International Nuclear Medicine Consensus on the Clinical Use of Amyloid Positron Emission Tomography in Alzheimer’s Disease

＜正＞Objectives: The exact location of sentinel node on lymphoscintigraphy is difficult to find due to poor anatomical landmark. We tested the feasibility of using bone scan as an anatomical reference for lymphoscintigraphy. Methods: Twenty-seven patients with breast cancer have undergone lymphoscintigraphy on the next day of bone scan. About 24 hours after 370 MBq of Tc-99m MDP injection, 5.1±0.9 MBq of Tc-99m antimony sulfide colloid （ASC） were injected and followed by imaging until the sentinel node were visualized. Detection of sentinel node using gamma probe were done after lymphoscintigraphy. Peak counts of background were measured in anterior chest and axilla using gamma probe before injection of Tc-99m ACS. Peak counts of sentinel lymph node were also measured. Results: Sentinel nodes were detected in 24 out of 27 patients. In 3 missing cases, injections were technically failed. Surgeons easily figured out the sentinel nodes both on lymphoscintigraphy and surgical field, and background signals were not affected. The ratio of peak counts between sentinel node and background was 5.0±2.5. This procedure saved the time and efforts of patients to visit hospital for preoperative bone scan prior to admission. Conclusion: Lymphoscintigraphy on the next day after bone scan in patients with breast cancer is a convenient and useful method for locating the sentinel node.

Alzheimer’s disease(AD)is the main cause of dementia,with its diagnosis and management remaining *** positron emission tomography(PET)has become increasingly important in medical practice for patients with *** integrate and update previous guidelines in the field,a task group of experts of several disciplines from multiple countries was assembled,and they revised and approved the content related to the application of amyloid PET in the medical settings of cognitively impaired individuals,focusing on clinical scenarios,patient preparation,administered activities,as well as image acquisition,processing,interpretation and *** addition,expert opinions,practices,and protocols of prominent research institutions performing research on amyloid PET of dementia are *** the increasing availability of amyloid PET imaging,a complete and standard pipeline for the entire examination process is essential for clinical *** international consensus and practice guideline will help to promote proper clinical use of amyloid PET imaging in patients with AD.

目的 (18F-氟戊基)三苯基季磷盐(18F-FPTP)是一种新的有应用前景的心肌PET显像剂,其在心肌细胞内积聚量多且肝脏清除快。该研究小组在小鼠体内做了18F-FPTP PET显像的代谢区室分析和两项线性分析,即用于生成参数图的线性最小二乘法（LLS）和基函数法（BFM）分析。此外,还研究了动态分析的最小动态显像时间,并进行了计算机模拟。方法对心肌梗死（MI）小鼠模型（n=12）进行18F-FPTP动态PET显像（18 min）和CT显像。在左心室、正常心肌和梗死区勾画感兴趣区（ROI）。比较两室模型（速率常数k1和k2;2室2参数）和不可逆摄取的三室模型（速率常数k1-k3;3室3参数）的拟合优度,并进行部分容积和溢出校正（Vα和α=1-Va）。LLS和BFM用于基于ROI和基于体素的动力学参数估算,估算结果与相应代谢区室模型标准的基于ROI的非线性最小二乘法（NLS）结果进行比较。通过计算机模拟研究估算方法的统计学特点。结果 2室2参数模型最适于描述18F-FPTP的动力学表现。正常心肌的平均k1、k2和Va值分别是6.8ml·min-1·g-1、1.1/min和0.44,而梗死组织的分别是1.4 ml·min-1·g-1、1.1/min和0.32。动态显像10 min的数据即足够用于估算各项参数。LLS和BFM的估算结果与ROI水平的NLS的结果有很好的相关性（k1:1.22x-0.30,r2=0.90;y=1.26x+0.00,r2=0.92）。动态参数图形（αk1,k1,k2,Va）的局部分布具有生理相关性。在计算机模拟中,LLS和BFM比NLS的特征性更强。结论快速的动力学特征和心肌较高的特异性摄取使18F-FPTP能在显像开始的最初10 min动态显像内,利用2室2参数模型进行定量分析。LLS和BFM能在体素水平进行参数估算,这2种方法可用于定量评价18F-FPTP在心肌中的分布,进而研究其在不同条件、不同疾病模型和病种中进行心肌显像的表现。

目的制备同型的~(（99）Tcm和~(（188）Re标记的叶酸-甘氨酸-甘氨酸-半胱氨酸-谷氨酸（叶酸-GGCE）,并通过小鼠肿瘤模型评价其用于叶酸受体（FR）靶向分子显像的可行性。方法采用固相肽合成法合成叶酸-GGCE,并用99Tcm或188Re标记。放化纯测定采用放射性高效液相色谱法。99Tcm-叶酸-GGCE的生物分布研究在同时注射或不注射过量的游离叶酸的情况下完成,小鼠肿瘤模型分别为荷FR阳性的KB细胞模型和荷FR阴性的HT1080细胞模型。188Re-叶酸-GGCE的生物分布研究在荷KB细胞小鼠肿瘤模型上完成。静脉注射99Tcm-叶酸-GGCE后,采集2种小鼠肿瘤模型的系列平面图像。小鼠肿瘤模型注射99Tcm-叶酸-GGCE或188Re-叶酸-GGCE（同时注射或不注射过量的游离叶酸）后,进行系列microSPECT/CT图像采集。结果99Tcm和188Re标记的叶酸-GGCE的标记率和标记后4 h内的放化纯均>95%。荷KB细胞模型在注射99Tcm-叶酸-GGCE后1、2、4 h,其肿瘤组织的放射性摄取值分别为16.4、23.2和17.6%ID/g,如果同时注射过量的叶酸,肿瘤组织的摄取可被抑制97.4%。注射后4 h,肿瘤与血液、肝、肺、肌肉和肾的T/NT比值分别是54.3、25.2、38.3、97.8和0.3。荷KB细胞模型在注射188Re-叶酸-GGCE后2、4、8、16 h,其肿瘤组织的放射性摄取值分别为17.4、21.7、24.1和15.6%ID/g。注射后8 h,肿瘤与血液、肝、肺、肌肉和肾的T/NT比值分别是126.8、21.9、54.8、80.3和0.4。注射99Tcm或188Re标记的叶酸-GGCE后,系列平面图像和microSPECT/CT图像均见KB细胞肿瘤组织的放射性摄取较高,肿瘤显像清晰。肿瘤对显像剂的摄取在同时注射过量叶酸时可被完全抑制。结论同型99Tcm和188Re标记的叶酸-GGCE可被FR阳性肿瘤高摄取,且T/NT比值较高。2种显像剂均具有良好的靶向能力,在FR阳性肿瘤的诊断和疗效观察中有一定的应用价值。

Porous 2D materials with high conductivity and large surface area have been proposed for potential electromagnetic interference(EMI)shielding materials in future mobility and wearable applications to prevent signal noise,transmission inaccuracy,system malfunction,and health ***,we report on the synthesis of lightweight and flexible flash-induced porous graphene(FPG)with excellent EMI shielding *** broad spectrum of pulsed flashlight induces photo-chemical and photo-thermal reactions in polyimide films,forming 5×10 cm^(2)-size porous graphene with a hollow pillar structure in a few *** resulting material demonstrated low density(0.0354 g cm^(−3))and outstanding absolute EMI shielding effectiveness of 1.12×10^(5) dB cm^(2) g^(−1).The FPG was characterized via thorough material analyses,and its mechanical durability and flexibility were confirmed by a bending cycle ***,the FPG was utilized in drone and wearable applications,showing effective EMI shielding performance for internal/external EMI in a drone radar system and reducing the specific absorption rate in the human body.

Objectives To evaluate the age-related one-year major adverse cardiocerebrovascular events （MACCE） after percutaneous coronary intervention （PCI） in acute myocardial infarction （AMI）. We analyzed the association between age and one-year MACCE after AMI. Methods A total of 13,104 AMI patients from Korea Acute Myocardial Infarction Registry-National Institue of Health （KAMIR-NIH） between November 2011 and December 2015 were classified into four groups according to age （Group I, 〈 60 years, n = 4199; Group II, 60-70 years, n = 2577; Group III; 70-80 years, n = 2774; Group IV, ≥ 80 years, n = 1018）. Patients were analyzed for one-year composite of MACCE （cardiac death, myocardial infarction, target vessel revascularization, cerebrovascular events） after AMI. Results The one-year MACCE in AMI were 3.5% （Group I）, 6.3% （Group II）, 9.6% （Group III） and 17.6% （Group IV）. After adjustment for confounding para- meters, the analysis results showed that patients with AMI had incremental risk of one-year MACCE [Group II, adjusted hazard ratios （aHR） = 1.224, 95% CI： 0.965-1.525, P = 0.096; Group III, aHR = 1.316, 95% CI： 1.037-1.671, P= 0.024; Group IV, aHR = 1.975, 95% CI： 1.500-62.601, P〈 0.001） compared to Group I. Especially, cardiac death in the composite of primary end point played a major role in this effect （Group II, aHR = 1.335, 95% CI： 0.941-1.895, P= 0.106; Group III, aHR = 1.575, 95% CI： 1.122-2.210, P= 0.009; Group IV, aHR = 2.803, 95% CI： 1.937-4.054, P 〈 0.001）. Conclusions Despite advanced techniques and medications for PCI in AMI, age still exerts a powerful influence in clinical outcomes. Careful approaches, even in the modem era of developed cardiology are needed for aged-population in AMI intervention.

Background There are numerous but conflicting data regarding gender differences in outcomes following percutaneous coronary intervention(PCI). Furthermore, gender differences in clinical outcomes with acute myocardial infarction(AMI) following PCI in Asian population remain uncertain because of the under-representation of Asian in previous trials. Methods A total of 13,104 AMI patients from Korea Acute Myocardial Infarction Registry-National Institute of Health(KAMIR-NIH) between November 2011 and December 2015 were classified into male(n = 8021, 75.9%) and female(n = 2547, 24.1%). We compared the demographic, clinical and angiographic characteristics, 30-days and 1-year major adverse cardiac and cerebrovascular events(MACCE) in women with those in men after AMI by using propensity score(PS) matching. Results Compared with men, women were older, had more comorbidities and more often presented with non-ST segment elevation myocardial infarction(NSTEMI) and reduced left ventricular systolic function. Over the median follow-up of 363 days, gender differences in both 30-days and 1-year MACCE as well as thrombolysis in myocardial infarction minor bleeding risk were not observed in the PS matched population(30-days MACCE: 5.3% vs. 4.7%, log-rank P = 0.494, HR = 1.126, 95% CI: 0.800-1.585;1-year MACCE: 9.3% vs. 9.0%, log-rank P = 0.803, HR = 1.032, 95% CI: 0.802-1.328;TIMI minor bleeding: 4.9% vs. 3.9%, log-rank P = 0.215, HR = 1.255, 95% CI: 0.869-1.814). Conclusions Among Korean AMI population undergoing contemporary PCI, women, as compared with men, had different clinical and angiographic characteristics but showed similar 30-days and 1-year clinical outcomes. The risk of bleeding after PCI was comparable between men and women during one-year follow up.
