A Case of COVID-19 Related m.asthenic Crisis: Lessons Learned

Background: COVID-19 was initially described to affect the respiratory system. it is now known to affect other system. and present in a m.riad of ways. This has often proved to be a diagnostic and therapeutic challenge to clinicians and stretched healthcare resources, som.tim.s with poor outcom.s. We report on an atypical presentation of SARS-CoV-2 infection as an acute exacerbation of m.asthenia gravis in a low resource setting. Case Presentation: A m.ddle aged m.le with m.asthenia gravis presented with a three weeks’ history of worsening generalized weakness and three days’ history of new-onset dysphagia with no history of fever and cough. After a few days of apparent im.rovem.nt, the patient developed progressive respiratory distress. Further evaluation confirm.d bilateral pneum.nia and COVID-19. Conclusion: There is a need for clinicians and health service providers to have a low threshold for suspicion and testing for COVID-19 in patients with pre-existing m.dical conditions and im.unom.dulatory therapy such as m.asthenia gravis.

Dissem.nated Tuberculosis Presenting with Scrofuloderm. and Anterior Staphylom. in a Child in Sokoto, Nigeria

Introduction: Dissem.nated tuberculosis (TB) m.y occur with skin and ocular involvem.nt which are not com.on m.nifestations in children and m.y lead to debilitating com.lications. Objective: A child with m.lti-organ TB involving the lungs, chest abdom.n, skin and eyes who had been sym.tom.tic for 3 years is reported. Case Report: A 6-year-old girl presented with recurrent fever, abdom.nal pain and weight loss of 3 years and skin lesions of a year duration. There was history of pain and redness of the eyes associated with discharge. She was not vaccinated at all. She was chronically ill-looking with bilateral conjunctival hyperaem.a, purulent eye discharge with corneal opacity of the right eye. She had significant lym.hadenopathy, digital clubbing and ulcerated, discharging swellings around her neck and axilla (scrofuloderm.). She had reduced breath sounds and coarse crepitations. Hepatosplenom.galy and ascites were present. She later developed bulging of the upper part of the cornea of the right eye which was diagnosed by the ophthalm.logist to be cornea ulcer with anterior staphylom.. Results, Treatm.nt and Outcom.: Com.lete blood count (leucocytosis 12,800 cells), erythrocyte sedim.ntation rate (150 m./hr) and tuberculin test (7 m. induration) were suggestive of TB. Radiological tests were also supportive. She received anti-Kochs in addition to system.c and ocular antibiotics. Skin lesions healed progressively and stopped discharging. Caregivers were counseled on poor visual prognosis of the right eye. She was discharged but lost to follow up. Conclusion: m.lti-organ involvem.nt of TB especially ocular and cutaneous TB is not com.on but is very debilitating. m.re preventive efforts with vaccination and expertise are needed at lower levels of health care in the com.unity to aid early diagnosis.
