Primary Lymphoma of the Larynx: A Case Report and Literature Review

Background: Primary non-hodgkin lymphoma of the larynx is a rare entity accounting for 1% of laryngeal tumors. Fewer than 100 cases have been reported in the literature. Case presentation: A 52-year-old man, presented with a two-month history of dysphonia. Nasofibroscopy showed left submucosal bulging of the laryngeal supraglottic floor. Cervical computed tomography showed extensive left supraglottic mass, with partial lysis of the thyroid cartilage. Direct laryngoscopy showed left submucosal bulging of the supraglottic floor. Transmucosal biopsies were performed. Histological study concluded to the presence of a diffuse large-cell B lymphoma. The patient was treated with chemotherapy followed by external radiotherapy. Evolution was favourable. Conclusion: Although primary laryngeal non-hodgkin lymphoma is rare, it should be considered in the differential diagnosis of a mass in the neck region, especially in the supraglottic area. Direct laryngoscopy with biopsy makes the diagnosis. Treatment and prognosis depend on the stage and the grade of the lymphoma.

Intracranial complications of sinusitis are rare. However, they have an important morbidity and mortality rate, and can be a source of disabling neurological sequelae in the absence of a rapid diagnosis and adequate treatment. We carry a retrospective study of 23 patients having sinusitis with intracranial complications, treated between 1996 and 2011. All patients underwent complete ENT and neurological examination, biological investigations and sinonasal and cerebral CT. An intraveinous large-spectrum antibiotherapy was administered to all patients. Twenty patients underwent surgery. It included evacuation of the intracranial collection, sinus drainage, with or without cranialization of the frontal sinus. Evolution was assessed on clinical biological and radiological criteria. Mean age was 25 years and sex-ratio was 3.6. Neurological signs were the most frequent symptoms. Rhinological signs were essentially purulent rhinorrhea (14 cases) and nasal obstruction (12 cases). Nasal endoscopy showed pus in the middle meatus in 10 cases. On CT, intracranial complications included subdural empyema (11 cases), extradural empyema (7 cases) and brain abscess (5 cases). Associated cerebral thrombophlebitis was noted in 4 cases. Three patients with extradural empyema had had an exclusive medical treatment. All other patients (20 cases) were operated. Clinical and radiological evolution was favorable after initial treatment in 14 cases (60.8%). Six patients required secondary surgery. Two patients have died despite intensive care. The intracranial complications of sinusitis are serious and source of important morbidity and mortality. Management should rapid and adequate, combining effective antibiotic therapy and eventually neurosurgical treatment.

Otosclerosis: Contribution of Computed Tomography and Radio-Anatomical Correlations

Otosclerosis is an osteodystrophia of the otic capsule, responsible for hearing loss by blocking of the stapes footplate. The purpose of this study was to evaluate the reliability of computed tomography (CT) in the positive diagnosis of otosclerosis, to compare the radiological findings to those intraoperatively, and to seek a correlation between imaging data and postoperative audiometric results. This is a retrospective study of 60 patients having otosclerosis, treated in our department between 2009 and 2011. All patients underwent an otoscopic examination, pure tone and vocal audiometry, impedancemetry and petrous CT scan. At petrous CT scan, a footplate thickening was found in 9 cases. Labyrinthine bone hypodensity was noted in 51 cases. Sensitivity of CT in the diagnosis of otosclerosis was 100%. Regarding the operative findings and their correlation with imaging, we found that among the 7 facial canal procidences reported intraoperatively, CT had identified 6, with a sensitivity of 86%. Of the 53 facial nerve in normal position intraoperatively, CT had only identified 34, with a specificity of 64%. Regarding the ossicles, there were 6 cases of abnormalities of the incus, among them 3 were identified on CT, with a sensitivity of 50%. The 54 patients with normal ossicular chain intraoperatively were all identified as such on CT, with a specificity of 100% (p = 0.001). For correlation between imaging and postoperative results, it was found that patients with extended otosclerosis had lower postoperative audiometric improvement than those having localized disease (p < 0.05). Computed tomography is necessary for the diagnosis of otosclerosis. It also helps, with good sensitivity and specificity, seeking for anatomical variants that the surgeon could possibly encounter during surgical procedure. Britholites with the general formula Ca10-xLnx（PO4）6-x（SiO4）xF2, （0≤x≤6） are considered to be promising matrices for the confinement of the by-products in the nuclear industry. A thermodynamic study showed that the stability of these compounds de-creased as the substitution rate increased. The present work was an attempt to gain more information about the structural changes in-duced by the substitution, in order to understand the observed stability decrease. The samples were successfully synthesized as a sin-gle-phase apatite by a solid-state reaction between 1200 and 1400 °C. The structural refinement indicated that the La3＋ions preferen-tially occupied the 6（h） sites. A progressive shift of F-along the c-axis outside its ideal position occurred as a result of the substitution increase. This might be the cause of the observed stability decrease, especially as the energies of the La-O, La-F and Si-O bonds are higher than those of Ca-O, Ca-F and P-O. The distribution of La3＋ between the two non-equivalent sites was confirmed by the charge distribution method.

Structural,elastic,electronic and optical properties of the Pt3Zr intermetallic compound are investigated using first principles calculations based on the density functional theory(DFT)within the generalized gradient approximation(GGA)and the local density approximation(LDA).The Pt3Zr compound is predicted to be of cubic L12 and hexagonal D024 *** calculated equilibrium ground-state properties(lattice parameters a and c,bulk modulus B and its pressure derivative B',formation enthalpy ΔH)of the Pt3Zr compound,for both cubic and hexagonal phases,show good agreement with the experimental results and other theoretical *** constants(C11,C12,C13,C33,C44,and C55)are *** predicted elastic properties such as Young's modulus E and shear modulus GH,Poisson ratioν,anisotropic ratio A,Kleinman parameter ξ,Cauchy pressure(C12-C44),ratios B/C44 and B/G,and Vickers hardness Hv indicate the stiffness,hardness and ductility of the *** characteristic parameters such as Debye temperature θD and melting temperature Tm are *** properties such as density of states(DOS)and electronic specific heat γ are also *** calculated results reveal that the Fermi level is on the psedogap for the D024 structure and on the antibonding side for the L12 *** optical property functions(real partε1(ω)and imaginary part ε2(ω)of dielectric function),optical conductivity σ(ω),refraction index n(ω),reflectivity R(ω),absorption α(ω)and extinction coefficients k(ω)and loss function L(ω)are also investigated for the first time for Pt3Zr in a large gamme of energy from 0 to 70 eV.

The introduction of metals into vitreous matrices is the origin of various interesting phenomena;in particular,the presence of copper ions in glass has been the subject of considerable research because of its numerous *** ion-exchange process is primarily used to introduce copper ions into glass *** thermoluminescence(TL)of silicate glass was studied to evaluate its potential as gamma-sensitive material for dosimetric applications;the effect of copper doping on the thermoluminescent sensitivity was investigated using the Cu-Na ion-exchange technique for different concentrations and doping conditions,over a wide dose range of 10 mGy to 100 *** results showed that Cu doping significantly improved the sensitivity of the glasses to gamma *** the ion-exchange,two peaks appeared in the glow curves at approximately 175 and 230°C,respectively,which possibly originated from the Cu^(+) centers,along with a weak TL peak at around 320℃.We also attempted to explain the origin of the observed thermoluminescence by exploiting the Electron paramagnetic resonance(EPR)*** results clearly show quenching of the TL emission with increasing copper *** present work indicates that the thermoluminescence response of these glasses to gamma rays can be reasonably measured in the range of 0.001-100 *** study also facilitates the understanding of the basic TL mechanism in this glass system.

Traditional farming in the mountainous region of Bangladesh and its modifications

Shifting cultivation is a traditional farming system practiced in the tropical mountainous areas. Although it has been widely perceived as an economically inefficient and environmentally harmful agricultural production system, recent science reviews, however, indicate that the deleterious impacts of shifting cultivation on environment may have been overestimated. Despite the pressures of agricultural intensification in areas where shifting cultivation occurs, farmers across the tropics still maintain this traditional farming system. The objective of this study was to explore existing traditional shifting cultivation practices and their various modifications including the innovative farming techniques developed by farmers in the Chittagong Hill Tracts(CHTs), the mountainous region of Bangladesh, and examine their importance with respect to recent socio-economic and environmental changes. The study revealed that shifting cultivation still exists as the most dominant farming method which supports livelihoods and culture of the hill ethnic people. However, demand for more food and household income to meet livelihood needs of an increasing population combined with a rapid deterioration of soil and water quality over decades contributed to development of innovative farming practices through fallow land farming, crop substitution, agroforestry and homestead gardening in the CHTs. Through these farming techniques farmers maintain a strong relationship with traditional knowledge system embedded in the ageold shifting cultivation practices. Today state policies and market forces act in favor of replacement of traditional farming with intensive cash crop agriculture. It seems that disappearance of traditional farming practices from the hills may threaten local biodiversity and food security. It may be recommended that shifting cultivation should be encouraged in areas where they have potential for contributing to preservation of native biodiversity and ecosystem services, and
