National trends in total hip arthroplasty for traumatic hip fractures:An analysis of a nationwide all-payer database

BACKGrOUND Hemiarthroplasty(HA)has traditionally been the treatment of choice for elderly patients with displaced femoral neck *** treatment for younger,ambulatory patients is not as *** hip arthroplasty(THA)has been increasingly utilized in this population however the factors associated with undergoing HA or THA have not been fully *** To examine what patient characteristics are associated with undergoing THA or *** determine if outcomes differ between the *** We queried the Nationwide Inpatient Sample(NIS)for patients that underwent HA or THA for a femoral neck fracture between 2005 and *** NIS comprises a large representative sample of inpatient hospitalizations in the United *** Classifications of Disease,Ninth Edition(ICD-9)codes were used to identify patients in our *** variables,hospital characteristics,payer status,medical comorbidities and mortality rates were compared between the two *** logistic regression analysis was then performed to identify independent risk factors of treatment *** Of the total 502060 patients who were treated for femoral neck fracture,51568(10.3%)underwent THA and the incidence of THA rose from 8.3%to 13.7%.Private insurance accounted for a higher percentage of THA than *** increased most in urban teaching hospitals relative to urban non-teaching *** length of stay(LOS)was longer for *** mean charges were less for HA,however charges decreased steadily for both *** had a higher mortality rate,however,after adjusting for age and comorbidities HA was not an independent risk factor for ***,private insurance was an independent predictor for treatment with *** There has been an increase in the use of THA for the treatment of femoral neck fractures in the United States,most notably in urban *** and THA are decreasing in total charges and LOS.

Trichomerium is a genus of foliar epiphytes with the appearance of sooty moulds,mostly occurring on the surface of living leaves and apparently gaining their nutrients from insect *** have ascostromata with setae and develop on a loosely interwoven mycelial mass of dark brown hyphae,while asci have a bitunicate appearance with hyaline *** this study,we made 16 collections of Trichomerium from *** were isolated,and the LSU and ITS rDNA gene regions *** analysis indicated that the Trichomerium species form a monophyletic clade within Chaetothyriales and warrant the introduction of a new family *** support for the Chaetothyriales is 100%and clearly separates Trichomeriaceae from Capnodiales which are morphologically very similar.A detailed account of Trichomerium is provided and we describe and illustrate three new species based on morphological and molecular *** propose that *** is adopted as the generic type of Trichomerium because it has been impossible to obtain the holotype specimen of *** and also no molecular data exists in worldwide databases for this species or genus.

Blindly inserted nasogastric feeding tubes and thoracic complications in intensive care

Purpose of review: This article reviews the thoracic complications from malpositioned blindly inserted nasogastric feeding tubes in mechanically ventilated patients in intensive care and the methods to check the position and promote safe placement of the feeding tubes. recent findings: Malpositioned feeding tubes are not included in risk management databases. The reported incidence is 1-3% and more than half occur in mechanically ventilated patients. Eighty three mechanically ventilated patients were reported with malpositioned nasogastric tubes and 66% of them developed serious thoracic complications. Pneumothoraces accounted for 80% of thoracic complications that were evenly distributed between tubes with and without stylet. repeated misplacements appear to increase the risk. Non-radiological confirmation of the position of the tube has suboptimal performance. Protocols to place feeding tubes and new technology are promising candidates. Summary: Malpositioned nasogastric feeding tubes are underreported and associated with serious thoracic complications in mechanically ventilated patients. We need more data to answer whether we can afford to prevent them.

In optical microscopy,the slow axial scanning rate of the objective or the sample has traditionally limited the speed of volumetric ***,by conjugating either a movable mirror to the image plane in a remote-focusing geometry or an electrically tuneable lens(ETL)to the back focal plane,rapid axial scanning has been ***,mechanical actuation of a mirror limits the axial scanning rate(usually only 10–100 Hz for piezoelectric or voice coil-based actuators),while ETLs introduce spherical and higher-order aberrations that prevent high-resolution *** an effort to overcome these limitations,we introduce a novel optical design that transforms a lateral-scan motion into a spherical aberration-free axial scan that can be used for high-resolution *** a galvanometric mirror,we scan a laser beam laterally in a remote-focusing arm,which is then back-reflected from different heights of a mirror in the image *** characterize the optical performance of this remote-focusing technique and use it to accelerate axially swept light-sheet microscopy by an order of magnitude,allowing the quantification of rapid vesicular dynamics in three *** also demonstrate resonant remote focusing at 12 kHz with a two-photon raster-scanning microscope,which allows rapid imaging of brain tissues and zebrafish cardiac dynamics with diffraction-limited resolution.

Achieving superior performance of nanoparticle systems is one of the biggest challenges in *** major phenomena,occurring during the reactions,hinder the development of the full potential of nanoparticle catalysts:sintering and contamination with carbon containing species,sometimes called ***,we demonstrate that Ir nanocrystals,arranged into periodic networks on hexagonal boron nitride(h-BN)supports,can be restored without sintering after contamination by persistent *** restoration yields the complete removal of carbon from the nanocrystals,which keep their crystalline structure,allowing operation without *** findings,together with the possibility of fine tuning the nanocrystals size,confer this nanoparticle system a great potential as a testbed to extract key information about catalysis-mediated oxidation *** the case of the CO oxidation by O2,reaction of interest in environmental science and green energy production,the existence of chemical processes not observed before in other nanoparticle systems is demonstrated.

Effect of weekend admission on geriatric hip fractures
