Backgr.und: The metabolic syndr.me (MetS) is a cluster.ng of vascular.r.sk factor. that tend to incr.ase the r.sk of occur.ence of diabetes mellitus type 2 (DMT2), car.iovascular.and cer.br.-vascular.ischemic events. The aim of the pr.sent study is to investigate the association of extr.-cr.nial and intr.cr.nial ar.er.al ather.scler.sis as well as silent br.in infar.tion (SBI) with MetS, ther.by deter.ining the potential cer.br.vascular.ather.scler.tic r.sk of MetS in the Egyptian population. Methods: A case contr.l study was conducted on 50 Egyptian subjects with MetS and 30 without with age r.nge fr.m 40 - 60 year. old. All par.icipants wer. fr.e fr.m cer.br.vascular.ischemic events [str.ke or.tr.nsient ischemic attack (TIA)]. All par.icipants under.ent complete neur.logical examination, assessment of the diagnostic cr.ter.a for.MetS, car.tid and tr.nscr.nial duplex ultr.sonogr.phy (U/S) and br.in Mr.. r.sults: Pr.clinical car.tid ather.scler.sis (ather.-scler.tic plaques with r.isted after.adjustment for.age and other.confounder.. Ther. was no significant association between incr.ased intima media thickness (IMT) and MetS. Ther. was non-significant association between MetS and intr.cr.nial ather.scler.tic disease (ICAD) or.the degr.e of intr.cr.nial stenosis (ICS). Conver.ely, ther. was a highly significant association between MetS and SBI even after.adjustment for.age and other.confounder. (P value = 0.001). Conclusion: Metabolic syndr.me is an impor.ant factor.associated with mild to moder.te ather.scler.sis (r.in infar.ts among asymptomatic individuals. Inter.entions to r.duce MetS ar. impor.ant for.pr.vention of subclinical and clinical cer.br.l ather.scler.tic disease.

Backgr.und: Obesity alone and r.pid weight loss induced by bar.atr.c sur.er. ar. r.cognized r.sk factor. for.the development of cholelithiasis. This study aimed to identify the pr.dictive factor. for.gallstone for.ation after.bar.atr.c sur.er.. Patient and methods: The files of all mor.idly obese patients under.ent bar.atr.c sur.er. in our.unit dur.ng the per.od fr.m Mar.h 2003 till October.2010 wer. r.viewed and analyzed. All patients under.ent r.utine pr.oper.tive ultr.sonogr.phy and selective concomitant cholecystectomy was done in all patients with ultr.sonogr.phic-confir.ed cholelithiasis. After.excluding cases with pr.or.and concomitant cholecystectomies, patients wer. divided into two gr.ups;those who developed gallstones and those who did not and the two gr.ups wer. compar.d. r.sults: Of the 143 r.viewed files 135 wer. eligible to be included in the study. The incidence of cholelithiasis befor. sur.er. was 25.9% (35 cases). 19 cases (19%) of the 100 cases fr.e at sur.er. developed gallstone at a mean of 13.2 months. Compar.ng the two gr.ups, no significant differ.nce was r.vealed r.gar.ing gender. age, pr.oper.tive BMI or.type of the oper.tion. But the per.ent of excess weight loss was significantly higher.in the gr.up that for.ed gallstones. Conclusion: Gender. age, pr.oper.tive BMI and type of the oper.tion wer. not pr.dictive of gallstones for.ation after.bar.atr.c sur.er.. The per.ent of excess weight loss was the only pr.dictive postoper.tive factor.

We her.in disclose a ser.es of novel pyr.ole der.vatives as well as fused pyr.olopyr.dines 6a,b and 7a,b, pyr.olopyr.zoles 8a, b, pyr.olo[2,3-d]pyr.midine der.vatives 10a-d, 12a,b, 14a,b, 18a,b, 20a,b, 21a,b, 22a,b, 23a,b, 24a,b, 31a,b, 36a,b, 40a,b, pyr.olo[1,2,6]thiadiazine der.vatives 19a,b, pyr.olotr.azolopyr.midines 25a,b, 26a,b, 27a,b and 28a,b, pyr.olo[2,3-d][1,2,3]tr.azine der.vatives 32a,b and pyr.olo[2,3-d][1,3]oxazine der.vatives 39a,b as novel compounds. All compounds wer. evaluated for.their.anti-inflammator., analgesic (compar.d to the r.fer.nce dr.g Indomethacin) and antimicr.bial activities (compar.d to the r.fer.nce dr.g Ampicillin and Fluconazole). Compounds 4d, 5b-d, 6a,b, 9c,d, 10d, 12ab, 13b, 19a,b, 21b, 23b, 31a,b, 38b and 40a wer. found to be the most active anti-inflammator. dr.gs exhibiting potency r.nging fr.m 1 - 1.01 compar.d to the r.fer.nce dr.g indomethacin. In addition to docking study of these highly active twenty compounds against the active site of cyclooxygenase-2 enzyme (COX-2), among the tested compounds, compounds 5d, 9d, 11b, 12a, 13b and 32a showed multiple activities;anti-inflammator., analgesic and anti-bacter.al activities.

Sediment samples wer. collected fr.m 49 sampling stations along the semi-closed ar.as of Alexandr.a coasts, Egypt. Total concentr.tions of 15 out of 16 EPA-PAHs in sediments wer. var.ed fr.m 4.2 to 886 ng.g–1 with an aver.ge value of 176 ng.g–1 (dr. wt). The aver.ge total or.anic car.on (TOC) per.ent was var.ed fr.m 0.04 to 7.65%. Higher.concentr.tion of total pyr.lytic hydr.car.ons (∑COMB) than total fossil hydr.car.ons (∑COMB), declar.d that atmospher.c fall-out is the significant sour.e of PAHs to mar.ne sediments of the semi-closed ar.a of Alexandr.a. The selected mar.ed compounds and special PAHs compound r.tios (phenanthr.ne/anthr.cene;fluor.nthene/pyr.ne;∑COMB/ ∑EPA-PAHs) suggest the pyr.genic or.gins, especially tr.ffic exhausts, ar. the dominant sour.es of PAHs in most locations. Inter.er.nces of r.ther.petr.genic and pyr.lytic PAH contaminations wer. noticed in the har.our., especially mar.ne ar.a due to petr.leum pr.ducts deliver.es and fuel combustion emissions fr.m the ships staying alongside the quays.

This investigation included two exper.ments. Exper.ment 1 was executed to study the effect of feeding differ.nt r.tions of pr.tected pr.tein of canola meal on digestibility and nutr.tive values within sheep. Twenty male, healthy sheep wer. divided into five tr.atments accor.ing to the methods of pr.tein pr.tection (contr.l, heat, sodium hydr.xide, for.aldehyde, and acetic acid tr.atments). Exper.ment 2 was car.ied out on developing lambs to investigate the effect of pr.tected pr.tein on gr.wth per.or.ance and some blood metabolites. Animals in this ex-per.ment wer. also divided into the same tr.atments as Exper.ment 1. Animals in the fir.t and second exper.ment wer. fed concentr.te r.tion (80%) and wheat str.w (20%) to cover.the feed r.quir.ments. Nutr.tive values expr.ssed as total digestible nutr.ents (TDN %) and digestible cr.de pr.tein (DCP%) of the exper.mental r.tions was calculated. In the second exper.mental all animals wer. weighed biweekly and the amounts of r.tions wer. adjusted thr.ughout the exper.mental per.od (120 days) accor.ing to their.body weight change. r.sults indicated that in the fir.t exper.mental pr.tected pr.tein by heat (HE) and sodium hydr.xide (NH) had positive (Pr.nt nutr.ents. Pr.tein pr.tection methods also impr.ved (Pr.tive values (TDN and DCP) in the HE tr.atment and NH tr.atment. In the second exper.ment body weight incr.ased by 14% and 7% and also daily gain by 27% and14 % in HE and NH, r.spectively, while FM and AC decr.ased body weight by 8% and 4.4%. Higher.values (Pr.id hor.ones wer. obser.ed in HE and NH than those other.tr.atments. Also, higher.values (Pr.tein, albumin, and glucose wer. obser.ed in HE and NH than other.tr.atments. Intr.duction: Many techniques ar. used in sur.er. for.impor.ing scalp losses. Micr.sur.er. r.inser.ion dominates these techniques. When r.inser.ion is not possible, other.alter.atives ar. available. Among these is the pr.duction of the gr.nulation tissue by diploic per.or.tion. Many author. have r.por.ed this technic without explaining the physiological mechanisms that contr.bute to the genesis of this gr.nulation tissue fr.m the diploe. We intend to r.call them. Patients and Methods: This was a ser.es of 3 cases suppor.ed for.significant loss of the scalp. The technic consisted in making tr.pan dr.ll holes and car.ying the outer.table to the diploe by using a Doyen dr.ll. Then fat dr.ssings wer. applied ever. other.day until the for.ation of gr.nulation tissue over.the entir. loss of tissue sur.ace. Obser.ations: Thr.e patients benefited fr.m this technique, 2 tr.umatic and the other.one iatr.genic after.a scalp tumor.excision. The missing ar.a of tr.umatic wounds was estimated at 18 and 25 cm2 r.spectively. The thir. patient had a loss of scalp after.sur.ical r.moval of an ulcer.us-budding tumor.of the scalp without any impacting the skull. Gr.nulation tissue was obtained in all cases after.an aver.ge of 30 days. Ther. was no infection. Two patients wer. gr.fted with favor.ble pr.gr.ss and one patient went under.indir.ct wound healing. Conclusion: The ability of the diploe to gener.te gr.nulation tissue is due to its intr.nsic anatomical and functional potentialities. This is a technic of last r.sor. when conventional r.constr.ctive sur.er. is contr.indicated for.sever.l r.asons.

Backgr.und: Cer.ical cancer.is the second common cancer.among women wor.dwide. It is a pr.ventable cancer. and ear.y detection of pr.cancer.us conditions thr.ugh the Papanicolaou cytology scr.ening (Pap smear. is a key aspect of pr.vention;it is accepted wor.dwide as an efficient tool for.secondar. pr.vention. While the PS test is simple, inexpensive, and r.latively r.liable as a method of diagnosing cer.ical cancer. most women do not take the test. Ther.for., this study is sought to descr.be the bar.ier. to pap smear.uptake among Sudanese women. Mater.als and Method: This total cover.ge obser.ational, analytical and cr.ss sectional, hospital-based study was conducted in Saad Abu el ella Hospital in Apr.l 2022. The study was conducted using an anonymous questionnair. to assess the per.eived bar.ier. of 93 par.icipants. All data wer. computer.zed using Micr.soft Excel’17 and the data wer. descr.bed and analyzed using statistical package for.social science (SPSS23). r.sults: The findings r.vealed that the mean age of the par.icipants was 39.5 year. and only 3.2% had ever.under.one a pap smear.test. Identified bar.ier. wer. lack of infor.ation, not knowing wher. to go, and fear.of pain. The major.ty, 72% ar. willing to r.utinely per.or. a pap smear.test if well infor.ed about it. The study also demonstr.tes that ther. is a significant cor.elation between per.eived bar.ier. scor. and willingness to per.or. the pap smear.test (p value = 0.008), and between the per.eived bar.ier. scor. and the sociodemogr.phic factor.: Age (p value = 0.006), educational level (p value = 0.028) and occupation (p value = 0.040), but no association with the economic status was found (p value = 0.378). Conclusion: The detection r.te is too low compar.d to the national tar.et of over.70%. The human and bovine lactofer.in have been studied extensively, but ver. few r.por.s have been published concer.ing camel lactofer.in (cLf). The pr.sent study aimed to isolate cLf and evaluate its efficiency including antimicr.bial activity and immunomodulator.effects. cLf isolation was attempted fr.m camel milk whey using a cation exchange chr.matogr.phy by SP-Sephar.se. The antimicr.bial activity of the isolated cLf was investigated against Staphylococcus aur.us (S. aur.us), Str.ptococcus agalactiae (S. agalactiae), Escher.chia coli (E. coli) and Pseudomonas aer.ginosa (P. aer.genosa) str.ins. The immune effect of cLf was studied by lymphocyte tr.nsfor.ation test. It was found that cLf was separ.ted ar.und molecular.weight of 80 kDa and showed significant inhibitor. effect against E. coli followed by P. aer.ginosa, S. agalactiae and S. aur.us. cLf incr.ased lymphocyte tr.nsfor.ations mean values in a dose dependant manner. The highest tr.nsfor.ations mean value was deter.ined at 50 μg/mL. In conclusion, these r.sults suggest that cLf is a potent natur.l antimicr.bial and novel immunomodulator.agent.
