Progressive Symmetrical Erythrokeratoderma-Like Psoriasis: A New Case Report

Background: Psoriasis, a chronic, systemic, inflammatory disease with prominent skin involvement, affects approximately 2% - 4% of the world population. Common variants of psoriasis are plaque psoriasis, inverse psoriasis, guttate psoriasis, erythrodermic psoriasis, pustular form either palmoplantar pustular psoriasis or generalized pustular psoriasis, nail psoriasis, and psoriatic arthritis. Progressive Symmetrical Erythrokeratoderma (PSEK) is a rare genetic disorder, characterized by fixed, well-defined erythematous and hyperkeratotic plaques distributed predominantly on the elbows, knees, trunk, and dorsal surfaces of hands and feet. Clinically has the same presentation to psoriasis especially at early onset and could be confused with psoriasis but histopathological findings and progression of the psoriatic disease can differentiate between both conditions. Aim: To document a new variant of a severe, recalcitrant type of psoriasis with a history of recurrent attacks of exacerbations and partial remissions especially in lesions involving lower extremities that are clinically PSEK-like in presentation, but histopathologically consistent with psoriasis. Case report: A 12-year-old childhood male, known case of Down’s syndrome, presented to our clinic with a history of severely pruritic skin rashes involving the perioral area, corners of the mouth, bilateral elbows, dorsal hands, scrotum, and both lower extremities for 6 years duration. The rashes gradually progressed with time to form fixed lesions in the last 2 years. He was received multiple treatment modalities, including topical steroids, topical vitamin D derivatives, and narrowband UVB phototherapy without significant improvement and the lesions became more worsened over time. A series of new 2,5-disubstituted-l,3,4-oxadiazole and 1,2,4-triazole derivatives were synthesized by hetero- cyclization of acid hydrazide I and thiosemicarbazide derivative 2. Furthermore, the acyclic C-nucleoside analogs were prepared by cyclization of their corresponding sugar hydrazones by reaction with acetic anhydride. The antimicrobial activity of the prepared compounds was evaluated and some of the synthesized compounds revealed good activities against fungi.

Prostatic Abscess Due to Idiopathic Granulomatous Disease: A Rare Complication of a Chameleon Disorder —Prostatic Abscess Due to Idiopathic Granulomatous Disease

Background: Prostatic abscesses are usually diagnosed in the setting of bacterial prostatitis. Rarely, they reveal or complicate granulomatous prostatitis (GP). Four cases of idiopathic xanthogranulomatous GP have been described previously and the present case report is the first of typical idiopathic variety. The case: A 60-year-old man presented with urine retention that was associated with pyuria and massively enlarged prostate. Cystoscopy revealed prostatic abscess (PA) that was opened. Urine and prostatic culture were negative for bacteria. Prostatic biopsy revealed multiple non-caseating granulomata surrounded by lymphocytes, plasma cells yet without foamy histiocytes, parasites and vasculitis. Special stains were negative for vasculitis, fungiand acid-fast organisms. The patient was treated with Solumedrol 1 g intravenously daily for 3 days followed by Prednisone 1 mg/kg/day for 1 month followed by gradual tapering till discontinuation by 3rd month. Moreover, he had received Mycophenolate mofetil (MMF) 1 g twice/daily. By the end of 2nd month;he was asymptomatic and without pyuria. Repeat cystourethroscopy and MRI scan of the prostate showed near normal prostate. In Conclusion: Idiopathic GP can present with PA that requires proper drainage and since it is a locally hyperimmune disease with genetic predisposition;MMF therapy will be maintained for a total of 2 years to prevent future disease-relapse.

Computational Analysis for Solving the Linear Space-Fractional Telegraph Equation

Over the last few years, there has been a significant increase in attention paid to fractional differential equations, given their wide array of applications in the fields of physics and engineering. The recent development of using fractional telegraph equations as models in some fields (e.g., the thermal diffusion in fractal media) has heightened the importance of examining the method of solutions for such equations (both approximate and analytic). The present work is designed to serve as a valuable contribution to work in this field. The key objective of this work is to propose a general framework that can be used to guide quadratic spline functions in order to create a numerical method for obtaining an approximation solution using the linear space-fractional telegraph equation. Additionally, the Von Neumann method was employed to measure the stability of the analytical scheme, which showed that the proposed method is conditionally stable. What’s more, the proposal contains a numerical example that illustrates how the proposed method can be implemented practically, whilst the error estimates and numerical stability results are discussed in depth. The findings indicate that the proposed model is highly effective, convenient and accurate for solving the relevant problems and is suitable for use with approximate solutions acquired through the two-dimensional differential transform method that has been developed for linear partial differential equations with space- and time-fractional derivatives.

This paper presents a study of the segmentation of medical *** paper provides a solid introduction to image enhancement along with image segmentation *** the first step,the morphological operations are employed to ensure image detail protection and *** objective of using morphological operations is to remove the defects in the texture of the ***,the Fuzzy C-Means(FCM)clustering algorithm is used to modify membership function based only on the spatial neighbors instead of the distance between pixels within local spatial neighbors and cluster *** proposed technique is very simple to implement and significantly fast since it is not necessary to compute the distance between the neighboring pixels and the cluster *** is also efficient when dealing with noisy images because of its ability to efficiently improve the membership partition *** results are performed on different medical image ***(Us),X-ray(Mammogram),Computed Tomography(CT),Positron Emission Tomography(PET),and Magnetic Resonance(MR)images are the main medical image modalities used in this *** obtained results illustrate that the proposed technique can achieve good results with a short time and efficient image *** results on different image modalities show that the proposed technique can achieve segmentation accuracies of 98.83%,99.71%,99.83%,99.85%,and 99.74%for Us,Mammogram,CT,PET,and MRI images,respectively.

Patterns of brain injuries associated with maxillofacial fractures and its fate in emergency Egyptian polytrauma patients

Purpose： Maxillofacial injuries are commonly encountered in the practice of emergency medicine. More than 50% of patients with these injuries have multisystem trauma that requires coordinated manage- ment between emergency physicians and surgical specialists in oral and maxillofacial surgery, otolar- yngology, plastic surgery, ophthalmology, and trauma surgery. The aim of this study is to identify the patterns of brain injuries associated with maxillofacial trauma and its outcome. Methods： This descriptive study （cross-sectional） was carried out among 90 polytrauma patients with maxillofacial fractures attending the Emergency Department at Suez Canal university Hospital and fulfilling our inclusion and exclusion criteria. Results： This study demonstrates the relation between type of maxillofacial fracture and type of trau- matic brain injuries in which the majority of patients with epidural hemorrhage presented with mid face fractures （60%）, while the minority of them presented with upper and lower face fractures （20% for each of them）. The majority of patients with subdural hemorrhage were associated with mid face fractures （75%）, the majority of patients with brain contusions associated with mid face fractures （75%）, and all of the patients presented by pneumocephalus were associated with mid face fractures （100%）. Conclusion： The results of this study confirm the value of quick diagnosis and early intervention, which is fundamental to prevent morbidity as well as mortality especially with regards to prevention of traumatic brain injury as even a short duration of hvpoxia and edema will lead to significant permanent neurological deficits.

Efficient Multiobjective Genetic algorithm for Solving Transportation, Assignment, and Transshipment Problems

This paper presents an efficient genetic algorithm for solving multiobjective transportation problem, assignment, and transshipment Problems. The proposed approach integrates the merits of both genetic algorithm (GA) and local search (LS) scheme. The algorithm maintains a finite-sized archive of non-dominated solutions which gets iteratively updated in the presence of new solutions based on clustering algorithm. The use clustering algorithm makes the algorithms practical by allowing a decision maker to control the resolution of the Pareto set approximation. To increase GAs’ problem solution power, local search technique is implemented as neighborhood search engine where it intends to explore the less-crowded area in the current archive to possibly obtain more nondominated solutions. The inclusion of local search and clustering algorithm speeds-up the search process and also helps in obtaining a fine-grained value for the objective functions. Finally, we report numerical results in order to establish the actual computational burden of the proposed algorithm and to assess its performances with respect to classical approaches for solving MOTP.
