Impact of IPMOE on nursing tasks in the medical ward: A time-motion study

Introduction:The In-patient Medication Order Entry System (IPMOE) was first implemented in the medical ward of Princess Margaret Hospital,Hong *** was a local developed close-loop system including prescription,dispensing and administration *** on its impact on nursing tasks would be important for practice improvement and subsequent system ***:The study was conducted to quantify the nursing times across medication-associated tasks for paper-based MAR and computer-based IPMOE,including change in the tasks and time patterns before and after IPMOE ***:This was a prospective observation study in medical wards before (Jan 2014-Jun 2014) and after (Mar 2015-Jun 2015) the implementation of *** conducted 8-hr observation studies of individual nurses with a customized application to time various pre-categorized nursing *** inferences and interrupted time series analysis was performed to identify the change in the intercept and trends over time after ***:The average number of medication-related tasks was significantly reduced from 61.07 to 29.81,a reduction of 31.26 episodes per duty (P < 0.001,95% CI 22.9-39.63).The time for the medication-related tasks was reduced from 32 min (SD =21.57) to 26.57 min (SD =11.35) and the medication administration time increased from 37.93 min (SD-14.78) to 44.37 min (SD 19.45),but there was no overall significant difference in the time spent on each duty (P =0.315) between the two *** improving trend in the delayed effect was observed (P =0.03),which indicated a run-in period for new application was needed in clinical ***:Our study had shown the time motion observation could be applied to measure the impact of the IPMOE in a busy clinical *** classification of activities,validation,objective measurement and longitudinal evaluation,the method could be applied in various systems as well as different clinical settings in

Hong Kong has a long historical record of harmful algal blooms(HABs).In the 1980s-1990s,HABs were mainly pollution-related and most of the events happened in estuaries and enclosed embayment such as Tolo Harbour and Port *** major cause of HABs in 1980s-1990s was closely related to nutrients enrichment;included but not limited to changes in the concentration and ratio of soluble N and P in *** major causative organisms of HABs in Hong Kong by then were meso-plankton such as selected species of diatoms and dinofl ***,zooplankton was also a cause of red tides(the common name of HABs).There has been gradual change after the *** is attributed to higher and higher influences from the region,namely the Zhujiang(Pearl)River *** are increasing influences from Chinese mainland due to extraordinary social and economic growths during the past 20 *** the past10 years,HABs in Hong Kong was mainly subject to regional impacts in the Zhujiang River *** the duration period and covered areas have been enlarged which overwhelmed the localized influences of stream pollution and self-contamination of aquaculture *** flagellates of uncommon happening become dominant species in HAB ***,the seasonal impact of Noctiluca scintillans in late winter to mid spring remained.

Image-guided drawing can compensate for a lack of skill but often requires a significant number of repetitive strokes to create *** automatic stroke synthesis methods are usually limited to predefined styles or require indirect manipulation that may break the spontaneous flow of *** present an assisted drawing system to autocomplete repetitive short strokes during a user's normal drawing *** draw over a reference image as usual;at the same time,our system silently analyzes the input strokes and the reference to infer strokes that follow the user's input style when certain repetition is *** can accept,modify,or ignore the system's predictions and continue drawing,thus maintaining fluid control over *** key idea is to jointly analyze image regions and user input history to detect and predict *** proposed system can effectively reduce the user's workload when drawing repetitive short strokes,helping users to create results with rich patterns.

Verification of tropical cyclones(TC)wind structure forecasts from global NWP models and ensemble prediction systems(EPSs)

Forecasting wind structure of tropical cyclone(TC)is vital in assessment of impact due to high winds using Numerical Weather Prediction(NWP)*** usual verification technique on TC wind structure forecasts are based on grid-to-grid comparisons between forecast field and the actual ***,precision of traditional verification measures is easily affected by small scale errors and thus cannot well discriminate the accuracy or effectiveness of NWP model *** this study,the Method for Object-Based Diagnostic Evaluation(MODE),which has been widely adopted in verifying precipitation fields,is utilized in TC’s wind field verification for the first *** TC wind field forecast of deterministic NWP model and Ensemble Prediction System(EPS)of the European Centre for Medium-Range Weather Forecasts(ECMWF)over the western North Pacific and the South China Sea in 2020 were evaluated.A MODE score of 0.5 is used as a threshold value to represent a skillful(or good)*** is found that the R34(radius of 34 knots)wind field structure forecasts within 72 h are good regardless of DET or *** performance of R50 and R64 is slightly worse but the R50 forecasts within 48 h remain good,with MODE exceeded *** R64forecast within 48 h are worth for reference as well with MODE of around *** study states that the TC wind field structure forecast by ECMWF is skillful for TCs over the western North Pacific and the South China Sea.

Morphological variability and genetic diversity in five species of Skeletonema(Bacillariophyta)

Twelve strains of Skeletonema from Chinese coastal waters and another strain from the West Sea of Korea were examined using light microscopy(LM),scanning and transmission electron microscopy(SEM,TEM),and small subunit(SSU)rDNA sequence *** identified four species of Skeletonema:*** Sarno et Zingone,***(***)Bethge,*** Sarno et Kooistra,***- icum Cleve,as well as a Skeletonema-like *** are the first records of ***,*** and *** in *** Skeletonema-like species,MMDL5067,was isolated from the estuary of the changjiang *** morphologic and phylogenetic char- acteristics of this species are not correspond to those of any previously published Skeletonema *** intercalary fultoportula pro- cesses(IFPPs)has long open tubes,and the intercalary rimoportula process(IRP)is short and located inside the rim of the IFPPs. Phylogenetic analysis revealed that MMDL5630 clustered with *** in the NJ and MP trees,but its IRPs differed from those of previously described specimens of *** IRP is partially open,with an elongated opening at the base of the tube,and is longer than the *** specimens of *** and *** also have longer tubular IRPs than the *** to pre- viously described specimens of *** and ***,these two specimens we identified have shorter *** results reflect the ambiguity and diversity of Skeletonema *** are also significant for the future identification and management of red tides caused by Skeletonema in the seas around China.

Resection of T4 hepatocellular carcinomas with adjacent structures, is it justified？

BACKGROUND： T4 hepatocellular carcinoma （HCC） with invasion to adjacent structure（s） may require resection of not only the tumor but also the invaded structure（s）. This study aims to assess whether such combined resection for T4 HCC is justifiable. METHODS： Adult patients with T4 HCC were divided into three groups. Group 1： tumors and invaded adjacent structures were resected together if histopathologically confirmed tumor invasion; group 2： same as group 1 but histopathologically confirmed tumor adhesion; group 3： tumor resection only. Group comparisons were made. RESULTS： Totally 144 patients were included in the study. There were 71, 14 and 59 patients in groups 1, 2 and 3, respec- tively. The groups were comparable in demographics, compli- cation and survival. Ten hospital deaths occurred （5, 0 and 5 in groups 1, 2 and 3, respectively; P=0.533）. The 5-year overall survival （hospital mortality excluded） was 17.8% in group 1, 14.3% in group 2, and 28.9% in group 3 （P=0.191）. The 5-year disease-free survival was 10.4% in group 1 and 14.5% in group 3 （no data for group 2 yet）（P=0.565）. On multivariate analysis, macrovascular invasion and poor differentiation were risk factors for survival whereas combined resection did not impact patients＇ survival. CONCLUSIONS： Combined resection achieved survival outcomes similar to tumor resection only. Patients with tumor invasion and those with tumor adhesion had comparable survival after combined resection. At centers with the required expertise, combined resection should be attempted to treat T4 HCCs with clinically suspected invasion of adjacent structures.

Women's experiences of formula feeding their infants:an interpretative phenomenological study

Objective: This study aimed to explore the experiences of women in the process of formula feeding their infants. The World Health Organization has emphasized the importance of breastfeeding for infant health. After decades of breastfeeding promotions,breastfeeding rates in Hong Kong have been rising consistently; however, the low continuation rate is alarming. This study explores women's experiences with formula feeding their infants, including factors affecting their decision to do ***: A qualitative approach using an interpretative phenomenological analysis(IPA) was adopted as the study design. Data were collected from 2014 to 2015 through individual in-depth unstructured interviews with 16 women, conducted between 3 and 12 months after the birth of their infant. Data were analyzed using ***: Three main themes emerged as follows:(1) self-struggle, with the subthemes of feeling like a milk cow and feeling trapped;(2) family conflict, with the subtheme of sharing the spotlight; and(3) interpersonal tensions, with the subthemes of embarrassment,staring, and innocence. Many mothers suffered various stressors and frustrations during breastfeeding. These findings suggest a number of pertinent areas that need to be considered in preparing an infant feeding ***: The findings of this study reinforce our knowledge of women's struggles with multiple sources of pressure, such as career demands, childcare demands, and family life after giving birth. All mothers should be given assistance in making informed decisions about the optimal approach to feeding their babies given their individual situation and be provided with support to pursue their chosen feeding method.

Investigation of the genetic etiology in male infertility with apparently balanced chromosomal structural rearrangements by genome sequencing

Apparently balanced chromosomal structural rearrangements are known to cause male infertility and account for approximately 1%of azoospermia or severe ***,the underlying mechanisms of pathogenesis and etiologies are still largely ***,we investigated apparently balanced interchromosomal structural rearrangements in six cases with azoospermia/severe oligospermia to comprehensively identify and delineate cryptic structural rearrangements and the related copy number *** addition,high read-depth genome sequencing(GS)(30-fold)was performed to investigate point mutations causative of male ***-pair GS(4-fold)revealed additional structural rearrangements and/or copy number changes in 5 of 6 cases and detected a total of 48 ***,the breakpoints caused truncations of 30 RefSeq genes,five of which were associated with ***,the breakpoints disrupted 43 topological-associated *** disruptions or potential dysregulations of genes,which play potential roles in male germ cell development,apoptosis,and spermatogenesis,were found in all cases(n=6).In addition,high read-depth GS detected dual molecular findings in case MI6,involving a complex rearrangement and two point mutations in the gene ***,our study provided the molecular characteristics of apparently balanced interchromosomal structural rearrangements in patients with male *** demonstrated the complexity of chromosomal structural rearrangements,potential gene disruptions/dysregulation and single-gene mutations could be the contributing mechanisms underlie male infertility.

Retrohepatic vena cava deroofing in living donor liver transplantation for caudate hepatocellular carcinoma

The removal of tumor together with the native liver in living donor liver transplantation for hepatocellular carcinoma is challenged by a very close resection margin if the tumor abuts the inferior vena *** is in contrast to typical deceased donor liver transplantation where the entire retrohepatic inferior vena cava is included in total *** we report a case of deroofing the retrohepatic vena cava in living donor liver transplantation for caudate hepatocellular *** order to ensure clear resection margins,the anterior portion of the inferior vena cava was *** right liver graft was inset into a Dacron vascular graft on the back table and the composite graft was then implanted to the recipient inferior vena *** this technique,we observed the no-touch technique in tumor removal,hence minimizing the chance of positive resection margin as well as the chance of shedding of tumor cells during manipulation in operation.
