BACKGROUnD We previously reported national 30-d readmission rates of 27% in patients with decompensated cirrhosis(DC).AIM To study prospective interventions to reduce early readmissions in DC at our tertiary *** Adults with DC admitted July 2019 to December 2020 were enrolled and randomized into the intervention(InT) or standard of care(SOC) arms. Weekly phone calls for a month were completed. In the InT arm, case managers ensured outpatient follow-up, paracentesis, and medication compliance. Thirty-day readmission rates and reasons were *** Calculated sample size was not achieved due to coronavirus disease 2019;240 patients were randomized into InT and SOC arms. 30-d readmission rate was 33.75%, 35.83% in the InT vs 31.67% in the SOC arm(P = 0.59). The top reason for 30-d readmission was hepatic encephalopathy(HE, 32.10%). There was a lower rate of 30-d readmissions for HE in the InT(21%) vs SOC arm(45%, P = 0.03). There were fewer 30-d readmissions in patients who attended early outpatient follow-up(n = 17, 23.61% vs n = 55, 76.39%, P = 0.04).COnCLUSIOn Our 30-d readmission rate was higher than the national rate but reduced by interventions in patients with DC with HE and early outpatient follow-up. Development of interventions to reduce early readmission in patients with DC is needed.

Does Resisted Sled Towing Improve the Physical Qualities of Elite Youth Soccer Players of Differing Maturity Status?

Purpose Sled towing has been shown to be an effective method to enhance the physical qualities in youth *** aim of this study was to evaluate the impact of a 6-week sled towing intervention on muscular strength,speed and power in elite youth soccer players of differing maturity *** Seventy-three male elite youth soccer players aged 12-18 years(Pre-Peak Height Velocity[PHV]n = 25;Circa-PHV n = 24;Post-PHV n = 24)from one professional soccer academy participated in this *** assessments(10 m and 30 m),countermovement jump and isometric mid-thigh pull were undertaken before(T1)and after(T2)a 6-week *** training intervention consisted of 6 weeks(2xper week,10 sprints over 20 m distance)of resisted sled towing(linear progression 10%-30%of body mass)during the competitive *** regression models analysed differences between T1 and T2 within each maturity *** There were minimal changes in strength,speed and power(P=0.35-0.80)for each maturity group across the 6-week *** there were changes with greater certainty,they are unlikely to represent real effect due to higher regres-sion to the mean(RTM).Conclusion It appears that a 6-week sled towing training programme with loadings of 10%-30%body mass only maintains physical qualities in elite youth soccer players pre-,circa-,and *** research is required to determine the effectiveness of this training method in long-term athletic development programmes.

Quantitative proteomic determination of diethylstilbestrol action on prostate cancer

Diethylstilbestrol （DES） has a direct cellular mechanism inhibition on prostate cancer. Its action is independent from the oestrogen receptors and is preserved after a first-line hormonal therapy. We aimed to identify proteins involved in the direct cellular inhibition effects of DES on prostate cancer. We used a clonogenic assay to establish the median lethal concentration of DES on 22RV1 cells. 22RV1 cells were exposed to standard and DES-enriched medium. After extraction, protein expression levels were obtained by two-dimensional differential in-gel electrophoresis （2D-DIGE） and isotope labelling tags for relative and absolute quantification （iTRAQ）. Proteins of interest were analysed by quantitative RT-PCR and western blotting. The differentially regulated proteins （P〈0.01） were interrogated against a global molecular network based on the ingenuity knowledge base. The 2D-DIGE analyses revealed DES-induced expression changes for 14 proteins （〉 1.3 fold; P〈0.05）. The iTRAQ analyses allowed the identification of 895 proteins. Among these proteins, 65 had a modified expression due to DES exposure （i.e., 23 overexpressed and 42 underexpressed）. Most of these proteins were implicated in apoptosis and redox processes and had a predicted mitochondrial expression. Additionally, ingenuity pathway analysis placed the OAT and HSBP1 genes at the centre of a highly significant network. RT-PCR confirmed the overexpression of OAT （P=0.006） and HSPB1 （P=0.046）.

The establishment of a highly efficient planktivore, Silver carp(Hypophthalmichthys molitrix), in the Mississippi River Basin has been documented as having negative impacts on native planktivore populations. Silver carp have the potential, to alter the trophic food web through negative interactions with native planktivores in large river systems. Due to increasing Silver carp ranges, interactions between Silver carp and other native planktivores is likely to increase. One abundant native planktivore that is often found in similar habitats as Silver carp is Gizzard shad(Dorosoma cepedianum). Potential competition for resources exists because of overlapping habitat use and forage. Given the similarities between these two planktivores, we sought to evaluate daily growth rates and determine hatch dates of age-0 Silver carp and Gizzard shad in the Middle Mississippi River. Silver carp and Gizzard shad were collected from the Middle Mississippi River using an electrified surface trawl from June through September with sizes ranging from 21—65 mm total length for Gizzard shad and 24—85 mm total length for Silver carp. A sample of age-0 fish(n=697) was collected at the end of the growing season including 292 Gizzard shad and 405 Silver carp. Size structure(total length in mm) of Silver carp(49.05±0.41; ± SE) was significantly greater(T= –16.43, df=695, Phan Gizzard shad(38.95 ± 0.45). hatch dates were similar between Silver carp(July 26 th to August 18 th) and Gizzard shad(July 28 th to August 19 th). Growth of Silver carp(2.24 mm per day) was significantly greater(Phan Gizzard shad(1.72 mm per day). Mortality was similar between Silver carp(28.1%)and Gizzard shad(25.1%) at this early life stage. The faster growth rates in Silver carp result in Silver carp entering the winter season larger in size with likely an increased capacity to consume plankton and decreased predation ultimately leading to higher recruitment. Typical clinical symptoms of chronic pancreatitis are vague and non-specific and therefore diagnostic tests are required, none of which provide absolute diagnostic certainly, especially in the early stages of disease. Recently-published guidelines bring much needed structure to the diagnostic work-up of patients with suspected chronic pancreatitis. In addition, novel diagnostic modalities bring promise for the future. The assessment and diagnosis of pancreatic exocrine insufficiency remains challenging and this review contests the accepted perspective that steatorrhea only occurs with > 90% destruction of the gland.

Self-assembled bright luminescent hierarchical materials from a tripodal benzoate antenna and heptadentate Eu(III) and Tb(III) cyclen complexes

The europium heptadentate coordinatively unsaturated (Eu(III)) and the terbium (Tb(III)) 1,4,7,10-tetraazacyclododecane (cyclen) complexes 1 and 2 were used in conjunction with ligand 3 (1,3,5-benzene-trisethynylbenzoate) to form the supramolecular self-assembly structures 4 and 5;this being investigated in both the solid and the solution state. The resulting self-assemblies gave rise to metal centered emission (both in the solid and solution) upon excitation of 3, confirming its role as a sensitizing antenna. Drop-cased examples of ligand 3, and the solid forms of 4 and 5, formed from both organic and mixture of organic-aqueous solutions, were analyzed using Scanning Electron Microscopy, which showed significant changes in morphology;the ligand giving rise to one dimensional structures, while both 4 and 5 formed amorphous materials that were highly dense solid networks containing nanoporous features. The surface area (216 and 119 m2·g^-1 for 4 and 5 respectively) and the ability of these porous materials to capture and store gases such as n2 investigated at 77 K. The self-assembly formation was also investigated in diluted solution by monitoring the various photophysical properties of 3–5. This demonstrated that the most stable structures were that consisting of a single antennae 3 and three complexes of 1 or 2 (e.g., 4 and 5) in solution. By monitoring the excited state lifetimes of the Eu(III) and Tb(III) ions in H2O and D2O respectively, we showed that their hydration states (the q-value) changed from -2 to 0, upon formation of the assemblies, indicating that the three benzoates of 3 coordinated directly to the each of the three lanthanide centers. Finally we demonstrate that this hierarchically porous materials can be used for the sensing of organic solvents as the emission is highly depended on the solvent environment;the lanthanide emission being quenched in the presence of acetonitrile and THF, but greatly enhanced in the presence of methanol.

Feed Form Affects Growth and Stomach Ulcers in Yearling Horses

Fourteen yearling standardbred horses were used to test the hypothesis that hay and grain fed as a complete total mixed ration （TMR） cube diet （C） would result in greater average daily gain （ADG）, feed efficiency （gain to feed ratio, G/F） and apparent digestibility in horses on the TMR diet compared to horses fed the same weight and proportion of hay cubes and grain fed separately （diet HG）. Both diets consisted of 75% forage and 25% ground oats. The forage in both the plain hay cubes and the complete cubes was 80% alfalfa and 20% endophyte-free tall rescue. An additional hypothesis that stomach ulceration would not differ between horses fed either of the two diet treatments was also tested by comparing upper gastric endoscopies before and after 70 days of feeding the experimental diets. Horses fed diet C had greater ADG （P 〈 0.046） while G/F between the two treatments was not significantly different （P 〉 0.065） but showed a trend to be higher in the horses consuming diet C. Apparent digestibility of dry matter and crude protein was not different between the diet treatments, and no major gastric ulceration was found in horses consuming either diet treatment. The results of this study found that a complete cubed diet of 75% hay and 25% oats did not cause stomach ulceration while achieving an acceptable growth rate in yearling horses.
