Edge enhancement is derived from lack of accurate result from edge detection techniques. The image which is captured from long distances carries a lot of noise and blur which causes edge discontinuity. Although some novel algorithms which are based on cellular neural network, fuzzy enhancement and binary morphology have shown accuracy in order to obtain refined edge but still the problem of edge discontinuity arises. Eliminating discontinuity of edge a hybrid technique is proposed based on pixel neighbors pattern analysis PNPA. In the technique Canny operator for initial edge detection, PNPA operation for edge enhancement are performed for remote sensing satellite image successively. The visual and subjective evaluation shows that the proposed PNPA operation can effectively eliminate the influence of edge discontinuity which occurred due to noise and blurr in original captured image, as comparing to existing edge segmenting processes.

Soluble sugar is a key quality trait of soybean *** developed rapid and economic extraction and quantification methods for seed soluble sugars using an ultra-high performance liquid chromatography system with a refractive index *** evaluated the soluble sugar compositions of 1164 soybean accessions collected from diverse ecoregions and grown in multiple locations and *** soluble sugar(TSS)content was influenced by accession type,year of cultivation,and *** mean contents of fructose,glucose,sucrose,raffinose,stachyose and TSS were 3.31,5.21,55.60,6.60,35.47,and 106.19 mg g^(-1),*** highest mean TSS content(108.71 mg g^(-1)) was observed in accessions from Northern Region of *** contained higher contents of sucrose,raffinose,and TSS,whereas landraces had a higher content of *** accessions with mean TSS contents>130 mg g^(-1) were identified as elite soybean *** was correlated with sucrose,raffinose,stachyose,protein,oil and total *** main soluble sugar components were correlated with latitude and longitude,indicating that the geographical origin of the accessions affected their seed soluble sugar *** developed methods and elite identified accessions can be used in the food and feed industry and in soybean breeding programs aimed at improving soybean seed nutrition.

AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver ***:We studied retrospectively all patients who underwent PD in our centre between January 2002and December *** A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic *** cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were *** primary outcome measure was hospital mortality *** outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival ***:Only 67/442 patients(15.2%)had cirrhotic *** blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic *** stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group *** hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with ***:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver *** is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension.

Purpose: To study the association between carotid intima media thickness (CIMT) and high sensitivity CRP (hs-CRP) level as markers for PAD in elderly patients with DM. Subjects: A case-control study on 90 participants aged 60 years and older divided into 60 cases (30 patients with DM alone and 30 patients with DM and comorbidities) and 30 healthy controls. All were assessed by measuring CIMT, ankle brachial index (AB), and markers for cardiovascular disease such as high-sensitivity CRP (hs-CRP), total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), and low density lipoprotein (LDL). Results: hs-CRP levels showed statistically significant difference being highest among patients with DM and comorbidities and lowest among controls (P < 0.001). Also, symptoms of PAD were significantly higher among cases than controls. ABI was able to detect PAD in many asymptomatic patients. Color changes were present in only 43.30% (n = 26) of positive PAD cases while delayed wound healing, claudication pain, rest pain, cold extremities, and trophic changes were present in 23.30% (n = 14), 16.71% (n = 10), 16.71% (n = 10), 45.00% (n = 27), and 21.7% (n = 13). Using logistic regression analysis revealed that DM, CIMT, and hs-CRP were independent predictors for PAD (OR = 4.194, 7.236, 1.003;P value = 0.044, 0.25, 0.031) after adjustment of other coronary risk factors such as sex, smoking, hypertension, TC, and TG. Conclusion: Diabetic elderly have higher prevalence of asymptomatic PAD thannon-diabetics using solely ABI. DM, CIMT, and hs-CRP are independent predictors for the occurrence of PAD. Hs-CRP levels are highest among diabetics with comorbidities.

AIM:To evaluate the impact of mass vaccination against the hepatitis B virus (HBV) in Egypt,and to search for vaccinee asymptomatic breakthrough HBV infection and its ***:Seven hundred serum samples from vaccinated children and adults (aged 2-47 years) were used for quantitative and qualitative detection of HBsAb by *** hundred and sixty serum samples representing undetectable or low or high HBsAb were screened for markers of active HBV infection (HBsAg,HBcAb (IgG) and HBeAb by ELISA,plus HBsAg by AxSYM) and HBV-DNA genotyping by nested multiplex PCR and by DNA ***:It was found that 65% of children aged 2-4 years,and 20.5% aged 4-13 years,as well as 45% adults were good responders to HBV vaccination mounting protective level *** responders were 28%,59.5% and 34%,and non-responders were 7%,20% and 21% respectively,in the three studied *** of asymptomatic HBV infections were HBsAg detected by ELISA in 2.5% vs 11.39% by *** markers were HBcAb (IgG) in 1.38%,HBeAb in 0.83%,and HBV-DNA in 7.8%.All had HBV genotype E ***:It is concluded that HBV vaccine is efficient in controlling HBV infection among children and *** vaccine breakthrough infection was by HBV genotype E.A booster dose of vaccine is recommended,probably four years after initial vaccination.

Introduction: Nowadays the more accepted surgical option for treating early breast cancer is breast conserving surgery. The main challenge in this type of surgery is to get free safety margins without need of second surgical operation, so many breast surgeons have started to depend on intraoperative frozen sections to ensure free safety margins. Aim of work: To assess our policy that we prefer to depend on intraoperative frozen section analysis to get free safety margin in breast conserving surgery from the 1st surgery, and its oncologic outcome. Patients and Methods: This is a retrospective study conducted in Oncology Center—Mansoura University (OCMU), where the data of 219 patients with breast cancer, who were managed by breast conserving surgery with intraoperative frozen section analysis of the safety margins, was analyzed. Results: The intraoperative frozen section analysis of safety margin was negative from the start in 183 (83.6%) patients, while it was positive in 36 patients (16.4%). Intraoperative decision of margin re-excision was applied for 29 patients (13.2%) in order to reach negative margin, modified radical mastectomy was offered for 4 patients (1.8%), while nipple sparing mastectomy with immediate breast reconstruction using latissimus dorsi flap was offered for 3 patients (1.4%). The postoperative paraffin results were typical with intraoperative frozen section analysis results in 216 patients (98.6%) and different results were obtained in only 3 patients (1.4%) who were managed by modified radical mastectomy in a second operation. Only 4 patients had local recurrence (1.8%) during the period of follow-up duration which was ranged from 1 to 86 months with mean ± SD (22.3 ± 14.1). AIM:To investigate the clinicopathological features and the significance of different prognostic factors which predict surgical overall survival in patients with gastric ***:This retrospective study includes 80 patients diagnosed and treated at gastroenterology surgical center,Mansoura University,Egypt between February 2009 to February *** factors were assessed by cox proportional hazard ***:There were 57 male and 23 *** median age was 57 years(24-83).One,3 and 5 years survival rates were 71%,69% and 46% *** median survival was 69.96 *** the follow-up period,13 patients died(16%).Hospital morbidity was reported in 10 patients(12.5%).The median number of lymph nodes removed was 22(4-41).Lymph node(LN) involvement was found in 91% of *** R0 resection,depth of wall invasion,LN involvement and the number(> 15) of retrieved LN,LN ratio and tumor differentiation predict *** multivariable analysis,tumor differentiation,curability of resection and a number of resected LN superior to 15 were found to beindependent prognostic ***:Surgery remains the cornerstone of *** differentiation,curability of resection and a number of resected LN superior to 15 were found to be independent prognostic *** LN dissection does not increase the morbidity or mortality rate but markedly improves long term survival.

AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.
