AIM: To investigate the clinical efficacy of leukocytapheresis (LCAP) in patients with active ulcerative colitis (UC), and to elucidate the mechanisms by determining the changes in the cytokine levels in the peripheral blood and of the functions of the peripheral blood leukocytes in these ***: The subjects were 19 patients with active UC, with a mean clinical activity index (CAI) of 9.2. The LCAP was conducted using Cellsorba E. In each session of LCAP, 2-3 L of blood at the flow rate of 30-50 mL/min was processed. The treatment was carried out in approximately 1-h sessions, once a week, for 5-10 wk. Blood samples for determination of the cytokine levels were collected from the inflow side of the column (site of dehematization; at the start of LCAP) and outflow side of the column (at the end of LCAP). Blood samples for the determination of reactive-oxygen-producing cells were collected from the peripheral blood before and after ***: LCAP resulted in clinical improvement in all the 19 patients of UC recruited for this study. Remission (CAI: ≤4) was noted in 15 (79%) of the 19 patients. The blood level of the pro-inflammatory cytoline IL-6 was found to be decreased following treatment by LCAP, and the level of the anti-inflammatory cytokine IL-10 at the outflow side of the LCAP column was found to be significantly elevated as compared to that at the inflow side of the column. The reactive-oxygen-producing granulocytes in the peripheral blood of UC patients was increased as compared to that in healthy persons and the increase was found to be decreased following treatment by ***: LCAP exerted a high therapeutic efficacy in patients with active UC. Our findings suggest that LCAP is associated with enhanced production of the inhibitory cytokine IL-10 to indirectly inhibit the functions of the inflammatory leukocytes, and that inflammation is also considerably attenuated by the direct removal of reactiveoxygen-producing neutrophils from th

A 74-year-old woman presented with symptoms consistent with hyperadrenocorticism and hyperca- techolaminism. She had a cushingoid appearance and her cortisol level was elevated. Her serum dopamine and noradrenalin levels were also elevated. Computed tomography detected a left adrenal mass measuring 3.5 cm × 3.0 cm in diameter. Metaiodobenzylguanidine scintigraphy was negative. Unexpectedly, the serum Serum carcinoembryonic antigen (CEA) level waselevated. Fluorodeoxyglucose positron emission tomography showed increased uptake in the adrenal tumor only, with a maximum standardized uptake value of 2.8. Selective venography and blood sampling revealed that the concentrations of cortisol, catecholamines and CEA were significantly elevated in the vein draining the tumor. A diagnosis of CEA-producing benign adenoma was made. After preoperative management, we performed a combined lateral and anterior transperitoneal laparoscopic adrenectomy. Her vital signs remained stable during surgery. Histopathological examination revealed a benign adenoma. Her cortisol, catecholamine and CEA levels normalized immediately after surgery. We present, to the best of our knowledge, the first case of CEA-producing adrenal adenoma, along with a review of the relevant literature, and discuss our laparoscopic surgery techniques.

The purpose of this multicenter study was to evaluate the clinical performance of an ultrahigh molecular weight polyethylene (UHMWPE) fiber cable for re-attachment of the osteotomized greater trochanter in hip surgery. Included in the study were 85 hips that had undergone surgery with greater trochanter osteotomy, including 50 hip arthroplasty procedures and 35 hip osteotomies. The osteotomized greater trochanter was reattached using one or more UHMWPE fiber cables. The bone union and displacement of the greater trochanter were assessed in radiographs for up to 12 months after surgery. Non-union of the osteotomy site occurred in 4.7% of the cases. In approximately 90% of the cases, displacement was less than 2 mm at up to 12 months after surgery. The UHMWPE fiber cable was a good biomaterial for reattaching the osteotomized greater trochanter and may also be an option for osteosynthesis procedures.

Computed tomography characteristics and complete resection of large thymoma in a cat

Thymoma is an intrathoracic malignant tumor occurring rarely in cats. Although surgery is the treatment of choice in feline thymomas,complete resection may be difficult in cases that the large vessels are involved into the ***,diagnostic imaging techniques including computed tomography( CT) are essential for the treatment of feline thymomas. This report aimed to present the CT characteristics and successful removal of very large thymoma in a cat with exfoliative *** 8-year-old spayed mongrel cat was referred to our hospital with chief complaints of chronic anorexia,alopecia,and desquamation. Tachypnea,tachycardia,and exfoliative dermatitis were observed in the first presentation. Thoracic radiography showed a very large mass in the craniomediastinal region. In the CT findings,the mass included multiple small cysts,and forced up the intrathoracic trachea,cranial vena cava,brachiocephalic trunk and left subclavian artery as well as heart and *** the 6th day after,the patient underwent a median sternotomy and diaphragmatic incision to expose the mass. The mass was completely resected with right phrenic nerve by using cautery and LigaSure. No postoperative complications were observed. The histopathologic diagnosis was made as a thymoma. Postoperative progress of the patient was excellent without the exfoliative *** enlarged thymoma could be completely resected in this patient,and the exfoliative dermatitis was postoperatively controlled. The paraneoplastic immune disorder was suggested to cause the dermatitis. CT has a great impact on the indication of surgery for the feline thymomas.

Hepatocellular carcinoma (HCC) is generally observed as a massive tumor originated from one lobe in *** is the treatment of choice in dogs with a massive HCC. However,complete resection is technically difficult in the cases with an extremely enlarged HCC pressing or involving the large vessels such as caudal vena cava and main hepatic veins. This study aimed to report the divisional hepatic lobectomy for complete resection of very large HCC and successful treatment of paraneoplastic hypoglycemic seizures in a dog.A 12-year-old spayed Yorkshire terrier was referred to our hospital with chief complaints of seizures and a very large hepatic mass. In the first presentation,serum liver enzymes levels increased,and serum glucose concentration was 29mg/dl. Computed tomography (CT) angiography revealed that a huge mass occurred in the right medial,quadrate,and left medial hepatic lobes and flattened caudal vena cava and main portal vein. The patient underwent surgical *** mass was approached with midline celiotomy,caudal median sternotomy,and bilateral paracostal laparotomy. En bloc resection of central and left divisional lobes including the mass was accomplished with surgical ligation of hepatic hilar vessels and hepatic veins. The hypoglycemic seizures were postoperatively resolved,and no clinical signs were *** hypoglycemic seizures were thought to be associated with the insulin-like growth factors (IGF). The right divisional lobes could serve the sufficient hepatic function. Divisional hepatic lobectomy based on CT angiography is very useful for the treatment of canine HCCs.

We present a rare case of rapidly enlarging inflamematory hepatocellular adenoma (IHCA) in a 60-year-old Japanese man. Screening abdominal computed tomography (CT) for the fatty liver patient revealed a 1.7-cm liver mass in the anterior segment of the liver. After 19 months, the lesion had rapidly enlarged to 6 cm in diameter and the patient was referred to our hospital. On perflubutane microbubble contrast-enhanced ultrasonography, the tumor showed a characteristic centripetal filling pattern in the vascular phase. We performed hepatic anterior segment resection because we could not rule out malignant tumor. Histopathological examination showed hyperplasia of mildly atypical hepatocytes and sinusoidal dilatation with marked inflammatory cell infiltration. Immunohistological staining revealed positive staining for serum amyloid A and C-reactive protein;therefore, we diagnosed this tumor as IHCA. The patient remains alive 42 months after operation without evidence of recurrence.
