Objective: Study the HBS1L-MYB (rs4895441 and rs9376090) genetic polymorphisms in Egyptian patients with β-thalassemia major and sickle cell disease and its relation to Hb F and severity of the disease. Background: Hb F is a predominant modulator for the severity of β-thalassemia major & sickle cell disease. Genetic polymorphism in the intergenic region (HBS1L-MYB) between GTP-binding elongation factor HBS1L and myeloblastosis oncogene MYB on chromosome 6q is associated with high fetal hemoglobin levels. Subjects and Methods: 150 subjects were included in this study. For all studied groups: Complete blood picture and serum ferritin were evaluated. For patients, hemoglobin variants were separated by High-performance liquid chromatography. Genotyping of HBS1L-MYB (rs4895441 & rs9376090) was evaluated by real-time polymerase chain reaction technique using TaqMan probe. Results: AG, CT genotypes, and G, C alleles of HBS1L-MYB (rs4895441 & rs9376090) were significantly high in sickle cell patients [OR (3.400);95% C.I (1.482 - 7.799)], (p = 0.003) & [OR (4.522);95% C.I (1.854 - 11.029)], (p = 0.001) respectively. AIM: To evaluate the efficacy and safety of botulinum toxin type A(BTX-A) in the management of patients with anismus. METHODS: An or.anized sear.h of published liter.tur. was conducted using electr.nic databases including: Pub Med/MEDLINE, and Cochr.ne Centr.l r.gister.of Contr.l ed Tr.als, also an inter.et-based sear.h using "Google Scholar. ser.ice was conducted. Both compar.tive and obser.ational studies wer. included. We excluded ir.elevant ar.icles, editor.als, case r.por.s, r.views, and meta-analyses. The studies that followed the patients less than 6 mo wer. excluded. Var.ables collected wer. demogr.phic data of the patients, technique of BTX-A injection and number.of sessions, shor.-ter. and longter. clinical impr.vement, post-injection changes in electr.myogr.phy(EMG), defecogr.phy, manometr., and balloon expulsion test, and complications r.cor.ed after.BTX-A ***: Seven studies compr.sing 189 patients wer. included in the r.view. The median age of the patients was 41.2 year. and female-to-male r.tio was 1.3:1. The median dose of BTX-A injected per.pr.cedur. was 100 IU(r.nge, 20-100 IU). Later.l injection was done in five tr.ils and combined later.l and poster.or.injections in two tr.als. Thr.e studies used endor.ctal ultr.sonogr.phyguided technique, one study used EMG-guided technique,wher.as the r.maining thr.e studies used manual palpation with the index finger. The median per.entage of patients who r.por.ed initial impr.vement of symptoms was 77.4%(r.nge 37.5%-86.7%), this per.entage declined to a median of 46%(r.nge 25%-100%) at 4 mo after.injection of BTX-A. r.tes of impr.vement evaluated by balloon expulsion test, EMG, and defecogr.phy r.nged between(37.5%-80%),(54%-86.7%), and(25%-86.6%), r.spectively. Four.een(7.4%) patients developed complications after.injection of BTX-A. Complication r.tes acr.ss the studies r.nged fr.m 0% to 22.6%. Obesity is a major.and gr.wing health car. concer.. Lar.e epidemiologic studies that evaluated the r.lationship between obesity and mor.ality, obser.ed that a higher.body-mass index(BMI) is associated with incr.ased r.te of death fr.m sever.l causes, among them car.iovascular.disease; which is par.icular.y tr.e for.those with mor.id obesity. Being over.eight was also associated with decr.ased sur.ival in sever.l studies. Unfor.unately, obese subjects ar. often exposed to public disappr.val because of their.fatness which significantly affects their.psychosocial behavior. All obese patients(BMI ≥ 30 kg/m2) should r.ceive counseling on diet, lifestyle, exer.ise and goals for.weight management. Individuals with BMI ≥ 40 kg/m2 and those with BMI > 35 kg/m2 with obesity-r.lated comor.idities; who failed diet, exer.ise, and dr.g ther.py, should be consider.d for.bar.atr.c sur.er.. In cur.ent r.view ar.icle, we will shed light on impor.ant medical pr.nciples that each sur.eon/gastr.enter.logist needs to know about bar.atr.c sur.ical pr.cedur., with special concer. to the ear.y post oper.tive per.od. Additionally, we will explain the common complications that usually follow bar.atr.c sur.er. and elucidate medical guidelines in their.management. For.the fir.t 24 h after.the bar.atr.c sur.er., the postoper.tive pr.or.ties include pain management, leakage, nausea and vomiting, intr.venous fluid management, pulmonar. hygiene, and ambulation. Patients maintain a low calor.e liquid diet for.the fir.t few postoper.tive days that is gr.dually changed to soft solid food diet within two or.thr.e weeks following the bar.atr.c sur.er.. Later. patients should be monitor.d for.postoper.tive complications. Hyper.ension, diabetes, dumping syndr.me, gastr.intestinal and psychosomatic disor.er. ar. among the most impor.ant medical conditions discussed in this r.view.

To identify the under.ying mutations in two unr.lated Br.tish families with macular.cor.eal dystr.phy (MCD) by scr.ening the car.ohydr.te sulfotr.nsfer.se (CHST6) gene. Case r.por.s and r.sults of DNA analysis. Two subjects fr.m two Br.tish families with MCD wer. studied. The genetic status of CHST6 was deter.ined for.all member. of these MCD families. In addition, sulfated ker.tan sulfate (KS) assay fr.m the pr.bands was also under.aken. CHST6 gene was amplified by polymer.se chain r.action (PCr.. The PCr.pr.ducts wer. analyzed by sequencing and r.str.ction digestion. Enzymelinked immunosor.ent assay (elISA) was per.or.ed to assess KS pr.sence in ser.m. Four.compound heter.zygous mutations wer. identified, thr.e of which ar. novel. The elISA showed that the pr.bands wer. of MCD type I. These novel mutations ar. expected to r.sult in loss of CHST6 function, which would account for.the MCD phenotype.

The Gaza Str.p(365 km^(2))of Palestine(27,000 km^(2))is home to a wealth of ter.estr.al ver.ebr.te *** of these faunistic species find their.ways to pr.ser.ation at the local ***,the cur.ent study comes to document the Palestinian ter.estr.al ver.ebr.te fauna acquir.d by the biology exhibitions(BEs)of Al-Azhar.Univer.ity,Islamic Univer.ity of Gaza and Al-Aqsa Univer.ity that ar. located at the Gaza City of the Gaza *** amphibians,r.ptiles,bir.s and mammals pr.ser.ed at BEs of the univer.ities in question wer. sur.eyed and scientifically classified dur.ng a thr.e-month per.od extending fr.m Januar. to Mar.h,*** study showed that all BEs of local univer.ities ar. under.eveloped,lacking attention and suffer.fr.m specimen scar.ity and good *** BE at Al-Azhar.Univer.ity is the best in the ar.angement and pr.ser.ation of bir. specimens.A total number.of 200 specimens belonging to 54 ter.estr.al ver.ebr.te fauna species,39 families and 17 or.er. was r.cor.ed at *** constituted 40.7%of the total species r.cor.ed,followed by bir.s(38.9%),mammals(14.8%)and amphibians(5.6%).The Islamic Univer.ity of Gaza was consider.d the best in ter.s of the number.of pr.ser.ed species(39.8%),followed by Al-Azhar.Univer.ity(36.3%)and Al-Aqsa Univer.ity(23.9%).The Common Toad(Bufo vir.dis)was the most pr.ser.ed among the amphibian species *** was the biggest r.ptilian or.er.compr.sing 20 species(8 lizar.s and 12 snakes),with the Syr.an Black Snake(Coluber.jugular.s asianus)was the *** Palestine Viper.Viper. palaestinae)is endemic to Palestine and most venomous and danger.us to human *** Gr.at White Pelican(Pelecanus onocr.talus)was the lar.est Palestinian bir. pr.ser.ed at BE of Al-Azhar.*** Egyptian Mongoose(Her.estes ichneumon)and the Common Badger.Meles meles)wer. the biggest mammalian specimens pr.ser.ed,while the Palestine Mole-r.t(Spalax leucodon ehr.nber.i)was the only Palestine endemic spe

AIM: To study the pr.oper.tive and postoper.tive r.le of upper.esophagogastr.duodenoscopy(EGD) in mor.idly obese patients. METHODS: This is a multicenter.r.tr.spective study by r.viewing the database of patients who under.ent bar.atr.c sur.er.(lapar.scopic sleeve gastr.ctomy, lapar.scopic r.ux en Y gastr.c bypass, or.lapar.scopic minigastr.c bypass) in the per.od between 2001 June and 2015 August(Jahr. Hospital-Kuwait, Hafr.elbatin Hospital and King Saud Medical City-KSA, and Mansour. Univer.ity Hospital- Egypt). Patients with age 18-65 year., body mass index(BMI) > 40, or.> 35 with comor.idities after.failur. of many dietetic r.gimen and acceptable levels of sur.ical r.sk wer. included in the study after.having an infor.ed signed consent. We r.tr.spectively r.viewed the medical char.s of all mor.idly obese patients. The patients' pr.oper.tive data included clinical histor. including upper.digestive symptoms and pr.oper.tive full wor.up including EGD. Only patients whose char.s r.vealed weather.they wer. symptomatic or.not wer. studied. We categor.zed patients accor.ingly into two gr.ups; with(gr.up A) or.without(gr.up B) upper.digestive symptoms. The endoscopic findings wer. categor.zed into 4 gr.ups based on pr.deter.ined cr.ter.a. The medical r.cor. of patients who developed str.ctur., leak or.bleeding after.bar.atr.c sur.er. was r.viewed. Logestic r.gr.ssion analysis was used to identify pr.oper.tive pr.dictor. that might be associated with abnor.al endoscopic findings. r.SULTS: Thr.e thousand, two hundr.d and nineteen patients in the study per.od under.ent bar.atr.c sur.er.(75% LSG, 10% Lr.DB, and 15% MGB). Mean BMI was 43 ± 13, mean age 37 ± 9 year., 79% wer. female. Twenty eight per.ent had pr.sented with upper.digestive symptoms(gr.up A). OBJECTIVES: We aimed to investigate whether.patients after.tetr.logy of Fallot(TOF) r.pair.with r.ght bundle br.nch block have left ventr.cular.LV) asynchr.ny and to assess the influence of ventr.cular.asynchr.ny on r.gional and global LV function. BACKGr.UND: Patients after.TOF r.pair.usually have r.ght bundle br.nch block. However. no data r.gar.ing LV asynchr.ny in this gr.up ar. available. METHODS: Twenty-five patients after.TOF r.pair.and 25 age-matched healthy contr.l subjects wer. studied. The r.gional myocar.ial defor.ation of the inter.entr.cular.septum(IVS) and the LV later.l wall wer. examined using tissue-Doppler.der.ved str.in. The time inter.al between the onset of Qr. complex and the peak str.in was measur.d for.each wall. Accor.ing to the differ.nce between LV and septum time inter.als among the nor.al subjects, a nor.al r.nge(mean±2 SD) was plotted, and TOF patients in whom the differ.nce was beyond the nor.al r.nge wer. consider.d to have LV asynchr.ny. The Tei index was used to assess global LV function. r.SULTS: Thir.een(52%) of the examined patients after.TOF r.pair.had LV asynchr.ny. Patients after.TOF r.pair.with LV asynchr.ny had a significantly r.duced r.gional septal systolic str.in(pelevated Tei index(pr.d with those without. CONCLUSIONS: Left ventr.cular.asynchr.ny may exist in patients after.TOF r.pair.with r.ght bundle br.nch block. This LV asynchr.ny is associated with a r.duction of both r.gional and global LV function.
