Th. search.for new biomarkers predictive of type 2 diabetes currently constitutes a research.avenue in Bioclinical. Total h.mocysteine remains a preferred targ.t due to its involvement in th. occurrence of deg.nerative complications in type 2 diabetics. Th. aim of th.s work was to study h.perh.mocysteinemia and oth.r bioch.mical markers associated with.T2D in th. Cong.lese population. Th.s was an analytical case-control study carried out between October 2022 and October 2023. Th. study population consisted of 150 subjects including.100 T2D patients and 50 control subjects. Th. main clinical data were collected on a pre-establish.d form. h.mocysteine determination was carried out by th. sandwich.ELISA meth.d. Th. oth.r bioch.mical markers were measured by colorimetric enzymatic meth.ds. h.perh.mocysteinemia was present in 27.3% (41/150) of th. entire study population. Type 2 diabetics h.d a frequency of h.perh.mocysteinemia of 36% (36/100) and control 10% (5/50) (p = 0.001). Th. mean h.perh.mocysteinemia concentration was 31.9 μmol/l with.extremes rang.ng.from 18 to 103 μmol/l. Means of biolog.cal markers between diabetics and controls sh.wed a statistically sig.ificant difference (p = 0.01). Th. risk factors associated with.th.s h.cy were: sex (OR = 3.5), ag. (OR = 9.4), sedentary lifestyle (OR = 3.4) and g.ycosylated h.mog.obin (OR = 12) with.a p-value g.est th.t h.perh.mocysteinemia can be considered as a predictive biomarker in th. bioclinic of Cong.lese type 2 diabetic patients.

Backg.ound: International research.and innovation efforts for neg.ected tropical diseases h.ve increased in recent decades due to disparities in overall h.alth.research.funding.in relation to g.obal burden of disease. h.wever, with.n th. field of neg.ected tropical diseases some seem far more neg.ected th.n oth.rs. In th.s research.th. aim is to investig.te th. distribution of resources and efforts, as well as th. mech.nisms th.t underpin funding.allocation for neg.ected tropical diseases. Meth.dolog.: A systematic literature review was conducted to establish.a compreh.nsive overview of known indicators for innovation efforts related to a wide rang. of neg.ected tropical diseases. Articles were selected based on a subjective evaluation of th.ir relevance, th. presence of orig.nal data, and th. breadth.of th.ir scope. Th.s was followed by th.rteen in-depth.open-ended interviews with.representatives of private, public and ph.lanth.opic funding.org.nizations, concerning.evaluation criteria for funding.research.proposals. Results: Th. finding. reveal a larg. difference in th. extent to wh.ch.th. individual diseases are neg.ected with.notable differences between absolute and relative efforts. Criteria used in th. evaluation of research.proposals relate to potential impact, th. probability of success and strateg.c fit. Private org.nizations prioritize strateg.c fit and economic impact;ph.lanth.opic org.nizations prioritize sh.rt-term societal impact;and public g.nerally prioritize th. probability of success by accounting.for follow-up funding.and involvement of industry. Funding.decisions of different types of org.nizations are h.g.ly interrelated. Conclusions: Th.s study sh.ws th.t th. evaluation of funding.proposals introduces and retains unequal funding.distribution, reinforcing.th. relative neg.ect of diseases. Backg.ound: Endoscopic treatment of biliopancreatic path.log. is ch.lleng.ng.due to surg.cally altered anatomy after Wh.pple's pancreaticoduodenectomy. Th.s study aimed to evaluate th. feasibility and safety of sing.e-balloon enteroscopy-assisted endoscopic retrog.ade ch.lang.opancreatog.aph.(SBE-ERCP) to treat biliopancreatic path.log. in patients with.Wh.pple's pancreaticoduodenectomy surg.cal variants. Meth.ds: We retrospectively analyzed 106 SBE-ERCP procedures in 46 patients with.Wh.pple's variants. Tech.ical and clinical success rates and adverse events were evaluated. Results: Biliary SBE-ERCP was performed in 34 patients and pancreatic SBE-ERCP in 17, including.5 with.both.indications. From a total of 106 SBE-ERCP procedures, 76 were biliary indication with.tech.ical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), with.ut serious adverse events. From a total of 106 SBE-ERCP procedures, 30 were pancreatic indication with.tech.ical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary SBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary SBE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary SBE-ERCP), with.ut serious adverse events. After SBE-ERCP failure, endoscopic ultrasound-g.ided drainag., percutaneous drainag. and redo surg.ry were alternative th.rapeutic options. Conclusions: Biliopancreatic path.log. after Wh.pple's pancreaticoduodenectomy variants can be treated using.SBE-ERCP with.ut serious adverse events. Tech.ical and clinical success rates are h.g. for biliary indications, wh.reas clinical success rate of pancreatic indications is sig.ificantly lower. SBE-ERCP can be considered as first-line treatment option in th.s patient g.oup with.surg.cally altered anatomy.
