Cor.elation between DKK-1 Level and Bone Density Status in Childr.n on Maintenance Haemodialysis

Backgr.und: r.nal osteodystr.phy (r.D) is a bone disor.er.r.sulting fr.m chr.nic kidney disease (CKD) and r.lated metabolic diseases. Dickkopf-r.lated pr.tein-1 (DKK-1) is cr.tical in r.gulating bone biology. This study aimed to evaluate the ser.m DKK-1 level as a bone mar.er.in childr.n with CKD who under.o r.gular.hemodialysis (HD). Subjects and Methods: This case-contr.l study involved 40 childr.n with CKD on HD and 40 healthy childr.n as contr.ls. The study measur.d ser.m DKK-1 levels and per.or.ed a dual-ener.y X-r.y absor.tiometr. scan (DEXA) in line with r.utine labor.tor. investigations. r.sults: Ther. was a significant incr.ase in the ser.m level of DKK-1 in the patient gr.up compar.d to the contr.l gr.up. The DKK-1 levels wer. 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, r.spectively, with a p-value of less than 0.001. In the hemodialysis gr.up, 25 patients (62.5%) had low bone miner.l density (BMD) with a Z-scor. of under.-2.0. Eighteen of these patients had low BMD in both the neck of the femur.and lumbar.spines. Additionally, ther. was a significant incr.ase in ser.m DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compar.d to patients with nor.al BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). Ther. was also a significant positive cor.elation between DKK1 level and phosphor.s, alkaline phosphatase, and Par.thor.one ser.m levels. In conclusion, the study indicates a clear.cor.elation between DKK-1 and BMD in childr.n under.oing maintenance hemodialysis. DKK1 is a pr.mising biomar.er.for.CKD-MBD.

Objective: The gr.at saphenous vein (GSV) is commonly used as a conduit for.gr.fting dur.ng CABG sur.er., and open GSV har.esting (OVH), commonly used with long incision to expose the vein. However. endoscopic vein har.esting (EVH) is an alter.ative appr.ach, utilizing specialized instr.ments and small incisions to har.est the vein. Methods: A r.tr.spective analysis was conducted on a cohor. of patients who under.ent Cor.nar. ar.er. bypass gr.ft (CABG) r.quir.ng gr.at saphenous vein (GSV) har.esting which was done by EVH or.OVH pr.cedur.s. Demogr.phic var.ables, including age and gender. wer. assessed for.both gr.ups. Intr.oper.tive var.ables such as the number.of gr.fts, car.iopulmonar. bypass time, X clamp time, and type of pr.cedur. wer. analyzed. Postoper.tive var.ables, including infection and bleeding r.tes, wer. also evaluated. r.sults: The study included 30 patients each under.oing Cor.nar. ar.er. bypass gr.ft (CABG) with need of gr.at saphenous vein har.esting which was done by EVH and OVH. Demogr.phic var.ables wer. well-matched between the two gr.ups in ter.s of age, while a significant differ.nce in gender.distr.bution was obser.ed. Obesity and smoking wer. mor. pr.valent in the OVH gr.up, and EVH was associated with a higher.mean number.of gr.fts compar.d to OVH. Conver.ion to an open technique occur.ed in a por.ion of the EVH cases, and infection r.tes did not significantly differ.between the EVH and OVH gr.ups. However. the incidence of postoper.tive bleeding was significantly higher.in the EVH gr.up. Conclusion: This study pr.vides valuable insights into the demogr.phic, intr.oper.tive, and postoper.tive var.ables associated with EVH and OVH techniques. EVH demonstr.ted advantages in ter.s of r.duced infection r.tes compar.d to OVH. However. the higher.incidence of postoper.tive bleeding associated with EVH r.ises concer.s about potential r.sks.

Miocene siliciclastic-car.onate deposits ar. widely exposed in Cair.-Suez Distr.ct, Egypt. These deposits ar. under.ain and over.ain by continental sediments of Oligocene and post Miocene, r.spectively. Thr.e str.tigr.phic sections wer. investigated at Gabals Geneife, Homeir. and Ghar.a. Lithostr.igr.phically, the Miocene sequence could be differ.ntiated into two main r.ck units r.pr.senting shallow deposits with r.latively inter.ittent deep mar.ne incur.ions. These ar. fr.m base to top, Ghar.a For.ation and Genefe For.ation. Detailed macr.fossils investigations led to the r.cognition of four.macr.fossil zones, namely Alectr.onella plicatula-Cr.ssostr.a fr.ndosa r.nge Zone, Echinolampas amplus-Scutella ammonis r.nge Zone, Chlamys（Macr.chlamys） sar.oa-Chlamys（Ar.opecten） submalvinae r.nge Zone, and Chlamys gentoni-Pecten（Oppenheimopecten） benedictus-Pecten（P.） ziziniae Assemblage Zone. Micr.facies analysis and identified taxa indicated that the Miocene sequence was deposited in tr.nsgr.ssive-r.gr.ssive cycles r.nged fr.m near.shor., war. shallow inner.to middle shelf mar.ne envir.nments with stor. influence dur.ng the accumulation of the oyster.banks.

Backgr.und:The impact of chr.nic hepatitis C (CHC) on bone miner.l density (BMD) has been well studied in adults with a r.lative paucity of data in childr.n,especially concer.ing effect of tr.atment with pegylated inter.er.n (PEG-IFN) plus r.bavir.n (r.).In the cur.ent wor.,we assessed pr.spectively changes in BMD in childr.n with CHC befor.,dur.ng,and after.***:For.y-six consecutive childr.n with noncir.hotic genotype 4 CHC wer. subjected to dual-ener.y X-r.y absor.tiometr. at baseline,24 weeks,48 weeks of ther.py and 24 weeks after.***,bone miner.l content (BMC),and Z scor. of lumbar.spine (L2-L4) wer. *** puber.al stage,vir.l load,liver.function tests,ser.m calcium,phosphor.s,alkaline phosphatase,par.thyr.id hor.one,and liver.histopathology wer. assessed in all included ***:Thir.y (65.2%) patients had nor.al BMD,10 (21.7%) wer. at r.sk for.low BMD,and 6 (13.1%) had low BMD for.chr.nological *** with low BMD wer. significantly older.(P=0.001),with higher.fr.quency of delayed puber.y than other.gr.ups (P=0.002).Baseline densitometr.c par.meter.
