Do statins reduce the mortality rate in stroke patients treated with systemic thrombolysis in a 5-year single-center study?

The present study investigated the association between pre-treatment with a cholesterol-lowering drug(statin) or new setting hereon and the effect on the mortality rate in patients with acute ischemic stroke who received intravenous systemic thrombolysis. During a 5-year period(starting in October 2008), 542 consecutive stroke patients who received intravenous systemic thrombolysis with recombinant tissue plasminogen activator(rt-PA) at the Department of Neurology, University Hospital Schleswig-Holstein, Campus Lübeck, Germany, were included. Patients were characterized according to statins. The primary endpoint was mortality;it was assessed twice: in hospital and 3 months after discharge. The secondary outcome was the rate of symptomatic intracerebral hemorrhage. Of the 542 stroke patients examined(mean age 72 ± 13 years;51% women, mean National Institutes of Health Stroke Scale(NIHSS) score 11), 138 patients(25.5%) had been pretreated with statin, while in 190 patients(35.1%) statin therapy was initiated during their stay in hospital, whereas 193(35.6%) never received statins. Patients pre-treated with statin were older and more frequently had previous illnesses(arterial hypertension, diabetes mellitus and previous cerebral infarctions), but were comparably similarly affected by the stroke(NIHSS 11 vs. 11;P = 0.76) compared to patients who were not on statin treatment at the time of cerebral infarction. Patients pretreated with statin did not differ in 3-month mortality from those newly treated to a statin(7.6% vs. 8%;P = 0.9). Interestingly, the group of patients pretreated with statin showed a lower rate of in hospital mortality(6.6% vs. 17.0;P = 0.005) and 3-month mortality(10.7% vs. 23.7%;P = 0.005) than the group of patients who had no statin treatment at all. The same effect was seen for patients newly adjusted to a statin during the hospital stay compared to patients who did not receive statins(3-month mortality: 7.1% vs. 23.7%;P < 0.001). Efficient maximum power point tracker based on neural network and sliding-mode control for buck converters

This paper presents detailed design steps of an effective control system aiming to increase the solar energy harvested via photovoltaic power-generation *** design of an intelligent maximum power point tracker(MPPT)supported by a robust sliding-mode(SM)controller is discussed in this *** proposed control scheme is designed to track the MPP and provide a smooth system response by removing the overshoot in the load current during any variation in the connected *** a system is suitable for DC-DC buck converter *** study begins with modelling the buck converter for a continuous current mode *** reference voltage of the tracking system is produced by the proposed neural network(NN)*** proposed intelligent MPPT integrated with an SM controller is simulated in a MATLAB®/Simulink®*** simulation results are analysed to investigate and confirm the satisfaction level of the adopted four-serially connected PV-modules *** system performance is evaluated at a light intensity of 500 W/m^(2) and an ambient temperature of 25°*** only the proposed NN algorithm guarantees the MPP tracking response by delivering 100 W at a resistive load of 13Ω,and 200 W at a load of 6.5Ω,respectively,with 99.77%system ***,this simultaneously demonstrates a current spike of~0.5 A when the load is varied from 50%to 100%.The integrated SM controller demonstrates a robust and smooth response,eliminating the existing current spike.

BACKGROUND： Laparoscopic splenectomy is considered the gold standard for resecting normal-to-moderately bigger spleens in benign conditions, and in addition could be tried for patients with malignant splenic disorders. However, the safety of laparoscopic splenectomy in patients with hyper- splenism is not well-known. This study aimed to investigate the efficacy and safety of laparoscopic splenectomy for pa- tients with hypersplenism secondary to liver cirrhosis by com- paring with the open splenectomy.

Effect of vehicle safety recalls on the perception towards other vehicles from the same country of origin

A vehicle safety recall significantly impacts the demand for these specific manufacturer *** effect of this recall on the other manufacturers from the same country of origin(COO) that are not involved in the recall is poorly *** purpose of this study is to examine the impact of a vehicle safety recall on other vehicle brands originating from the same *** study uses a case study related to Toyota recalls and the impact of the crises on the attitudes toward three other Japanese brands-namely,Honda,Nissan,and Mazda.A questionnaire was conducted with 375 participants who are familiar with the *** findings from the questionnaire indicated that product-harm crises have an indirect negative effect on brands from the same ***,brands from the transgressor’s COO should not make the transgressor’s betrayal salient,which might hurt their *** example,if Honda makes the betrayal of Toyota salient through comparative advertising,Honda’s efforts might hurt the company directly through consumers’ image of Japanese *** research should examine the extent to which comparative advertising impacts non-transgressing brands that originate from the same country as the transgressing *** current results suggest that the non-transgressing companies might even assist the transgressing company to recover from such crises if consumers perceive a strong association between the non-transgressing brands and transgressing brands.

Analytical approximate solution for nonlinear space-time fractional Klein Gordon equation

The fractional derivatives in the sense of Caputo and the homotopy analysis method are used to construct an approximate solution for the nonlinear space-time fractional derivatives Klein-Gordon equation. The numerical results show that the approaches are easy to implement and accurate when applied to the nonlinear space-time fractional derivatives Klein- Gordon equation. This method introduces a promising tool for solving many space-time fractional partial differential equations. This method is efficient and powerful in solving wide classes of nonlinear evolution fractional order equations.

Multiple sclerosis: New insights and trends

Multiple sclerosis(MS) is the most famous autoimmune disease attacking the central nervous system. It attacks people from age 20–50 years old and the females' attacks double than males' attacks. MS is an autoimmune disease affecting principally the central nervous system that cause nerve sheath demyelination followed by axon damage and paralysis. MS symptoms include muscle weakness, weak reflexes, muscle spasm, difficult in move, miss-coordination and unbalance with others. There are many factors may be responsible for MS: microbial, viral, smoking, stress, environmental toxins, contaminated diet, and gout. MS is wide spread in the populations in North Europe and this related to lack of vitamin D due to decrease of sunlight exposure. MS biomarkers include nitric oxide, interleukin-6, nitric oxide synthase, fetuin-A and osteopontin. MS is not a genetic disease where MS occurs when human leukocyte antigen system related genes are changed in chromosome 6. The physiology of MS is monitored by activation of immuneinflammatory, oxidative, and nitrosative stress pathways. MS is including two main steps:(1) myelin sheath destruction and formation of lesions and,(2) inflammation. Four types of MS can be distinguished: relapsing-remitting, primary progressive, secondary progressive and progressive relapsing. Nine treatments have been accepted for relapsingremitting MS type: interferon b-1a, interferon b-1b, mitoxantrone, natalizumab, glatiramer acetate, fingolimod, dimethyl fumarate, teriflunomide, and alemtuzumab, however,the only treatment used is mitoxantrone for progressive MS but many of MS treatments side effects are recorded. Complementary treatments also used in MS treatments such as:vitamin D, Yoga, medicinal plants, oxygen therapy, acupuncture and reflexology.

The organic Rankine cycle(ORC)coupled with a linear Fresnel reflector(LFR)utilizes a low-grade heat *** article presents the study of a small-scale LFR-ORC power-generation plant under the climatic conditions of Almatret,*** mathematical modelling is performed using thermodynamic equations and simulations are conducted to evaluate the optical performance of the LFR system and thermal performance of the ORC ***-62 is used as a heat-transfer fluid(HTF)in the solar field,whereas NOVEC^(TM) 649 is used as the working fluid in the ORC power *** LFR is integrated with a thermal-storage unit based on a two-tank system and stores the solar thermal energy via a heat *** thermal-energy output of the receiver tube of the LFR system is 108 kW and thermal losses are 7.872 kW during the peak time operation of a day at 1:00 *** mechanical power output of the ORC turbine is 7.296 kW using the specific design conditions and the two-tank thermal-storage system adds 4 operation hours to the power plant after sunset.

Protective effect of natural products and hormones in colon cancer using metabolome:A physiological overview

Globally, the third cause of males cancer and the fourth cause of females cancer is colon cancer(CC). In Egypt, high CC percentage occurs in children and in individuals below 40 years of age. The complete loss of biological enzyme function is the main cause of CC and consequently CC increased in smoking and pollution exposure. The aim of this review is to focus on the application of metabolome as a physiological tool that can play an important role in preventing CC incidence by natural products and hormones. The dietary factors, intestinal micro-flora and endogenously produced metabolites are the main three causes that produce free radicals in the colon. A correlation occurs between the enzyme activity and CC polymorphisms or property. Nowadays metabolome is applied with the progress of different analytical methods, data bases and tools for cancer predication and stimulation especially in CC cases. Metabolism is defined as intracellular chemical reactions that produce chemical substances and energies sustaining life. Metabolic pathway networks are also composed of links that are defined as transformation of chemical structures between two metabolites and an enzyme reaction. The most important advantage of metabolome is its ability to analyze metabolites from any source, regardless of origin, where the application of liquid chromatography combined with mass spectra in metabolome analysis to a series of cancer cell lines that were progressively more tumorigenic due to the induction of 1,2,3 or 4 oncogenes to cell lines could be a metabolome example application. In conclusion, natural products and hormones are very important in preventing CC in humans and animal models where both natural products and hormones play a significant and important effect in regulating physiological process especially in CC cases. In this situation, metabolome must increase in its application in the future for the diagnosis of CC cases.
