The Jordan River, also referred to as Nahr Al Sharieat in Arabic, is a long river in the Middle East that flows from north to south through the Sea of Galilee to the Dead Sea. The Jordan River is bordered by the Golan Heights and the Hashemite Kingdom of Jordan in the east and by israel and the Palestinian West Bank (Judea and Samaria) in the west. Soil tunnels, including those in the Jordan River watershed and on the israel, Syria, and Lebanon borders, have a long history of use for warfare, as invasion pathways, smuggling, and storage of rockets, missiles and ordnance and are causes of serious political tension between the countries. Soil tunnel construction and destruction often has adverse environmental and human health impacts in the Jordan River landscape. Due to agricultural wastes, the discharge of untreated sewage, and diversion of saline springs into the river water there has been serious deterioration in the water quality in the lower courses of the Jordan River. The primary objective of this research is to encourage the development of a multi-country clean-up, mitigation, and protection plan for the Jordan River.

Antiplatelet agents reduce liver fibrosis by inhibiting platelet activation and platelet-derived growth factor production. Previous cross-sectional epidemiological studies suggest that the use of aspirin is related to reduced liver fibrosis. The Aspirin-Myocardial Infarction Study(AMIS) aims to examine this relationship in a multicenter,randomized, double-blind and placebo-controlled trial. The existing clinical trial of aspirin was conducted to study the benefit of one gram aspirin daily among 4 524 individuals who had experienced at least one documented myocardial infarction. The aspartate aminotransferase(AST)-to-Platelet Ratio Index(APRI) was calculated at baseline and annually from the platelet count and AST levels. Participants in the AMIS trial had a mean baseline APRI of 0.34±0.36, and only 1% individuals had APRI scores higher than 1.0, a common cutoff for cirrhosis. The daily use of aspirin was associated with an increase, rather than a reduction of APRI, by 0.007 per year(95% CI0.002-0.015, P=0.12). The use of aspirin did not significantly affect platelet counts. In a sensitivity analysis of individuals with probable significant fibrosis at baseline(APRI≥0.7), the aspirin group had a sustained reduction in APRI over time, although this change was not significant compared to that in the placebo group. In the AMIS trial, the daily use of high-dose aspirin did not significantly affect APRI, a surrogate index of liver fibrosis. This study highlights the need for de novo clinical trials to investigate the potential benefit of antiplatelet agents on liver fibrosis.

Pediatric autoimmune neuropsychiatric disorders associated with or without streptococcal and other bacterial infections (PANDAS/CANS) are emerging as a featured pediatric disorder. Although there is some controversy regarding treatment approaches, especially related to the behavioral sequelae, we have hypothesized in other published work that it is characterized by the rapid onset of Reward Deficiency Syndrome (RDS) in children. We propose utilizing a multi-systems biological approach involving the coupling of genetic addiction risk testing and pro-dopamine regulation (KB220/POLYGEN®) to help induce “dopamine homeostasis” in patients with PANDAS, especially those with known DNA-induced hypodopaminergia. This case study examines a 12-year-old Caucasian male with no prior psychiatric issues who presented with a sudden onset of severe anxiety, depression, emotional liability, and suicidal ideation. The patient underwent genotyping and the genetic addiction risk score (GARS) testing, which revealed risk polymorphisms in the dopamine D2 (-DRD2/ANKK (Taq1A), OPRM1 (A/G), DRD3 (C/T), and MAOA (4R) genes. These polymorphisms have been linked to hypodopaminergia. The patient was subsequently placed on research ID-KB220ZPBMPOLY (POLYGEN®), and albeit the possibility of bias, based upon self and parental assessment, a marked rapid improvement in psychiatric symptoms was observed. In the second phase of treatment (102 days utilizing KB220), the patient received standard antibody testing, which was positive for Lyme. Antibacterial therapy started immediately, and KB220z was discontinued to provide a wash-out period. A monotonic trend analysis was performed on each outcome measure, and a consistently decreasing trend was observed utilizing antibacterial therapy. Our recommendation, albeit only one case, is to utilize and further research a combined therapeutic approach, involving precision-guided DNA testing and pro-dopamine regulation along with antibacterial therapy, as well as

Review and Analysis: Successful Use of Soil Tunnels in Medieval and Modern Warfare and Smuggling

For more than 2500 years, soil tunnels have been used in warfare and smuggling. Initially tunnels were utilized to attack fortresses that were underlain by unconsolidated (non-bedrock) soil materials. Later tunnels provided housing and served as smuggling corridors. The medieval warfare undermining technique involved digging soil tunnels with wooden or beam props to hold up the soil ceilings. Then flammable material, such as hay or straw, was put in the tunnel and set on fire. The fire burnt the support beams which collapsed the soil tunnel ceilings and undermined the overlying perimeter wall. Later gunpowder and dynamite replaced fire when attempting to collapse a tunnel, fortress or perimeter defense. Modern warfare soil tunnels were the pathways used to move troops, weapons and supplies to the other side of a border or wall for surprise attacks. Most of the soil tunnels were placed in easy-to-dig unconsolidated soil materials that had a low water table and were not subject to flooding. Eventually, machinery was used to drill through bedrock permitting deeper and longer tunnels for troop movement or smuggling. However, when drilling through bedrock under international borders, the process creates both noise and vibrations which were often detected by the enemy. Once discovered the tunnels were often collapsed by blowing up the tunnel, injection of gas, filling with water or wastewater, or inserting barriers. A series of case studies will be examined with the goal of determining soil and site criteria required to permit successful tunneling. The most restrictive soil and geologic conditions will be identified as well as potential mitigation methods used to overcome the site restrictions will be documented. Countries with warfare or smuggling issues along their borders, such as israel and United States, need to identify the sections of the border most likely to be undermined by soil tunnels. We refute the controversial statement that addiction is not a brain disorder. Extensive peer-reviewed studies support the underlying neurobiological and neurogenetic basis of addiction’s “disease model”. In the 70s and 80s, a few clinical scientists suggested that it is possible to use behavioral training to teach controlled drinking. However, this controversial model failed drastically and increased labeling and stigmatization. Additionally, it was unhelpful in the search for treatment. Instead, we assert that addiction is a neuropsychiatric disorder characterized by a recurring desire to continue taking substances despite harmful physical and mental consequences. Work from our laboratory in 1995 supported the Reward Deficiency Syndrome (RDS) concept based on a common neurogenetic mechanism (hypodopaminergia) that underlies all substance and non-substance addictions. Non-substance addictions include behaviors like pathological gambling, internet addiction, and mobile phone addiction. Certain impulsive and compulsive behaviors or the acute intake of psychoactive substances result in heightened dopaminergic activity, while the opposite, hypodopaminergia, occurs following chronic abuse. Patients with Substance Use Disorder (SUD) can have a genetic predisposition compounded by stress or other epigenetic insults that can impact recovery. Relapse will occur post-short-term recovery if dopaminergic dysfunction remains untreated. Addiction, a brain disorder, requires treatment with DNA-directed pro-dopamine regulation and rehabilitation.
