Usefulness of contrast-enhanced ultrasonography in determining treatment efficacy and outcome after pancreatic cancer chemotherapy

AIM:To investigate if contrast-enhanced ultrasonography(CE-US) is useful for determining treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy by assessing changes in intratumor hemodynamics using CE-US with a contrast ***:The subjects were 34 patients with unresectable advanced pancreatic cancer treated by ***-US was assessed after every treatment(course) completion under the same conditions,and patients were divided into two groups according to the intratumor enhancement pattern:Vascular rich(R) group and vascular poor(P) ***:After the second course of treatment,R group in intratumor hemodynamics had 18 patients,and P group had 16 *** reduction rates of serum CA19-9 level after chemotherapy which decreased to half or less of the baseline level were 2/15(0.1%) in P group,but 11/16(69%) in R group(P = 0.006).When the mean number of courses of chemotherapy and outcome were compared,P group had a mean number of courses of 4.9(R group,10.2) and mean survival time(MST) of 246 d(R group,402 d),showing that outcome was significantly better in R group(P=0.006).CONCLUSION:CE-US revealed that the change in intratumor blood flow correlated with both serum CA19-9 level and *** with serum CA19-9 that decreased to less than half the baseline level,and patients with an abundant intratumor blood flow,had a significantly better ***,CE-US is potentially useful for evaluating treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy.

Cytapheresis for pyoderma gangrenosum associated with inflammatory bowel disease:A review of current status

Pyoderma gangrenosum(PG)is a neutrophilic dermatosis clinically characterized by the presence of painful skin ulcerations with *** it is frequently associated with inflammatory bowel diseases,including ulcerative colitis,gastroenterologists should be familiar with the disease including therapeutic ***,we have conducted a review focusing on the cytapheresis for PG in cases of inflammatory bowel diseases.A literature search was conducted to extract studies published in the last 20 years,with information on demographics,clinical symptoms,treatment,and the clinical course from a total of 22 cases reported and our recent *** most patients,cytapheresis was associated with improvement or resolution of PG after failure of conventional therapeutic options such as corticosteroids,antibiotics,immunosuppressive agents and *** on the information summarized,cytapheresis is helpful in the majority of patients with PG refractory to medical treatment associated with inflammatory bowel diseases and could be further studied in a multicenter,randomized trial.

Childhood-onset inflammatory bowel diseases associated with mutation of Wiskott-Aldrich syndrome protein gene

AIM To screen primary immunodeficiency,Wiskott-Aldrich syndrome(WAS),and chronic granulomatous disease(CGD) among children with inflammatory bowel disease(IBD).METHODS This was a single-center retrospective study. Eighteen children with IBD were investigated. We analyzed their expression of Wiskott-Aldrich syndrome protein(WASP) in lymphocytes and superoxide generation in phagocytes using flow cytometry. When the expression of WASP or superoxide generation was low or absent,we performed genetic analysis to determine the cause of this. RESULTS Eighteen patients were classified as having ulcerative colitis(n = 10),Crohn's disease(n = 5),or IBDunclassified(n = 3). In total,three patients revealed low expression of WASP associated with a WAS gene c.1378 C>T ***460 Ser mutation,which has previously been reported as a pathogenic mutation in WAS and X-linked thrombocytopenia. However,with respect to the major symptoms of WAS,none of these three patients showed either thrombocytopenia or increased susceptibility to infection,but one patient showed generalized eczema. No CGD patients were discovered in this *** Despite the lack of typical clinical manifestations of WAS,low expression of WASP could be associated with the pathogenesis of a subtype of IBD patients.

Azimuthal anisotropy of direct photon is measured in S_(NN)^(1/2)=200GeV Au+Au collisions at RHIC- *** photon is one of the most effective probes to study properties of hot dence medium at initial state(also QGP state)of heavy ion collisions because photons almost do not interact strongly with any other particles caused by its long mean free path and they keep their conditions when they axe *** statistical and systematic errors,the elliptic flow parameter(v_2)of direct photon is consistent with zero. Direct photon v_2 is estimated by hadron decay photon contamination are subtracted from inclusive photon v_2 in intermediate to high transverse momentum(p_T)region(0 to 10GeV/c)for 3 centrality selections(20% steps)and minimum bias.

Dear Editor, Disorders of sex development （DSDs） are a group of conditions in which chromosomal, gonadal, or anatomical sex is atypical.1 DSD in genetic males （46,Xy DSD） primarily results from impaired androgen production or action or perturbed genital morphogenesis？ The current understanding of the genetic basis of 46,Xy DSD remains fragmentary. For example, although more than 15 genes have been implicated in the development ofnonsyndromic hypospadias,2 one of the most common forms of 46,Xy DSD,~ mutations in these genes account for 〈20% of the cases.3

Can we go beyond INDCs： Analysis of a future mitigation possibility in China, Japan, EU and the U.S.

The most important achievement in the Paris Agreement is to set up emission reduction target by commitment submitted by each country/ region with the form INDC. However the emission reduction target inside INDCs could not match with the emission pathway for the global to keep a global temperature rise this century well below 2 ℃ and to drive efforts to limit the temperature increase even further to 1.5 ℃ above pre- industrial levels. It is important for countries to do more than their commitment in INDCs, especially in near term. This paper looks at the near term trend for possibilities for further emission reduction compared with INDCs, and found there do have the possibility. It concludes there is space for the four countries/regions to do more than their INDCs submitted to UNFCCC, and see the light towards to a global 2 ℃ target.

Endoscopic ultrasound-guided choledochoduodenostomy in patients with failed endoscopic retrograde cholangiopancreatography

Endoscopic ultrasonography （EUS）-guided biliary drainage was performed for treatment of patients who have obstructive jaundice in cases of failed endoscopic retrograde cholangiopancreatography （ERCP）. In the present study, we introduced the feasibility and outcome of EUS-guided choledochoduodenostomy in four patients who failed in ERCR We performed the procedure in 2 papilla of Vater, including one resectable case, and 2 cases of cancer of the head of pancreas. Using a curved linear array echoendoscope, a 19 G needle or a needle knife was punctured transduodenally into the bile duct under EUS visualization. Using a biliary catheter for dilation, or papillary balloon dilator, a 7-Fr plastic stent was inserted through the choledochoduodenostomy site into the extrahepatic bile duct. In 3 （75%） of 4 cases, an indwelling plastic stent was placed, and in one case in which the stent could not be advanced into the bile duct, a naso-biliary drainage tube was placed instead. In all cases, the obstructive jaundice rapidly improved after the procedure. Focal peritonitis and bleeding not requiring blood transfusion was seen in one case. In this case, pancreatoduodenectomy was performed and the surgical findings revealed severe adhesion around the choledochoduodenostomy site. Although further studies and development of devices are mandatory, EUS-guided choledochoduodenostomy appears to be an effective alternative to ERCP in selected cases.

Transpapillary incision of refractory circumscript pancreatic duct stricture using wire-guided snare forceps

Endoscopic therapy of pancreatic duct(PD)strictures using balloon dilation and pancreatic duct stent(PS) placement has been reported to improve the severity of abdominal pain in selected patients with chronic pancreatitis(CP).However,some strictures are refractory and require frequent PS exchange to control *** describe two cases of successful endoscopic PD incision for difficult PD stricture using a wireguided *** snare is partially opened within the strictured pancreatic duct while applying current,thus incising the *** both cases were successful without complications we do not advocate that this method be used routinely because of the potential for severe complications,***,ductal perforation or pancreatic parenchymal *** order to prevent these complications,we developed a wire-guided technique under fluoroscopic *** think this procedure may be useful in patients with short,straight PD *** further study is required,this approach may have potential for selected patients with refractory PD strictures due to CP.

Primary liposarcoma of gallbladder diagnosed by preoperative imagings: A case report and review of literature

A 49-year-old Japanese woman was referred to our department because of high fever and a huge abdominal mass. Computed tomography （CT） and magnetic resonance （MR） imagings revealed a tumor, about 30 cm in diameter, occupied the right hepatic lobe and the peritoneal cavity. Abdominal angiography showed that the tumor was fed mainly by the cystic artery. We preoperatively diagnosed angiosarcoma of the gallbladder and performed tumor resection with cholecystectomy because the tumor was almost casplated, however the posterior wall of the gallbladder attached to the tumor firmly. Histologically, the tumor was composed of spindle cells including lipoblasts with cellular pleomorphism, which were also detected in the muscular layer of the gallbladder. We finally diagnosed pleomorphic liposarcoma of the gallbladder. At 10 mo and 29 mo after the first operation, she underwent two more operations because of recurrence. Now she has a good quality of life 3 years and 6 mo after the first operation.
