.Moreover,the proposed approach is compared with other traditional approaches,and the findings confirmed the superiority of the proposed approach in terms of all the evaluation metrics.The minimum accuracy achieved by the proposed approach is(98.5%),and the maximum accuracy is(99.8%)when different test cases are included in the evaluation experiments.

Wind power is one of the sustainable ways to generate renewable energy.In recent years,some countries have set renewables to meet future energy needs,with the primary goal of reducing emissions and promoting sustainable growth,primarily the use of wind and solar power.To achieve the prediction of wind power generation,several deep and machine learning models are constructed in this article as base models.These regression models are Deep neural network(DNN),k-nearest neighbor(KNN)regressor,long short-term memory(LSTM),averaging model,random forest(RF)regressor,bagging regressor,and gradient boosting(GB)regressor.In addition,data cleaning and data preprocessing were performed to the data.The dataset used in this study includes 4 features and 50530 instances.To accurately predict the wind power values,we propose in this paper a new optimization technique based on stochastic fractal search and particle swarm optimization(SFSPSO)to optimize the parameters of LSTM network.Five evaluation criteria were utilized to estimate the efficiency of the regression models,namely,mean absolute error(MAE),Nash Sutcliffe Efficiency(NSE),mean square error(MSE),coefficient of determination(R2),root mean squared error(RMSE).The experimental results illustrated that the proposed optimization of LSTM using SFS-PSO model achieved the best results with R2 equals 99.99%in predicting the wind power values.

Arrhythmia has been classified using a variety of methods.Because of the dynamic nature of electrocardiogram(ECG)data,traditional handcrafted approaches are difficult to execute,making the machine learning(ML)solutions more appealing.Patients with cardiac arrhythmias can benefit from competent monitoring to save their lives.Cardiac arrhythmia classification and prediction have greatly improved in recent years.Arrhythmias are a category of conditions in which the heart's electrical activity is abnormally rapid or sluggish.Every year,it is one of the main reasons of mortality for both men and women,worldwide.For the classification of arrhythmias,this work proposes a novel technique based on optimized feature selection and optimized K-nearest neighbors(KNN)classifier.The proposed method makes advantage of the UCI repository,which has a 279-attribute high-dimensional cardiac arrhythmia dataset.The proposed approach is based on dividing cardiac arrhythmia patients into 16 groups based on the electrocardiography dataset’s features.The purpose is to design an efficient intelligent system employing the dipper throated optimization method to categorize cardiac arrhythmia patients.This method of comprehensive arrhythmia classification outperforms earlier methods presented in the literature.The achieved classification accuracy using the proposed approach is 99.8%.

Conjunctival flap advancement with or without scleral graft for hypotony maculopathy after trabeculectomy

AIM: To investigate the outcomes of adaptive conjunctival flap advancement surgical techniques with or without scleral graft for patients undergoing revision surgery after trabeculectomy.METHODS: This retrospective study included 28 eyes of 28 subjects from December 2015 to April 2018. A group of 15 eyes underwent conjunctival advancement only while the other 13 eyes did conjunctival advancement with partial thickness scleral graft. In both study groups, we considered the intraocular pressure(IOP) as our primary outcome and visual acuity(VA) as our secondary outcome. Both were assessed pre-and post-operatively till the last followup possible point. Additionally, we classified the patients according to complete and qualified success criteria.RESULTS: The mean age of the conjunctival advancement group was 36.87±19.25 y, while it was 44.08±18.04 in the other group. In both study groups, the mean IOP significantly increased after revision surgery at 1, 2, 3 mo and the last follow-up visit when compared to prior the surgery(Peline in both groups, there was no significant difference between IOP improvement in 1 mo(P=0.263), while the difference was significant in 2 mo(P=0.03), 3 mo(P=0.02) and in the final follow-up visit postoperatively. However, this difference was not significant regarding VA(P=0.5).CONCLUSION: The both adaptive techniques of conjunctival advancement and conjunctival advancement with scleral graft are effective for treating patients with lateonset hypotony. Yet, more prospective studies are needed to assert upon these results.

What Factors Influence the Choice of Anesthesiology in a Moroccan Medical School?

Objective: To determine factors influencing anesthesiology choice among resident doctors at the medical school of Marrakech. Materials and Methods: We have conducted a descriptive cross-sectional study based on an anonymous questionnaire. Results: A total of 406/672 questionnaires were returned, with a female/male sex ratio of 1.07. The duration of the training (OR: 3.3;CI 95%: 1.74 - 6.23;p ellectual challenge (OR: 3.02;CI 95%: 1.69 - 5.37;p elationship (OR: 2.22;CI 95%: 1.02 - 4.84;p: 0.04), and financial aspects (OR: 2.14;CI 95%: 1.09 - 4.21;p: 0.02) were independent factors that influenced the choice of anesthesiology. Conclusion: we recommend the succeeding: 1) Support students in their choice;2) Correct misconceptions about certain specialties;3) Promote clinical clerkship;4) Encourage mentorship;5) Increase the salary of at-risk specialties.

Objective:To investigate the antioxidant and the antibacterial activities of crude extract from aerial part of Polygonum maritimumL.(Polygonaceae)(P.maritimum)and to find new actives biomolecules.Methods:The whole plant was collected from the Rechgoune coast(West of Algeria),and methanolic crude extract of aerial parts ofP.maritimum(PMCE)was prepared.The extract was tested against different bacterial strain and tested for his ability to neutralize free radical(DPPH)and to scavenge the H2O2.Results:PMCE had a very high content of total phenol,which was(352.49依18.03)mg/g dry weight,expressed as gallic acid equivalent.PMCE exhibited excellent antioxidant activity,as measured using DPPH and H2O2 scavenging assays.It also showed a high antibacterial activity against gram-positive bacterial strains:Bacillus cereus,Bacillus subtilisandStaphylococcus aureus with an highest MIC of 120μg/mL.Conclusions:The antioxidant and antibacterial activity of the PMCE is probably due to phenolic compounds present in the extract.The contemporary presence of antioxidant and antibacterialactivities in the PMCE suggests that this plant may be a source of bioactive substances with multifaceted activity.

Noninvasive assessment of hepatic fibrosis in Egyptian patients with chronic hepatitis C virus infection

AIM: To evaluate the accuracy of specific biochemical markers for the assessment of hepatic fibrosis in patients with chronic hepatitis C virus (HCV) infection. METHODS: One hundred and fifty-four patients with chronic HCV infection were included in this study; 124 patients were non-cirrhotic, and 30 were cirrhotic. The following measurements were obtained in all patients: serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin, total bilirubin, prothrombin time and concentration, complete blood count, hepatitis B surface antigen (HBsAg), HCVAb, HCV-RNA by quantitative polymerase chain reaction, abdominal ultrasound and ultrasonic-guided liver biopsy. The following ratios, scores and indices were calculated and compared with the results of the histopathological examination: AST/ALT ratio (AAR), age platelet index (API), AST to platelet ratio index (APRI), cirrhosis discriminating score (CDS), Pohl score, G teborg University Cirrhosis Index (GUCI). RESULTS: AAR, APRI, API and GUCI demonstrated good diagnostic accuracy of liver cirrhosis (80.5%, 79.2%, 76.6% and 80.5%, respectively); P values were: ely. Among the studied parameters, AAR and GUCI gave the highest diagnostic accuracy (80.5%) with cutoff values of 1.2 and 1.5, respectively. APRI, API and GUCI were significantly correlated with the stage of fibrosis (P ely), while CDS only correlated significantly with the stage of fibrosis (P 0.05). In addition, we found significant correlations for the AAR, APRI, API, GUCI and Pohl score between the non-cirrhotic (F0, F1, F2, F3) and cirrhotic (F4) groups (P values: ely; CDS did not demonstrate significant correlation (P > 0.05).
